Long-Term Surgical Outcome of 1057 Gastric GISTs According to 7th UICC/AJCC TNM System : Multicenter Observational Study From Korea and Japan

        Kim, Min-Chan,Yook, Jeong-Hwan,Yang, Han-Kwang,Lee, Hyuk-Joon,Sohn, Tae-Sung,Hyung, Woo-Jin,Ryu, Seung-Wan,Kurokawa, Yukinori,Kim, Young-Woo,Han, Sang-Uk,Kim, Hyung-Ho,Park, Do-Joong,Kim, Wook,Lee, Sa Williams & Wilkins Co 2015 Medicine Vol.94 No.41

Abstract

The aim of this study was to evaluate the treatment and prognosis of gastric gastrointestinal stromal tumors (GISTs) according to the 7th UICC/AJCC tumor-node-metastasis (TNM) system and the modified National Institutes of Health (NIH) risk classification. The study cohort consisted of 1057 patients with gastric GIST who underwent surgery between January 2000 and December 2007 from 13 institutions in Korea and 2 in Japan. Clinicopathologic characteristics, surgical outcomes, recurrence, and 5-year recurrence-free survival were evaluated.

The mean age of the patients was 58.6 years. Thirty patients (2.8%) had distant metastasis preoperatively. Median tumor size was 4.0 cm. Complete resection (R0 resection) was achieved in 1018 patients (96.3%). Eighty-six patients (8.1%) had postoperative complications, and 2 patients (0.2%) died within 30 days after surgery. According to the 7th UICC/AJCC TNM system, 5-year recurrence-free survival rates were 95% to 99% in stage I, 94.1% in stage II, 74.1% in stage IIIA, 48.6% in stage IIIB, and 50.0% in stage IV patients. On survival analysis of high-risk patients according to the TNM system, the 5-year recurrence-free survival rates were 91.6% in stage II, 74.1% in stage IIIA, and 48.6% in stage IIIB patients. Independent factors of recurrence following surgery for gastric GIST were gender, tumor size, mitotic count, and radicality on multivariate analysis.

The treatment outcome and prognosis of gastric GIST in Korea and Japan seem more favorable compared to those in Western countries. Compared to the modified NIH risk classification, the 7th UICC/AJCC TNM system is more reflective of the 5-year recurrence-free survival of patients with gastric GIST.

p53 Nuclear Accumulation as a Possible Biomarker for Biological Radio - dosimetry in Oral Mucosal Epithelial Cells

        KIM, JONG IL,Son, Young Sook,Kim, Jin,Kim, Tae Hwan,Kim, Youn Young,Yook, Jong In 생화학분자생물학회 2002 BMB Reports Vol.34 No.2

Cellular response to ionizing radiation is affected by cell types, radiation doses, and post-irradiation time. Based on the trypan blue dye exclusion assay in normal oral mucosal cells (OM cells), a 48 h post-irradiation was sufffcient and an adequate time point for the evaluation of radiation sensitivity Its LD_(50) was approximately 1.83 Gy To investigate possible biomarkers useful for the biological radiodosimetry of normal epithelial cells (p53, c-fos, cyclin Dl, cdc-2, pRb) EGF receptor phosphorylation and Erk activation were evaluated at different radiation doses and different post-irradiation times. Frorn 0.5 Gy, p53 was accumulated in the nucleus of basal cells of the OM raflk culture at 4 h post-irradiation and sustained up to 24 h post-irradiation, which suggests that radiation-induced apoptosis or damage repair was not yet completed. The number of p53 positive cells and biosynthesis of p53 were correlated with radiation doses. Both cyclin D1 and c-fos were only transiently induced within 1 h post-irradiation. Cyclin D1 was induced at all radiation doses. However, cfos induction was highest at 0.1 Gy, approximately 7.3 fold more induction than the control, whose induction was reduced in a reverse correlation with radiation dose. The phosphorylation pattern of cdc-2 and pRb were unaffected by radiation. In contrast to A431 tails overexpressing the EGF receptor approximately 8.5 fold higher than normal epithelial, the OM cells reduced the basal level of the EGF receptor phosphorylation in a radiation dose dependent fashion. In conch~sion, among radiation-induced biomolecules, the p53 nuclear accumulation may be considered for the future development of a useful marker far biological radiodosimetry in normal epithelial tissue since it was sustained for a longer period and showed a dose response relationship. Specific c-fos induction at a low dose may also be an important finding in this study It needs to be studied further for the elucidation of its possible connection with the low dose radio-adaptive response.

Totally Laparoscopic Distal Gastrectomy with ROUX-EN-Y Reconstruction for Treatment of Duodenal Ulcer Obstruction

        Kim, Min-Gyu,Kim, Beom-Su,Kim, Tae-Hwan,Kim, Kap-Choong,Yook, Jeong-Hwan,Oh, Sung-Tae,Kim, Byung-Sik 대한위암학회 2010 Journal of gastric cancer Vol.10 No.2

Because of advancement of medical treatment, surgical management of gastric or duodenal ulcer was indicated for treatment of perforation, massive hemorrhage and obstruction. The distal gastrectomy including ulcer was known as principle method of duodenal ulcer obstruction, but actually many surgeons have performed only bypass surgery for the difficulty of formation of duodenal stump. In our case, 61-year-old male with repetitive duodenal ulcer obstruction transferred with obstruction due to deformities and inflammations of duodenal ulcer. We had performed totally laparoscopic distal gastrectomy with ROUX-EN-Y reconstruction using the clear visibility of laparoscopy and fine dissections of harmonic scalpel. The patient started soft diet on postoperative day 5 and discharged on postoperative day 8. He returned to work after discharging immediately.

Leafy Vegetable Mix Supplementation Improves Lipid Profiles and Antioxidant Status in C57BL/6J Mice Fed a High Fat and High Cholesterol Diet

        Kim, Mi-Yeon,Cheong, Sun-Hee,Kim, Min-Hee,Son, Chan-Wok,Yook, Hong-Sun,Sok, Dai-Eun,Kim, Jin-Hee,Cho, Yong-Sik,Chun, Hye-Kyung,Kim, Mee-Ree The Korean Society of Food Science and Nutrition 2009 Journal of medicinal food Vol.12 No.4

Daily consumption of an antioxidant-rich leafy vegetable mix (LVM) was assessed for beneficial effects on plasma lipid profiles, tissue lipid peroxidation, and oxidative DNA damage in C57BL/6J mice fed a high fat and high cholesterol diet (20% fat and 1% cholesterol, wt/wt) for 4 weeks. The LVM contained beet leaf, angelica, red leaf lettuce, dandelion, green cos lettuce, lollo rosso, romaine lettuce (12.5%, respectively), scotch kale, and red kale (6.25%, respectively). The mice (n=16) were randomly divided into either the control (high fat and cholesterol diet without LVM) or the LVM (high fat and cholesterol diet with 8% LVM supplement) groups after a 1-week acclimation. Lipid peroxidation as measured by thiobarbituric acid-reactive substances in the plasma, liver, heart, and kidney was significantly lower. Antioxidants (glutathione and $\beta$-carotene) and antioxidant enzyme activities (glutathione peroxidase, glutathione reductase, and superoxide dismutase) were improved in mice fed LVM diet. In the comet assay, tail extent moment, olive tail moment, and tail length were significantly less in the hepatocyte and lymphocyte DNA of the LVM group, indicating the beneficial effect of LVM on the resistance of hepatocytes and lymphocytes DNA to oxidative damage. Findings from the present study suggest that dietary supplementation with LVM may be useful for protecting cells from lipid peroxidation and oxidative DNA damage.

      • Hydrodynamic Interaction Analysis of Floating Multi-body System

Kim, Young-Bok,Kim, Moo-Hyun,Kim, Yong-Yook,Kim, Young-Hun Korean Society of Ocean Engineers 2011 International journal of ocean system engineering Vol.1 No.4

Recently, several problems have occurred in the space, infra-structure, and facility of the contiguity of existing harbors due to the trend of enlarged container vessels. In this regard, the Mobile Harbor has been proposed conceptually in this study as an effective solution for these problems. The concept is that of a transfer loader that transfers containers from a large container ship to the harbor on land, and is a catamaran type floating barge. The catamaran-type vessel is well known for its advantage in maneuverability, resistance, and effectiveness for working on board. For the safe and effective operation of the two floating bodies (a container ship and the mobile harbor in the near sea detached from the quay), robot arms, novel crane systems, and pneumatic fenders are specially devised with an additional mooring facility or DP (dynamic positioning) system. In this study, this concept is to be verified through comparison and simulation studies under various environmental conditions. It is shown that the proposed concept is in general feasible but there are several areas for further investigation and improvement.
