        경안면 괴사성 근막염 : 증례보고

        Necrotizing fasciitis is defined as infectious disease showing extensive necrosis of the superficial fascia with widespread involvement of the surrounding tissues and concurrent systemic toxicity. It is found commonly in the extremities, the trunk and the perineum, but shows extremely rare occurrence in the well vascularized area such as head and neck area. Occurred in the head and neck area, it is called "cervicofacial necrotizing fasciitis" (CFNF). It is a fatal disease which carries a mortality rate up to 60%. Early diagnosis, aggressive surgical debridement of necrotic tissues and massive antibiotics therapy are essential for achieving a favorable outcome. We present a case of cervicofacial necrotizing fasciitis in 35-year-old male patient with literature review.

        재난의 경험과 사건공동체의 구성 - 1972년 남한강 홍수와 원주 재해위 활동가들의 기억을 중심으로 -

        원주는 한국 협동조합운동의 중심지이다. 1965년 원주교구가 설정되고, 초대 주교로 지학순 신부가 선임된다. 지학순 주교는 사회운동가 장일순과 함께 다양한 사회운동을 펼치면서 지역 사회 발전을 위해서 노력한다. 두 사람은 지역의 낙후된 경제와 반민주적 정치 현실이 한국사회의 구조적 문제로부터 기인한다고 생각했다. 이들이 다양한 문제를 해결할 수 있는 방안으로 생각한 것이 협동조합운동이었다. 1972년 8월 남한강 유역에 홍수가 발생하고, 많은 사람들이 피해를 입었다. 지학순 주교는 외국의 구호 기관에 복구 자금을 요청했고, 이 복구 자금으로 민 관 학이 연계된 재해대책사업위원회를 구성한다. 재해위는 수해 피해에 대한 단순복구 작업이 아닌 기존의 모습과 다른 새로운 공동체를 만들고자 했다. 협동조합운동은 새로운 공동체 구성의 핵심적 요소가 되었다. 재해위는 박정희 정부의 감시와 억압, 기존 체제의 반발 등을 겪었다. 비록 재해위가 1970년대 재해위가 추진한 남한강 사업이 성공했다고 단정할 수 없지만, 당시의 경험은 많은 사람들에게 경험의 체득되어 이후 다양한 곳에서 그 힘을 발휘하게 된다. 더불어 원주가 현재 한국 협동조합운동의 중심지가 되었던 원동력이 되었다. Wonju is the center of Korean cooperative movement. In 1965, the Diocese of Wonju was established, and Father Ji Hak-sun was appointed as the first bishop. Bishop Ji Hak-sun strived for the development of the local community as he carried out various social movements with the social movement activist, Jang Il-sun. The two thought that the region’s lagging economy and anti-democratic political reality stemmed from structural problems in Korean society. It was cooperative movement that they considered as a way to solve various problems. In August 1972, a flood occurred in the Namhan River basin, and many people were damaged. Bishop Ji Hak-sun requested recovery funds from a foreign aid agency, and this restoration fund constituted the Relief and Rehabilitation Committee, which linked the public-government-academia together. The Relief and Rehabilitation Committee wanted to create a new community different from the existing one, rather than simply recovering from flood damage. The cooperative movement has become a key element in the formation of a new community. The Relief and Rehabilitation Committee suffered from the surveillance, repression, and backlash of the existing regime, Park Jeong-hee government. Although we cannot conclude that the Namhan River project promoted by the Relief and Rehabilitation Committee in the 1970s was successful, but the experience at the time was acquired by many people, and after that, the power was exerted in various places. In addition, it became the driving force to make Wonju become the center of Korean cooperative movement.

      • 돈육의 지방산과 아미노산 조성 및 관능 특성에 미치는 미생물제제 및 고구마 사일리지 급여 효과

        3원교잡종(LY×D) 돼지를 이용하여, 60일 동안 미생물제제 K를 3% 급여한 A 처리구와 O를 3% 급여한 B 처리구 및 가고시마흑돼지에게 10% 고구마 사일리지를 급여한 C 처리구로 각 처리구별로 30두씩 공시하여 사양 시험 후 돈육의 지방산, 아미노산 조성 및 관능적 특성에 미치는 효과를 분석하였다. 가열육의 관능검사에서는 B 처리구가 다른 처리구들에 비하여 유의적으로 높은 값을 보였다(p<0.05). 처리구 간에 지방산 조성 변화를 보면 palmitic과 palmitoleic acid 함량은 C 처리구가 다른 처리구들에 비하여 유의적으로 높았다(p<0.05). Aspartic, threonine, serine, glutamic, isoleucine과 phenyl alanine 함량은 C 처리구가 다른 처리구들에 비하여 유의적으로 높았다(p<0.05). Proline, alanine, cystine, valine, methionine, leucine과 histidine 함량은 B 처리구가 다른 두 처리구들에 비하여 유의적으로 낮은 아미노산 함량을 보였다(p<0.05). A total of 90 pigs were used to investigate the effect of feeding microbes and sweet potato silage on physico-chemical characteristics of pork loin. About 60 kg pigs were randomly alloted into one of three experimental diet groups (A: Supplemented with 3% K microbe; B: Supplemented with 3% O microbe; C: Supplemented with 10% sweet potato silage). Pigs were slaughtered at approximately 110 kg live weight and fatty acid, amino acid and sensory characteristics were measured in pork loin. In cooked meat, sensory evaluation was significantly higher in B compared to other treatments (p<0.05). The palmitic and palmitoleic acid content of C treatment were significantly higher than those of other treatments (p<0.05). In amino acid composition, aspartic acid, threonine, serine, glutamic acid, isoleucine and phenyl alanine level were significantly higher in C treatment compared to other treatments (p<0.05).

      • 산삼 배양액 급여가 돈육의 이화학적 특성에 미치는 영향

        Berkshire를 이용하여 70일간 산삼 배양액 급여 시 육의 이화학적 특성에 미치는 영향은 다음과 같다. 산삼 배양액의 급여에 의해 육의 수분, 조단백, 조지방, 회분, pH, 가열감량, 콜레스테롤 함량, 육색, 조직감의 응집성과 검성, 포화지방산 및 불포화지방산의 비율은 차이를 보이지 않았다. 보수력, 지방색 L^(*)값, 지방산 중 linoleic acid(18:2), arachidonic acid(20:4), 필수지방산의 함량, 아미노산 중 threonine, glutamic acid, valine, isoleucine, leucine, histidine, lysine 및 총아미노산 함량은 산삼 배양액의 급여에 의해 감소하였다. 전단가, 조직감의 표면경도, 경도, 씹힘성, 지방산 중 stearic acid(18:0), oleic acid(18:1) 함량, 아미노산 중 serine, glycine, cystine, methionine, tyrosine 및 phenylalanine의 함량은 산삼 배양액의 급여에 의해 증가하였다. 본 연구결과를 종합하면 산삼 배양액의 급여는 돈육의 경도를 증가시키는 효과 외에 뚜렷한 이화학적인 차이를 나타내지 않았다. 이러한 이유는 급여된 산삼 배양액 내에 함유된 생리활성 물질의 함량이 낮고, 급여수준이 낮은 결과로 사료되어진다. A total of 60 pigs (Berkshire) were used to investigate the effect of dietary artificial culture medium of wild ginsengs (CMG) on physico-chemical characteristics of pork. About 60 kg pigs were randomly alloted into one of two experimental diet groups (C:commercial diet feed; T:commercial diet + 1 L CMG per day for 70days). Pigs were slaughtered at approximately 110 kg live weight, and chemical composition and physico-chemical characteristics were measured in pork loin. Moisture, crude protein, crude fat and ash were not significantly different by dietary artificial culture medium of wild ginsengs. Water-holding capacity was decreased by dietary artificial culture medium of wild ginsengs, while shear force was increased. However, pH, cooking loss and cholesterol contents were not significantly different between two different treatments. In color, L^(*) of fat was decreased by dietary artificial culture medium of wild ginsengs. Brittleness, hardness and chewiness were increased in dietary artificial culture medium of wild ginsengs group. In fatty acid, stearic acid (18:0) and oleic acid (18:1) were higher in dietary artificial culture medium of wild ginsengs group, whereas linoleic acid (18:2) and arachidonic acid (20:4) were higher in control group. In amino acid, serine, glycine, cystine, methionine, tyrosine and phenylalanine were higher in dietary artificial culture medium of wild ginsengs group, whereas threonine, glutamic acid, valine, isoleucine, leucine, histidine and lysine were higher in control group.

        백서 척수후근절로부터 슈반세포의 효과적인 체외 배양법

        Schwann cells(SCs), an important component of the peripheral nervous system, intract with nerous to mutually support growth and replication for the peripheral nerve regentation. Recently, ading SCs to the lumen of guidance channel is widely tried to improve regeneration or to make regeneration possible over otherwise irreparable gaps. however, it is not easy to isolate and multiplicate SCs as much as enough to help the axonal regeneration. For the allogeneic SCs source for tubular nerve guidance, we developed a little bit improved technique of harvesting and multiplicating SCs. by culturing dispersed dorsal root ganglia in specially designed medium with growth factors and serial processing, we repeatedlly generate relatively homogenous SC cultures. Our technique was compared with other methods of literature using immunostaining methods such as GFAP, S100, BDNF and the total SC count assessment at different time interval after primary culture.

      • 골반 교정운동이 요부형태의 천골각 및 통증에 미치는 효과

        The purpose of this study was to examine the effects of pelvis corrective exercise for low back pain sampled from high school baseball players who complained their low back pain. Lumbar shape, and pain responses were examined before and after treatment to identify the effects of pelvis corrective exercises. The number of subjects were 39 consisted of four groups: control group, active goroup, assistive group, and active/assistive combined group. The time periods of treatments were 30 minutes per day, 5 times a week, total 18 weeks. Lumbar X-ray, DITI(Digital Infrared Thermographic Imaging) were utilized before and after treatment for data collection. Kruskal-Wallis Analysis was used to compare the differences of lumbar shapes as well as the differences of pain responses in between groups. The results of this study were as follow: 1. The effects on lumbar sacral angle among tests of lumbar shape were found most forcefully in assistive group and active/assistive combined group next. 2. The effects on pain responses were shown in rank such as active/assistive combined group first, assistive group second, and active group third. Therefore, pelvis corrective exercises could be regarded as an effective method to correct body type and relieve low back pain.

      • 백서 발치와의 치유과정에서 Dexamethasone이 미치는 영향

        백서 발치와의 치유과정에서 dexamethasone이 미치는 효과에 대해 관찰하고자 생후 2개월의 Sprague-Dawley계의 백서 24마리의 발치와를 이용하여 실험을 시행하였다. 실험군과 대조군 모두 발치를 용이하게 하고 외상을 최소화 하기위해 발치전에 0.2g/kg β-APN을 음용수에 용해시켜 5일간 투여하였고, 발치후에는 감염방지를 위해 1.5㎖/kg ampicillin을 하루에 한번씩 근육주사하였고 실험군에서는 0.2㎖/kg dexamethasone을 하루에 2회씩 3일 동안 근육주사를 통하여 투약하였다. 각 군은 발치후 1,3,7,15 일 간격으로 3마리씩 희생시켜 조직학적인 검경과 TRAP 염색에 의한 파골세포 활성의 관찰을 시행하여 다음과 같은 결과를 얻었다. 1. 전반적인 치유양상은 실험군과 대조군 공히 유사한 양상이었으나 실험군이 대조군보다 골성회북이 지연되었다. 2. 파골세포의 활성은 대조군에서는 3일째까지 증가되고 이후에 감소되는 양상이었으나 실험군에서는 7일째까지 증가되었다가 이후에 감소된 양상을 보였다. 3. 골조직의 변화에서 골주형성이 7일째에 관찰되었으나 실험군이 대조군보다 미약하였고 파골세포의 활성은 7일째를 제외하고 양군간에 통계학적으로 유의성있는 차이를 보였다(p<0.05,p<0.01). 이상의 연구를 미루어 볼 때 dexamethasone은 생체조직 치유과정에서 발치와의 치유와 파골세포의 활성에 관여하여 전반적인 치유양상을 지연시키는 것으로 나타났다. The purpose of this study was to evaluate the effect of dexamethasone on the healing aspect of gingiva and alveolar bone after extraction. Extracted socket of 24 Sprague-Dawley rat was used. To extract easily and minimize injury, β-APN 0.2 g/kg / day soluted in mimeral water was administrated for 5 days before extraction in both group. Ampicillin 1.5 ㎖/kg i.m.,q.d. was administered for preventing infection after teeth extraction in both group, and dexamethasone 0.4mg/kg/day was injected for 3 days in experimental group. 3 rats on each day was sacrificed on 1, 3, 7, 15 days after extraction. Histologic examination and the activity of osteoclasts by tartrate resistant acid phosphatase was observed. The results were as follows: 1. The Overall healing pattern was similar with both the experimental and control group, but in experimental group osseous healing was delayed. 2. The activity of osteoclasts increased to day 3 and then decreased after day 3 in the experimental group. In comparison to the control group, the experimental group showed increased appearance to day 7 and then decreased appearance following day. 3. Regarding to the change of osseous tissue, the activity of osteoblasts was shown at day 7, but osteoclastic activity of the experimental group was less than that of the control group. The osteoclastic activity was statistically significant between two groups except day 7 (p<0.05, p<0.01). In conclusion, the effects of dexamethasone for healing of extraction socket were considered as limiting the activity of osteoclasts, and the healing of extraction socket was delayed.

      • 퍼지하드웨어 시스템의 구성기법에 관한 연구

        본 연구는 일반적으로 사용되고있는 아날로그형 퍼지제어기를 디지탈 하드웨어 시스템으로 구성하는 기법을 제안한다. 이를 위해 퍼지제어기의 기본 기능인 퍼지화, 추론규칙, 비퍼지화 등을 분석하고, 퍼지데이타의 취급에 적합하도록 세그먼트와 페이지로 이원화된 메모리를 구성하였다. 이를 토대로 퍼지제어에 필요한 퍼지명령어를 추출하고, 이것들에 적합한 명령어 형식과 디지탈 하드웨어 시스템의 구성기법안을 제안하였다. 또한 제안된 시스템을 VHDL로 기술하여 시뮬레이션을 행한 결과, 그 타당성을 확인하였다.
