Claudin-7 is Highly Expressed in Chromophobe Renal Cell Carcinoma and Renal Oncocytoma

Claudin-7 has recently been suggested to be a distal nephron marker. We tested the possibility that expression of claudin-7 could be used as a marker of renal tumors originating from the distal nephron. We examined the immunohistochemical expression of claudin-7 and parvalbumin in 239 renal tumors, including 179 clear cell renal cell carcinoma (RCC)s, 29 papillary RCCs, 20 chromophobe RCCs, and 11 renal oncocytomas. In addition, the methylation specific-PCR (MSP) of claudin-7 was performed. Claudin-7 and parvalbumin immunostains were positive in 3.4%, 7.8% of clear cell RCCs, 34.5%, 31.0% of papillary RCCs, 95.0%, 80.0% of chromophobe RCCs, and 72.7%, 81.8% of renal oncocytomas, respectively. The sensitivity and specificity of claudin-7 in diagnosing chromophobe RCC among subtypes of RCC were 95.0% and 92.3%. Those of parvalbumin were 80.0% and 88.9%. The expression pattern of claudin-7 was mostly diffuse in chromophobe RCC and was either focal or diffuse in oncocytoma. All of the cases examined in the MSP revealed the presence of unmethylated promoter of claudin-7 without regard to claudin-7 immunoreactivity. Hypermethylation of the promoter might not be the underlying mechanism for loss of its expression in RCC. Claudin-7 can be used as a useful diagnostic marker in diagnosing chromophobe RCC and oncocytoma.

<P><B>Background</B></P><P>Appropriate immunosuppressive therapy for patients with idiopathic membranous nephropathy (MN) remains controversial. The effect of mycophenolate mofetil (MMF) versus cyclosporine (CsA) combined with low-dose corticosteroids was evaluated in patients with idiopathic MN in a multi-center randomized trial (NCT01282073).</P><P><B>Methods</B></P><P>A total of 39 biopsy-proven idiopathic MN patients with severe proteinuria were randomly assigned to receive MMF combined with low-dose corticosteroids (MMF group) versus CsA combined with low-dose corticosteroids (CsA group), respectively, and followed up for 48 weeks. Complete or partial remission rate of proteinuria and estimated glomerular filtration rate (eGFR) at 48 weeks were compared.</P><P><B>Results</B></P><P>The level of proteinuria at baseline and at 48 weeks was 8.9 ± 5.9 and 2.1 ± 3.1 g/day, respectively, in the MMF group compared to 8.4 ± 3.5 and 3.2 ± 5.7 g/day, respectively, in the CsA group. In total, 76.1% of the MMF group and 66.7% of the CsA group achieved remission at 48 weeks (95% confidence interval, −0.18 to 0.38). There was no difference in eGFR between the two groups. Anti-phospholipase A2 receptor Ab levels at baseline decreased at 48 weeks in the complete or partial remission group (<I>P</I> = 0.001), but were unchanged in the no-response group. There were no significant differences between the two groups in changes in the Gastrointestinal Symptom Rating Scale and Gastrointestinal Quality of Life Index scores from baseline to 48 weeks.</P><P><B>Conclusion</B></P><P>In combination with low-dose corticosteroids, the effect of MMF may not be inferior to that of CsA in patients with idiopathic MN, with similar adverse effects including gastrointestinal symptoms.</P><P><B>Trial Registration</B></P><P>ClinicalTrials.gov Identifier: NCT01282073</P>

To present our initial experience with single port access (SPA) laparoscopic temporary clipping of uterine arteries during SPA laparoscopic myomectomy (LM) We performed the SPA-LM with temporary clipping of both uterine arteries in 7 patients from January 1, 2013 to April 30, 2013. All cases of SPA-LM with temporary clipping of both uterine arteries were performed by a single surgeon. We analyzed the data to determine the outcomes of the SPA-LM with temporary clipping of both uterine arteries. A total of 7 consecutive patients have undergone the SPA-LM with temporary clipping of both uterine arteries, for benign uterine fibroids regardless of the BMI or previous abdominal or pelvic surgery. There was no case of placing additional trocars during the surgery. The median age and pre-operative size of uterine fibroids on transvaginal sonography were 33 years (25-42) and 6.9 cm (4.3-12.0). During the surgery, the median operation time and estimated blood loss were 156 min (122-220) and 150 ml (100-550). The median value of Hb drop after surgery on day 1 and hospital stay were 2.3 g/dl (0.3-3.8) and 3 days (2-5). During the procedure there were no case of uterine artery occlusion or rupture and patients are doing well postoperatively. The SPA-LM with temporary clipping of both uterine arteries was safe and feasible.
Long Term Follow-up Results of External Beam Radiotherapy as Primary Treatment for Retinoblastoma

<P>The authors reviewed their experiences of external beam radiotherapy (EBR) as an initial treatment in retinoblastoma patients to determine its long-term effect on subsequent tumor control and complications. A total of 32 eyes in 25 patients that underwent EBR for retinoblastoma were reviewed retrospectively. The patients consisted of 21 boys and 4 girls of median age at treatment of 7.1 months. Radiation doses ranged from 35 to 59.4 Gy. The 10-yr ocular and patient survivals were 75.4% and 92.3%, respectively. Nine of the 32 eyes progressed; 7 of these were enucleated and 2 were salvaged by focal treatment. According to the Reese-Ellsworth classification, 4 of 5 eyes of Group II, 13 of 16 Group III eyes, 2 of 4 Group IV eyes, and 5 of 7 Group V eyes were retained, and of the 32 eyes, 13 had visual acuity better than 20/200. Eleven patients experienced a radiation-induced complication. No patient developed a second malignancy during follow-up. Despite the limited number of patients enrolled, EBR may provide a mean of preserving eyeball and vision for some advanced lesions.</P>

To determine whether use of high-intensity focused ultrasound (HIFU) ablation combined with cisplatin, as compared with use of HIFU ablation or cisplatin alone, facilitates tumor necrosis and decreases tumor volume in animal models. The cervical cancer cell line SiHa was cultured and injected subcutaneously on female BALB/c nude mice. Mice (n = 8 per group) were monitored daily for tumor development and post-treatment complications. Animals that developed tumors were randomized to four groups: HIFU ablation and cisplatin (IP injection, 100 mg/mouse twice a week) treatment, cisplatin treatment alone, HIFU ablation alone, or control. The dimensions of the tumor were measured transcutaneously using a vernier caliper. Triphenyl tetrazolium chloride(TTC) staining was done to investigate the area of coagulation necrosis in the tumors. Of the 32 animals that developed tumors, 25 had tumors possible to be measured through subcutaneous palpation at pretreatment period (control, n= 7; HIFU ablation, n= 7; cisplatin, n= 6; combination of HIFU ablation and cisplatin, n= 5). In the group receiving HIFU ablation with cisplatin, tumor volume decreased significantly when compared with that of the other groups including control and HIFU ablation or cisplatin alone. On the TTC staining, we found that necrosis was induced in central zone of the tumors where ablated by HIFU, and chemotherapy enhanced the effect of HIFU ablation in combination therapy group. HIFU ablation combined with cisplatin facilitates decrease in tumor volume and increases in tumor necrosis, and could be useful as another option in treatment of cervical cancer patients.
Unreamed intramedullary interlocking nail 을 이용한 경골 간부 골절의 치료

In general, blood supply of the long bones contributes three ways-nutrient, metaphyseal and periosteal vessels. Its vascular damage is caused by major trauma or extensive soft tissue det-achment. When the diaphysis of long bone is reamed, endosteal blood supply is eliminated. In our hospital, 23 selected cases of the diaphyseal fracture of tibia have been treated by manipulative reduction, unreaming of the medullary canal and fixation of fracture fragments with an interlocking intramedullary nail for preservation its vascularity as possible. We analized the patients from Jan. 1992 to May. 1993, who were followed up for a mean 12 months. Most of cases were acute, 18 of these cases were open fracture and main cause of the injury was traffic accidents. Overall, 90% of the patients were judged to have good or excellent result based on both clinical and radiological parameters, and the average bony union time was 17 weeks. The complications were mainly infection(3 cases), loosening or breakage of the locking screws(3 cases) and delayed union(1 case). The merits of unreamed interlocking nailing were relatively easy procedure, short operation time, decreased compications, rigid fixation and early rehabilitation. We concluded that unreamed intramedullary interlocking nailing is a useful method to treat the tibia shaft fractures, especially open type.
Genetic profiling to predict recurrence of early cervical cancer

We compared the prediction powers for disease recurrence between gene set prognostic model and clinical prognostic model developed in a single large population to see whether genetic quantitative approach will have significant prognostic role in early cervical cancer patients who underwent radical hysterectomy with or without adjuvant therapies. Gene set model to predict disease free survival of early cervical cancer was developed using DASL assay dataset from the cohort of early cervical cancer patients who were treated with radical surgery with or without adjuvant therapies at the Samsung Medical Center of Sungkyunkwan University School of Medicine in Seoul, Korea, between January 2002 and September 2008. Clinical prediction model was also developed in the same cohort. Adequate DASL assay profiles were obtained in 300 patients and we selected 12 genes for the gene set model. When the proportions of patients were categorized as having a low or high risk by the prognostic scoring using these genes, the Kaplan-Meier curve showed significant different recurrence rate between two groups. Clinical model was developed using FIGO stage as well as post-surgical pathological findings. In multivariate Cox regression analysis of prognostic models, gene set prognostic model showed higher hazard ratio for recurrence of 9.95 with 95% CI of 5.48-18.1 when compared with the clinical prognostic model, which showed the hazard ratio of 1.96 with 95% CI of 1.29-2.97. We could find that genetic quantitative approach may have better performance of predicting recurrence in early cervical cancer patients.a

<P>Abstract</P><P>Aim: </P><P>To better predict treatment responses for managing bulky cervical carcinoma with neoadjuvant chemotherapy (NAC).</P><P>Methods: </P><P>The expression of p-STAT3 was analyzed by immunohistochemistry using paraffin-embedded pretreatment cervical biopsy tissues. The study included 29 patients with bulky IB to IIA cervical squamous cell carcinoma treated with NAC.</P><P>Results: </P><P>Twenty (69.0%) of 29 patients were scored as p-STAT3-positive. Pathological response to chemotherapy (complete response or residual tumor with less than 3 mm stromal invasion) was observed in eight patients (27.6%). The p-STAT3-positive patients had a longer disease-free survival compared to p-STAT3-negative patients (<I>P</I> = 0.03), though they had more frequent clinical nodal involvement (<I>P</I> = 0.046).</P><P>Conclusion: </P><P>Pretreatment assessment of p-STAT3 expression may provide additional information for the identification of patients with cervical cancer who have a favorable prognosis.</P>
