Background: Current guidelines recommend using nirmatrelvir-ritonavir for coronavirus disease 2019 (COVID-19) treatment, but its potential drug interactions and contraindications limit its applicability in certain categories of patients. The aim of the study was to evaluate the real-world effectiveness of molnupiravir and nirmatrelvir-ritonavir in managing COVID-19 among hospitalized patients. Methods: We conducted a retrospective cohort study among hospitalized COVID-19 patients who received molnupiravir or nirmatrelvir-ritonavir and did not require baseline supplemental oxygen from February 2022 to January 2023. We compared the effectiveness of molnupiravir and nirmatrelvir-ritonavir with a focus on disease progression. Results: The study included 401 high-risk, hospitalized adult COVID-19 patients who received molnupiravir or nirmatrelvir-ritonavir. No significant difference was found in disease progression, the composite outcome of disease progression (4.0% vs. 1.4%, P = 0.782), and O2 supplementation via nasal prong (21.8% vs. 14.8%, P = 0.115) between the patients treated with molnupiravir and those treated with nirmatrelvir-ritonavir. This finding was similar after 1:1 propensity-score matching. In the multivariate analysis, molnupiravir treatment was not significantly associated with progression to severe disease. Conclusion: In conclusion, our findings suggest that similar to nirmatrelvir-ritonavir, molnupiravir has a distinct potential role in COVID-19 treatment, transcending its current perceived status as only a secondary option.
The First Case of Monkeypox in the Republic of Korea

A rapid outbreak of monkeypox is ongoing in non-endemic countries since May 2022. We report the first case of monkeypox in the Republic of Korea. This occurred in a 34-year-old male patient who traveled to Europe in June 2022. On the day of his return to the Republic of Korea (June 21, 2022), the patient presented with a genital lesion. The results of the monkeypox real-time polymerase chain reaction tests were positive in the penile ulcer, oropharyngeal and nasopharyngeal specimens. The patient subsequently developed fever and skin rash after hospital admission. Careful history taking along physical examination should be conducted in the patients who have epidemiologic risk factors for monkeypox. Moreover, appropriate specimens should be obtained from lesions and tested for the monkeypox virus.

<▼1><P>Supplemental Digital Content is available in the text</P></▼1><▼2><P><B>Abstract</B></P><P><I>Coxiella burnetii</I> is a common cause of blood culture–negative infective endocarditis (IE). Molecular detection of <I>C burnetii</I> DNA in clinical specimens is a promising method of diagnosing Q fever endocarditis. Here, we examined the diagnostic utility of Q fever polymerase chain reaction (PCR) of formalin-fixed heart valve tissue from patients with blood culture–negative IE who underwent heart valve surgery. Clinical and laboratory data of patients with blood culture–negative IE who underwent heart valve surgery during a 6-year period and for whom biopsy tissues were available were reviewed retrospectively. Blood culture–positive IE patients who underwent heart valve surgery within the last 3 years were used as controls. Heart valve samples were cultured and also subjected to histological examination and PCR for Q fever, brucellosis, and bartonellosis. Data from 20 patients with blood culture–negative IE and 20 with blood culture–positive IE were analyzed. Eight cases of blood culture–negative IE were PCR-positive for <I>C burnetii</I> (40%; 95% confidence interval, 19–64). No specimen was PCR-positive for brucellosis or bartonellosis. Histologically, 4 of 8 specimens with a positive Q fever PCR result were characterized by clusters of multinucleated giant cells without a fibrin ring. None of 20 patients with blood culture–negative IE received anti-<I>Coxiella</I> antibiotic therapy due to lack of clinical suspicion. Six-month mortality was higher in the Q fever PCR-positive group than in the Q fever PCR-negative group [38% (3/8) vs 0% (0/12), <I>P</I> = .049). Of the 20 patients with blood culture–positive IE, none yielded a positive Q fever PCR result for valve tissue. Approximately 40% of patients with culture-negative IE who received heart valve surgery were PCR-positive for Q fever; patients without clinical suspicion suffered high mortality. These data suggest that Q fever IE in patients with culture-negative IE is often missed in routine clinical practice.</P></▼2>

This study explains an agricultural mobile commerce case using Oauth Based Real-Time Information Sharing Technique according to the growth of the agricultural e-commerce market. Considering the characteristics of the agricultural market, it can confirm differential values with the existing agricultural mobile commerce market according to the functions and characteristics of its technique. We compare variables for introducing agricultural mobile commerce with the existing mobile commerce. Trust through information sharing with real-timefarming diaries, suitability and perceived ease of use variables are positively influenced, but influence on completeness and authority of information variables are relatively insufficient. Through this, we explained differential values of agricultural mobile commerce and suggested an applicable business model.

<P>In order to investigate the possibility of using mixed rare earth (Nd<SUB>1/3</SUB>Eu<SUB>1/3</SUB>Gd<SUB>1/3</SUB>)Ba<SUB>2</SUB>Cu<SUB>3</SUB>O<SUB>7-x</SUB> (NEG123) as the superconducting layer of the HTS coated conductor, the NEG123 thin film was deposited epitaxially on LAO(100) single crystal and IBAD_YSZ metal templates by pulsed laser deposition. Systematic studies were carried out to investigate the influences of deposition parameters of PLD on the microstructure, texture and superconducting properties of NEG-123 coated conductor. Deposition at oxygen partial pressure of 600 mTorr was needed to routinely obtain high quality NEG123 films with J<SUB>C</SUB>'s (77 K) over 2 MA/cm<SUP>2</SUP> and Tc's over 90 K (DeltaT ~ 2 K) . We verified from magnetization study that the NEG123 has an improved in-field Jc as the field increases at temperatures between 10 K and 77 K compared with Gd123. The J<SUB>C</SUB> (77 K, self field) and the value of onset T<SUB>C</SUB> of NEG123 thin film on LAO substrate was 4.0 MA/cm<SUP>2</SUP> and 92 K, respectively. This is the first report, to the best of our knowledge, of coated conductors with NEG123 film as the superconducting layer which have Ic and Jc over 40 A/cm-width and 1.6 MA/cm<SUP>2</SUP> at 77 K, self field. This study shows the possibility of using NEG123 film as the superconducting layer of the HTS coated conductor which can be used in high magnetic field power electric devices.</P>
RCP 4.5 기후변화 시나리오 기반의 낙동강 유역의 강우-유출 탄성도 분석

기후 변화는 기후를 구성하는 대기, 해양, 생물, 육지 등의 다양한 구성 요소에 작용하여 자연 생태계와 인간의 사회경제 활동에 큰 영향을 미친다. 기후 변화의 영향을 예측하고 방어대책을 마련하는 것은 변화된 기후에 적응하기 위한 중요한 연구가 될 것이다. 본 연구에서는 IPCC(Intergovermental Panel on Climate Change)에서 개발된 RCP 4.5 시나리오를 이용한 낙동강 권역의 강우-유출 탄성도 분석을 실시하였다. 제어적분 200년의 남한 상세 기후변화 시나리오를 수집하여 낙동강권역에 대해 강우량을 산정하고, Tank 모형을 통해 산정된 유출량을 2가지 시나리오(계절, 연별)로 추출하여 탄성도 분석을 실시하였다. 본 연구의 결과를 통해 우리나라의 기후가 2100년까지 매우 습한 형태 초기 1.129 (0.851~1.523), 중기 1.075(0.756~1.302), 후기 1.043(0.882~1.325)의 기후로 변해 갈 것이라고 예상 하였다. 본 연구의 결과를 통해 기후 변화에 따른 수자원 관리 및 효율적인 수리구조물 적용 방안 및 변화된 기후에 대한 빠른 적응에 대한 연구에 활용 할 수 있을 것이다. Climate change affects the natural ecosystem and human socioeconomic activities by acting on various constituents such as the atmospheric, oceanic, biological, and land constituents of the climate. Predicting the impacts of ongoing climate change will be an important factor in adapting to the climate of the future. In this study, precipitation-streamflow elasticity analysis of the Nakdong River area was conducted using the RCP 4.5 scenario developed by the IPCC (Intergovernmental Panel on Climate Change). Precipitation and streamflow in the Nakdong River area was analyzed using monthly, seasonal, and yearly data. Results found that the climate would become very humid climate by 2100. Results of this study can be applied to adaptation of climate change, management of water resources and efficient utilization of hydraulic structures.
