      • Purge & Trap-GC를 이용한 의약품 필름코팅 정제 중 잔류용제에 관한 연구

        의약품은 약물을 생체에 적풋하기 위하여 유효성분의 효과가 언제나 일정하게 확보되고 사응에 편리하도록 만들어지는 것이므로 유효썽분 이외에 약효에 영향을 주지 않는 성분이 첨가되는 경운가 많다. 이 때 사용되는 용매들은 제피의 광택 및 건쪼시간의 단축 등을 위하여 휘발점이 낮을 용매들이 주로 사용되어진다. 본 연구는 의약품 필름코팅정제 중 잔류용매 4종(chlorofonr benzen, trichloro ethylen, 1,4-dioxane)에 대한 변형된 pirge & trap-GC 장치를 이용한 동시분석방법을 개발하였으며, 각 표준품의 RSD 값은 chloroform 3.03%, benzen 3.17%, trichloroethylen 3.69% and 1,4-dioxane 3.41%였다. 또한 시중 유통중인 의약품 50종에 대하여 잔류웅매 양을 측정하였으며, 검출되는 잔류용매는 한 건도 없었다. This study nras carried out to develope the analytical method for the mixture of chlorefonn, benzen, trichloroethylen and 1,4-dioxane simultaneously and determine the remainingorgauic solvents in coating tablets by Purge & Trap-GC. The results were as follouFs ; 1. Chloroform, benzen, trio:tloroethylen and 1,4-dioxane separated by tenax #5 trap by HP-624GC column by terrlperature programming. The peaks were separated completely at retentiontime of 6.88min for chloroform, 8.21min for benzen, 10.38miu for trichloroethylen and 11.95minfor 1,4-dioxane. 2. Standard RSD were individually chloroform 3.03%, benzen 3.17%, trichloroethylen 3.69%and 1,4-diorane 3.41%. 3. 60 samples were not detrcted chloroform, benzen, trichloroethylen and 1,4-dioxane.

        Romo1 is a mitochondrial nonselective cation channel with viroporin-like characteristics

        <P>Reactive oxygen species (ROS) modulator 1 (Romo1) is a nuclear-encoded mitochondrial inner membrane protein known to regulate mitochondrial ROS production and to act as an essential redox sensor in mitochondrial dynamics. Although its physiological roles have been studied for a decade, the biophysical mechanisms that explain these activities of Romo1 are unclear. In this study, we report that Romo1 is a unique mitochondrial ion channel that differs from currently identified eukaryotic ion channels. Romo1 is a highly conserved protein with structural features of class II viroporins, which are virus-encoded nonselective cation channels. Indeed, Romo1 forms a nonselective cation channel with its amphipathic helical transmembrane domain necessary for pore-forming activity. Notably, channel activity was specifically inhibited by Fe<SUP>2+</SUP> ions, an essential transition metal ion in ROS metabolism. Using structural bioinformatics, we designed an experimental data–guided structural model of Romo1 with a rational hexameric structure. We propose that Romo1 establishes a new category of viroporin-like nonselective cation channel in eukaryotes.</P>

      • Recurrence after Curative Resection of Small Hepatocellular Carcinoma: Histology as Potent Prognostic Factors

        Aims: Hepatocellular carcinoma (HCC) is one of the leading causes of cancer-related death worldwide despite the improvement in diagnostic methods and treatment modality. Curative treatment is recommended for patients with small HCCs and good liver function based on Milan criteria, but nevertheless the risk of tumor recurrence is still high in these selected patients. Therefore, we aimed to identify the factors that could predict HCC recurrence after resection focusing on the histologic findings. Methods: A total of 99 HCC patients who had undergone resection from June 2003 to December 2015 with HCCs ≤ 3cm and total number of tumors ≤ 3 were retrospectively reviewed. Patients with child pugh grade A were included in the study. Histologic features including gross pattern, Edmonson grade, microvascular invasion, capsule invasion, free resection margin for tumor while presence of cirrhosis in non-tumor lesion were analyzed. Predictive efficacy of HCC recurrence were also analyzed using clinical variables and lab findings including tumor markers. Results: Forty patients (40.4%) exhibited HCC recurrence following resection. In univariate analysis, factors associated with recurrence were presence of DM, increased baseline PIVKA level, prolonged INR and histological microvascular invasion, high Edmonson grade (grade4), cirrhosis. In multivariate analysis, presence of DM, increased PIVKA level and high Edmonson grade were associated with HCC recurrence. In subgroup analysis, presence of microvascular invasion and increased PIVKA level were associated with early recurrence (≤2 years) (both, P< 0.05) while presence of DM and low albumin level tended to be associated with late recurrence ( >2 years). When recurrence free survival was analyzed, presence of DM and microvascular invasion (both, P<0.05) remained as associated factors. Conclusions: Our findings show that in addition to tumor marker, histologic findings and presence of DM are important prognostic factors that could help in identifying patients who will recur from resection.

ABSTRACT In the case of hemodynamically stable broad complex tachycardia, it is essential to evaluate the causes and to diagnose correctly in the selection of appropriate management and drugs. We report two neonates diagnosed with idiopathic ventricular tachycardia, which is extraordinarily rare. One presented with idiopathic fascicular ventricular tachycardia (right bundle branch block pattern with a superior axis), and the other presented with right ventricular outflow tract ventricular tachycardia (left bundle branch block pattern with an inferior axis). These two forms are representative of benign ventricular tachycardia. No features of cardiovascular shock were observed. While the recommended initial drug treatments are different, the conditions were well controlled by propranolol without the development of any adverse events. There was no recurrence of arrhythmia for several months in the outpatient clinic.

        HS 1-Associated Protein X-1 Is Cleaved by Caspase-3 During Apoptosis

        Caspase-3 (CASP3) plays a key role in apoptosis. In this study, HAX-1 was identified as a new substrate of CASP3 during apoptosis. HAX-1 was cleaved by CASP3 during etoposide-(ETO) induced apoptosis, and this event was inhibited by a CASP3-specific inhibitor. The cleavage site of HAX-1, at Asp<sup>127</sup>, was located using N-terminal amino acid sequencing of in vitro cleavage products of recombinant HAX-1. Overexpression of HAX-1 inhibited ETO-induced apoptotic cell death. It also inhibited CASP3 activity. Together, these results suggest that HAX-1, a substrate of CASP3, inhibits the apoptotic process by inhibiting CASP3 activity.

        Comparison of Clinico-Physiologic and CT Imaging Risk Factors for COPD Exacerbation

        <P>To date, clinico-physiologic indices have not been compared with quantitative CT imaging indices in determining the risk of chronic obstructive pulmonary disease (COPD) exacerbation. We therefore compared clinico-physiologic and CT imaging indices as risk factors for COPD exacerbation in patients with COPD. We retrospectively analyzed 260 COPD patients from pulmonary clinics at 11 hospitals in Korea from June 2005 to November 2009 and followed-up for at least one year. At the time of enrollment, none of these patients had COPD exacerbations for at least 2 months. All underwent clinico-physiologic and radiological evaluation for risk factors of COPD exacerbation. After 1 yr, 106 of the 260 patients had at least one exacerbation of COPD. Multiple logistic regression analysis showed that old age, high Charlson Index, and low FEV<SUB>1</SUB> were significant in a clinico-physiologic model, with C-statistics of 0.69, and that increased age and emphysema index were significant in a radiologic model, with C-statistics of 0.64. The difference between the two models was statistically significant (<I>P</I> = 0.04 by bootstrap analysis). Combinations of clinico-physiologic risk factors may be better than those of imaging risk factors in predicting COPD exacerbation.</P>

        Single incision laparoscopic totally extraperitoneal hernioplasty: lessons learned from 1,231 procedures

        Purpose: Although there are many articles about single incision laparoscopic (SIL) hernioplasty, a large-scale study or article about its long-term outcome has not been reported yet. The aim of this study is to assess short- and long-term outcomes of SIL totally extraperitoneal (TEP) hernia repair with large number of cases. Methods: A prospectively collected database containing details of 1,231 procedures in 1,129 consecutive patients who underwent SIL-TEP hernia repair between June 2010 and December 2017 at a single institution was retrospectively analyzed. SIL-TEP hernia repair was performed using a glove single port device and standard laparoscopic instruments. Recurrence rate of SIL-TEP hernia repair was analyzed by a telephone questionnaire. Results: Among 1,129 patients, 1,027 (91.0%) had unilateral hernia and 102 (9.0%) had bilateral hernia. There were 12 (1.1%) conversions to single or 3 ports laparoscopic transabdominal preperitoneal hernioplasty or Lichtenstein repair. Mean operative time was 40.3 minutes for unilateral hernia and 61.6 minutes for bilateral hernia. Intraoperative complication rate was 21.8%. Most intraoperative complications were peritoneum or sac tearing (20.1%). Postoperative complications occurred in 97 (8.6%) cases, most of which were minor morbidity except for 1 mesh infection. Five-year recurrence rate was 4%. Conclusion: SIL-TEP hernia repair is safe and technically feasible with acceptable short- and long-term outcomes. Large-scale randomized controlled trials comparing SIL-TEP hernia repair with conventional laparoscopic TEP are needed to confirm these results.

        인슐린 비의존형 당뇨병환자의 혈장아미노산 농도에 관한 연구

        N/A In 105 non-insulin dependent diabetics, composed of 20 under-weight, 66 non-obese, and 19 obese diabetics admitted to Severance Hospital, Yonsei University College of Medicine, plasma amino acid levels were measured by HPLC (High Performance Liquid Chromatography) in the fasting state before control of the blood sugar level. The following results were observed: 1) Plasma levels of branced chain amino acids-leucine, isoleucine, and valine-were higher in the diabetic group than in the control group (p<0.05) 2) Plasma levels of tyrosine, phenylalanine, tryptophan, glutamate, and serine were also higher in the diabetic group than in the control group (p<0.05). 3) The plasma level of threonine was lower in the diabetic group than in the control group (p<0.05). 4) In under-weight, non-insulin dependent diabetics, plasma levels of threonine, lysine, and alanine were lower than in the eontrol group (p<0.05). It could be concluded that in non-insulin dependent diabetics, both the uncontrolled state of the diabetes and the nuritional state influnence the changes of plama amino acid levels.
