Differential regulation of p53 and p21 by MKRN1 E3 ligase controls cell cycle arrest and apoptosis.

<P>Makorin Ring Finger Protein 1 (MKRN1) is a transcriptional co-regulator and an E3 ligase. Here, we show that MKRN1 simultaneously functions as a differentially negative regulator of p53 and p21. In normal conditions, MKRN1 could destabilize both p53 and p21 through ubiquitination and proteasome-dependent degradation. As a result, depletion of MKRN1 induced growth arrest through activation of p53 and p21. Interestingly, MKRN1 used earlier unknown sites, K291 and K292, for p53 ubiquitination and subsequent degradation. Under severe stress conditions, however, MKRN1 primarily induced the efficient degradation of p21. This regulatory process contributed to the acceleration of DNA damage-induced apoptosis by eliminating p21. MKRN1 depletion diminished adriamycin or ultraviolet-induced cell death, whereas ectopic expression of MKRN1 facilitated apoptosis. Furthermore, MKRN1 stable cell lines that constantly produced low levels of p53 and p21 exhibited stabilization of p53, but not p21, with increased cell death on DNA damage. Our results indicate that MKRN1 exhibits dual functions of keeping cells alive by suppressing p53 under normal conditions and stimulating cell death by repressing p21 under stress conditions.</P>

<P><B>Background</B></P><P>Identification of poor prognostic factors in papillary thyroid carcinoma (PTC) patients is important for the patients' care and follow-up. We can sometimes see small tumor clusters without desmoplasia and no evidence of lymphatic emboli around the main tumor mass of PTC. We termed this form of tumor clustering, 'tumor sprouting,' and determined whether these tumors correlate with lymphovascular invasion, lymph node metastasis, and recurrence.</P><P><B>Methods</B></P><P>We analyzed a total of 204 cases of papillary thyroid macrocarcinoma. Number, size and distance from the main tumor of the tumor sprouting were observed and analyzed with clinicopathologic characteristics.</P><P><B>Results</B></P><P>Tumor sprouting was observed in 101 patients. Presence of tumor sprouting was significantly associated with positive resection margin (p=.002), lymphovascular invasion (p=.001), lymph node metastasis (p<.001), and recurrence (p=.004). Univariate analysis of recurrence-free survival revealed that tumor multiplicity (p=.037), positive resection margin (p=.007), lymphovascular invasion (p=.004), lymph node metastasis (p<.001), and tumor sprouting (p=.004) were poor prognostic factors. In multivariate analysis, positive resection margin was an independent poor prognostic factor of recurrence.</P><P><B>Conclusions</B></P><P>In conclusion, tumor sprouting is significantly correlated with lymph node metastasis and recurrence. Evaluation of tumor sprouting in PTC patients could be helpful in predicting tumor recurrence or lymph node metastasis.</P>

A 22-year-old female presented with erythematous macules and papules on her both palms and erythematous scaly papules on whole body that developed 6 days ago. She had been treated with psoriasis vulgaris since about 10 years ago and kept in good condition recently. Before the skin lesion developed, she had history of direct contact with the infant with hand-foot-mouth disease 1 week ago. On histopathologic examination, spongiosis and exocytosis of inflammatory cells were shown in epidermis and lymphocytic infiltration in dermis. Considering early stageof enterovirus infection, real time polymerase chain reaction was performed and coxsackievirus A16 was identified. Eczema coxsackium caused by coxsackievirus A16 was finally diagnosed. Eczema coxsackium is caused by enterovirus and presented with fever, sore throat and generalized cutaneous eruption. It usually occurred in children but rarely reported in adult who had underlying disease such as Darier’s disease, eczema or human immunodeficiency virus infection . Herein, we report a rare case of eczema coxsackium in adult patient after direct contact with hand-foot-mouth disease patient.
A case of CD30 positive anaplastic large cell lymphoma occurred in patient of mycosis fungoides

CD30 positive anaplastic large cell lymphoma is characterized by constant expression of CD30 and anaplastic cytologic feature. It usually experience an indolent disease course and favorable prognosis. Large cell transformation of mycosis fungoides can show similar clinical and histopathologic findings with CD30 positive anaplastic large cell lymphoma. But it shows poor prognosis and needs more aggressive treatment, so differential diagnosis of those two diseases should be required. Differential diagnosis is made by overall consideration of clinical presentation, past history, histopathologic findings and disease course but the distinguishment is still difficult. A 29-year-old Korean male presented with erythematous nodule on his left leg. He was diagnosed as mycosis fungoides 10 years ago and had been treated with phototherapy continuously. On histopathologic examination and immunochemistry of the nodule, more than 75% of CD30 positive large T cell shown in the specimen. With favorable clinical course, CD30 positive anaplastic large cell lymphoma was finally diagnosed. Herein, we report a case of CD30 positive anaplastic large cell lymphoma in patient with mycosis fungoides differentially diagnosed from large-cell transformation of mycosis fungoides.
P252 : Analysis of dermatologic consultations in neonatal intensive care unit patients

Background: There have been several studies about neonatal skin diseases. However, there has been few study analyzing dermatologic consultations of neonate intensive care unit(NICU) patients. Objectives: The aim of this study is to investigate consulted cutaneous disorders of neonates in NICU and their clinical courses after consultation. Methods: The authors retrospectively reviewed the medical records of patients who were consulted at our department of dermatology during their NICU admission periods for 5 years(2009-2014). Results: A total of 44 neonates were enrolled and 25 of them were preterm babies. They were composed of 20 males and 24 females. The mean age was 4.3(0.15-18) weeks and the mean gestational age was 33.3(24-41) weeks. Infectious disease was the most common (n=9, 20.4%), followed by eczema (n=8, 18.2%) and disorder of skin appendages (n=8, 18.2%). Congenital disorders were consulted in 12 patients (27.3%). Pressure alopecia was the most common disease entity (n=6, 13.3%), followed by cutaneous candidiasis (n=5, 11.1%). Conclusion: This study showed dermatologic problem of neonate in NICU showed characteristic distribution. Both understanding about specialized condition of neonate in NICU and collaboration with pediatrician seem to be necessary to solve the dermatologic problem of neonates.
Cutaneous metastasis of renal angiosarcoma occurred on the face

A 66-year-old male present with multiple erythematous nodules on his forehead, right cheek and left temple developed one month ago. He had been diagnosed as renal angiosarcoma with liver and bone metastasis and underwent right nephrectomy and adjuvant chemoradiation therapy. On histopathologic examination performed on his left temple, infiltration of basophilic epitheloid cells and irregularly anastomosing vascular structures dissecting collagen bundles and extravasated red blood cells were shown in entire dermis. High-power view showed atypical endothelial cells lining vascular lumen and pleomorphic, heterochromic tumor cells. Immunohistochemical stain of CD34 and factor VIII-related antigen showed positivity. On the basis of clinical and histopathological examinations, metastatic angiosarcoma was finally diagnosed. Angiosarcoma is highy malignant tumor which usually occurs on the skin of the head and neck area and is related with old age, lymphedema, trauma or radiotherapy. It frequently metastasizes from skin to internal organ, but metastasis from internal organ to skin is rarely reported. In Korean dermatologic literature, 2 cases of metastatic cutaneous angiosarcoma, from heart and lung relatively, were reported. Therefore, we report a rare case of metastatic cutaneous angiosarcoma from kidney.
A case of multiple nevoid hypertrichosis accompanied with clinodactyly

Nevoid hypertrichosis is a rare disorder, characterized by localized terminal hairs on normally pigmented skin. It usually presents at birth or soon after with marked female predilection. Etiology of this disease is not yet clarified but mosaicism during embryogenesis was suggested as possible cause of this phenomenon. Accompanied partial lipoatrophy and bony abnormalities has been reported occasionally. It usually presents as solitary lesion, but rarely, multiple patches can develop. We called it multiple nevoid hypertrichosis and need differential diagnosis with other disease such as congenital hypertrichosis lanuginose and congenital generalized hypertrichosis terminalis. An 18-month-old female child visited our clinic due to black, coarse hairs on her left periocular area, left upper back and left leg. Histopathologic examination demonstrated terminal hair follicles in normal skin. Except clinodactyly of left fifth finger, no other neurologic or bony abnormalities were found. She was diagnosed to multiple nevoid hypertrichosis and following-up regularly without other treatments.
스테이션학습을 활용한 자기주도 학습 - “직업/일”을 주제로 한 독일어 수업의 예 -

Autonomes Lernen ist kein neuer Begriff in der Fremdsprachendidaktik. Dennoch ist die Einplanung von offeneren Lernformen im Unterricht, die die Lernerautonomie unterstützen, ist für viele Lehrende immer auch mit Bedenken verbunden. Vielleicht ist aber dieser Zweifel dadurch begründet, dass man nicht genau weiß, wie das Konzept autonomen Lernens in der Unterrichtpraxis konkret umzusetzen ist. Im vorliegenden Beitrag wird versucht, die Grundideen autonomen Lernens herauszustellen und anhand eines Unterrichtsbeispiels das “Stationenlernen” als eine Umsetzungsmöglichkeit für diese Ideen aufzuzeigen. Beim Stationenlernen ist das Lernen ein aktiver Prozess. Lernprozesse führen dabei für jeden Lernenden zu unterschiedlichen Ergebnissen. In der vorgestellten Unterrichtseinheit wurde das Thema “Beruf/ Arbeit” in 10 Teilbereiche - eine Aufgabe im Plenum und neun Lernstationen - aufgeteilt und zu jedem Teilbereich wurde ein Materialangebot erstellt, das von den Lernenden selbständig bearbeitet werden soll. Die Materialien wurden im Klassenraum an verschiedenen Lernstationen ausgelegt. Die Lernenden gingen in Gruppen von Station zu Station und bearbeiteten die Materialien. Meine Erfahrung zeigt, dass diese Unterrichtsmethode den unterschiedlichen Lerntypen entgegenkommt und den Lehrenden ermöglicht, sich einzelnen Schülern mehr zu zuwenden, und viele Unterrichtszugänge einzubringen, die im lehrerzentrierten Unterricht nur wenig Platz finden können.
