P057 : Acral lentiginous melanoma arsing from small and medium sized congenital melanocytic nevi

Background: Congenital melanocytic nevi (CMN) are classified as small, medium, and giant according to their size. Giant CMN are well known risk factors for the development of melanoma. But, the risk of melanoma in small and medium sized CMN is controversial and there is few study for evaluation of melanoma risk in patient with small and medium sized CMN in Asian patients. Objectives: We investigate to evaluate a relationship between small and medium sized CMN and acral lentiginous melanoma (ALM). Methods: A total of 73 patients with histologically confirmed ALM who were received surgical treatment and follow up more than 1year were included. We divided them into 2 groups according to arising or not from the CMN (CMN group = 18, Non-CMN group = 55) and compare the both groups in clinicopathological features. Also, clinicopathological feature was compared in each subgroup of CMN group according to size. Results: The proportion of small or medium sized CMN in the patients with ALM (24.7%) is higher than those in previous studies (7.9-15%). CMN group showed earlier onset age (47.89±10.67) than those of non-CMN group (59.40±14.89) (p=0.034). There is no different other clinicopathologic parameters between CMN group and Non-CMN group. In CMN group, more larger CMN tends to have larger primary tumor size (p=0.024) and deeper tumor depth (p=0.048). Conclusion: Our data shows that ALM arising from CMN occurs at earlier age and small and medium sized CMN have a higher malignant potential than we expect.
