        시각적 쾌적성과 에너지 성능분석에 의한 오피스 창호의 적정 투과율 선정

        At present, with the development of using glass in building, visual amenity and energy saving were more concerned. However, it is found at glass performance evaluation that there is more consideration on energy saving than occupant visual amenity. Therefore, this aim of study is to select optimum and minimum transmittance by analysis visual amenity and energy performance used in office glass. The process of the study is as follows: 1)selected 6 transmittances as evaluated value from current film maker in office building window and analysis on construction case. 2) set up Mock-up model, then select 17 evaluated values.3)analyzed cooling-heating load, light energy, daylight glare of office building with the different transmittance change. 4) selected proper transmittance of office building through subjective evaluation of Mock-up experiment, energy performance and glare analysis. As a result of subjective evaluation, the comfort transmittance is from 40% to 60% . cooling-heating load is about 50%, and on glare evaluation the proper transmittance is from 20% to 40%.

        오피스 창호의 適定 및 最少 透過率 선정에 관한 연구

        The purpose of this study is to select the optimal and minimum transmittance for visual comfort in office buildings. This study progressed as follows. The first, we selected 5 films with various transmittance based on the current film makers whose films being used on the windows of office building nowadays and the case study of construction. And then we choose 6 kinds of transmittance as evaluation object including plain glass. The second, we made a mock-up models having different transmittance on the southern windows with model of real scale office building. The third, we selected 17 pairs of adjectives for the evaluation of visual comfort on interior or exterior conditions with transmittance factor. The fourth, subjective evaluation experiment was done using selected evaluation adjectives and the result was analyzed. The results are as follows ; 1) As a result of the factor analysis, we extracted two factor, [cleanㆍvarious] and [amenityㆍappropriateness] image, from the 16 evaluation items. 2) As a result of the multi regression analysis, it is concluded that the transmittance of the windows is the high effective physical item to two factor, [cleanㆍvarious] and [amenityㆍappropriateness], in simultaneous. 3) As a result of the average score analysis of evaluation item by transmittance, the first factor, [cleanㆍvarious], get a more points as the transmittance is increased. And the second factor, [amenityㆍappropriateness], get a most high score at 50% transmittance and decreased with higher transmittance than 50%. 4) Finally, the optimal transmittance range is concluded in 40%~60% and the minimum transmittance appropriate for the office building is 30%~60%.

        비타민 D와 만성 질환과의 관련성

        Background: Vitamin D deficiency is prevalent worldwide, not only in patients with osteoporosis but also in normal adults. Because the situation is especially severe in Korea. So, we take a look at the degree of vitamin D deficiency in Korea and the relationship between chronic diseases which are common in primary care and vitamin D level. Methods: 200 patients above the age of 20, who were admitted to the Department of Family Medicine at Soonchunhyang University from June of 2009 to May of 2010 were chosen. Questionnaires regarding the patients' drug history and afflicted disease and the average serum 25-hydroxyvitamin D, 1,25-dihydroxyvitamin D levels were obtained and we looked into their relationships to the diagnosed chronic disease or possessed chronic disease such as like diabetes, hypertension, osteoporosis, hyperlipidemia. Results: In our study a serum 25(OH)D level lower than 80 nmol/dL (32 ng/mL) was considered vit D deficient. 127 patients (63.5%) were deficient in vit D and 126 patients (73%) were beyond adequacy. The serum 25(OH)D showed a significant positive relationship with total cholesterol (P=0.01), triglyceride (P=0.049), High density lipoprotein-cholesterol (P=0.001), and Low density lipoprotein-cholesterol (P=0.007). The serum 1,25(OH)₂D₃ showed a significant negative relationship with Triglyceride (0.018). The serum 25(OH)D level was not shown any significant effect by Fasting plasma glucose, HbA1c, Total cholesterol, Triglyceride, High density lipoprotein, Low density lipoprotein. The serum 25(OH)D showed no significant relationship with the presence of existing diseases as diabetes, hypertension, hyperlipidemia, osteoporosis or newly diagnosed as same above at admitted. also, the serum 1,25 (OH)₂D₃ showed a significant negative relationship with the presence of existing diseases as diabetes, hypertension, hyperlipidemia, osteoporosis or newly diagnosed as same above at admitted. Preexisting treatment history or newly diagnosed at admission of diabetes (P=0.003) and hypertension (P=0.009) showed a significant inversely proportional correlation to the serum 1,25 (OH)₂D₃ level and of hyperlipidemia showed a significant directly proportional relationship to the serum 1,25 (OH)₂D₃ level (P=0.008). Conclusion: About a two-thirds of those surveyed is found to have vitamin D deficiency. Vitamin D, in addition to its role in bone metabolism, seems to have an effect on the genesis of hypertension and diabetes. So, it is considered that a further prospective study on the clinical usefulness of vitamin D will be needed for chronic diseases increases due to the increase of the elderly population. 연구배경: 본 연구는 전 세계적으로 비타민 D 영양 상태가 골다공증 환자뿐만 아니라 정상 성인에서도 불충분하고, 우리나라의 비타민 D 불충분은 특히 심하다고 알려져 있다. 이 연구는 우리나라의 비타민 D 불충분의 정도와 일차의료 현장에서 접할 수 있는 만성 질환과 비타민 D 혈중 수준과의 연관성을 밝히고자 하였다. 방법: 2009년 6월부터 2010년 5월까지 순천향대학교병원 가정의학과에 입원한 20세 이상의 200명의 환자들을 대상으로 연구를 시행하였다. 입원 환자들에게 설문지를 통해 과거 병력 및 약물복용력 등을 조사하였고, 혈청 25-hydroxyvitamin D, 1,25-dihydroxyvitamin D 등을 측정하여 평균을 구하였으며 과거 병력 중 당뇨병, 고혈압, 골다공증, 고지혈증의 만성 질환이 있거나 만성 질환을 진단받은 경우와 비타민 D의 관련성을 알아보았다. 결과: 본 연구에서 혈청 25(OH)D 농도가 80 nmol/dL (32 ng/mL) 이하일 때 비타민 D 불충분으로 봤을 경우 200명의 입원 환자 중 63.5%인 127명이 비타민 D 불충분으로 나타났고, 적정도(Adequacy)에 미치지 못하는 경우는 126명(73%)인 것으로 나타났다. 혈청25(OH)D의 경우 총콜레스테롤(P=0.01), 중성지방(P=0.049), 고밀도콜레스테롤(P=0.001), 저밀도콜레스테로(P=0.007)와 유의미한 양의 상관관계를 보이는 것으로 나타났다. 또한 1,25(OH)₂D₃와 중성지방은 유의미한 음의 상관관계를 보이는 것으로 나타났음을 알 수 있다(P=0.018). 공복 혈당(Fasting plasma glucose), 당화혈색소(HbA1c), 총 콜레스테롤(Total cholesterol), 중성지방(Triglyceride), 고밀도 콜레스테롤(High density lipoprotein cholesterol), 저밀도 콜레스테롤(Low density lipoprotein cholesterol)은 혈청 25(OH)D 농도에 유의한 영향을 미치지 못하는 것으로 나타났다. 혈청 25(OH)D의 경우 만성 질환인 당뇨병, 고혈압, 고지혈증, 골다공증의 과거 병력이 있거나 입원 시 진단 받은 경우에 있어서 유의미한 상관관계를 보이지 않는 것으로 나타났다. 또한 1,25(OH)₂D₃의 경우 당뇨병, 고혈압의 과거력이 있거나 입원 시 진단 받은 경우와 유의미한 음의 상관관계를 보이는 것으로 나타났음을 알 수 있다. 당뇨병, 고혈압, 고지혈증, 골다공증은 혈청 25(OH)D에 의미있는 영향을 미치지 않는 것으로 나타났다. 당뇨병(P=0.003), 고혈압(P=0.009)으로 치료를 받았거나 새로 진단 받은 경우 1,25(OH)₂D₃ 농도에 반비례의 유의한 영향을 미치는 것으로 나타났고, 고지혈증(P=0.008)의 경우에서는 1,25(OH)₂D₃ 농도에 비례하게 유의한 영향을 미치는 것으로 나타났음을 알 수 있다. 결론: 조사대상자의 약 2/3에서 비타민 D 부족이 있는 것으로 나타나고 있다. 비타민 D는 골대사와 관련된 역할 이외에 고혈압과 당뇨 발생에 있어서도 영향을 미치는 것으로 보이며, 향후 노인 인구증가에 따른 만성 질환의 증가와 함께 비타민 D 사용의 임상적인 유용성에 대한 전향적인 연구가 필요할 것으로 판단된다.

        Advanced Lipid Extraction Method for the Determination of the Phospholipase D Activity

        Phospholipase D is a ubiquitous enzyme that plays an important role in various lipid mediated cellular signaling pathways and produces rare phospholipids, phosphatidylethanol or phosphatidylbutanol, instead of phosphatidic acid with unique catalytic activity transphosphatidylation in the presence of primary alcohols. The reaction products, phosphatidylethanol or phosphatidylbutanol are used as markers of in vitro phospholipase D activity in many studies. For the sensitive detection of the phospholipase D products, we developed an advanced lipid extraction method that facilitates recovery of the compounds. With the new method, the activity change of phosaholipase D by agonists could be detected more easily and the recovery rate was also increased. The increase of detected enzyme activity change was about double fold compared to the conventional lipid extraction method. This method provides selective force for the phospholipase D products in the extraction procedure.

      • 면역적격 환자에서 발생한 폐 크립토코쿠스증 5예

        폐 크립토코쿠스증은 최근 스테로이드 제제를 포함한 면역억제제의 사용으로 그 발생이 증가하고 있으며, 면역적격 환자들에게서 발생할 수 있다. 일부에서는 뇌수막염이나 패혈증과 같은 치명적인 전신감염을 초래할 수 있어, 이에 대한 진단 및 적절한 치료가 중요하다. 본 증례에서는 면역저하의 증거가 없는 환자들에서 다양한 폐질환의 양상으로 발병한 폐 크립토코쿠스증 5예를 경험하여 문헌고찰과 함께 보고하는 바이다. Cryptococcal infections occur frequently in patients with cell-mediated immune deficiency, such as acquired immune deficiency syndrome, organ transplants, hematologic malignancy patients and immunocompromised patients who receiving cancer chemotherapy or corticosteroid therapy. In these conditions, cryptococcosis can cause serious infections requiring systemic antifungal therapy such as meningoencephalitis and sepsis. However, pulmonary cryptococcosis in immunocompetent hosts may have self-limited course and improve without treatment. We experienced various pulmonary manifestations of pulmonary cryptococcosis in five immunocompetent patients. We diagnosed three cases of isolated pulmonary cryptococcosis and two cases of disseminated cryptococcosis involving lungs. All cases were treated with intravenous injection of amphotericin B during hostpital care, and maintained with oral fluconazole after discharge. During the follow-up, clinical symptoms and radiologic features were markedly improved.

        Parathyroid Carcinoma: Outcome Analysis of 16 Cases from a Single Center

        Purpose: Parathyroid carcinoma is an extremely rare malignancy, but there is still a lack of understanding of the natural course and prognostic implications of this disease. In this study, we reviewed the clinicopathological characteristics of patients with parathyroid carcinoma treated at our institution. Methods: Patients with parathyroid carcinoma were included in this study. We reviewed the clinicopathological characteristics of patients with parathyroid carcinoma treated at our institution, and evaluated patient outcomes and prognostic factors. Results: A total of 16 patients with parathyroid carcinoma were included in this study. The median follow-up duration was 61.5 months (range, 7–215 months), and seven patients developed recurrence (7/16, 43.8%). Four patients had died from parathyroid carcinoma (4/16, 25.0%) at the time of the analysis. Increased serum intact parathyroid hormone (iPTH) level during follow-up was an independent risk factor for recurrence after making adjustments for multivariate analysis. Trends of increasing serum calcium and iPTH levels after surgery were associated with the development of recurrence. Conclusion: We observed a good survival rate after initial surgery for parathyroid carcinoma. However, the rate of recurrence was relatively high and early detection of recurrence is critical. Close monitoring during follow-up is necessary, and a serial check for serum calcium and iPTH level may prove useful. When steadily increasing serum calcium and iPTH results are observed, careful evaluation for recurrence is needed.

      • Hippocalcin increases phospholipase D2 expression through extracellular signal-regulated kinase activation and lysophosphatidic acid potentiates the hippocalcin-induced phospholipase D2 expression

        <P>We have previously isolated a 22 kDa protein from a rat brain which was found to be involved in activating phospholipsae D (PLD), and identified the protein as hippocalcin through sequence analysis. Nevertheless, the function of hippocalcin for PLD activation still remains to be resolved. Here, we proposed that hippocalcin was involved in extracellular signal-regulated kinase (ERK)-mediated PLD2 expression. To elucidate a role of hippocalcin, we made hippocalcin transfected NIH3T3 cells and showed that the expression of PLD2 and basal PLD activity were increased in hippocalcin transfected cells. We performed PLD assay with dominant negative PLD2 (DN-PLD2) and hippocalcin co-transfected cells. DN-PLD2 suppressed increase of basal PLD activity in hippocalcin transfected cells, suggesting that increased basal PLD activity is due to PLD2 over-expression. Hippocalcin is a Ca<SUP>2+</SUP>-binding protein, which is expressed mainly in the hippocampus. Since it is known that lysophosphatidic acid (LPA) increases intracellular Ca<SUP>2+</SUP>, we investigated the possible role of hippocalcin in the LPA-induced elevation of intracellular Ca<SUP>2+</SUP>. When the intracellular Ca<SUP>2+</SUP> level was increased by LPA, hippocalcin was translocated to the membrane after LPA treatment in hippocalcin transfected cells. In addition, treatment with LPA in hippocalcin transfected cells markedly potentiated PLD2 expression and showed morphological changes of cell shape suggesting that increased PLD2 expression acts as one of the major factors to cause change of cell shape by making altered membrane lipid composition. Hippocalcin-induced PLD2 expression potentiated by LPA in hippocalcin transfected cells was inhibited by a PI-PLC inhibitor, U73122 and a chelator of intracellular Ca<SUP>2+</SUP>, BAPTA–AM suggesting that activation of hippocalcin caused by increased intracellular Ca<SUP>2+</SUP> is important to induce over-expression of PLD2. However, downregulation of PKC and treatment of a chelator of extracellular Ca<SUP>2+</SUP>, EGTA had little or no effect on the inhibition of hippocalcin-induced PLD2 expression potentiated by LPA in the hippocalcin transfected cells. Interestingly, when we over-express hippocalcin, ERK was activated, and treatment with LPA in hippocalcin transfected cells significantly potentiated ERK activation. Specific inhibition of ERK dramatically abolished hippocalcin-induced PLD2 expression. Taken together, these results suggest for the first time that hippocalcin can induce PLD2 expression and LPA potentiates hippocalcin-induced PLD2 expression, which is mediated by ERK activation. J.
