Now a days, hematopoietic stem cell transplatation can be used as an effective treatment modality for bone marrow failure syndromes such as aplastic anemia, malignancies, and certain metabolic disease. Hematopoietic stem cells are the population of cells capable both of self renewal and differentiation into a variety of hematopoietic lineages. It is suggested that clinical practice in the areas of bone marrow transplantation and gene therapy might rely on the ex vivo expansion of hematopoietic stem/progenitor cells. The enumeration of cell expression of CD34 and granulocyte macrophage colony forming unit (CFU-GM) are used for assessing the efficacy of human hematopoietic stem cell enrichment technique. The colony stimulating factors (CSF) stimulate the proliferation and differentiation of progenitor cells. We pursued a series of experiments to define the proper conditions for the expansion of hematopoietic stem cells in the short term liquid suspension culture of bone marrow cells and to define optimal combination of colony stimulating factors(CSFs). Liquid cultures were initiated with 1x10 bone marrow mononuclear cells (BM MNCs), and innoculated with 1x10 CD 34 cells, which were isolated from BM MNCs by flow cytometry, in 3 well petri dishes with the various hematopoietic growth factors (HGF). The recombinant human HGFs used are 100ng/mL of stem cell factor (SCF), 100 ng/mL of granulocyte colony-stimulating factor (G-CSF), 100 ng/mL of granulocyte-macrophage colony-stimulating factor (GM-CSF), 100ng/mL of interleukin-3 (IL-3) and combination of SCF 100ng/mL, GM-CSF 100ng/mL, IL-3 100ng/mL. At the end of the culture, colony forming cells were evaluated by semisolid clonogenic assay(CFU-GM) and CD34+ cells were enumerated with flow cytometry. On the third day of the culture, CD34+ cells were expanded 14.98 fold with the addition of SCF, 7.82 fold with G-CSF, 11.12 fold with GM-CSF, 15.53 fold with IL-3 and 18.32 fold with SCF, GM-CSF and IL-3. On the seventh day of the culture, CD34+ cells were expanded 5.82 fold with SCF,5.91 fold with G-CSF,13.94 fold with GM-CSF, 6.43 fold with IL-3 and 9.45 fold with SCF, GM-CSF and IL-3. In CFU-GM assay of the three day culture, CFU-GM counts were 37.5 with SCF, 74.1 with G-CSF, 90.5 with GM-CSF, 126.7 with IL-3, and 125.8 with SCF, GM-CSF and IL-3. The CFU-GM counts of the seven day culture are 31.4 with SCF, 44.6 with G-CSF, 42.3 with GM-CSF, 55.2 with IL-3, and 90.2 with SCF, GM-CSF and IL-3. The three day culture of BM MNCs was more effective than the seven day culture of CD34+ cell expansion and CFU-GM expansion. The results of this study suggest that short term culture of bone marrow cells could expand hematopoietic progenitor cells with the addition of HGFs. Especially, three day culture of BM MNCs with the addition of SCF, GM-CSF and IL-3 might be the most efficient in this system.
Case Reports : Adrenalectomy for Metastatic Disease to the Adrenal Gland from Gastric Cancer

Metastases to the adrenal glands are frequently found at autopsy. In practice, adrenal metastases have generally been accepted as evidence of blood-borne systemic disease. So, clinically curable adrenal metastases is a rare malady. The role for surgical resection in adrenal metastases has not been clearly defined. A 45-year-old man initially underwent total gastrectomy with D2 lymph node dissection for treating his advanced gastric cancer. A solitary adrenal metastases was resected 1 year later. The patient has survived for 3 years and no further evidence of disease was found on his last follow-up examination. We report here on this case to show that for selected cases, surgical resection of adrenal metastases is feasible and this procedure may extend survival for metastatic gastric cancer patients.
Darapladib Binds to Lipoprotein-Associated Phospholipase A2 with Meaningful Interactions

Lipoprotein-associated phospholipase A2 (Lp-$PLA_2$) is a crucial enzyme in atherosclerosis as a potential drug target. The most remarkable Lp-$PLA_2$ inhibitory drug is Darapladib. We determined the binding pose of Darapladib to Lp-$PLA_2$ through docking study. Darapladib formed two hydrogen bonding interactions with the side chain of Tyr160 and Gln352 and several pi-pi interactions with aromatic and aliphatic hydrophobic residues of Lp-$PLA_2$. It is known that the dietylpropan-amine moiety of Darapladib has influence on the improvement of its oral bioavailability and we supposed this in our docking results.

Background Gaucher disease (GD) is an autosomal recessive disorder characterized by excessive accumulation of glucosylceramide in multiple organs. This study was performed to determine the detection rate of GD in a selected patient population with unexplained splenomegaly in Korea. Methods This was a multicenter, observational study conducted at 18 sites in Korea between December 2016 and February 2020. Adult patients with unexplained splenomegaly were enrolled and tested for β-glucosidase enzyme activity on dried blood spots (DBS) and in peripheral blood leukocytes. Mutation analysis was performed if the test was positive or indeterminate for the enzyme assay. The primary endpoint was the percentage of patients with GD in patients with unexplained splenomegaly. Results A total of 352 patients were enrolled in this study (male patients, 199; mean age, 48.42 yr). Amongst them, 14.77% of patients had concomitant hepatomegaly. The most common sign related to GD was splenomegaly (100%), followed by thrombocytopenia (44.32%) and, anemia (40.91%). The β-glucosidase activity assay on DBS and peripheral leukocytes showed abnormal results in sixteen and six patients, respectively. Eight patients were tested for the mutation, seven of whom were negative and one patient showed a positive mutation analysis result. One female patient who presented with splenomegaly and thrombocytopenia was diagnosed with type 1 GD. The detection rate of GD was 0.2841% (exact 95% CI, 0.0072‒1.5726). Conclusion The detection rate of GD in probable high-risk patients in Korea was lower than expected. However, the role of hemato-oncologists is still important in the diagnosis of GD.

Multiple myeloma is a cancer of plasma cells, a type of white blood cell present in bone marrow. Cytomegalovirus(CMV) is an important pathogen after allogeneic transplantation. However, few studies have examined CMV reactivation after autologous hematopoietic stem cell transplantation (AHSCT) to treat multiple myeloma(MM). Here, we present our case of CMV reactivation following AHSCT in MM with extramedullary plasmacytoma. 52-years-old female patient visited emergency room due to left arm pain. Her pain started 2weeks ago. Her pain aggravated after bumping to the wall. Serum protein electrophoresis showed peak in gamma region with 4.2g and bone marrow aspiration has 48.2% of immature plamsa cell. Initial serum free kappa light chain were 3,760mg/L. Chest PA showed pleura based plasmacytoma and multiple osteolytic lesion in left humerus and pathologic fracure in same bone. So, she was diagnosed lgG kappa tyep multiple myleoma. She received nail fixation operation in the left humerus. After surgery, she treated with four cycles of thalidomide, cyclophosphamide and dexamethasone(TCD). Her plasmacytoma and marrow plasmacytosis disappeared and serum immunofixation electrophoresis turned to negative. Patient completely recovered after transplantation. CMV Ag were detected on D17 after AHSCT, so intravenous ganciclovir started but CMV Ag copy number did not turned negative. Cidofovir were started after 12days treatment of ganciclovir because of CMV Ag were successively increased. CMV Ag turned to negative following two time injection of cidofovir. A retrospective single center paper reported CMV reactivation after AHSCT in MM and it is relatively common. CMV viremia was frequently detected in CMV seropositive patient. CMV surveillance should be considered, especially when tandem transplantation is performed using combination chemotherapy with highdose melphalan. This is not the case of tandem transplantation but CMV surveillance can be useful in clinical setting.
