There are many optimal design packages, but they are big ones and they have only a few kinds of optimal algorithm coded with Fortran and it is sometimes necessary for user to write down some codes into their packages. So it is hard for user to learn how to use and customize them. More over, there are no commercial home-made software for optimum design. So, in this paper, several famous optimum algorithms are coded and modulized with C^++ which is known as a suitable computer language in order to build up more algorithms into one computer software. All algorithms developed with C++ here were tested for comparison with optimization tool box of MATLAB and are superior to MATLAB.

Background: It is known that the prevalence and social burden related to chronic obstructive pulmonary disease is increasing. Previous survey published at 2008 demonstrated that 13.4% of subjects aged over 40 years had airfiow obstruction (FEV1/FVC <0.7) in Korean population. However, current status of prevalence and characteristics of elderly who have obstruction airway disease (OAD) is not well investigated yet. Methods: A total of 2566 adults (1109 of male and 1457 of female; mean age 73.4±6.4) were included from public health survey during 3 years. Pulmonary function was measured by portable fiow-volume device. A standard questionnaire (CAT score), smoking status, respiratory symptoms such as cough, sputum, dyspnea, and comorbid illness were collected. Results: The prevalence of person with OAD which was defined as FEV1/FVC <0.7 was 18.5% (Male 29.2%, Female 10.4%). Mean FEV1 change was -33mL per year in both men and women. There was significant correlation between age and prevalence of OAD. Former or current smoking was significantly more common in group of OAD. Among persons with OAD, 56.2% have FEV1 below 80% of predicted. The proportion of person according to GOLD stage A-D was 50.6%, 37.9%, 4.6%, and 6.9% respectively. Comparing GOLD stage A with B, persons with GOLD stage B were tent to have low BMI and comorbid illness such as hypertension and diabetes. In multivariate analysis, factors associated with higher CAT score in OAD were advanced COPD stage, history of smoking, and female gender. Conclusions: Our data indicate that the prevalence of COPD in elderly is still high and large number of person is undiagnosed and untreated.
Allergic Bronchopulmonary Aspergillosis Presenting as Recurrent Mass-like Consolidation

Allergic bronchopulmonary aspergillosis (ABPA) is a hypersensitive disease showing various radiographic and clinical manifestations. Its clinical course has not been fully understood. Here I describe a case of a 23-year-old immunocompetent man with frequently relapsing ABPA. He was asthmatic. He visited our hospital because of a chronic cough. Laboratory examination showed eosinophilia with increased total and Aspergillus-specific IgE as well as positive skin reaction to Aspergillus fumigatus. Radiologic feature was a dense consolidation. Histology showed organizing pneumonia with eosinophilic infiltration. On the diagnosis of ABPA, he was treated with systemic steroid and itraconazole. Although treatment response was excellent, he suffered from recurrent ABPA three times thereafter in the form of fleeting mass-like consolidation.
Endobronchial mucormycosis successfully treated with bronchoscopic cryotherapy: A case report

Endobronchial mucormycosis is devastating fungal disease and a major threat in immunocompromised patients. The mortality rate is high, and surgical debridement of necrotic tissue along with antifungal therapy is required for cure. However, there are so many hurdles to apply the surgical resection as a therapeutic strategy. We describe a case of endobronchial mucormycosis in a liver transplant recipient who was successfully treated with bronchoscopic cryotherapy and liposomal amphotericin B. Given that there is extremely little information on mucormycosis treated with bronchoscopic cryotherapy so far, the current experience has valuable clinical significance suggesting that bronchoscopic cryotherapy with antifungal treatment can be a promising alternative therapeutic approach for endobronchial mucormycosis, especially in cases of mucormycosis that surgery may not be feasible due to various factors such as the extent and location of the lesions and poor medical condition of the patient.
