        Diagnostic Significance of Brainstem Auditory Evoked Potentials in Microvascular Decompression of Patients with Hemifacial Spasm or Trigeminal Neuralgia

        The purpose of this study was to analyse brainstem auditory evoked potentials (BAEP) wave change data during microvascular decompression (MVD). The nerve function of Cranial Nerve VIII is at risk during MVD. Intraoperative monitoring of BAEP can be a useful tool to decrease the danger of hearing loss. Between January and December 2009, 242 patients had MVD for hemifacial spasm (HFS) and trigeminal neuralgia (TN). Among intraoperative BAEP changes, amplitude of V-V’ was the most frequently observed during cerebellar retraction and decompression step of the MVD procedure. 138 patients (57%) had no BAEP change while 104 patients (42.98%) had BAEP change. 69 patients (28.5%) had Type A-I, 16 patients (6.6%) had Type A-II, 5 patients (2.1%) had Type B, and 13 patients (5.37%) had Type C. MVD is a surgical procedure to relieve the symptoms (e.g. pain, muscle twitching) caused by compression of a nerve by an artery or vein. During BAEP intraoperative monitoring, the surgical step is important in interpreting the changes of wave V. Several potential mechanisms of injury may affect the cochlear nerve, and complete loss of BAEP is often associated with postoperative hearing loss. Intraoperative BAEP monitoring may provide an early warning of hearing disturbance after MVD.

        Background: Pregnancy affects the course of Graves' Disease (GD), and patients who initially maintain euthyroid function into their middle trimester with minimum doses of antithyroid drugs become exacerbated after delivery. Even patients who are completely cured, requiring no treatment during pregnancy, can relapse after delivery. In this study, we examined the postpartum changes in the thyroid functions of patients with GD, and attempted to determine the factors contributing to these changes. Methods: The study subjects were recruited from pregnant women visiting our outpatient clinic for routine prenatal evaluations. 45 women previously diagnosed with GD, who had been treated and cured with hyperthyroidism, and were no longer taking any thyroid medications, were evaluated for 1 year post delivery. Results: Among 45 patients, 20 (44.4%) developed thyroid disorders following delivery. Postpartum thyroiditis (PPT) developed in 8 patients (17.8%), and GD developed in 12 (26.0%). The onset of the PPT disease 3.1 ± 1.4 months following delivery, which was significantly earlier than the 6.7 ± 2.7 months required for the post delivery onset of GD (p=0.003). The TBII values, measured during the thyrotoxic state in each womaen, were negative in women with PPT and positive in 71.4% of women with GD (p=0.030). The duration of treatment for hyperthyroidism prior or pregnancy, the number of recurrences, and the time interval without treatment, were not associated with the development of postpartum thyroid disorders. Whereas, the mean number of past pregnancies for women who developed PPT was 3.9 ± 2.1, and was significantly higher than the 2.2 ± 1.7 for women developing no thyroid dysfunctions (p=0.044). In 13 women their initial onset of GD occurred within one year postpartum, 7 (53.8%) having had a recurrence, which was significantly higher than in women whose disease onset occurred unrelated to delivery (5 of 32 women: 15.6%). Conclusion: Women with GD developed postpartum thyroid dysfunctions in 44.4% of cases. Women whose initial disease onset occurred within one year postpartum had higher recurrences of GD, and women who developed PPT had a history of higher gravidity compared to the euthyroid women postpartum. Therefore, if women with GD develop postpartum thyroid dysfunctions, the diagnosis should be made, and a treatment modality planned, following careful considerations of the patients' past obstetric history, changes in clinical manifestations and the TBII values

      • Contributions of intramolecular and intermolecular energy changes to strain-induced enthalpy relaxation in uniaxially drawn poly(lactic acid) films

        <P><B>Abstract</B></P> <P>Blends of poly(lactic acid)s (PLAs) containing three different concentrations of <SMALL>D</SMALL>-isomer (i.e., 1, 4, and 10 mol%; referred to as PLA1, PLA4, and PLA10, respectively) with poly(<SMALL>DL</SMALL>-lactic acid) (PDLLA) were prepared via solution blending. The three PLAs were miscible with PDLLA, as determined by dynamic mechanical analysis. Drawn films of PLA1/PDLLA, PLA4/PDLLA, and PLA10/PDLLA blends exhibited endothermic peaks just above the glass transition temperature, which is evidence of strain-induced enthalpy relaxation. A thermodynamic model for the miscible blend system was applied to determine contributions of intramolecular and intermolecular energy changes to strain-induced enthalpy relaxation for PLA1, PLA4, and PLA10; the resultant values suggested that intramolecular energy changes are mainly responsible for strain-induced enthalpy relaxation.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Drawn films of PLA blends exhibit endothermic peaks just above glass transition temperature. </LI> <LI> For PLA containing less than 4 mol% of <SMALL>D</SMALL>-isomer, almost 60% of the endothermic heat originates from intramolecular energy change. </LI> <LI> As the <SMALL>D</SMALL>-isomer content of PLA increases, the contribution of intramolecular energy change to the endothermic heat increases. </LI> <LI> Overall, intramolecular energy change is primarily responsible for strain-induced enthalpy relaxation. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        離婚家庭 靑少年을 위한 仲裁프로그램의 開發

        The divorce rates are increasing and children of divorced parents are exposed to various social, phychological problems compared to those children living together with their own parents. The main purpose of this research project lies in developing intervention programs for counseling children of families having divorced parents. The study is mainly composed of two parts. The first part deals with the investigation of psychological characteristics of children of divorce-parents families. The second part documents the effect and actual status of intervention programs for counseling those children of divorce-parents families in order to reduce the level of negative psychological features(such as anxiety, aggression, depression, self-esteem and achievement motivation). A total of 1,082 middle school students from 11 boy's middle schools and 7 girl's middle schools are the object of the survey research of the first part of the research project. In this sample are included 605 students of normal family(362 boys and 243 girls), 256 families with one or both parents deceased(159 boys and 97 girls) and 221 students of divorce-parents families(140 boys and 81 girls). In the second part of the research, we provided intervention programs for counseling children of divorce-parents families. A total of 15 students (7 boys and 8 girls) participated in this program in which every student was given two intervention sessions in a four-night-five-day schedule. This intervention program consists of 15 stages and is characteristic of a group counseling program. All of the 15 students participating in this intervention program were given guestionnaires measuring levels of children's attitudes and self-perception(CASP) on divorce before and after the intervention program activity, this trying to figure out whether there were any significant differences in their CASP between before and after the program. One of the findings were there were some differences between boy students an girl students. For boy students, it was found that there are significant differences between before and after the program with the exception of anxiety, aggression, social self-esteem and family self-esteem. For girl students, there existed significant differences except family self-esteem. Judging from these findings it is concluded that the intervention program developed for this research works effective as we originally designed to work. However, further researches should be done in order to elaborate the reasons of the differences between boy and girl students.

        지문 및 장문의 변화와 염색체의 변화를 관찰한 결과를 요약하면 다음과 같다. 1. 염색체는 G-trisamy를 나타내었다. 2. 지문의 형태에서 Ulnar Loop가 증가하였다. 3. Total ridge count는 감소하였다. 4. 장문에 있어서 palmar transverse flexion crease에 Simian type의 빈도가 높았다. Mongolism is the commonest syndrome encountered in patients with Autosomal anomaly. G_21 trisomy, associated with mental retardation. Accurate diagnosis of the nature of the chromosomal anomaly is important in order to he ablc to advise the patients upon the risk of involvement of next children, After Nehemiah, on the other hand, dermatoglyphics has been applied to medical field and We observed somewhat different finger prints or palmar creases in mentally retarded children. The purpose of the present investigation was to find out these variations in dermatoglyphics and chromosomal patterns in patients with Down's syndrome compared to normal developed children. Several results were obtained as follows. 1. The chromosome map showed the trisomy of G group. 2. The total ridge count (TRC) was declined and the frequency of ulnar loop was high in the finger prints of these patients. 3. The symian type was frequently appeared in palmar crease.

        Background: Vitronectin is one of major cell- adhesive glycoprotein in mammalian serum and plasma ; the other is fibronectin. It is a mixture of 65 and 75kDa protein in plasma which promotes spreading of a variety of cultured cells ,inhibits the cytotoxicity of membrane attacks complex C5b-7 and modulates thrombin-antithrombin III activity. Human plasma and serum contain 10-40mg of vitronectin per 100ml,but only a few mg or less of vitronectin can be isolated with a 0.5-20% recovery efficiency through very long processes or with expensive,commercially available monoclonal antibody. In this study, we purified the vitronectin from human plasma and produced anti-vitronectin polyclonal antibody using purified vitronectin. Methods: Vitronectin was purified from the human plasma by heparin-sepharose column and its efficacy was measured by cell spreading assay. Anti-vitronectin polyclonal antibody produced and confirmed by ELISA and immunoblotting. Results: This procedures produced about 0.8mg vitronectin from 100ml human plasma within 2 days. 1) Purified vitronectin promoted spreading of HepG2 hepatoma cells on substrates with a half maximal activity at 0.lug/ml. 2) In SDS-PAGE analysis of purified protein, 2 bands were found and their molecular weights were 75kDa and 65kDa,respectively. 3) The immunoblotting assay showed that the bands of molecular were same site as SDSPAGE analysis. Conclusion: Simple,rapid purified vitronectin by heparin-sepharose column and anti-vitronectin antibody may facilitate the elucidation of vitronectin function and action mechanism in human body.
