        저위교합 유구치에서의 공간확장술을 이용한 외과적 접근

        저위교합이란 능동적 맹출기 동안이나 이후 해당치아의 교합면 방향으로의 성장이 멈추어 다른 치아들보다 하방에 존재하는 것을 말한다 저위교합치의 결과로 침하치의 만기잔존, 부정교합, 증가된 치아우식 감수성과 치주질환의 가능성, 계승치의 이동 등이 나타날 수 있다. 따라서 조기진단 및 적절한 처치가 필요하다. 저위교합치의 치료는 저위교합 정도나 후속 영구치의 발육정도에 따라 보존에서 발치까지 다양하다. 영구 계승치가 없거나 부분적인 침하치가 치주적으로 건전한 경우, 공간문제를 발생하지 않는 경우 보존적인 접근이 추천되기도 한다. 그러나 계승치가 존재하고 맹출이 지연되는 경우, 국소적인 염증반응과 관련되는 경우 교정적인 치료가 필요한 경우에는 발거가 필요할 수도 있다. 특히 저위교합된 유구치의 상방으로 인접치가 경사되어 악궁장경에 문제를 초래할 위험이 있는 경우 외과적인 발치가 필요할 수도 있으며 경사가 심한 경우 발치를 위한 외과적 접근이 힘들 수 있다. 심하게 경사된 인접치로 인하여 저위교합치의 외과적 발거가 힘들 환아에서 가철성 교정장치를 이용하여 공간확보를 진행 하였으며 보다 용이하게 치아를 발거할 수 있었다. Infraclusion may be defined as teeth that stop their relative occlusal movement in the dental arches during or after the period of active eruption and then remain under the occlusal plane. Delayed exfoliation, malocclusion, increased susceptibility to dental caries and periodontal disease of both the neighboring teeth and retained molar, and dislocation of' the successor are the consequencces of infraclusion of primary molars. Therefore, early diagnosis and appropriate treatments are necessary. The therapeutic approach of' the infracluded teeth varied from preservation to extraction. The teeth with simple infraclusion without any signs of interference with occlusal and jaw development may be examined periodically with fellow-up check and radiographically. However, if the infracluded tooth interferes with normal eruption of successor or shows any sign of delayed resorption, or the tipping of adjacent teeth or supraeruption of op-posing teeth is expected, the teeth inflicted should be extracted and appropriate measures should be provided in order to maintain the normal development of occlusion and dentition. The adjacent teeth which have been collapsed over a infracluded deciduous teeth can disturb the arch length perimeter. In such cases, surgical approach might be necessary, although it would be difficult when teeth are severly leaned. However, an easier surgical access have been obtained by space regaining procedures, in young patients whose arch length has been shortened due to the infracluded teeth.

        Ursolic acid supplementation decreases markers of skeletal muscle damage during resistance training in resistance-trained men: a pilot study

        Ursolic acid (UA) supplementation was previously shown to improve skeletal muscle function in resistance-trained men. This study aimed to determine, using the same experimental paradigm, whether UA also has beneficial effects on exercise-induced skeletal muscle damage markers including the levels of cortisol, B-type natriuretic peptide (BNP), myoglobin, creatine kinase (CK), creatine kinase-myocardial band (CK-MB), and lactate dehydrogenase (LDH) in resistance-trained men. Sixteen healthy participants were randomly assigned to resistance training (RT) or RT+UA groups (n=8 per group). Participants were trained according to the RT program (60~80% of 1 repetition, 6 times/week), and the UA group was additionally given UA supplementation (450 mg/day) for 8 weeks. Blood samples were obtained before and after intervention, and cortisol, BNP, myoglobin, CK, CK-MB, and LDH levels were analyzed. Subjects who underwent RT alone showed no significant change in body composition and markers of skeletal muscle damage, whereas RT+UA group showed slightly decreased body weight and body fat percentage and slightly increased lean body mass, but without statistical significance. In addition, UA supplementation significantly decreased the BNP, CK, CK-MB, and LDH levels (p<0.05). In conclusion, UA supplementation alleviates increased skeletal muscle damage markers after RT. This finding provides evidence for a potential new therapy for resistance-trained men.

      • HCV, Acute, LT : The Seroprevalence and Survey of the Risk Factor of Hepatitis a Infection in Korean Twenties

        Background/Aims: We aimed to determine the factors affecting the prevalence of hepatitis A(HAV) antibody and also tried to investigate the current prevalence of HAV antibodies in Korean their twenties. Methods: Blood samples of 298 subjects from solitary hospital in Pohang, the subjects were military soldiers in their twenties. They were assayed for anti-HAV-IgG. And economical and environmental data were obtained using a questionnaire and we performed an analysis to find affecting factors. Results: Overall anti-HAV prevalence was 24.79% in their twenties, this result had a huge change compared with the result of reports in 1970s. And also, HAV infection was associated with a variety of environmental and behavioral factors. Such as, rural resident (adjusted odds ratio [OR] = 10.7 P=0.00), presence of flies in the kitchen (OR=31.51 P=0.00), and the size of house (OR=1.002 P=0.00) were independently associated with higher prevalence of anti-HAV-IgG. And the educational pattern of father (OR=0.41 P=0.01), and hand washing after defecation (OR=0.47 P=0.00) were indepen dently associated with lower prevalence of anti-HAV-IgG. Conclusions: Even though, this study did not show the defi- nite risk factors, but we found several suspicious relating factors. Also, the differences in socioeconomic status could be considered as a risk factor for hepatitis A infection. To decrease spreading HAV, improvements in educational programs are needed.

        A Numerical Analysis on the Curved Bileaflet Mechanical Heart Valve (MHV) : Leaflet Motion and Blood Flow in an Elastic Blood Vessel

        In blood flow passing through the mechanical heart valve (MHV) and elastic blood vessel, hemolysis and platelet activation causing thrombus formation can be seen owing to the shear stress in the blood. Also, fracture and deformation of leaflets can be observed depending on the shape and material properties of the leaflets which is opened and closed in a cycle. Hence, comprehensive study is needed on the hemodynamics which is associated with the motion of leaflet and elastic blood vessel in terms of fluid-structure interaction. In this paper, a numerical analysis has been performed for a three-dimensional pulsatile blood flow associated with the elastic blood vessel and curved bileaflet for multiple cycles in light of fluid-structure interaction. From this analysis fluttering phenomenon and rebound of the leaflet have been observed and recirculation and regurgitation have been found in the flow fields of the blood. Also, the pressure distribution and the radial displacement of the elastic blood vessel have been obtained. The motion of the leaflet and flow fields of the blood have shown similar tendency compared with the previous experiments carried out in other studies. The present study can contribute to the design methodology for the curved bileaflet mechanical heart valve. Furthermore, the proposed fluid-structure interaction method will be effectively used in various fields where the interaction between fluid flow and structure are involved.

        High-dose Versus Low-dose Cyclophosphamide in Combination with G-CSF for Peripheral Blood Progenitor Cell Mobilization

        Background: To compare the mobilizing effects and toxicities of two different doses of cyclophosphamide (CY) plus lenograstim (glycosylated G-CSF), we performed a prospective randomized study by enrolling patients suffering with either high-risk Non-Hodgkin`s lymphoma (NHL) or breast cancer undergoing ablative chemotherapy. Methods: The NHL patients received 4 cycles of CHOP and the breast cancer patients received 2-3 cycles of FAC (FEC) adjuvant chemotherapy. Then, the patients were randomly allocated to receive CY 4g/㎡ (arm A) or 1.5g/㎡ (arm B) in combination with lenograstim. Large volume leukapheresis was carried out and it was continued daily until the target cell dose of 2×10(6) CD34+ cell/kg was reached. Results: Twenty-seven patients were enrolled in the study. The median number of leukaphereis sessions actually performed was 2.5 sessions in arm A and 3 sessions in arm B. The target cell dose was obtained with the median number of one leukapheresis session in both arms of the study (p=0.09). The collected number of CD34+ cells in the leukapheresis products was higher in arm A than arm B (22.4 vs. 9.9×10(6)/kg, respectively, p=0.05). Grade III or IV leukopenia was present in 14/15 patients (94%) in arm A and in 1/12 patients (8%) in arm B (p<0.0001). Grade III or IV thrombocytopenia was present in 8/15 patients (54%) in arm A, but this was not present in any patients of arm B (p=0.0004). Neutropenic fever occurred in 6/15 patients (40%) in arm A, and in 1/12 patients (8%) in arm B (p=0.09). The hematological recovery of the leukocytes and platelets after transplantation was not statistically different between the two doses. Conclusion: Low-dose CY plus lenograstim is a safe and effective mobilizing regimen.

        Sympathetic Block for Treating Primary Erythromelalgia

        Primary erythromelalgia is a rare condition that’s characterized by erythema, an increased skin temperature and burning pain in the extremities. The pain is often very severe, and treating erythromelalgia is frustrating and difficult. We report here on the case of a 12-year old girl with primary erythromelalgia in both lower extremities. The pain was refractory to medical treatment, but a bilateral sympathetic block with lidocaine and triamcinolone resulted in relief from the pain. Our experience with this disease demonstrates that sympathetic blocks are effective in improving the symptoms and they may be attempted on erythromelalgia patients who do not respond to other treatments, including medication and epidural blocks.

        Transcriptome analysis of non-small cell lung cancer and genetically matched adjacent normal tissues identifies novel prognostic marker genes

        <P>Lung cancer can be classified into two different types, non-small cell lung cancer (NSCLC) and small cell lung cancer. Up to 85% of lung cancer cases are NSCLC. To identify prognostic marker genes for NSCLC, we conducted 10 pairs of genetically matched transcriptome (NSCLC and adjacent normal tissues obtained from 10 patients) analysis using next-generation sequencing. Pathway analysis revealed that the genes associated with the cell cycle are highly upregulated in NSCLC. Integrative analysis of two independent NSCLC studies, 71 pairs (GSE40419) and 58 pairs (TCGA-LUAD), also confirmed that cell cycle-related genes are mostly upregulated in NSCLC. All three independent NSCLC transcriptomes concordantly identified 10 potential tumor-marker genes that are highly expressed as well as significantly altered in NSCLC. A quantitative real-time polymerase chain reaction experiment validated the altered expressions of the genes. Among them, downregulation of MFAP4 and AGER and upregulation of SPP1 genes in NSCLC compared to normal tissues were significantly associated with poor prognosis as evaluated by Kaplan-Meier survival and receiver operating characteristic analyses. A further analysis indicated that the expression ratio of MFAP4 and SPP1 or AGER and SPP1 can be used as potential prognostic marker genes for NSCLC.</P>
