        Benoist씨 장치를 이용한 소아의 하악 과두 골절의 치험례 : A CASE REPORT

        소아에서의 하악 골절은 성인에서보다 드물다. 그러나, 어릴수록 강직과 성장장애의 가능성이 크며 성인만큼 악간고정을 잘 견뎌내지 못한다. 반면에, 골절은 소아에 있어 더 빨리 치유되고 합병증도 적다. 소아에서 하악 골절은 종종 하악의 acrylic splint therapy 단독 또는 eyelet wire 와 악간 고정을 같이 사용하여 성공적으로 치료될 수 있다. 발생할 수 있는 심각한 합병증으로는 유착과 성장장애가 있다. 이러한 합병증의 빈도와 심 각성은 좀더 짧은 기간의 악간고정과 긴밀한 술 후 관리에 의해 감소될 수 있다. 특히 소아 환자가 악간고정을 잘 견디지 못하는 경우, 악간고정의 기간이 길어지는 경우, 환자의 자발적인 운동요법이 어려운 경우, 악간고정이 불필요하고 개구장애 만을 일으킨 미약한 증상의 경우에는 동기부여에 의한 물리적 하악 운동요법이 사용될 수 있다. 본 증례에서는 하악 정중부의 외상으로 인하여 양측성 과두 골절이 일어난6세 여자 환아를 대상으로 상,하악에 elastic을 이용하여 하악의 개구, 측방 전방운동에 대해 훈련을 가능하게 하는 Benoist씨 장치를 이용한 능동적 하악 운동 요법으로 7개월 간의 정기 검진을 통하여 하악 운동량의 증진, 과두 유착의 방지, 골개조의 치료효과를 보였다. Mandibular fracture is less common in children than in adults. However children are more susceptible to ankylosis and developmental disorders, and don't respond as well to intermaxillary fixation compared to adults. On the other hand, bone fracture is healed more quickly in children and complications are scarce. Mandibular fracture in children is usually treated successfully with acrylic splint therapy with or without the use of eyelet wires and intermaxillary fixation. Severe complications that include ankylosis and developmental disorders may occur. The frequency and severity of such complications can be mitigated with a shorter duration of intermaxillary fixation and good post-operative care. Encouraging mandibular physical therapy by increasing patient motivation may be necessary in such cases where the patient's response is poor and the duration of intermaxillary fixation increases; when the patient is unable to undergo physical therapy, or when intermaxillary fixation is not necessary with the patient showing only minor symptoms such as trismus. In this case report, a 6 year-old girl with bilateral condylar fracture was treated with elastic in both the upper and lower jaws to allow mandibular physical therapy using a Benoist's appliance, which allows opening, lateral, and protrusive retrusive movements of the mandible. A 7-month follow-up showed beneficial therapeutic effects such as increased mandibular movement and prevention of condylar ankylosis.

      • 천연두 백신 접종 후 발생된 급성 심근염 1예

        Acute myocarditis complicating coronary vasospasm after smallpox vaccination is very rare complication that can be generally developed within less than 30 days after vaccination. Clinically and biochemically, this disorder is often mimic and highly suggestive of acute myocardial infarction. We report 21 years old man that had a history of a smallpox vaccination 3 weeks ago and prodromal symptoms(fever, malaise etc) a few days ago. hIe visited to our hospital because of abrupt onset of severe substernal chest pain, dyspnea and diaphoresis. At admission, electrocardiography was ST elevation in lead Ⅰ, aVL, V4, V5, and V6. The CK-MB level was 68 U/L, LDH level was 760 U/L, Troponin-Ⅰ was 24.64 ng/㎖, and Troponin-T was positive. 2D-echocardiograpy was normal left ventricular function, Clinically we presumed this disorder as acute myocardial infarction and we promptly performed the coronary angiography and revealed focal spasm in distal left coronary artery territory without critical stenosis in both coronary arteries. We diagnosed this disorder as coronary vasospam complicating acute myocarditis that is different with acute myocardial infarction, and we prescribed nitroglycerin and nifedipine. After discharge, he is now well without any symptoms and has been following up monthly at our outward department for 6 months.

      • 盛土에 의한 橋臺의 側方移動에 대한 硏究

        This Study investigated a possibility of lateral movement in the reconstruction field of Nongso bridge at Nongso-ri, Joochon-myeon, Kimhae. The methods applied in this study were the judgement formula and the Finite Element Method(FEM). The conclusions are shown in the following. 1) The application of the FEM, as well as the judgement formula resulted in the lateral movement. 2) Bridge abutment and foundation must be constructed after completing consolidation. 3) The occurrence possibility of lateral movement should be judged by the FEM that can re-present field conditions. 4) Lateral movement will take place in the case. Hence, settlement of proper countermeasure method should be followed by the construction.

      • 원광 뇌졸중 등록 자료를 통한 허혈성 뇌졸중의 임상 양상과 위험인자

        Background: To investigate clinical features and risk factors of stroke patients who lived in Korean rural area, we analysis Wonkwang Stroke Registry and compared with other studies. Methods: We filled out Stroke Registry in the consecutive patients with acute ischemic stroke from march 1997 to February 1999. We analyzed demographic data, risk factors and subtypes of stroke. All patients were divied into 3 groups according to their age. Stroke subtype was classified into large artery atherosclerosis(LAA),small vessel occlusion(SVO), cardioembolism(CE), stroke of other determined etiology(SOE), and stroke of undetermined etiology(SUE) by TOAST(Trial of Org 10172 in Actue Stroke Treatment). Results: Five hundred nine patients were included in Wonkwang Stroke Registry from march 1997 to February 1999. Mean age was 65.47±0.48 years old. The ratio of males to females was 1 : 0.78. SVO was the most frequent subtype(49%) of ischemic stroke, and then followed by LAA(27.5%), CE(7%), SOE(8.1%), and SUE(8.4%). The internal carotid artery(ICA) territory was involved in 59.3% patients. Vertebrobasilar artery territory involvement was noted in 37.5%, and 3.2% of patients showed multiple artery territory involvement. All patients had risk factor of ischemic stroke except for 14 patients. Mean number of risk factors was 1.7/patient. Hypertension(62.1%) was the most frequent risk factor, and follwed by smoking(26.3%), diabetes mellitus(25.9%), and hyperlipidemia(25.55%). Conclusions: These results showed that the proportion of ischemic stroke subtype was not concordant with other studies. It may be caused by different mode of living and ethnic difference. This study may afford further information on the characteristics of Korean rural area stroke.

      • 잔류진동 억제를 위한 가상모드 입력성형기 개선

        Input shaping is an efficient tool to eliminate residual vibration caused by motion of mechanical systems. However, the presence of input shapers may increase the rise time of system. This paper is concerned with the rise time reduction when using input shaping. To this end, this paper proposes an input shaper design method for an undamped single mode vibration system using virtual modes, which are not actual mode but reflected in the design process. This paper proposes a new input shaper which has multiple impulses as if a single mode system has more than one modes: one actual mode and several virtual modes. The natural frequencies of the virtual modes are design parameters to change the rise time of the system. This paper discusses the characteristics of the 1 virtual mode and 2 virtual mode input shapers. The performance of the proposed input shapers is tested by experiments.

      • 스테이지 이동 반력에 의한 베이스 진동 저감기법 개발

        A variety of precise stages are adopted for precise machinery. To secure vibration isolation, those precise stages are mounted on a heavy base structure supported by compliant springs. Then the base structure is often subjected to vibration due to the reaction force induced by the stage motion. This paper investigates the dynamic behavior of base structures as well as a vibration reduction technique. A 6 DOF dynamic model is established to investigate the vibration of positioning stage and base structure. A new input shaper is devised to eliminate both the residual vibrations in base structure. Simulations show that the proposed input shaper effectively remove the residual vibrations in base structure.

      • 제주도 연안 초망 어선의 소음에 관한 연구

        제주도 연안에서 이루어지는 초망 어업에서 수중음을 이용하여 멸치 어군을 유효하게 집어하기 위한 기초 자료를 얻기 위하여 어선의 주기관을 공회전시켰을 때의 선상과 어군이 유집되어 투망되는 수심 4m지점과 함께 수심 10m층의 수중 소음을 측정 조사하였다. 이때 사용한 선박의 공회전은 전진시의 800 rpm과 후진시의 1000 rpm으로 그 결과는 다음과 같다. 1. 기관의 회전수 800 rpm에서 선수(B), 갑판(D), 선미(S), 연돌 위(F), 기관실 내(E)의 소음 준위는 각각 90, 93, 106, 107, 101 dB 이었으며, 기관 회전수 1,000 rpm에서의 소음 준위는 각각 93, 95, 110, 109, 103dB이었다. 2. 회전수를 800rpm으로 할 경우 100, 300, 500, 1000, 1500, 2000 Hz에서 선수의 소음 준위는 4m인 경우 각각 92, 95, 102, 97, 94, 92dB 이었으며, 수심 10m인 경우 87, 88, 99, 96, 94, 93 dB이었고, 선체 중앙에서는 4m인 경우 94, 91, 100, 96, 94, 92dB, 10m인 경우 93, 92, 99, 95, 93, 91dB이었다. 선미에서는 4m인 경우 98, 94, 100, 97, 94, 92 dB이었으며 10 m인 경우 92, 93, 99, 96, 93, 92 dB로 나타났다. 3. 후진시 사용하는 회전수 1000 rpm으로 할 경우 100, 300, 500, 1000, 1500, 2000 Hz에서 선수 소음 준위는 4m인 경우 각각 86, 91, 98, 95, 93, 92 dB로 나타났고, 10m인 경우 88, 87, 99, 96, 94, 92 dB로 나타났으며, 선미에서는 4m인 경우 88, 88, 99, 96, 93, 92 dB, 10m인 경우 88, 86, 99, 96, 94, 92 dB로 나타났다. In order to obtain a basic data for alluring a school of fish, anchovy with emitting underwater sound, an experimnet for measuring noise level was carried out on no-load operation of a engine. noise level was measured at the depths of four meters and ten meters and analyzed. Then, the revolution per minute of the ship used at this field experiment was 800 including 1000. The results were followed. 1. On 800 rpm, noise levels on board were 90, 93, 106, 107 and 101 dB in the order of bow, deck, stern, funnel, funnel and engine room. In addition, 93, 95, 110, 109 and 103 dB on 1000 rpm, respectively. 2. on 800 rpm, underwater noise levels of the depth of four meters at the bow were 92, 95, 102, 97, 94 and 92dB at the frequencies of 100, 300, 500, 1000, 1500 and 2000 Hz, respectively and those of the depth of ten meters were 87, 88, 99, 96, 94 and 93 dB. Underwater noise levels of the depth of four meters outside the engine room were 94, 91, 100, 96, 94 and 92 dB at the frequencies of 100, 300, 500, 1000, 1500 and 2000 Hz, respectively and those of the depth of ten meters were 93, 92, 99, 95, 93 and 91 dB. Underwater noise levels of the depth of four meters at the stern were 98, 94, 100, 97, 94 and 92 dB at the frequencies of 100, 300, 500, 1000, 1500, and 2000 Hz, respectively and those of the depth of ten meters were 92, 93, 99, 96, 93 and 92 dB. 3. On 1000 rpm, underwater noise levels of the depth of four meters at the bow were 86, 91, 98, 95, 93 and 92 dB at the frequencies of 100, 300, 500, 1000, 1500 and 2000 Hz, respectively and those of the depth of ten meters were 88, 87, 99, 96, 94 and 92 dB. Underwater noise levels of the depth of four meters at the stern were 88, 88, 99, 96, 93 and 92 dB at the frequencies of 100, 300, 500, 1000, 1500 and 2000 Hz, respectively and those of the depth of ten meters were 88, 86, 99, 96, 94 and 92 dB.
