A Multi-Center Educational Research Regarding Breastfeeding for Pediatrics Residents in Korea

Purpose: Pediatricians have a significant responsibility to educate mothers about the importance of breastfeeding. However, there have been minimal efforts in the courses of resident training in Korea. The purpose of this study is to evaluate the change in knowledge and attitude before and after a 4-week breastfeeding educational intervention among multicenter residents. Methods: Prospective interventional educational research was designed for residents at eight training hospitals in Korea. Institutional reviews were obtained in each hospital. The education curriculum consisted of 14 courses regarding breastfeeding theory and practice. These materials were used to teach pediatric residents for 4 weeks. Knowledge-based tests were administered before the course, and re-tests were administered after the course using different test items of similar levels. Test scores and survey responses were compared before and after the intervention. Results: A total of 73 residents (1st year 20, 2nd year 23, 3rd year 16, and 4th year residents 14) from eight training hospitals completed the intervention. Their average age was 30.3±2.9 years, 17 (23.3%) were male, 22 (30.1%) were married, and eight had more than one child of their own. The mean pre-test score was 61.8±13.4 and the mean post-test score was 78.3±7.5 (P<0.001). The inter-grade difference in the score was significant in the pre-test (P=0.005), but not significant in the post-test (P=0.155). There were more responses of obtaining confidence after the intervention (P<0.001). Conclusion: In our study, pediatric residents showed improvement in their knowledge and confidence level after 4 weeks of the breastfeeding curriculum. 의사 김필순의 생애와 독립운동

2019 marks the 100th anniversary of the March First Movement, As a result, the interim government of the Republic of Korea was established and it was busy to inform the international community. The representative figure was Kim Gyu-sik, a well-known fact. However his brother-in-law, Kim Pil-soon, a great doctor and independence activist who silently performed his role, was buried in history. This article does not focus on the fact that Kim Pil-soon was one of the first seven licensed medical doctors in Korean history, and that he was one of the leading figures in the Korean medical community. He revealed how he devoted himself to the independence movement, not resting on his stable career as a doctor, but not ignoring the reality of his country at that time. In addition to being a doctor who was responsible for the hygiene of the immigrant Koreans after the exile in China, he was able to expand the scope of independent movement through his activities as an independent activist in Northern Manchuria. It is a remaining task to study about Kim Pil-soon's offsprings staged anti-Japanese struggles throughout China after Kim's death.

2003년도 해양스포츠 체험활동을 통한 소아건강교실 사례 발표

부산에 소재한 3개 초등학교 청소년단체 소속의 초등하교 3학년부터 6학년까지 구성된 172명을 대상으로 청소년단체 지도자, 초등학교교사, 자원봉사자, 의사와 영양사가 1박 2일간의 해양스포츠 체험활동 을 통한 건강교실을 시행하였다. 대상자의 체질량지수, 체지방량을 측정하여 비만정도를 평가하였으며, 칼로리 섭취에 대한 강의를 시행하였고, 설문지 조사를 통해 대상자의 식이 섭취 습관에 대한 조사를 시행하였고, 본 건강교실에 대한 만족도를 측정하였다. 대상자중 남자에서는 40%, 여자에서는 25.69%에서 비만 경향을 보였다. 대상자의 대부분에서 시행된 건강교실에 긍정적 반응을 보였으며, 향후 체계적인 구성과 철저한 사전계획이 병행되어 본 증례 보고와 같은 건강교실을 시행한다면, 비만아동에서 거부감 없이 비만에 대한 교육을 실시할 수 있으며, 정상 체중 아동군에서도 비만에 대한 예방교육을 시행할 수 있는 프로그램이 될 것이다. 172 elementary school students involved this health program. This subjects were learened about obesity that affect health problem, ideal eating habits, calories that contained in the children's favorite foods by doctors and nutritionist. And this stubjects were checked heights, weight, body fat percent. Obesity is 40% in the boys and 25.69% in the girls. Most subjects were satisfactory with this health program. So we will effectively educate obese children without resistance and prevent normal weight children from obesity through structured and well-designed health program that similar to this program.

자궁내막암 환자에서 복강경하 자궁적출술과 복식 자궁적출술의 비교 연구

목적 : 자궁내막암 환자에서 시행된 복강경 수술의 임상적 결과를 평가하고 수술적 지표 및 재발률에 대하여 기존의 개복 수술과 비교하고자 하였다. 연구 방법 : 1997년 8월부터 2003년 11월까지 저자들은 임상적 병기 I기인 79명의 자궁내막암 환자에서 복강경하 자궁적출술과 골반 및 부대동맥 림프절 절제술을 시행하였다. 총 79명의 환자 중, 수술적 병기 I기와 II기로 판명된 74명의 환자를 연구 대상으로 하였다. 대조군으로는 같은 기간 동안에 개복수술을 시행한 환자중 복강경 수술군과 병기가 같은 168예를 선정하였다. 결과 : 평균 수술 기간, 수혈량은 양군에서 비슷하였으며, 평균 재원 기간은 복강경 수술군에서 유의하게 짧았다. 수술전후 및 만성 합병증의 발생은 개복 수술군에서 유의하게 증가되었으며, 획득한 림프절의 수는 복강경 수술군에서 유의하게 더 많았다. 복강경 수술군에서 1예, 개복 수술군에서는 2예에서 재발하였고, 2년 무병 생존률은 복강경 수술군과 개복 수술군에서 각각 97.5%와 98.6%으로 유의한 차이는 관찰되지 않았다. 결론 : 자궁내막암 치료로서 복강경 수술은 기존의 개복술을 대신할 수 있는 안전하고 효과적인 시술 방법이다. 개복 수술과 비교해 볼 때, 재발 및 생존율 면에서 차이가 없었지만, 향후 전향적이고 더 오랜 추적기간을 갖는 연구가 필요하다고 사료된다. Objective : To evaluate the outcomes of laparoscopic surgery and to compare surgical parameters and recurrence rate of these with those of conventional abdominal surgery in patients with endometrial cancer. Methods : From August 1997 to November 2003, we have performed 79 cases of LAVH (laparoscopically assisted vaginal hysterectomy) with or without lymph node dissection. Laparoscopic approach was adapted in patients with FIGO clinical stage I by imaging study. Of the 79 patients, 74 patients who were proved to be surgical stage I and II were enrolled in this comparative study. As a control group, We selected 168 cases for the laparotomy group of the same stages. Results : The mean duration of surgery, the amount of blood transfusion and hemoglobin chamges were similar in both the laparoscory and the convertional alparotomy group. The mean duration of hospital stay was significantly shorter in patients treated by laparoscopic surgery (10.2 vs. 15.5 days). The number of lymph node obtained was significantly higher in the laparoscopy group. Two year recurrence-free survival rates were 97.5% in laparoscopy group and 98.6% in laparotomy group (p=0.763). Conclusion : Laparoscopic surgery for the treatment of early stage endometrial cancer is safe and effective altematives in terms of perioperative complications. Overall and recurrence-free survival did not differ significantly in both groups however long term risk for recurrence and survival has yet to be defined.

아세클로페낙 연질캡슬(클란자 에스 연질캡슬)의 개발

To develop and aceclofenac soft capsule, four preparations with various solubilizers were prepared and their dissolution test was carried out. Among four preparations tested, a preparation with ethanolamine was selected a formula of aceclofenac soft capsule (Clanza S^(™), since it showed the fastest dissolution rate. Bioequivalence of aceclofenac tablet, Airtal^(™)(Dae-Woong Pharmaceutical Co., Ltd.) and aceclofenac soft capsule, Clanza S^(™)(Korea United Pharmaceutical Co., Ltd.) was evaluated according to the guideline of KFDA. Fourteen normal male volunteers (age 20-25 years old) were divided into two groups and a randomized 2×2 cross-over study was employed. After oral administration of one tablet or capsule containing 100 ㎎ of aceclofenac, blood ws taken at predetermined time intervals and the concentration of aceclofenac in plasma wa determined with an HPLC method under UV detector. The pharmacokinetic parameters (C_(max), T_(max) and AUC_(t)) were calculated and ANOVA was utilized for the statistical analysis of parameters using logarithmically transformed AUC_(t), C_(max) and T_(max) between Airtal tablet and Clanza soft capsule were 2.89%, 0.18% and 43.0%, respectively. There were no sequence effects between two formulations in these parameters. The 90% confidence intervals using logarithmically transformed data were within the acceptance range of log(0.8) to log(1.250(e.g.log(0.81) - log(1.23) and log(0.89) - log(1.14)) for AUC_(t) and C_(max), respectively. Thus, the criteria of the KDFA guidelines for the equivalence was satisfied, indicating that Clanza S^(™) soft capsule is bioequivalent to Airtal^(™) tablet.

서울의 PPNG 발생 빈도(1991-1992)

The prevalence of PPNG among pretreated gonorrhea cases isolated at the STD clinic of Choong-Ku Public Health Center in Seoul has been studied and reported annually since 1981. In 1991, 123 strains of N.gonorrhoeae were isolated, among which 58(47.1%) were PPNG. In 1992, 98 starains of N.gonorrhoeae were isolated, among which 51(52.0%) were PPNG. In all, 109(49.3%) strains were found to be PPNG among 221 strains isolated between 1991-1992. The prevalence of PPNG in Seoul showed increased tendency till 1989, thereafter, it has been stationary or slightly decreasing.
