      • IL-7 Induces an Epitope Masking of<i>γ</i>c Protein in IL-7 Receptor Signaling Complex

        <P>IL-7 signaling via IL-7R<I>α</I>and common<I>γ</I>-chain (<I>γ</I>c) is necessary for the development and homeostasis of T cells. Although the delicate mechanism in which IL-7R<I>α</I>downregulation allows the homeostasis of T cell with limited IL-7 has been well known, the exact mechanism behind the interaction between IL-7R<I>α</I>and<I>γ</I>c in the absence or presence of IL-7 remains unclear. Additionally, we are still uncertain as to how only IL-7R<I>α</I>is separately downregulated by the binding of IL-7 from the IL-7R<I>α</I>/<I>γ</I>c complex. We demonstrate here that 4G3, TUGm2, and 3E12 epitope masking of<I>γ</I>c protein are induced in the presence of IL-7, indicating that the epitope alteration is induced by IL-7 binding to the preassembled receptor core. Moreover, the epitope masking of<I>γ</I>c protein is inversely correlated with the expression of IL-7R<I>α</I>upon IL-7 binding, implying that the structural alteration of<I>γ</I>c might be involved in the regulation of IL-7R<I>α</I>expression. The conformational change in<I>γ</I>c upon IL-7 binding may contribute not only to forming the functional IL-7 signaling complex but also to optimally regulating the expression of IL-7R<I>α</I>.</P>

        골화석증 환자에서 발생한 흉요추부 경막 내 신경초종

        신경초종은 Schwann 세포에서 기원하는 양성 종양으로 주로 두경부, 체간 및 상하지에서 발견되고, 흉요추부 경막 내에는 드물게 발생한다. 그리고 골화석증은 유전적 소인을 가지고 원인 불명인, 발생 빈도가 극히 낮은 골격계 질환으로 이에 대한 보고는 장관골 골절에 관한 것들이 대부분이며 척추에 대한 보고는 드물다. 더욱이 골화석증 환자에서 발생된 흉요추부 신경초종과 그 치료에 대해서는 아직 보고된 바가 없다. 저자들은 서서히 진행된 좌측 하지 근력 약화 및 감각 이상을 주소로 내원한 36세 여자 골화석증 환자에 발생한 흉요추부 신경초종을 후방 접근법에 의한 수술적 종양 절제술로서 우수한 결과를 얻었기에 문헌고찰과 함께 보고하는 바이다. Schwannoma is a benign tumor derived from Schwann cells which are usually found in the head, neck, trunk, upper and lower extremities, and rarely in the thoracolumbar junction. Also, osteopetrosis is an idiopathic genetic skeletal disease that rarely occurs. Most reports of osteopetrosis are about long bone fractures, but spinal tumor in osteopetrosis has been rarely reported. Furthermore, there is no report about thoracolumbar intradural Schwannoma and its treatment in patient with osteopetrosis. Thus, we report the clinical results of a thoracolumbar Schwannoma in a 36 year-old female osteopetrotic patient who had progressive weakness, and dysesthesia of the left lower limb. The patient underwent laminectomy and total excision of tumor through a posterior approach, and a favorable clinical outcome was achieved.

        Validation of the Korean Ankylosing Spondylitis Quality of Life Questionnaire

        Background: Measuring accurate and reliable scores of quality of life in patients with ankylosing spondylitis (AS) is important in both decision-making and treatment planning for the disease. Questionnaire, The ankylosing spondylitis quality of life (ASQoL), is one of the representative tools for assessing how seriously AS patients view their disease severity, activity, as well as their overall health. To make these types of questionnaires readable and understandable, local language translation of surveys should be required. A Korean version of the ASQoL questionnaire has accordingly been developed. This study assessed the Korean version of the ASQoL survey to evaluate the reliability and validity of it. Methods: Translation and reverse translation of the English ASQoL survey were conducted. A total of 120 consecutive AS patients received a mail including the Korean-translated 36-Item Short Form Survey (SF-36), the ASQoL survey, and the visual analog scale (pain). The coefficient of intraclass correlation and Cronbach’s alpha were computed, and factor analysis, as well as reliability assessments utilizing the kappa agreement statistics for each item, was undertaken. By analyzing the responses to SF-36 and ASQoL questionnaire utilizing Pearson’s correlation coefficient, construct validity was calculated. Results: Factor analysis was performed regarding pain, physical function, and mental function. The kappa statistic of agreement was larger than 0.6 for all items. The ASQoL questionnaire had adequate test and re-test reliability (0.814). Furthermore, Cronbach’s α, the internal consistency, was very good (0.877). The Korean-translated ASQoL questionnaire demonstrated a significantly strong correlation between the single domain and total SF-36 scores. Conclusions: The Korean version of the ASQoL questionnaire showed acceptable properties of measurement and successful translation. Thus, it can be said that the questionnaire is appropriate for evaluating the outcomes of Korean patients with AS.

        Relationships between Clinical Status and Gait Parameters in Ankylosing Spondylitis

        Background: This study aimed to identify the relationship between gait parameters and health-related quality of life (HRQOL) in patients with ankylosing spondylitis (AS). Methods: The study group comprised 134 patients with AS and 124 patients were enrolled as controls. All study participants underwent instrumented gait analysis and completed clinical questionnaires. The kinematic parameters of gait were walking speed, step length, cadence, stance phase, single support, double support, phase coordination index (PCI), and gait asymmetry (GA). For each patient, a visual analog scale (VAS; 0–10) score was used to assess back pain, 36-item short form survey (SF-36) questionnaire was administered to evaluate the HRQOL, and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) was calculated. Using kinematic parameters and questionnaires, statistical analyses were done to investigate significant differences between the groups. Relationship of gait kinematic data and questionnaires of clinical outcome was also evaluated. Results: Among the 134 patients with AS, 34 were women and 100 were men. In the control group, 26 were women and 98 were men. The patients with AS and control group patients had significant differences in terms of walking speed, step length, single support, PCI, and GA. However, such differences were not observed in cadence, stance phase, and double support (p > 0.05). In correlation analyses, gait kinematic parameters and clinical outcomes were significantly related with each other. In multiple regression analysis performed to identify predictive factors for clinical outcome, walking speed was found to predict VAS, and walking speed and step length were found to predict the BASDAI and SF-36 scores. Conclusions: Patients with and without AS had significant differences in the gait parameters. Correlation analysis showed significant correlation between the gait kinematic data and clinical outcomes. In particular, walking speed and step length successfully predicted clinical outcomes in patients with AS.

        Experimental investigation of macroscopic material nonlinear behavior and microscopic void volume fraction change for porous materials under uniaxial compression

        <P><B>Abstract</B></P> <P>The purpose of this study was to simultaneously analyze the nonlinear behavior and the void volume fraction change of polyurethane foam (PUF) under a uniaxial compressive load through micro-scale and macro-scale viewpoints. The influence of micro-scale factors on the macroscopic properties of PUF was analyzed, along with the interrelationships between micro-scale and macroscopic properties. The microstructure of PUF was observed by analyzing the density. Various micro-analyses were also conducted, including those for energy absorption, Young's modulus, and recovery behaviors. Internal structure changes such as those in density and degree of compressive strain were analyzed by measuring the void volume fraction of each region in the specimen under a uniaxial compressive load. The elastic, plateau, densification, and fracture regions of the compressive test stress-strain curve were divided, analyzed, and compared with microscopic analysis results. Based on the results of the macro-micro-scale experiment, we analyzed influence and change process of microstructures on the compressive behavior of the PUF specimen in detail. This study shows that internal and overall changes in the compression process of various porous materials can be predicted under a uniaxial compressive load.</P>

        Treatment of Thoracolumbar and Lumbar Unstable Burst Fractures by Using Combined and Posterior Surgery

        Purpose: The purpose of this research was to analyze the results of the combined and posterior approaches for treating thoracolumbar and lumbar burst fractures and to find an adequate method of treatment. Methods: We retrospectively analyzed the cases of 46 patients with unstable thoracolumbar and lumbar burst fractures who had been surgically treated. All cases were divided into two groups based on the operation method used. Eleven patients had undergone the combined approach, while 35 patients had undergone the posterior approach. Radiological and clinical evaluations were performed before surgery, after surgery, and at the final follow-up. Results: The stenotic ratios of the area occupied by the retropulsed bony fragments to the estimated area of the original spinal canal were 68.2% and 45.6% for the combined and the posterior approaches, respectively. No significant differences in the neurological improvement or the corrected state of the sagittal index were noted, but the patients who had been treated with the combined approach group had better results than those who had been treated with the posterior approach group in terms of correction and maintenance of the sagittal index. The average kyphosis corrections at the final follow-up were 15.3 degrees for the patients in the combined approach group and 10.0 degrees for those in the posterior approach group. Surgical time and estimated blood loss were all significantly higher for patients in the combined approach group. Conclusion: The combined and the posterior approaches showed similar results in the improvements of the neurologic state and the corrected state of the sagittal index. However, use of the combined approach is recommended for patients with severe kyphosis and with severe canal encroachment. [ J Trauma Inj 2016; 29: 14-21 ]

        This study suggests an optical method to measure cardinal of gait (step width, step length, and stride length) with position sensitive detector (PSD). The effect of reflector’s shape (flat and cylinder) on the PSD output voltage was examined for the application of the suggested system to real situations with a curved shape reflector (e.g. shoes). Various mathematical models were evaluated to find the optimal equation for the distance measurement. Considering the effect of shape on detected signal, the inverse polynomial model was developed. The suggested method is simple to operate, low in cost, small in size, and can evaluate gait parameters in real time. This method is expected to be useful in the field of rehabilitation and sport science

        유한요소법을 활용한 척추유합술 후 유합 전만각에 따른 인접 분절의 역학적 특성 평가

        후방 요추체간 유합술은 퇴행성 척추질환의 치료법으로서 불안정한 요추를 고정시키고 요추 전만곡을 교정하여 퇴행성 척추 질환의 진행을 늦추지만 인접 분절의 퇴행과 같은 합병증이 유발된다. 인접분절의 퇴행성 변화의 발병 요인 중 하나는 수술 이후 유합 인접 분절에 과도한 하중이 걸리기 때문이다. 따라서 과도한 하중을 낮추기 위한 방안으로 후방 요추체간 유합술 시 유합 분절사이 전만각이 유합 인접 분절에 미치는 영향에 관한 평가가 필요하다. 본 논문에서는 유한요소법을 활용하여 척추 거동별 인접 분절의 운동 범위와 유합 인접 추간판 및 유합 분절내의 종판과 케이지의 최대 등가응력 비교 분석하여 유합 분절사이 전만각에 따른 인접 분절의 역학적 특성을 평가하였다. Posterior lumbar interbody fusion (PLIF) is a treatment for degenerative spinal disease. It slows the progression of degenerative spinal disease by repairing the unstable lumbar spine and correcting lumbar lordosis. However, certain complications such as adjacent segment degeneration are further induced. A cause of degenerative change in the adjacent segment is the excessive load in that segment after surgery. In this study, using the finite element method, the range of motion of the adjacent segment for the lumbar motion and the maximum equivalent stress of the adjacent intervertebral disc to fusion as well as the endplate and the PLIF cages within the fusion segment were compared and analyzed. Based on this, the mechanical properties of the adjacent segment according to the lordosis angle between the fusion segments were evaluated.

        골밀도를 고려한 척추경 나사못의 최적 디자인 도출을 위한 실험적 연구

        본 논문에서는 퇴행성 및 특발성 척추 질환 치료에 사용되는 척추경 나사못의 최적 디자인을 제안하였다. 최적 디자인 도출을 위하여 서로 다른 골밀도의 인공 해면골과 다양한 척추경 나사못의 디자인에 대한 인발 시험을 수행하여 인발력(고정력)을 검토하였다. 우선, 총 9종의 일반형 나사못의 인발시험을 통하여 최적의 디자인을 도출하였으며, 여기에서 도출된 최적 일반형 나사못 디자인을 기반으로 이중 나사산형, 침투형, 복합형 척추경 나사못의 디자인을 설계 및 시제품을 제작하였다. 제작된 시제품과 글로벌 기업의 제품에 대한 인발 시험 수행 및 비교를 통하여 본 시제품의 우수성을 검증하였고, 골밀도에 따른 이중 나사산-피질골, 골 시멘트-해면골의 고정력에 미치는 영향에 대하여 검토하였다. 본 연구를 통하여 이중 나사산과 골 시멘트가 고정력에 지대한 영향을 미치며, 골다공증성골에서는 더욱 큰 효과를 보임을 알 수 있었다. 이를 통하여 골밀도를 고려한 최적의 척추경 나사못 디자인을 도출할 수 있었다. This paper proposes an optimal pedicle screw design for the treatment of spinal diseases. A pullout test was performed with various pedicle screw designs and artificial bones with different bone mineral densities, in consideration of the pullout force of the pedicle screw. First, the optimal design of the standard pedicle screw (S) was selected through a pullout test of the 9 types of standard pedicle screw, based on which the prototypes of the doublethread (D), penetration (P), and hybrid (H) pedicle screws were designed and produced. The D, P, and H pedicle screws were also subjected to pullout tests and compared with the products of a global medical device company. The effects of the double thread-cortical bone and bone cement-cancellous bone were investigated according to bone mineral density. The double thread and bone cement had significant effects on the fixation force to the osteoporotic bone. The optimal pedicle screw design was derived in consideration of bone mineral density in this study.
