Purpose: We describe a 54-year-old woman with systemic lupus erythematosus (SLE) who suddenly presented with chorea and had positive antiphospholipid antibodies. F-18 FDG PET showed abnormally increased glucose metabolism in bilateral putamen and primary motor cotex. Tc-99m ECD SPECt also showed abnormally increased regional cerebral blood flow in bilateral putamen. She was treated with corticosteroid and aspirin after which the symptoms improved. Four months later, follow up F-18 FDG PET showed improvement with resolution of hypermetabolism in bilateral putamen. This case suggests that striatal hypermetabolism is associated with chorea in SLE.

<P><B>Abstract</B></P> <P>In this study, we describe a method to predict 6-axis ground reaction forces based solely on plantar pressure (PP) data obtained from insole type measurement devices free of space limitations. Because only vertical force is calculable from PP data, a wavelet neural network derived from a non-linear mapping function was used to obtain 3-axis ground reaction force in medial-lateral (GRF<SUB>ML</SUB>), anterior-posterior (GRF<SUB>AP</SUB>) and vertical (GRF<SUB>V</SUB>) and 3-axis ground reaction moment in sagittal (GRF<SUB>S</SUB>), frontal (GRF<SUB>F</SUB>) and transverse (GRF<SUB>T</SUB>) data for the remaining axes and planes. As the prediction performance of nonlinear models depends strongly on input variables, in this study, three input variables – accumulated PP with respect to time, center of pressure (COP) pattern, and measurements of the opposite foot, which are calculable only with a PP device – were considered in order to improve prediction performance. To conduct this study, the golf swing motions of 80 subjects were characterized as unilateral movement and GRF patterns as functions of individual characteristics. The prediction model was verified with 5-fold cross-validation utilizing the measured values of two force plates. As a result, prediction model (correlation coefficient, <I>r=</I>0.73–0.97) utilized accumulated PP and PP data of the opposite foot and showed the highest prediction accuracy in left-foot GRF<SUB>V</SUB>, GRM<SUB>F</SUB>, GRM<SUB>T</SUB> and right-foot GRF<SUB>AP</SUB>, GRF<SUB>ML</SUB>, GRM<SUB>F</SUB>, GRM<SUB>T</SUB>. Likewise, another prediction model (<I>r</I>=0.83–0.98) utilized accumulated PP and COP patterns as input and showed the best accuracy in left-foot GRF<SUB>AP</SUB>, GRF<SUB>ML</SUB>, GRM<SUB>S</SUB> and right-foot GRF<SUB>V</SUB>, GRM<SUB>S</SUB>. New methods based on the findings of the present study are expected to help resolve problems such as spatial limitation and limited analyzable motions in existing GRF measurement processes.</P>

<P><B>Abstract</B></P> <P>Functionalized polymer filter (FPF) was fabricated using a synthesized terpolymer containing a quaternary ammonium moiety via electrospinning method to enhance the durability of air filters. Removal efficiency and durability of FPF were exhibited compared with those of polystyrene (PS) filter and a commercial electret filter. FPF showed the best performance than others, and the removal efficiencies were 89.5, 98.6, and 99.8% when the sodium chloride aerosol were sized in 100, 200, and 300 nm, respectively. In addition, FPF had almost no decrease in the removal efficiency after the treatment by isopropyl alcohol (IPA). On the other hand, the IPA-treated PS and electret filter showed the rapid decrease around 20–40% for the sodium chloride aerosol. The performance of FPF was also kept even after the water treatment. Furthermore, FPF also showed good antibacterial activity, a desirable characteristic for using the air filter without infections.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Functionalized polymer filter was fabricated for removal of ultrafine aerosol. </LI> <LI> Hydrophilicity and high dipole moment from the quaternary ammonium moiety were helpful to capture the MPPS. </LI> <LI> Durability of the filter was improved owing to the inherent characteristics of the functionalized terpolymer. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>
저장탱크 화재⋅폭발사고 사례연구 및 안전 확보방안에 대한 고찰

최근 화학물질 취급 사업장 등에서 화재⋅폭발사고가 빈번하게 발생하고 있다. 다량의 위험물을 취급하고 여러 공정⋅설비 등이 밀집되어있는 사업장에서 화재⋅폭발사고가 발생할 경우, 수많은 인명피해를 초래할 뿐만 아니라 인근 설비 등의 손상 또는 2차 사고의 위험이 있으므로 대형 사고로 이어질 가능성이 매우 크다. 그러므로 체계적인 안전조치를 통해 사고를 예방할 필요가 있으며, 이를 위해서는 과거 사례로부터 위험요인을 파악하여 이를 기반으로 안전대책을 수립하는 것이 필요하다. 이에 2019년부터 2022년 5월까지 사업장에서 사상자가 발생한 화재⋅폭발사고 30여 건을 분석한 결과, 주로 정비⋅보수작업과 같은 비일상 작업에서 사고가 잦았으며, 사고 설비로는 저장탱크에서 가장 많은 사고가 발생하였다. 특히, 인화성 물질 잔존 상태 또는 퍼지 미실시로 유증기 등이 잔류된 상태에서 화기작업 실시 등으로 화재⋅폭발사고가 빈번하게 발생되고 있는 것으로 나타났다. 이처럼 동일한 유형의 사고가 지속적으로 발생하고 있기 때문에, 재발방지를 위해 사고의 근본원인을 파악하는 것이 매우 중요하다. 이에 본 연구에서는 주 사고유형인 비일상작업 중 저장탱크 화재⋅폭발 사고사례를 분석하여 사고의 근본원인과 위험요인을 도출하였으며, 이를 기반으로 동종⋅유사사고 재발방지를 위한 안전 확보방안에 대해 고찰하였다.

<P><B>Background</B></P><P>Radiographic progression is reported to be highly heritable in rheumatoid arthritis (RA). However, previous study using genetic loci showed an insufficient accuracy of prediction for radiographic progression. The aim of this study is to identify a biologically relevant prediction model of radiographic progression in patients with RA using a genome-wide association study (GWAS) combined with bioinformatics analysis.</P><P><B>Methods</B></P><P>We obtained genome-wide single nucleotide polymorphism (SNP) data for 374 Korean patients with RA using Illumina HumanOmni2.5Exome-8 arrays. Radiographic progression was measured using the yearly Sharp/van der Heijde modified score rate, and categorized in no or severe progression. Significant SNPs for severe radiographic progression from GWAS were mapped on the functional genes and reprioritized by post-GWAS analysis. For robust prediction of radiographic progression, tenfold cross-validation using a support vector machine (SVM) classifier was conducted. Accuracy was used for selection of optimal SNPs set in the Hanyang Bae RA cohort. The performance of our final model was compared with that of other models based on GWAS results and SPOT (one of the post-GWAS analyses) using receiver operating characteristic (ROC) curves. The reliability of our model was confirmed using GWAS data of Caucasian patients with RA.</P><P><B>Results</B></P><P>A total of 36,091 significant SNPs with a <I>p</I> value <0.05 from GWAS were reprioritized using post-GWAS analysis and approximately 2700 were identified as SNPs related to RA biological features. The best average accuracy of ten groups was 0.6015 with 85 SNPs, and this increased to 0.7481 when combined with clinical information. In comparisons of the performance of the model, the 0.7872 area under the curve (AUC) in our model was superior to that obtained with GWAS (AUC 0.6586, <I>p</I> value 8.97 × 10<SUP>-5</SUP>) or SPOT (AUC 0.7449, <I>p</I> value 0.0423). Our model strategy also showed superior prediction accuracy in Caucasian patients with RA compared with GWAS (<I>p</I> value 0.0049) and SPOT (<I>p</I> value 0.0151).</P><P><B>Conclusions</B></P><P>Using various biological functions of SNPs and repeated machine learning, our model could predict severe radiographic progression relevantly and robustly in patients with RA compared with models using only GWAS results or other post-GWAS tools.</P><P><B>Electronic supplementary material</B></P><P>The online version of this article (doi:10.1186/s13075-017-1414-x) contains supplementary material, which is available to authorized users.</P>
Dermatofibroma occurred on finger: unusual location

Even though dermatofibroma (benign fibrous histiocytoma) is a common skin tumor presented as a solitary nodule on the extremities, shoulders or buttock of young adults, it is rarely seen on fingers. We report the first Korean case of dermatofibroma on finger. A 32-year-old man presented with a tender, solitary, about 0.5cm sized, firm skin colored nodule on the dorsum side of left index finger. The patient thought that it was an insect bite; however, the lesion continued for 6 months without any symptoms. There was no traumatic history. Our first impressions were picker’s nodule, spiradenoma, or leiomyoma. The skin biopsy revealed a poorly circumscribed proliferative dermal tumor. It is largely composed of interlacing fascicles of slender spindled cells, sometimes in a focal whorling arrangement. He was diagnosed with dermatofibroma on finger and the lesion was completely excised. Awareness of unusual clinical demonstration of dermatofibroma would help to avoid misdiagnosis and make early decision.
Pebbly-appearing papules of the multiple digits

Finger pebbles are known as the thickening of the skin on the dorsum of the fingers. This certain cutaneous manifestation could be an early marker of thick skin syndrome and a self-limiting marker of diabetic disease. Finger pebbles usually described as fine papillary structures over the dorsum of the interphalangeal and metacarpophalangeal joints, usually associated with diabetes mellitus. Herein, we report an interesting case of pebbly-appearing papules of the multiple digits without diabetes mellitus in Korea. A 15-year-old obese boy presented for evaluation of hyperkeratotic plaque on the dorsa side of both proximal and distal interphalangeal joint on right hand and foot. Closer view of plaque revealed diffuse pebbling appearance forming round shaped plaques on each joints. He had no complaint of pruritus or other symptoms. Fasting venous plasma glucose concentration and levels of hemoglobin A1c were 107mg/dL and 5.4% respectively, which were within the normal limits. The histologic findings revealed the epidermal hyperkeratosis and hypertrophy of the papillary dermis which was similar to that of chronically rubbed skin except for the absence of a markedly thickened stratum corneum. He was diagnosed with finger pebbles, and recommended the serum glucose level checkup periodically.
