        Thermus caldophilus GK24 (Tca) DNA polymerase is highly useful enzyme for amplifying DNA fragments by polymerase chain reaction (PCR). A plasmid, pTCA, is expression vector for Tca DNA polymerase gene in Escherichia coli under the control of tac promoter and its expression level is low. For the high expression, the DNA sequence coding for NH_2-amino acid sequence of Tca DNA polymerase was changed by PCR, and then an expression vector pTCAM was constructed. The activity of Tca DNA polymerase in E. coli harboring for pTCAM has enhanced nearly 6-fold/ml than that for E. coli harboring pTCA.

        A thermostable DNA polymerase was used in an in vitro amplification procedure, the polymerase chain reaction(PCR). Thermus caldophilus GK24(Tca) DNA polymerase greatly simplifies the procedure and, by enabling the amplificatioin reaction to be performed at higher temperature, significantly improves the specificity, yield and sensitivity. The optimal conditions for PCR were investigated in terms of pH range, MgCl_2, KCl and DTT concentration using the purified Tca DNA polymerase. The optimal pH for DNA amplification was found to be between 8.6, and 9.2 in 50mM Tris-HCl buffer. The optimal concentrations of MgCl_2 and KCl for PCR were 1-1.5mM and 10-50mM, respectively. The concentration of DTT above 0.5mM was sufficient to amplify template DNA by PCR. These conditions can be used to amplify a wide range of target sequences with excellent specificity.

        상악골과 비사골동에 이환된 백악질 골화성 섬유종(cemento-ssifying fibroma)의 치험례

        The cemento-ossifying fibroma is a benign fibro-osseous tumor that may behave in an aggressive pattern. Although found predominantly in the mandible, most of these unusual tumors may arise in the maxilla and paranasal sinuses. Especially these lesion in the maxilla and sinuses behave more aggressively and are more prone to recur compared to mandibular counterparts. Therefore, more radical approach may be recommended than expectant observation or curettage in the initial management of this neoplasm. This is a case report of cemento-ossifying fibroma extended to maxillary sinus and naso-ethmoidal area treated by wide surgical excision after repeated conservative treatments.

      • 越鞠丸合補中益氣湯이 電氣刺戟스트레스를 받은 mouse의 免疫機能에 미치는 影響

        본 실험은 越鞠丸合補中益氣湯의 항스트레효과를 조사하기 위하여 시행되었다. 越鞠丸合補中益氣湯이 전기자극으로 스트레스 상황을 유발시킨 mouse에게 끼친 B 임파구 면역계 변화를 mouse 혈청의 綿羊赤血球에 대한 적혈구 응집소가와 비장세포에서 B 세포 및 T 세포가 차지하는 백분율의 변화를 통하여 측정하였으며 이를 대조군과 비교 분석하였다. 혈청 항체가의 측정에서 스트레스 자극하에서 越鞠丸合補中益氣湯을 투여받은 그룹에서 약제를 투여받지 않은 그룹에 비하여 유의성 있는 차이를 보여(p<0.01) 면역 기능을 정상군의 수준으로 유지하는 결과를 얻었다. T 세포와 B 세포의 백분율의 측정에서도 스트레스 자극하에서 越鞠丸合補中益氣湯을 투여 받은 그룹에서 약제를 투여 받지 않은 그룹에 비하여 유의성 있는 차이를 보여(p,0.01) 면역 기능을 정상군의 수준으로 유지하는 결과를 얻었다. 실험결과들로 보아 越鞠丸合補中益氣湯이 체액성 면역 반응과 비장 내 면역 세포의 백분율을 정상의 수준으로 유지하는 효과를 보여 면역 기능의 이상에 응용할 수 있는 가능성을 제시한다고 사료된다. The present experiments were designed to study the influence of Walgookwhanhab-Bojoongikgitang on immune functions of Balb/c mice under stress condition. Walgookwhanhab-Bojoongikgitang was orally administered to the mice for 15 days. On the 10th day the mice were immunized with sheep red blood cells (SRBC) and then subjected to electric footshock for 5 days (2 sessions a day, 11 footshocks a 31 min-session). The immune responses to SRBC were determined by means of hemagglutination and B / T cell populations in the spleen were studied by FACS analysis on the 16th day. The results were as follows. 1. After electric footshock, mice became sluggish and crowded to one side of the cage. Increased antibody titer for SRBC, increased B cell population, and decreased 7 cell population in the spleen were also observed. These results confirm that electric footshock caused stress inducing immunological and behavioral changes in Balb/c mice. 2. Walgookwhanhab-Bojoongikgitang administration significantly antagonized the effect of electric footshock on the antibody titer for SRBC. As a result, antibody titers for SRBC in the mice treated with Walgookwhanga-Bojoongikgitang were maintained at the similar levels as those of control group mice even after the electric footshock. 3. Walgookwhanhab-Bojoongikgitang administration also antagonized the effect of electric footshock on the B / T populations in spleen. As a result, B and T populations in the mice treated with Walgookwhanhab-Bojoongikgitang were maintained at the similar levels as in the control group mice even after the electric footshock. Taken together, M algookwhanhab-Bojoongikgitang seem to help Balb/c mice to maintain their humoral immune response and immune cell populations at a normal range under the stress conditions, suggesting its possible therapeutic use as a immune function modulator.

      • Thermus flavus AT-62 균주의 내열성 DNA Ligase 유전자의 Cloning 및 발현

        The DNA fragment encoding DNA ligase from Thermus flovus AT-62 was cloned into Escherichia coli using polymerase chain reaction(PCR) method. Under tac promoter control, Thermus flavus AT-62(Tfl) DNA ligase was expressed in E colt. Tfl DNA ligase in E. coli was purified by simple methods like heat treating, DEAESephacel and Cellulose phosphate column chromatography. The purified enzyme had a molecular mass of about 77kDa, estimated by sodium dodecyl sulfate-polyacrylamide-gel electrophoresis. The purified Tfl DNA ligase was capable of catalyzing blunt-end ligation at 65℃.

        K 대학병원 서어비스에 대한 소비자 만족도

        Authors administered a questionnaire to 300 clients visited to the K University Hospital during August in 1992 to investigate the degree and related factors of consumer's satisfaction with the hospital services. Clients consisted of 82 in-patients and 158 out-patients, 52.4 percent of them was male. Four dimensions of satisfaction were assessed which were medical management, administration, environment and attitude. In-patients reported as dissatisfied with the waiting time during receipt, investigative medical procedures and pharmacy (64.7%), lack of conveniency facilities for patients and their guardians(48.8%) and provision of conveniency facilities in the ward(42.4%), unsanitary state of out- and in-hospital(41.5%). Out-patients reported dissatisfaction with the course of medical management and the time needed(70.3%), the waiting time during receipt, investigative medical procedures and pharmacy(68.3%), lack of conveniency facilities for patients and guardians(42.4%). But both in-patients and out-patients were satisfied with medical equipments, doctor's diagnosis reliability and medical effects, technician's attitude more than 50%. Canonical correlation analysis was done to find related sociodemographic factors to the four dimensions of satisfaction. Of the four canonical functions, the first was only statistically significant in in-patients. Its eigen value was 0.56 and its canonical correlation coefficient was 0.60. Satisfaction with environment and medical management was selected as important concept in dependent variables. Standardized canonical function of environment and medical management were 0.76 and 0.35 respectively. It was related with woman with high educational attainment and increasing number of hospital visits. In out-patients, the first, second and third function were statistically significant and their eigen values were 0.18, 0.4, 0.08 respectively which were small compared to those of in-patients. Their canonical correlation coefficient were 0.40, 0.35, 0.28 respectively. Low degree of environment but high degree of medical management satisfaction was noted in the first function which was related with old age, woman and professional occupation. Low degree of satisfaction with administration(second function) was related with city resident, non-professional occupation and young age. High degree of medical management but low degree of administration satisfaction(third function) was related with surgical visitor, man and non-professional occupation. The direction of hospital managerial strategies could be planned and general characteristics of dissatisfactory clients might be suggested with these results.

        임플란트 보철물에서 임시 접착용 시멘트의 인장결합강도 비교

        The major disadvantage of cement-retained implant-supported superstructure is difficulty of retrievability. Sometimes, it is recommended that proetheses be provisionally cemented to implant abutment in order to ebaluate occlusion, screw loosening and tissue reaction. The retention of the provisional luting cementedusually employed in this case is considered to be important. This study compared tensile bond strength of gold crowns cemented to machined titanium implant abutments with three provisional luting cements. Also the effect of petrolatum and thermocycling on tensile bond stength were evaluated. Tensile bond strength of provisional luting cement were as follows with the decreasing order of Eugenol (13.06±1.544 kgf), Temp-Bond (9.41±0.737 kgf), Cavex (8.04±0.974 kgf), and Temp-bond with petrolatum (5.28±0.800 kgf). The addition of petrolatum in Temp-bond decreased tensile bond strength significantly (p<0.01). After thermocycling of 1,500 cycles, the tensile bond strengths of the cements were decreased with the order of Nogenol (10.87±3.031 kgf), Temp-Bond (8.1±0.667 kgf), Cavex (7.33±1.103 kgf), Temp-bond with petrolatum (4.88±1.348 kgf). The mocycling decreased the bond strength of Temp-Bond significantly, with no effects no other cements (p<0.01).

      • 내동정맥루수술에 있어서 자가정맥과 인조혈관(PTFE)의 개존율 비교

        The internal arteriovenous fistula is essential treatment modality of end stage renal disease. The majority of the end stage renal disease patients depend on hemodialysis, including those waiting for kidney transplantation or those suffering from rejection after kidney transplantation to sustain their lives. The long team patency and low complication rate of internal arteriovenous fistula are important for the quality of patient's life. The aim of this study is to compare primary and secondary patency between native vein & polytetrafluoroethylene (PTFE) for internal arteriovenous fistula(AVF) for long - term hemodialysis. The influence of diabetes on the primary and secondary patency of internal arteriovenous fistula is also studied. All native vein & PTFE AVF constructed between June 1989 & December 1994 at Asan Medical Center were retrospectively reviewed. Wilcoxon test was used to compare the primary and secondary patency rates of native vein AVF(n:299) with PTFE AVF(n:154). The ratio of male to female was 318: 210(1.5:1). The diabetes patient was 184cases(34.8%). The primary patency rate of native vien showed no significant difference to that of PTFE AVF(p=0.0539). The secondary patency rate of native vein is superior to that of PTFE AVF(p=0.0428). Both of primary and secondary patency rate in internal arteriovenous fistula for non-DM patients were superior to DM patients(p=0.0008, p=0.0105). In conclusion, we can use the PTFE as an alternative choice of material for internal arteriovenous fistula if there is no suitable native veins. We could expect good patency rate under the appropriate indications. We must try salvage procedure first on the failing graft before making another internal arteriovenous fistula on the any other site, especially on the native vein cases. Diabetes is a negative factor on the primary and secondary patency of internal arteriovenous fistula.
