        The Effects of Multi-Growth Factors-Containing Cream on Post-Thyroidectomy Scars: A Preliminary Study

        Background: Growth factors play important roles in wound healing. However, the evidence for the effects of growth factors on post-thyroidectomy scars is limited. Objective: We performed a prospective study to assess the preventive and therapeutic effect of a multi-growth factor (MGF)-containing cream on post-thyroidectomy scars. Methods: Twenty-one patients with thyroidectomy scars applied MGF cream twice a day. We assessed the changes in erythema, pigmentation, skin elasticity, and skin hydration status using the erythema index, melanin index, cutometer, and corneometer, respectively. In addition, Vancouver scar scale (VSS) and patient satisfaction were assessed at 10 days after surgery (baseline), 2 weeks, 6 weeks, and 12 weeks after baseline. Results: The mean total VSS scores were significantly lower at 6 weeks (3.24±1.51 vs. 1.91±1.38) and 12 weeks (3.24±1.51 vs. 1.71±1.59) compared to the baseline. The degree of pigmentation was significantly lower at 12 weeks compared to the baseline, and the skin elasticity, and the skin hydration status were significantly higher at 12 weeks compared to the baseline. Over 85% of the patients were satisfied with the use of MGF cream without any adverse effect. Conclusion: MGF cream might have additive or supportive effect for scar formation after thyroidectomy.       • P132 : Comparison of non-ablative and ablative fractional laser treatments in a postoperative scar study

        Background: Recently, both non-ablative and ablative fractional laser (NFL and AFL) systems have attracted attention as potential therapies for the revision of thyroidectomy scars. Objectives: The present split-scar study was designed to directly compare the efficacy of these two methods for the treatment of post-thyroidectomy scars. Methods: 20 females with scarring 2 to 3 months post-thyroidectomy were enrolled. One half of the scar was treated with NFL and the other half was treated with AFL. In each case, 2 treatments were given at 2-month intervals. Clinical photographs were taken for evaluation. Independent clinician grading of improvement and patient satisfaction were measured on a quartile scale. Color and scar hardness were measured with a dermaspectrometer and durometer, respectively. Results: The mean clinical improvement grades for AFL and NFL were highly similar, 2.45±0.99 and 2.35±0.85, respectively, without statistical significance (p=0.752). However, NFL treatment resulted in statistically significant changes in erythema and pigmentation (p=0.035 and p=0.003, respectively), and skin hardness was significantly reduced after AFL treatment (p=0.026). Conclusion: Clinical improvement was not significantly different between the two systems; however, AFL was better at reducing scar hardness whereas NFL was superior for lightening color. These data suggest that a study assessing the feasibility of a combined approach for the revision of post-thyroidectomy scarring might be warranted.

      • Development of rheumatoid nodules after anti-tumor necrosis factor α treatment with adalimumab for rheumatoid arthritis

        A 40-year-old male presented with 4 months history of multiple various sized erythematous to violaceous subcutaneous nodules on both extensor surfaces of elbows, which was partially movable and showed no symptoms such as pruritus, pain, and tenderness. He was diagnosed as rheumatoid arthritis 7 years ago. A variety of therapeutic options all failed to show symptomatic improvement. Thus, the patient started adalimumab, a human monoclonal antibody to tumor necrosis factor (TNF)-α. Introduction of adalimumab injection improved signs and symptoms of RA gradually, however, the multiple subcutaneous nodules occurred in the extensorsides of the elbows two months after. A skin biopsy showed dense fibrinoid deposits in deep dermis and subcutis at low magnification. At high magnification, large palisading granulomas surrounding degenerated connective tissue and fibrin were noticed and mucin deposition was rarely observed. Overall, pathologic features were consistent with rheumatoid nodule. Rheumatoid nodule development is extremely rare side effect and only two cases after etanercept and adalimumab injection have reported so far. However, in the previous report caused by adalimumab injection, rheumatoid nodule was diagnosed by only ultrasonogram. Thus, to our knowledge, this is the first report of rheumatoid nodules confirmed by histopathologic diagnosis following the treatment with adalimumab.

        Filler injection is quite useful and promising for facial rejuvenation due to immediate effect and reduced risks compared with invasive surgery. However, various side effects have been reported in the literature, and polydimethylsiloxane (PDMS), one of the industrial silicones, can cause local reactions including granulomatous foreign body reactions, abscesses, draining sinuses, scarring, contractures, and deformities, and migration to distant locations and systemic complications have also been reported. A 32-year-old male presented with 3 years history of multiple pea-sized subcutaneous nodules on the forehead which showed no symptoms such as pruritus, pain, and tenderness. He had a history of PDMS injected for forehead wrinkles at the nonmedical beauty shop 7 years ago and the injected sites were getting bigger since3 years ago. A skin biopsy was done to figure out foreign body reactions, and the slide revealed foreign body granulomas in the superficial dermis. There are many different treatment options to reduce granulomas. Systemic corticosteroids are the most widely used and surgical removal could be an option although there are limitations of the aggressiveness and difficulty of fully removing the widely disseminated filler materials. And there is also a report about successful treatment with combination of Nonablative Fractional and Ablative Fractional Lasers.

      • The effects of the mechanical stretch on human dermal fibroblasts

        Background: Mechanical force plays a significant role in scar development which is supported by the fact that many high stretching tension areas tend to be keloids or hypertrophic scars. Furthermore, mechanical force has been shown to stimulate the transformation of dermal fibroblast into myofibroblasts, which is one of the important key mediators of fibrosis. However, the research about the effects of mechanical stretch on human dermal fibroblasts is still limited. Objectives: The aim of this study is to investigate the effects of mechanical stretch to dermal fibroblast. Methods: To generate the mechanical stretch, we made special mechanical cell stimulation kit which is a vacuum induced programmable biostretching system called biostretching machine. In order to analyze the effects of mechanical stretch to dermal fibroblast, the sinusoidal (60cycles min-1) mechanical stretch was applied to human dermal fibroblasts. Proteomics analysis was performed to compare the protein expressions between stretched fibroblasts and non-stretched fibroblasts. Results: HDFs with mechanical stretch showed further proliferation than HDFs without mechanical stretch. And there was a difference in protein expression between stretched fibroblasts and non-stretched fibroblasts. Conclusion: A better understanding of the effects of mechanical stretch on human fibroblasts will help the development of novel technologies that can prevent or reduce pathological scar formation in the future.

      • The effects of Multi-growth factors-containing cream after laser treatment: A double blinded, randomized, split-face controlled study

        Background: Patients who received laser treatments can experience transitory erythema, edema, crusts and erosions for several days. They need a recovery period and thus it is important to shorten the downtime through faster recovery. Growth factors play an important role in wound healing. However, the research about the effects of growth factors on skin recovery after laser treatments is still limited. Objectives: In this study, we aim to evaluate the effects of the multi-growth factors-containing cream after laser treatment Methods: Twenty patients were enrolled in the study. They underwent laser treatment using ablative fractionallaser and they were randomized with respect to which side of the face was applied with either multi-growth factors (MGF) containing cream or control cream. Satisfaction levels were assessed from patients. And the wound healing levels, swelling improvement levels and overall improvement levels were assessed by two blinded dermatologists. Results: Patients’ satisfaction levels tended to be higher in the study group compared to the control group at each visit. Wound healing levels, swelling reduction levels and overall improvement levels were significantly higher in study group than control group. Conclusion: MGF containing cream showed rapid wound healing rate and objective improvement compared with the control and showed higher satisfaction level from patients. Thus, MGF containing cream can be effectively used for faster wound healing after laser treatment.

      • 3D 객체 검색시스템을 위한 벤치마크 데이터 파일포맷

        멀티미디어와 인터넷 기술이 성숙되어감에 따라 3D 객체가 콘텐츠를 작성하는데 많이 이용되고 있고 더불어 3D 객체의 검색시스템에 대한 연구가 시작되고 있다. 이에 따라 검색시스템을 평가하기 위해서는 표준화된 3D 벤치마크 데이터가 필요하다. 본 논문에서는 검색용 3D 벤치마크 데이터 파일을 만드는데 기반이 되는 새로운 3D 그래픽스 파일포맷을 제안한다. 제안한 파일포맷은 렌더링보다 검색효율을 목표로 하기 때문에 기하학적인 정보이외의 다른 정보는 줄이고 검색 속도와 편리성을 함께 고려하였다.

      • 입자 물리 시뮬레이션을 위한 3차원 오브젝트 생성

        컴퓨터 산업의 발달로 인해 입자 물리 실험을 가상으로 시뮬레이션 할 수 있게 되었다. 입자 물리 시뮬레이션을 하기 위해서는 실제 환경과 같은 가상 환경을 생성 해야 한다. 가상 환경을 생성하기 위한 오브젝트는 크기, 위치 정보 이외에 내부적으로 물질의 속성과 같은 입자 물리 시뮬레이션에 필요한 정보도 포함하고 있어야 한다. 그러나 이러한 오브젝트 생성은 어려운 작업이며 많은 시간을 필요로 한다. 본 논문에서는 입자 물리 시뮬레이션에 필요한 오브젝트를 쉽게 생성 할 수 있는데 사용자 중심의 생성 방법을 제안하며, 오브젝트 생성 시 물질의 속성과 색상 정보를 포함하여 생성하는 입자 물리 오브젝트 생성기를 구현했다.
