임신합병증 예측에 있어 다운증후군 통합 선별검사 지표의 의의

목적 : 임신 제1삼분기와 2삼분기에 시행하고 있는 다운증후군 선별검사 지표들이 다른 임신합병증 예측에 있어서 갖는 의의를 알아보고자 한다. 대상 및 방법 : 2005년 1월부터 2006년 12월까지 만2년간 강남차병원 산부인과에서 산전진찰을 받으며 임신 제1삼분기에 PAPP-A와 NT, 2삼분기에AFP, hCG, Inhibin-A, 그리고 uE3로 다운증후군 선별검사를 시행 받고 분만한 3,121명의 산모와 이들의 신생아를 대상으로 하였다. 의무기록지 검토를 통해 산모의 나이, 임신합병증과 제태연령, 분만시와 분만 후의 합병증 유무, 그리고 integrated test 시기와 각 지표의 수치를 조사하여 SPSS 프로그램을 이용하여 정규성 검정과 t-test, 그리고 로지스틱 회귀분석을 시행하였다. 결과 : 다운증후군 선별검사의 표지물질들이 다른 임신 합병증에서도 이상소견을 보였는데, 특히 조산과 자간전증 시에 AFP 수치의 증가, hCG 증가, Inhibin-A 증가, PAPP- A 감소, NT 감소가 있었다. Inhibin-A는 자간전증, 저체중아 출산, 조산시에 임신 제2삼분기에 증가되어 있었는데 odds ratio는 각각 2.843, 1.446, 1.287이었다. AFP는 임신 24주 이전의 태아손실(odds ratio 2.868)과 조산(odds ratio 1.653)시에 임신 제2삼분기에 증가하였고, 임신 제1삼분기의 PAPP-A는 자간전증(odds ratio 0.51)과 조산(odds ratio 0.75)시에 감소함을 알 수 있었다. 결론 : 모든 산전 클리닉에서 시행하고 있는 다운증후군 선별검사의 지표 중 특히 임신 제2삼분기의 Inhibin-A와 AFP, 제1삼분기의 PAPP-A의 이상 수치는 태반기능 관련 임신합병증인 조산, 자간전증과 저체중아 출산의 동반 가능성이 높아 이를 이용하면 이들 질환의 고위험군을 분류할 수 있으며, 이런 산모를 대상으로 보다 주의깊은 산전관리와 상담이 가능할 것으로 생각된다. Purpose: To assess the value of first-trimester pregnancy-associated plasma protein-A (PAPP-A), nuchal translucency (NT) and second-trimester alpha-fetoprotein (AFP), human chorionic gonadotropin (hCG), unconjugated estriol (uE3), and inhibin-A in predicting pregnancy complications other than fetal aneuploidy. Materials and Methods: A retrospective study in 3,121 singleton pregnancies with integrated testing was performed at Kangnam CHA hospital between January 2005 and December 2006. Baseline characteristics, pregnancy outcomes, and serum marker levels were obtained by review of the medical records. We analyzed the data to identify associations between the integrated screening markers and adverse pregnancy outcomes. Statistical analyses were performed with the SPSS program. Results: In preterm labor and preeclampsia, high AFP, hCG, and inhibin-A levels and low PAPP-A and NT levels were found to be significantly correlated (P<0.05). Elevated second-trimester inhibin-A levels were associated with preeclampsia (odds ratio 2.843), low birth weight (odds ratio 1.446), and preterm labor (odds ratio 1.287), and while decreased first-trimester PAPP-A levels were associated with preeclampsia (odds ratio 0.51) and preterm labor (odds ratio 0.75). Conclusion: First- and second-trimester maternal serum markers screening can be used for predicting high-risk pregnancies.

소의 뇌로부터 Protein Farnesyl-Cysteine Carboxyl-Methyltransferase의 정제에 관한 연구

Protein farnesyl-cysteine carboxyl-methyltransferse (PFCCMT) has been known to be involved in posttranslational modification of several important signal transduction proteins in eukaryotes including ras superfamily, γ-subunit of heterotrimeric G protein, α-subunit of cGMP phosphodiesterase isolated from retinal rod, yeast mating pheromone, which raises the possibility that carboxyl methylation may play a central role in the regulation of stimulus-response coupling in eukaryotic cells. The enzyme has been purified from bovine brain membranes, utilizing a synthetic substrate, N-acetyl-S-farnesyl-L-cysteine (AFC) , as the methyl acceptor substrate. The membrane-bound enzyme was found to be most effectively extracted from bovine brain with the buffer containing 0.6% 3 [3-cholamidopropyl) -dimethylammonio) -1-propane- sulfonate(CHAPS). The solubilized enzyme was purified by Fast Protein Liquid Chromatography (FPLC) employing Superose 6 and Mono Q columns, and non-denaturing polyacrylamide gel electrophoresis(ND-PAGE) 115 fold with the final specific activity of 496 pmol of [methyl-^(14)C] group trasfer/min/mg protein, and single band could be visualized on sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE). The molecular weight of the enzyme appeared to be 76,000 dalton on NDPAGE and 38,000 dalton on SDS-PAGE, indicating that it is composed of two identical subunits. Additionally, AFC was found to act as inhibitor in high concentrations above 0.1 mM, and the almost same enzyme activity measurements with and without AFC intriguingly suggests that the enzyme exists in complex strongly bound to in vivo substrates in the cell.
