        Audiologic Results of Ossiculoplasty Using Malleus Footplate Assembly: The Comparison between Autologous Incus and Hydroxyapatite

        Introduction A disruption of the ossicular chain of the middle ear causes conductive hearing loss. Ossicular reconstruction is performed to restore hearings in those patients with congenital ossicular chain anomaly or ossicular chain disruption caused by chronic otitis media or cholesteatoma. A study from Japan with 52 auditory ossicular malformation revealed that 44% of the patients had a fixation of the stapes, 40% had incudostapedial joint defects followed by fixation of the malleus and/or incus and defects in the incudostapedial joint with a fixation of the stapes.1) One of the most common ossicular chain anomalies in Korea was a stapes footplate fixation (54.5%) followed by a mobile stapes footplate with other anomalies (19.7%) including a stapes without suprastructure and an incus long process, stapes without anterior crus and an incus long process.2) One of the most common ossicular chain anomalies is the absence of an incus with or without an intact stapes suprastructure. In incudostapedial deformed ears, ossiculoplasty is mandatory to restore the hearing process. Ossiculoplasty is also needed in cases with incus long process erosion with inflammation. As the most vulnerable ossicles, one report said that incus erosion is found in 80% of the chronic otitis media.3) Austin-Kartush classified ossicular chain disruptions into four categories after 1,151 operation on chronic otitis media. Group A ears with an intact malleus handle and a stapes suprastructure was found in 59.2% followed by group B ears with an intact malleus handle but an eroded stapes suprastructure group.4) In the group of a tympanic membrane and malleus preserved but a stapes suprastructure eroded, ossicular continuity can be restored between the stapes footplate and manubrium of the malleus with the method of malleus footplate assembly (MFA) using either the incus body or if the incus is unavailable, a biocompatible material such as hydroxyapatite (HA).5,6,7) Until now, MFA has proven to be the safe and effective way to restore hearings in such patients.8,9,10,11,12)Though successful results using MFA with either incus body or biocompatible material are reported world widely, there were a few reports on audiologic results after MFA ossiculoplasty in Korea. Authors reviewed our procedure and report hearing results of ossiculoplasty with MFA using either incus or HA. In this paper, a single-surgeon series of 32 MFA operations was analyzed. The cases were limited to autologous interposition grafts performed on Austin-Kartush group B ears in order to provide consistency within the analysis. Subjects and Methods Data collection A retrospective evaluation has been carried out on patients who underwent MFA ossiculoplasty between September 2001 and March 2010 in Seoul National University Hospital. A total of 55 patients with Austin-Kartush type B defect repaired with incus body or HA were identified. Of those, 20 were excluded due to a failure to recover operation records or a lack of no more than 1 year postoperative audiograms. Three patients who had undergone MFA with a homologous incus were excluded in order to provide consistency within the analysis. Finally, 32 cases were enrolled into the study. All procedures were conducted by a single surgeon. All data including previous operations, follow-up duration, age, sex, etiology, materials which were used in the operation, combined mastoidectomy, complications, and revision operations were analyzed. Eustachian tube function was analyzed according to the results of the Eustachian tube test. The status of the middle ear mucosa was described as either 'healthy' or 'inflamed' according to the operation records. Demographic data are presented in Table 1. Analysis of audiologic data Preoperative and postoperative hearing results were evaluated according to the guidelines of the Committee on Hearing and Equilibrium of the American Academy of Otolaryngology-Head Neck Surgery Foundation13) an... Background and Objectives: Absence of incus is one of the most common ossicular anomalies and also frequently found in chronic otitis media or chornic otitis media with cholesteatoma. Ossiculoplasty with malleus footplate assembly (MFA) can be an excellent option in the above situations. We reviewed our procedures and evaluated the hearing results of ossiculoplasty using the MFA. Subjects and Methods: This study retrospectively reviews a series of 32 patients who underwent MFA ossiculoplasty between September 2001 and March 2010 in an academic tertiary referral center. The autologous incus or hydroxyapatite (HA) was used as materials for reconstruction. Results: Thirteen of 32 patients (40.6%) had ossicular chain anomaly, 9 patients (28.1%) had congenital cholesteatoma and 8 patients (25.0%) had chronic otitis media with cholesteatoma and 2 patients (6.3%) had chronic otitis media. The patients with ossicular chain anomaly showed the greatest air-bone gap (ABG) closure. Overall, postoperative ABG was 17.7±11.7 dB at postoperative 6 months and 19.7±14.3 dB at postoperative 1 year, compared with preoperative ABG of 37.9±9.3 dB. There was no statistical difference of ABG at postoperative 12 months, 24 months between MFA using incus (n=17) and HA (n=15)(p=0.300, and p=0.563). There was no meaningful difference of postoperative ABG between the patients with combined mastoidectomy (n=10) and those without mastoidecmoy (n=22)(p=0.555). No extrusions occurred.

      • 전두유 생산을 위한 비지의 재활용 기술개발

        본 연구는 두유 제조 시 발생하는 부산물 인 비지를 활용한 전두유를 제조하기 위한 최적 효소처리 조건 개발을 목적으로 콩 전체를 이용한 전두유를 생산하기 위해서 비 지와 두유를 분리한 후 일정량 가수하여 효 소반응을 진행하였으며 10% 고형분의 효소 반응 비지유 페이스트를 제조하였다. 비지의 최적 효소반응 조건은 반응시간 60분(평균 입도 47㎛ ; 점도 1,130cp), 반응온도 50℃ (평균입도 46㎛), 효소사용량 0.2%(평균입 도 47㎛)였다. 호소반응 비지유와 두유를 혼합하여 멸균, 균질. 포장이 완료된 전두유 의 입도분석 결과는 1~150㎛ 범위. 평균 46㎛ 였다. 전두유는 고형분 10%, 점도 86cp. 단백질 함량 4.2g%로 대두분말 전두유, 일반두유보 다 영양학적, 물성적 품질 측면에서 건강음 료로 우수하였으며 색, 맛, 구매의사 등 관 능학적 측면에서도 우수하여 두유 제조 부 산물인 비지의 재활용은 매우 바람직하다고 판단된다. This study was carried out to develop the recycling technology for the production of whole soymilk from soywaste. Enzymic hydrolysis was conducted to produce whole soymilk containing 10% solid paste from whole soybean. Optimum condition of enzymic reaction for the production of soymilk was 60 min. of reaction time(average particle size 47um ; viscosity 1,130cp), 50'C of reaction temp. (average particle size 46rm), 0.2% of enzyme utilization (average particle size 47rm). The range of particle sizes of the mixture of enzyme reacted soywaste and soymilk were 1~150um and the average was 46um. The whole soymilk which have 10% solid, 86cp viscosity, 4.2% of protein was superior beverage to the general soymilk at the view points of nutritional, physicochemical and sensory characteristics.

        건조조건에 따른 꽁치과메기의 콜레스테롤 함량 변화

        꽁치 가공식품 개발 가능성을 타진하기 위해 경북 동해안 일대의 전통·향토식품인 과메기를 자연건조와 인공건조로 각각 15일 건조하여 콜레스테롤 함량변화에 따른 과메기의 품질변화 건조효과를 조사하였다. 그 결과, 생체 꽁치의 콜레스테롤 함량은 건물 시료당 56.0mg%이었으며, 자연건조 3일째에 급격히 감소(생체 대비 10%)하였고, 그 후 큰 변화가 거의 없었다. 인공건조는 서서히 감소(3.0%→6.6%)하였다. New drying method was tested for the quality control for Kwamegi, dry Pacific saury(Cololahis saira) in east coast, area of Kyungbuk province, Korea, Cholesterol content of raw fish was 56 mg% on dry basis, and decreased to 50.82 mg% rapidly the first 3 day and then, it was almost unchanged. However, the cholesterol content decreased slowly to 52.3 mg% during 15 days in new artificial drying.

      • 랫드에서 l-muscone의 급성독성 및 아급성독성시험 연구

        l-Muscone is synthesized for use as substitutive material of musk which is the active ingredient of woohwangchungsimwon. The objective of this investigation was to evaluate the acute and subacute toxicity of l-muscone in rats. In oral acute toxicity test, SPF Sprague-Dawley male and female rats were gavaged with l-muscone of two doses(0, 5.0g/㎏). No dead animal and abnormal autopsy findings were found in control and treated group. Body weights were slightly decreased in both sexes of rats treated with 5.0g/㎏. Therefore, oral LD_50 of l-muscone was consider to be higher than 5.0g/㎏ in male and female rats. In intraperitoneal acute toxicity test, rats were injected intraperitoneally with dosages of 0, 1,000, 1,316, 1,732, 2,279 and 3,000 ㎎/㎏. Decreased body weights and motor activities were observed at high dose group. Intraperitoneal LD_50 of l-muscone were 1,920 ㎎/㎏ in male and female rats. In the subacute study. l-muscone was administrated orally to both sexes of rats for 4 weeks as several doses(0, 10, 100 and 1,000 ㎎/㎏). There were neither dead animals nor significant changes of body weights during the experimental period. In addition, no differences were found between control and treated groups in clinical signs, urinalysis, hematology, serum biochemical analysis, and other findings. Above data suggest that no observed adverse effect level of l-muscone in rats might be over 1,000 ㎎/㎏/day in this study.

        Abstract - In this paper, we propose a bidirectional mobile Ad Hoc routing protocol based on AODV(ad-hoc on-demand distance victor routing) with QoS(quality of service) support. At presence, wireless mobile communication focuses on how to efficiently support mobility of users more than QoS guarantee. However, in order to satisfy requirement of various applications which have been or will be served, QoS guarantee between end point is a very important issue. The existing AODV routing protocol is implemented to use only BE (Best Effort) service. However, actual wireless mobile environment requires to reduce a waste of communication resources and meet with the real-time change of data paths by setting up routes with QoS support, considering available communication resources at each node in advance. Therefore, in this thesis, we propose an adaptive QoS support method using hello message in order to rapidly meet with the change of available communication resources and to smoothly perform route maintenance. For performance evaluation, we analyzed the average date reception rate. At that time, we used evaluation parameters such as node's mobility -stop time, hello message period, and packet priority, considering the node's mobility and the number of best effort/QoS data flows. As results, we found that with our proposed method the average data reception rate was increased 16.3% for 0 mobility-stop second case and 4.7% for 300 mobility-stop second case.

      • 光州地方의 酸性降雨에 관한 硏究

        In this report, for Kwangju I shall discuss the relationship between precipitation and the pH. I shall look at the air pollutant and the pH. Data from this study for precipitation and the air pollutant were observed by KMA and YEA. The precipitation and the air pollutant were observed and recorded every hour from 1991 to 1995. Results : Annual mean pH was 5.6. The pH for November and the pH between January and May were 5.6 or less. The rate of acid rainy day in annual mean precipitation days was 48.1%. The occurrence of acid rain was more than 50%. The data shows that half of all rain in a year, was acid rain. The mean pH for March was 5.4 and there were 18 times acid rain occurred among 26 rainy times. This shows that rate of acid rain reaches 69%, it is a serious problem. The acidity is highest in the spring time and then followed in order by fall, winter and summer. The mean pH of precipitation in spring is 5.5, it means low level of acid rain. Daily variation of the pH doesn't have any special meaning. The pH in the afternoon is generally low and the highest pH level of acid rain was 4, which was observed between 19:00 and 21:00 on the 10th of March, 1991. The pH interannual variation of rain had a tendency to be high in 1992. But in 1995, the amplitude of the pH was large, so we can expect a high pH level of acid rain. As precipitation amount grews, the pH of precipitation tended to be low. Although, there is a little corelation(r=0.106) between frequency of acid rain and amount of rain. The lasting of rain for long periods raised the frequency of acid rain and standardized the pH. As the temperature goes up, consistency of pollutant also went up. But it is more reasonable to assume that consistency was raised by human activity. The temperature effect on the acidity of rain is minimal. NO_2 (r=0.205) is the primary effector on the acidity of rain, and then fellowed in order by SO_2 (r=0.118), TSP (r=0.019). However in fact, TSP has little influence on the acidity of rain.

      • 랫드에서 신우황청심원의 급성 및 아급성독성시험

        The acute and subacute toxicity of New Woohwangchungsimwon(NWCH) which was used l-muscone as substitutive material of musk were investigated in S.D. rats. In intraperitoneal acute toxicity test, rats(Sprague-Dawley, SPF) were injected intraperitoneally with dosages of 0, 540, 750, 1,070, 1,500 and 3,000 ㎎/㎏. Body weights were significantly decreased at 540 ㎎/㎏ dose group in both sexes and abnormal autopsy findings were founded in both sexes at all treated groups. Intraperitoneal LD_50 of NWCH was 812.3 ㎎/㎏ in male and 872.3 ㎎/㎏ in female rats. In the subacute toxicity study, NWCH was administrated orally to both sexes of rats for 4 weeks as several doses(0, 320, 800 and 2,000 ㎎/㎏). There were neither dead animals nor significant changes of body weights during the experimental period. In addition, no differences were found between control and treated groups in clinical signs, urinalysis, hematology, serum biochemical analysis, and other findings. Above data strongly suggest that no observed adverse effect level of NWCH might be over 2,000 ㎎/㎏/day in this study.
