자가 면역성 간염에서 자가항체의 발현 양상과 스테로이드 치료의 효과

목적 : 최근 8년간 본원에서 경험한 자기면역성 간염의 혈액학적 특징과 스테로이드의 치료효과를 알아보고자 본 연구를 시행하였다. 방법 : 1996년 이후부터 2002년까지 충남대학교 병원에서 1999년 International Autoimmune Hepatitis Group(LAHG)에서 제시한 수정 진단 기준안에 의거하여 총 진단점수가 probable 이상에 해당하는 환자들을 대상으로 임상적 특징, 검사실 및 조직 소견과 스테로이드와 azathioprine의 치료 효과를 후향적으로 조사하였다. 결과 : 9예의 환자들은 여성이 78%, 40대가 많았고, 급성 발병이 33.3%, 피로감, 식욕부진 및 황달 등이 나타났으며, 항핵항체가 55.6%에서 그리고 항평활근 항체가 77.8%에서 양성이었고, 동반 질환은 류마치스 관절염이 2예, 특발성 혈소판 감소증 1예, Henoch-Sch nlein 자반증 및 그레브스병이 각각 1예 씩이었다. 7예에 대해서 스테로이드와 azathioprine으로 치료하여 생화학적 임상적 완해가 이루어져서 유효한 치료 결과를 얻었다. 결론 : 자가면역성 간염은 우리 나라에서 원인을 찾을 수 없는 만성 간질환의 경우, 특히 중년여성에서 다른 자가면역성 질환이 동반되었을 때 자가면역성 간염을 의심하여 자가항체 선별검사를 시행해 보아야 할 것으로 생각된다. 향후 다기관 연구를 통하여 좀 더 많은 증례를 모으고 전향적인 연구를 시행하여야 할 필요가 있다. Although autoimmune hepatitis(AIH) is common in western contries, some studties have been reported in Korea. The aim of the study was to analyze the clinical and histological features and expression pattern of the autoantibody and to examine clinical course of AIH in Korea. For 9 patients diagnosed as having AIH in our hospital since 1996, we reviewed medical records and analyzed clinical and laboratory findings retrospectively. They had a probable or definite AIH according to the revised criteria of International AIH group. The mean age was 42.7(21∼68)years and 7(77.8%) were female. Three of them(33.3%) showed acute hepatitis and five of them showed cirrhosis. Fatigue and anorexia were the most frequently complained symptoms(55.6%). Four patients(44.4%) were accompanied with other autoimmune diseases. Antinuclear antibody was detected in 5(55.6%) and anti-smooth muscle antibody was detected in 7(77.8%). Three patients showed focal picemeal necrosis and four showed periportal fibrosis. Among 7 patients who received immunosuppressive therapy, six patients(85.7%) showed initial response. and biochemical and serologic remission were achived in 5(71.4%) and 3(42.8%) patients, AIH seems to be partly responsible for chronic liver disease of unknown etiology in Korea. AIH should be suspected in female patients with unknown chronic liver disease of unknown etiolgy, especially accompanied with other autoimmune disease. The clinical characteristics of AIH may not be quite different from those of Western countries.

Two dimensional Fast DCT using Polynomial Transform without Complex Computations

본 논문은 2차원 Discrete Cosine Transform (2D-DCT)의 계산을 새로운 Polynomial 변환을 통하여 1차원 DCT의 합으로 변환하여 계산하는 알고리즘을 개발한다. 기존의 2차원 계산방법인 row-column 으로는 N×M 크기의 2D-DCT에서 3/2NMlog₂(NM)-2NM+N+M의 합과 1/2NMlog₂(NM)의 곱셈이 필요한데 비하여 본 논문에서 제시한 알고리즘은 3/2NMlog₂M+NMlog₂N-M-N/2+2의 합과 1/2NMlog₂M의 곱셈 수를 필요로 한다. 또한 기존의 polynomial 변환에 의한 2D DCT는 Euler 공식을 적용하였기 때문에 복소 연산이 필요하지만 본 논문에서 제시한 polynomial 변환은 DCT의 modular 규칙을 이용하여 2D DCT를 ID DCT의 합으로 직접 변환하므로 복소 연산이 필요하지 않다. This paper develops a novel algorithm of computing 2 Dimensional Discrete Cosine Transform (2D-DCT) via Polynomial Transform (PT) converting 2D-DCT to the sum of 1D-DCTs. In computing N${\times}$M size 2D-DCT, the conventional row-column algorithm needs 3/2NMlog$_2$(NM)-2NM+N+M additions and 1/2NMlog$_2$(NM) additions and 1/2NMlog$_2$(NM) multiplications, while the proposed algorithm needs 3/2NMlog$_2$M+NMlog$_2$N-M-N/2+2 additions and 1/2NMlog$_2$M multiplications The previous polynomial transform needs complex operations because it applies the Euler equation to DCT. Since the suggested algorithm exploits the modular regularity embedded in DCT and directly decomposes 2D DCT into the sum of ID DCTs, the suggested algorithm does not require any complex operations.

수영강의 수질오탁과 그것이 광안리 해수용장에 미치는 영향에 대하여

This research was conducted to evaluate the effects of polluted Suyeong River water on the water quality of Kwangan-Ri bathing beach. The quantity of pollutant loading of Suyeong River was determined in January 1979, and the directions of tidal currents and chemical constituents of the Suyeong Bay water were observed in May 1979. The results are as follows : The quantity of total pollutant loading which was discharged into Suyeong Bay is : 75.2ton/day; COD 96.9ton/day; SS 20.5ton/day; ammonia-nitrogen 12.4ton/day; nitrate-nitrogen 430kg/day; nitrite-nitrogen 85.1kg/day; phosphate-phosphorus 594kg/day and total heavey metals 3.01ton/day. Considering the tidal current, the polluted waters of Suyeong River flow off the Kwangan-Ri beach during the ebb tides, and flow into the Kwangan Ri beach during the flood tides. Consequently, the water quality of Kangan-Ri bathing beach is not suitable for bathing.

The pivotal role of PDGF and its receptor isoforms in adipose-derived stem cells.

<P>Platelet-derived growth factor (PDGF) is one of the growth factors that reportedly regulates cell growth and division of mesenchymal cells. Although PDGF isoforms and their receptors reportedly play a pivotal role in mesenchymal stem cell regulation, there is a paucity of literature reviewing the role of PDGF in adipose-derived stem cells (ASCs). Therefore, we summarized previous reports on the expression and functional roles of PDGF and its receptor isoforms in this review. In addition, we examined findings pertaining to underlying molecular mechanisms and signaling pathways with special focus on PDGF-D/PDGFRβ. ASCs only express PDGF-A, -C, -D, PDGFRα, and PDGFRβ. PDGFRα expression decreases with adipocyte lineage, while PDGFRβ inhibits white adipocyte differentiation. In addition, PDGFRβ induces proliferation, migration, and angiogenesis and up-regulates the expression of paracrine factors in ASCs. Although PDGF-B and -D mediate their functions mainly by PDGFRβ and ROS generation, there are many differences between them in terms of regulating ASCs. PDGF-D is endogenous, generates ROS via the mitochondrial electron transport system, and regulates the autocrine loop of ASCs in vivo. Furthermore, PDGF-D has stronger mitogenic effects than PDGF-B.</P>

Protective role of adipose-derived stem cells and their soluble factors in photoaging.

<P>As individuals age, the skin undergoes changes, such as irregular pigmentation, thinning and loss of elasticity, that are due to both genetic and environmental factors. These changes may worsen, progressing to precancerous and cancerous diseases. Various medical treatments and topical cosmeceuticals have been used to treat some symptoms of photoaging, however, the results have been less than satisfactory. Mesenchymal stem cells within the stromal-vascular fraction of subcutaneous adipose tissue, adipose-derived stem cells (ADSCs), display multi-lineage developmental plasticity and secrete various growth factors that control and manage the damaged neighboring cells. Recently, the production and secretion of growth factors has been reported as an essential function of ADSCs, and diverse regenerative effects of ADSCs have been demonstrated in the skin. For example, conditioned medium from ADSCs (ADSC-CM) stimulated both collagen synthesis and migration of dermal fibroblasts, which improved the wrinkling and accelerated wound healing in animal models. ADSC-CM also inhibited melanogenesis in B16 melanoma cells, and protected dermal fibroblasts from oxidative stress induced by chemicals and UVB irradiation. Therefore, ADSCs and soluble factors show promise for the treatment of photoaging, and this review introduces recent research developments of the ADSCs and ADSC-derived secretory factors regarding this issue.</P>

Association between serum vitamin D level and psoriasis: A clinical review in Korean patients in a single tertiary center

Background: Psoriasis is a chronic, inflammatory autoimmune disease. Vitamin D (Vit D) is a hormone that may have important immunomodulatory effects in psoriasis. Some studies have demonstrated a relationship between Vit D deficiency and psoriasis. Objectives: We aimed to compare serum levels of Vit D of patient with active psoriasis and non-patient seen at the outpatient clinic. Methods: Based on the retrospective chart reviews, a total of 181 patients between April 2018 and Aug 2019 were observed. The analysis of Vit D serum levels was performed by immune-assay and according to recent report, values < 20ng/ml were considered deficiency, from 20 to < 30ng/ml insufficiency and ≥ 30ng/ml sufficiency. Statistical data were evaluated by Chi-Square test and the values were considered significant when P < 0.05. Results: In psoriasis group (n=83, 26 females and 57 males), mean age was 42.04 and patients with serum Vit D deficiency was 59.04% (n=49). In non-psoriasis group (n=98, 46 females and 52 males), mean age was 43.60 and the frequency of serum Vit D deficiency was 71.43% (n=70). There was no statistically significant difference between the two groups (p=.080, > 0.05). However, serum Vit D level and the PASI showed a negative correlation (R=-0.134), although the relationship was weak. Conclusion: We observed that there was no significant differences in serum Vit D level between two groups. Further study is needed considering age, gender, comorbidity associated with disease severity.

Benign skin tumors on the plantar surface of the foot

Background: Tumors of the foot are rarely found in the general population. Most of the foot tumors are benign conditions, while malignant tumors are very rare to encounter. There are studies that have reported the relevance of mechanical stress and acral melanomas, but it’s relevance with benign tumors of the foot is still not clear. Objectives: To evaluate prevalence and correlation between types of tumors and weight bearing regions. Methods: A total of 87 patients were retrospectively evaluated. Based on the biopsy data, the type of tumor and its location in the foot were evaluated. Results: The distribution of benign tumors in weight bearing regions versus non-weight-bearing regions was calculated with the use of the chi-square godness-of-fit test. The p-value was less than 0.001 and the result was significant at p=<0.05. The most frequent anatomical location was sesamoids of 1st metatarsal bone (n=20, 22.99%), in order of frequency, tuberosity of calcaneus (n=18, 20.69%), heads of 2nd to 5th metatarsal bones (n=13, 14.94%). In terms of tumor type, among the non-tumors, the rates were higher in the order of epidermal cyst (n=26, 29.89%), dermatofibroma (n=9, 10.34%), hemangioma (n=8, 9.20%), eccrine poroma (n=6, 6.90%), ganglion cyst (n=3, 3.45%), vascular leiomyoma (n=2, 2.30%), lipoma (n=2, 2.30%). Conclusion: The data shows that understanding the relationship between the anatomical location and the type of tumor may be helpful in the diagnosis of tumor on the foot.
