Eisenmenger's syndrome describes a situation in which the left-to-right intracardiac shunt is reversed as a result of increased pulmonary vascular resistance. The patients with Eisenmenger's syndrome are poorly tolerated and markedly increased maternal mortality to 30 - 50% during pregnancy, Physiological change of pregnancy decreases systemic vascular resistance that further aggravates the bi-directional or left shunt associated with Eisemnenger's syndrome. We report a case of 37 weeks a pregnant woman with an urgent cesarean section under general anesthesia for preterm labor. She was a pregnant patient with Eisenmenger's syndrome and did not receive antenatal care during pregnancy. Anesthesia was induced with ketamine-midazolam-fentanyl and maintained with ketamine-midazolam. The patient was ventilated with 100% O₂. Blood pressure was maintained with intravenous phenylephrine, crystalloid solution, and blood. Phenylephhne was used to maintain systemic vascular resistance (SVR). Even though the patient was carried rapid and adequate medical management, the patient was transferred to the ICU in an intubated state and died 2 days after surgery. We conclude that the parturients with Eisenmenger's syndrome require more regular, careful antenatal care, monitoring, and adequate management.

Prolapse of the fallopian tube into the vaginal vault after hysterectomy is a rare complication. Since most of cases may resolve before detection or remain undiagnosed, the exact incidence is still unknown. Though most common sign of tubal prolapse is tender friable tissue in vaginal cuff, with the simptoms of lower abdominal pain, dysparenunia, vaginal discharge, or postcoital spotting. The definitive diagnosis should be confirmed histologically Recently, we experienced a case of fallopian tube prolapse after abdominal hysterectomy in a 41-year-old woman and report with a brief review of the literature.

하지수술에서 Ropivacaine-Fentanyl 경막외마취시 첨가한 Clonidine의 용량에 따른 효과

Background: Ropivacaine is an amino amide local anesthetic that has an advantage of a low-toxicity profile. Clonidine, an α_2 adrenergic agonist, is known to prolong and intensify anesthesia from epidural local anesthetics. The aim of this study was to evaluate the dose-response effects of added clonidine to ropivacaine-fentanyl epidural anesthesia for lower extremity surgery. methods: Forty-five patients undergoing lower extremity surgery were randomly allocated to three groups to be given the following agents by and epidural route: 0.75% ropivacaine 15 ml + fentanyl 50 ㎍ (group Ⅰ), 100 ㎍ (group Ⅱ), or 150 ㎍ (group Ⅲ). Onset and maximal height of sensory block, and duration of sensory and motor block were assessed. Also, blood pressure, heart rate and sedation score were measured. Results: Duration of sensory and motor block of groups Ⅱ and Ⅲ was significantly longer than that of group Ⅰ, but there was no difference between group Ⅱ and group Ⅲ. Two segment regression times were significantly different among the three groups. Onset of sensory block, blood pressure, heart rate, and sedation scores were not significantly different among the three groups. Conclusions: The addition of Clonidine 100 and 150 ㎍ prolonged duration of anesthesia more than 50 ㎍ with ropivacaine-fentanyl epidural anesthesia for lower extremity surgery. Keratin wastes are generated in excess of million tons per year worldwide and biodegradation of keratin by microorganisms possessing keratinase activity can be used as an alternative tool to prevent environmental pollution. For practical use of keratinase, its physicochemical properties should be investigated in detail. In this study, we investigated characteristics of keratinase produced by Xanthomonas sp. P5 which is isolated from rhizospheric soil of soybean. The level of keratinase produced by the strain P5 increased with time and reached its maximum (10.6 U/ml) at 3 days. The production of soluble protein had the same tendency as the production of keratinase. Optimal temperature and pH of keratinase were 40℃-45℃ and pH 9, respectively. The enzyme showed broad temperature and pH stabilities. Thermostability profile showed that the enzyme retained 94.6%-100% of the original activity after 1 h treatment at 10℃-40℃. After treatment for 1 h at pH 6-10, 89.2%-100% of the activity was remained. At pH 11, 71.6% of the original activity was retained after 1 h treatment. Although the strain P5 did not degrade human hair, it degraded duck feather and chicken feather. These results indicate that keratinase from Xanthomonas sp. P5 could be not only used to upgrade the nutritional value of feather hydrolysate but also useful in situ biodegradation of feather.
