        하악골 절제술후 재이식된 돌결자가골 재이식 : 치험 3례

        We described three cases of immediate reimplantation of a frozen-thawed autogenous mandible composed of a mixture of iliac bone, marrow, and particulate hydroxyapatite in tumors of the mandible. Acceptable outcomes were obtained in three patients who underwent immediate autogenous mandibular graft reconstruction. The conditions leading to successful outcome of the procedure are also discussed. Reimplantation of frozen autogenous lesioned mandible was performed in three patients with mandibular tumors. Two reimplanted grafts survived without complications following surgery. One case had postoperative infection that resolved with appropriate antibiotic treatment. There were no recurrences of the primary lesions. Satisfactory facial contour after surgery was achieved. These results are most promising, and we believe that, with further refinement, this technique will offer a new and acceptable modality for facial reconstruction in patients with cancer.

        치조제 증강을 위한 HTR 중합체 이식후 치유과정 : 증례보고

        The purpose of this study is to evaluate the effect of HTR?? (hard tissue replacement, Bioplant Inc, U.S.A) polymer on short-term healing as a grafting material for alveolar ridge augmentation. A 48-year-old female presented insufficient bone height and width for the placement of implants. HTR?? polymer was used for ridge augmentation. Bone biopsy was harvested 8 months after the ridge augmentation procedure. HTR?? polymer displayed rapid bone regeneration and mature lamellar and trabecular bone redevelopment. Clinical and histologic observation from the treatment of the patient presented suggest that HTR?? polymer seems to be a appropriate material for alveolar ridge augmentation.

        백서에서 치아 회분말과 치과용 연석고의 혼합매식술시 골막의 역할: 실험적 연구

        The purpose of this study was to evaluate the bone-forming capacity of the periosteum in calvaria of rats. The experiment was carried out in 49 rats. We exposed the calvaria and made 1㎝ diameter round full thickness defect at both sides of calvaria. In the left calvarial bone serving as control, the periosteum was removed after implantation of block, while in the right calvarial bone the periosteum remained intact as an experimental site. The histologic examination of bone response was performed after 1-, 2-, 4-, 6-,8-, 12-, 24-week implantation in calvaria of rats. We could observe the periosteal preservation favorably influenced the bone formation.

        가토에서 관절원판을 전방변위시킨 후 두개하악관절의 조직병리학적 변화

        To investigate the histopathologic changes of the rabbit craniomandibular joint (CMJ), anterior disc displacement (ADD) was surgically induced. Of 15 rabbits, 9 were classified as experimental group, 3 as control of group, and 3 as sham-operation group, respectively The experimental group (9 rabbit) was subdivided into 6 weeks, 12 weeks, and 20 weeks group, respectively 3 rabbits. In experimental group, the disc was displaced anteriorly at the right side(operation side) and left CMJ was not operated(non-operation side). Both CMJs were harvested at 6, 12, and 20 weeks following surgery in experimental group. The CMJ was harvested at 6 weeks following surgery in sham-operation group. The specimens were examined under light microscope after hematoxylineosin and verhoeff staining. The obtained results were as follows: 1.In midsagittal view of control and sham-operation group, there were no degenerative and hyperplastic changes in the CMJ including articular disc, mandibular condyle, and temporal fossa. 2.Disc was shortened anterior-posteriorly and deformed of biconcave shape in all experimental group. In 20 week experimental group, intermediate zone of disc width was almost same as the width of posterior band of disc. 3.Soft tissue hyperplasia at the posterior attachment area of the disc and condylar hyperplasia were seen in the all experimental groups, but the degree of the hyperplasia was greater at 12, 20 weeks than at 6 weeks. 4.The number of elastic fiber in the disc proper was decreased and the direction and morphology was chanced in all experimental groups compared with those of control of groups. 5.Fatty degeneration was shown in non-operation side of 20 weeks experimental group. These results suggest that induced ADD in craniomandibular joint of rabbits could lead to a reparative and degenerative changes of the affected joint similar to those described for osteoarthrosis and osteoarthritis in human.

        변형 LeFort Ⅲ 골절단술을 이용한 중안면 성장부전을 동반한 하악전돌증의 치험례

        Maxillary-malar deficiency is the most frequently occurring midface dentofacial deformity. Clinicaly patients with maxillary-malar deficiency exhibit malar and infraorbital rim deficiency and class III malocclusion. For treatment of these deformities, modified LeFort III osteotomy have been used. Modified LeFort III osteotomy advances maxilla with orbital rims and zygomatic bone anteroposteriorly. This is a case of patient who had severe mandibular prognthism with midface deformity. We performed modified LeFort III osteotomy for maxillary-malar advancement and simultaneous bilateral sagittal split ramus osteotomy for mandibular prognathism and autogenous iliac bone graft.

      • 貯藏中인 쌀(벼, 玄米, 自米 Parboil米)의 脂肪成分의 化學的 變化

        Rough rice(Chucheong) which had been harvested at the experiment station, Chung Buk National University on October 26, 1992, was used in this experiment. The rough rice was divided into two parts. A part of the rough rice was parbolied(B series) and another part was untreated(A series). Brown and polished rice was obtained from untreated and parboiled rice. Six different types of the rice stored at room temperature for about 10 months. During the storge time, moisture and crude lipid contents, free fatty acid values, peroxide values, TBA values and fatty acid compositions were investigated. The result was as follows. 1. The moisture contents of all the samples decreased remarkably during the storage periods. Polished rice showed the greatest decrease. The decreasing rate was pronounced between 3-7 mouth. 2. The crude lipid contents of the polished and brown rice increased during the storage periods. As the moisture contents decrease, the crude lipid contents was increased proportionally. The crude lipid contents of rough rice were rarely changed. 3. The free fatty acid values of all the samples increased during the storage periods. The increasing rate was pronounced between 5-9month. The free fatty acid values of polished rice was greater than those of brown and rough rice. 4. Peroxide values of all the samples increased during the storage periods. Especially, the increase in the peroxide values remarked between 5-9month. The peroxide values of untreated rice were greater than those of parboiled rice. 5. TBA values of all the samples increased during the storage periods. Its increase pattern were similar to the peroxide values. 6. Major fatty acids of the samples were oleic acid, and linoleic acrid, palmitic acid. A little of stearic acid, myristic acid, linolenic acid, and arachidic acid was found. The changes in fatty acid composition of the each samples were rarely noted during the storage periods.

        치아회분-석고 혼합제와 생흡수성 차단막을 이용한 임플란트 주변 결손부 수복의 조직학적 평가

        This study evaluated whether a particulate dentin-plaster of paris combination (PDPP) and bioresorbable membrane could be used to induce new bone to cover the partially exposed surface of titanium endosseous implants. Twelve AVANA (SooMin Synthesis Dental Materials Co., Korea) dental implants were placed in the ilium of mongrel dog. All implants were self-tapping screw implants, 10㎜ long and 4㎜ in diameter. A circumferential bony defect around the implant was made with surgical bur and the cervical 1/3 of the implant surface was exposed. Six implants were used as controls and the rest were covered with particulate dentin-plaster of Paris combination and Biomesh bioresorbable membrane (Samyang, Korea). The dogs were killed at 6 and 12 weeks. The defects treated with PDPP and Biomesh membrane were filled with newly formed bone and direct bone contact with the implant surface developed. Most of PDPP material was resorbed after 12 weeks. In histomorphometric study, there was a statistically significant difference in bone contact between two groups in 6 weeks (P < 0.05). This study showed that guided bony regeneration using PDPP and bioresorbable membrane induces new bone to form over the exposed portion of an implant.

        비강상악동 미분화암종

        Sinonasal undifferentiated carcinoma(SNUC) is an uncommon aggressive malignancy of the nasal cavity and paranasal sinuses. Since the initial recognition of SNUC as a distinct clinicopathological entity, treatment regimens have evolved to include the current recommendation of combined radical resection, radio-therapy and chemotherapy. Despite this aggressive therapy, outcomes have remained dismal, with the mean survival time still being less than 1 year after diagnosis. SNUC present with locally advanced disease, with a tendency for the tumor to spread from the paranasal siuses to the orbit and anterior cranial fossa. A case of SNUC occurred in the left maxilla showing an unusual clinical manifestations and a fulminant course of disease, is described with literatures review. A 77-year-old female patient presented with rapidly growing tumor-like bulging of the left maxilla and slight ulceration of the right upper posterior vestibule. At the first biopsy, diagnosis was a radicular cyst, but SNUC was made subsequently by the second biopsy 6 month later. Rapid and wide extension from the maxilla to the orbit and nasal cavity developed with infection. Although the time relationship is not clear, local invasion could be developed from localized disease of SNUC. Despite combined therapy with chemotherapy and radiotherapy, She has been expired 13 months after the first biopsy. Focal positivity of immunohistochemical stain(CK, EMA), which is a specific characteristic of SNUC, may serve as a factor showing unfavorable prognosis.

        안면 중앙부 골절에 관한 임상적 연구

        The purpose of this study was to investigate the epidemiology of midface fratures. We observed clinically 71 patients with midface fracture who visited the Department of oral and maxillofacial-surgery, Chosun University, Dental Hospital, from 1991 to 1994. The results obtained were as follows. 1. There was the highest age incidence in the third decade(29.6%). 2. There was the highest incidence in the summer(36.6%). 3. The most etiologic factor of midface fracture was traffic accidents(43.6%) and next factor was fall down(38.0%). 4. The highest incidence fracture was zygomatic arch(38.7%) and next fracture was ZMC fracture(31.5%). 5. Most midface fracture was treated within 2 weeks(86.7%). 6. Midface fractures was most frequently combined with mandibular fracture(15.5%), head injuries(14.1%),orthhopedic injuries(8.5),thorax & abdomen(5.6%). 7. The highest complication was the nerve injury(8명), and next complications were infection(3명), epiphora(3명) etc.

        레이저 치의학의 최근 연구동향

        As the laser becomes more widely available in recent years, the oral and maxillofacial surgeon will have greater opportunity to use it for the treatment of soft tissue and hard tissue pathologies of the mouth. However, little information has been published on the bone. Dental lasers serve as an adjunct to regular treatments and enhancing outcomes in still others. The laser replaces conventional therapies with equal or improved results. Lasers offer new treatment options that heretofore were not available. Laser dentistry is capable of significantly minimizing discomfort, enhancing recovery times, and improving the overall efficiency of the dental office. We describe the basic researches and the developimg laser devices.
