        Collagenase와 esterase가 상아질 접착강도와 nanoleakage에 미치는 영향

        본 연구는 상아질 접착계면에서 collagenase와 esterase가 접착강도와 극미세누출에 미치는 영향을 살펴보기 위해 시행하였다. 발치된 치아의 교합면 상아질에 Single Bond 2(SB)와 Clearfil SE bond(SE)를 사용하여 상아질-레진 접착시편을 제작하고,시편을 인산완충용액(PBS)에 24시간(I),또는 PBS(II),collagenase 용액(III),esterase 용액(IV)에 4주간 보관 한 후 질산은 용액으로 염색하였다. 시편의 미세인장접착강도(μTBS)와 질산은 침투 면적을 측정하여 다음과 같은 결과를 얻었다. 1. SB군의 접착강도가 II∼IV군에서 SE군에 비해 낮은 값을 나타내었다(p<O.05). SB군은 II∼IV군의 접착강도가 I군에 비해 낮은 값을 보였으나(p<O.05),SE군의 접착강도는 I∼IV군간에 차이를 보이지 않았다(p>O.05). 2. SB군의 질산은 침투 면적이 SE군에 비해 높았으며(p<O.05),SB군과 SE군에서 질산은 침투 면적은 I ∼ IV군간에 차이를 보이지 않았다(p>O.05). 3. 접착강도와 질산은 침투 면적은 SE군의 I, II,III군에서 음의 상관관계를 보였다(p<O.05). The purpose of this study was to evaluate the effects of collagenase and esterase on dentin bond strength and nanoleakage. Resin composites were bonded to occlusal dentin of premolars with Single Bond 2(SB) and Clearfil SE Bond(SE). After the microtensile specimens were prepared and stored in PBS for 24 hours(I) or, PBS(II), collagenase(III), esterase(IV) solution for 4 weeks, the specimens were stained with silver nitrate solution. Microtensile bond strength(μTBS) and silver penetration area were measured and, the results were as follows: 1. For group II, III, and IV, the bond strengths of SB were lower than those of SB(p<0.05). The bond strengths of SB II, III, and IV were lower than that of SB I(p<0.05). There was no difference among the bond strengths of SE I ∼ IV groups(p>0.05). 2. Silver penetration areas of SB were higher than those of SE for all storage groups(p<0.05). In SB and SE groups, there was no significant difference of silver penetration area among I ∼ IV groups(p>0.05). 3. SE I, II, and III showed inverse relationship between the bond strengths and the silver penetration areas(p<0.05).

        상아질 접착에서 collagenase와 esterase가 미세인장결합강도에 미치는 영향

        상아질-레진 접착강도에 대한 collagenase와 esterase의 영향을 살펴보기 위해, 소구치의 교합면 상아질에 Single Bond 2와 Clearfil SE Bond로 접착을 시행하고 미세 시편을 제작하여 PBS, collagenase 용액, esterase 용액에 4주간 보관한 후 미세인장결합강도를 측정, 비교하여 다음과 같은 결론을 얻었다. 1. 모든 보관 용액에서 Single Bond 2의 미세인장결합강도는 Clearfil SE Bond보다 유의하게 낮았다(p<0.05). 2. Single Bond 2의 미세인장결합강도는 collagenase군이 PBS군, esterase군보다 낮았다(p>0.05). 3. Clearfil SE Bond의 미세인장결합강도는 esterase군이 PBS군에 비해 낮았으나(p>0.05), collagenase군보다는 높았다(p>0.05). Collagenase군은 PBS군에 비해 유의하게 낮은 미세인장결합강도를 보였다(p<0.05). The purpose of this study was to evaluate the effect of collagenase and esterase on the microtensile bond strength (μTBS) in dentin bonding. After resin composites were bonded to occlusal dentin, μTBS specimens were formed and stored in PBS, collagenase, or esterase solution. After 4-week storage, μTBS was determined and, the results were as follows: 1. μTBS values of Single Bond 2 were lower than those of Clearfil SE Bond for all storage medium (p<0.05). 2. In single Bond 2 group, collagenase solution lowered bond strength more than PBS and esterase solution (p>0.05). 3. In Clearfil SE Bond group, esterase solution lowered bond strength more than PBS(p>0.05). Collagenase solution lowered bond strength more than esterase solution(p>0.05) and PBS(p<0.05).

        시온의 운명과 토라 : 공시적 관점에서 본 사 36-39장의 역할에 대한 재고찰 Reconsidering the function of Isaiah 36-39 from a synchronic perspective

        The last two decades of biblical interpretation has witnessed a dramatic shift in Isaiah studies from interpreting Isaiah as if it were three independent books to interpreting it as a single literary entity. Several scholars have sought to interpret Isaiah as a whole, while uncompromisingly recanting the fundamental premises of the standard historical critical approach. Special attention has been drawn to the literary structural, ideological, canonical and thematic facets set forth in the book of Isaiah, which function as key central clues for understanding the book of Isaiah as a whole. Further emphasis has been placed upon these signs to gain an insight into the way in which the book of Isaiah is read as a unitary entity. It is commonly agreed, in particular, that Isaiah 36-39 plays a crucial role in shedding light on how to see the entire book of Isaiah synchronically. This paper will present a full-fledged overview of a variety of interpretations focused on the unity of Isaiah, especially chapters 36-39. Then it will canvass how the word of God, Torah, in Isaiah 36-39 plays a pivotal role in understanding the overall message of the book of Isaiah that for Zion success or failure is solely dependent on hearing the word of God.

        Air abrasive technique 을 이용한 복합레진 수복 증례

        The air abrasive technique is a non-mechanical method by which teeth are treated before restoration and stains and calculi are removed from tooth surfaces using the kinetic energy of small particles. The air abrasive technique in dentistry was first introduced in the 1950's with as instrument called 'Airdent'. But, as the main restorative materials of the period were amalgam and gold, and the instrument's inability to control the flow of particles caused the particles to be spread throughout the clinics, widespread use was not possible. In the 1990's, as these techincal problems were solved and more interest in new restorative materials rose in an effort to preserve sound tooth structure, new developments took place in instruments related to the air abrasive technique. The air abrasive technique produces less pressure, vibration and heat that might cause patient discomfort and facilitates the preservation of sound tooth structure. It also reduces the need for anesthesia and is less harmful to the pulp. Other advantages include increase in dentin bonding strength of composite resin, lower possibility of saliva contamination and maintenance of a dry field. But there is not direct contact between the nozzle and the tooth, the operator cannot use his or her tactile sense and must rely solely upon visual input. Other disadvantages are : the tooth preparation depends on the operator's ability ; alpha-alumina particles, after bouncing off the tooth surface, cause damage to dental mirrors ; the equipment is expensive and takes up a certain amount of space in the clinic. The author conducted case report using the air abrasive technique on patient visiting the Department of Pediatric Dentistry at Seoul National University Dental Hospital and arrived at the following conclusions. 1. The tooth preparation capability of different air abrasive devices varied widely among manufacturers. 2. It was more effective in treating early caries lesions and stains compared to lesions where caries had already progressed to produce soft dentin. 3. The cold stream and noise caused by the evacuation system was a major cause of discomfort to pediatric patients. 4. As there is no direct contact with tooth surface when using the air abrasive technique for tooth preparation, considerable experience and skill is required for proper tooth preparation.

      • 신경섬유종증 환자에게서 발생한 특발성 혈소판 감소증 1예

        Idiopathic thrombocytopenic purpura is a disease characterized by thrombocytopenia in peripheral blood with positive anti-platelet antibody titer, no splenomegaly, the presence of megakarocyte. The causes of idiopathic thrombocytopenic purpura were usually related to autoimmune disease, infection and drugs. A 46-years-old man was admitted for evaluation of abdominal mass. He has many cafe-au-lait spots and nodules in almost skin for 30 years. Several years ago, he palpated mass in Lt. inguinal area. hIe has known the abdominal mass after checking abdomen CT in local clinic and visited our hospital for evaluation of abdominal mass. At admission, laboratory tests showed hemoglobin 12.5 g/dL, hematocrit 38.4 %, WBC 5,460 /㎣ (neutrophil 64.5%, lymphocyte 21.7%, monocyte 4.1%, eosinophil 5.4%), reticulocyte 0.7%, Platelet 52,000 /㎣, total protein 6.51 g/dL, albumin 3.80 g/dL, alkaline pohsphatase 161 IU/L, AST 110.7 IU/L, ALT 64.9 IU/L, total bilirubin 0.44 ㎎/dL, BUN 18.4 ㎎/dL, creatinine 0.79 ㎎/dL. Peripheral blood smear showed normocytic normochromic anemia, thrombocytopenia. Bone marrow aspiration smear showed normoplastic myeloid and erythroid precursor. The bone marrow biopsy section shows normocellular marrow (35%) for his age and mildly increasing myeloid cell lineage. We have experienced a case of idiopathic thrombocytopenic purpura in a patient being in neurofibromatosis and therefore reporting it with documentary records. 저자들은 신경섬유종증 환자에서 특별한 원인을 발견할 수 없었음에도 특발성 혈소판 감소증이 발생한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

      • 코일로 치료한 다발성 폐동정맥 기형 1예

        A 29 year old female patient came with grade NYHA Fc IV dyspnea of 1 month. No abnormality was heard during auscultation. Arterial blood gas analysis done due to shortness of breath showed a decreased PO_(2) of 64.6 mmHg. Computerized tomography(CT) was checked due to multiple nodules seen in both lower lung fields of the chest X-ray. CT revealed multiple pulmonary arteriovenous malformations in both the mid and lower lung fields. Dyspnea was uncorrected by the application of oxygen. Coil embolization showed improvement of PO2 and dyspnea. The CT follow up at 9 months after the procedure showed no progression of the arteriovenous malformation without any additional symptoms nor dyspnea.

        불소를 포함시킨 가철성 교정장치용 Resin의 불소 방출과 굴곡강도에 관한 연구

        For the purpose of investigating the continuous fluoride releasing from acrylic resin using removable appliances by incorporating fluoride into polymethylmethacrylate during the construction of the appliances, two kinds of fluoride were blended with acrylic powder and devided into five groups as follows. Group 1: no fluoride (control) Group 2: NaF 5 wt% Group 3: NaF 10 wt% Group 4: CaF₂ 5 wt% Group 5: CaF₂10 wt% Resin specimens were tested in vitro for fluoride release with fluoride electrode and flexural strength with Instron. The results were as follows; 1. After 24 hrs, released fluoride concentration(ppm) was the most in group 3(126.5±14.15) then, next, group 2(49.5±4.92), group 5(2.67±1.08), group 4(0.81±0.30),group 1(0.03±0.01) in order, but after 28 days, group 3(3.37± 0.31), group 5(1.25±0.25), group 4(0.74±0.19), group 2(0.68±0.07), group 1(0.03±0.01). 2. Fluoride releasing of NaF gruops were faster and more amount than CaF₂groups initially but decreased significantly, later. 3. Flexural strength(㎏/㎠ ) was the biggest in group 1(602.90±69.32), and then group 4(568.98±20.16), group 5(556.19±5.26), group 2(536.12±30.35), group 3(508.44±26.16) in order, the difference between control group and NaF GROUPS were statistically significant(p<0.05).

      • 유·초등학생 장애이해 교육

        특수교육이 장애학생을 포함한 모든 학생의 특성과 요구에 부응하는 통합교육으로 발전하기 위해서는 무엇보다도 장애인에 대한 편견과 차별이 없어야 한다. 장애인에 대한 편견과 부정적인 태도는 결국 장애인의 사회통합에 부정적인 영향을 미치기 때문에 우선적으로 해결해야 할 과제라고 할 수 있다. 본 연구에서는 「유ㆍ초등학생 장애이해 교육」자료를 개발ㆍ보급하여 장애인에 대한 편견과 차별을 해소하는데 그 목적을 두고, 전문가 협의회, 관련 문헌 탐색, 기존 장애이해 교육 자료 분석, 현장교사 설문 및 심층면담 등을 실시하였고, 이를 통해 얻은 연구 결과를 요약 정리하면 다음과 같다 첫째, 우리의 학교현장에서는 장애이해 교육 시간확보가 어려운 실정이다. 이런 현상은 교육경력이 낮은 교사일수록 또는 학교경영자와 일반교사의 이해가 부족할수록 더욱 심각하게 나타나는데, 이러한 문제를 해결하기 위해서는 국립특수교육원이나 시ㆍ도교육청에서 장애 이해 교육에 따른 시간확보 방안을 제공할 필요가 있다. 장애이해 교육의 필요성에 대한 학교경영자 및 일반교사의 인식 부족을 해결하기 위해서는 학교경영자와 일반교사가 특수교육과 장애인에 대한 이해를 넓힐 수 있도록 하는 다양한 연수프로그램이 마련되어야 한다. 둘째, 장애이해 교육에 활용할 수 있는 자료의 양이 부족할 뿐 아니라, 기존에 개발된 자료를 활용할 수 있는 체계적인 방안이 마련되어 있지 많다는 점이다. 따라서 장애이해 교육에 활용할 수 있는 다양한 자료가 개발되어야 하고, 기존에 개발된 자료는 쉽게 활용될 수 있도록 종류별로 정리되어야 하고, 활용할 수 있는 구체적인 방법도 제시할 필요가 있다. 셋째, 장애이해 교육이 학기초나 '장애인의 날'과 같은 특정기간에 편중되어 있다는 점이다. 장애인식 개선을 위한 교육은 년 중 계획을 세워 일정기간 단위로 실시하여야 한다. 넷째, 장애이해 교육에 활용되는 모델이나 이해교육 방법이 주로 감각장애(시각장애ㆍ청각장애) 또는 지체부자유학생을 중심으로 되어 있다는 점이다. 교육현장에서는 발달장애학생도 많기 때문에 발달장애학생을 모델로 하는 장애이해 교육 프로그램이나 교수-학습방법도 지속적으로 개발되어야 한다. 다섯째, 장애이해 교육방법이 다양하지 못하다는 점이다. 장애이해 교육이 효율적으로 이루어지기 위해서는 장애인의 다양한 특성이 고려되어야 한다. 그러기 위해서는 교수활동 방법 또한 이들의 특성을 고려하여 보다 다양하게 이루어져야 할 것이다. 위에서 제시한 장애이해 교육 실태를 토대로 본 연구보고서에서는 현장교사들이 쉽게 활용할 수 있도록 하기 위해서 1부, 2부로 나누어「유ㆍ초등학생 장애이해 교육자료」를 안내하였다. 제 1부 「장애이해 교육의 개관」에서는 장애이해 교육자료 개발의 필요성, 목적, 연구방법 및 절차, 장애이해 관련 제이론, 장애이해 교육의 현황 및 문제점, 개선방안 등을 기술하여 학교현장의 교사들이 장애이해 교육에 대해 보다 폭 법은 이해를 도모하도록 하였다. 제 2부 「장애이해 프로그램 및 교육자료」에서는 장애유형별 즉, 시각장애, 청각장애, 정신지체, 정서장애, 자폐성장애, 학습장애, 지체부자유 등으로 나누어 제시하여 교사들이 장애 이해 지도를 보다 쉽게 접근할 수 있도록 하였고, 아울러 기존에 개발된 장애이해 교육자료를 도서, 방송프로그램, 영화, 인터넷 웹사이트 등으로 분류 제시하여 학교현장에서 학생의 수준과 활동 내용에 따라서 다양하게 활용할 수 있도록 하였다. 본 프로그램은 「유ㆍ초등학생 장애이해 교육자료」의 한 예에 불과하다. 학교 현장의 지도 교사는 프로그램의 수준을 상향 또는 하향 조정하여 지도할 수 있으며, 생활 주변의 다양한 소재로 프로그램의 내용을 수정 또는 대체할 수 있다. 나아가 창의적으로 새로운 상황을 전개할 수 있도록 하는 노력도 필요하겠다. 2004년에는 「중ㆍ고등학생 장애이해 교육자료」가 개발되어 학교현장에 보급될 계획이다. In order for special education to develop into an inclusive education system that meets the characteristics and needs of all children, including those who are disabled, there must be no prejudices and discriminations against children who are physically or mentally challenged. As the biases and negative attitudes toward persons with disabilities could eventually exert negative influences on their integration into society, these tasks must be effectively taken care of above all else. This study, with the aim of dissolving prejudice and discrimination against the persons with disabilities by developing and disseminating materials for the "Education for Non-disabled Preschoolers and Elementary School Children to Understand Disabilities", has had consultations with experts, researched related books and other documents, analyzed the existing educational materials for understanding individuals with disabilities and conducted surveys on and interviews with teachers in the field of education. The results of the study are summarized as follows. First, the fact remains that it is difficult to secure time to educate on how to understand the children with disabilities at the nation's schools. Such a phenomenon occurs more seriously among teachers with less educational experiences as well as school managers and general education teachers without adequate understanding of the children with disabilities. To resolve this problem, either the Korea Institute for Special Education or the municipal and provincial educational agencies need to provide ideas to secure time to educate the understanding of those who are challenged. In order to solve the lack of understanding of the children with disabilities among school managers and general education teachers, various programs must be created and implemented to train the school administrators and regular teachers to expand their understanding of the persons with disabilities. Second, not only is the amount of materials available for the education of the children with disabilities insufficient, but moreover there are no systematic measures to make the most of those materials that are already developed. Therefore, diverse resources must be developed to use for educating the understanding disabilities, and already developed materials must be classified into categories for easy use along with concrete methods for utilizing such resources. Third, education for understanding the children with disabilities is concentrated on specific periods, such as the early semester or the "Day for the Persons with Disabilities". Education to improve the perception of disabilities should be conducted in periods under an annual program. Fourth, the models or methods used to educate for understanding the persons with disabilities are centered mainly on the individuals with sensory injured (with visual or hearing impairments) or the persons with physically disabled. At educational sites, there are many children with developmentally disabled and so educational programs and teaching-learning methods to educate the understanding of the children with disabilities should be modeled after the children with developmentally challenged. Fifth, the methods to educate the understanding of the persons with disabilities are not diverse. For the efficient education of the understanding disabilities, the diverse characteristics of the persons with disabilities should be taken into account. To be successful in this matter, more diverse methods of teaching activities also need to be used by considering the characteristics of the people with disabilities. On the basis of the reality of developing an understanding and proper awareness of those who are disabled as presented above, this study report guides the "Education for non-disabled preschoolers and elementary school children to understand disabilities" in Part 1 and Part 2 for easy use by teachers on active duty. In Part 1, titled as "A General Survey of Educating the Understanding of the Children with Disabilities", I have described the necessity and purpose of educating the understanding of the children with disabilities, its study methods and procedures, various theories on educating the understanding of the children with disabilities, the current situations and problems of educating the understanding of the children with disabilities and their improvement measures to deepen the understanding about educating the understanding disabilities by teachers on active duty. In Part 2, titled the "Programs to Educate the Understanding Disabilities and Educational Materials," I have classified types of disabilities into visual impairment, hearing impairment, mental retardation, emotional disorder, autism, learning disability and the physical impairment for easy approach to educate the understanding disabilities by teachers. At the same time, I re-categorized the already developed materials to educate the understanding disabilities into books, broadcasting programs, movies, Internet web-sites and others for diverse use at schools according to the children' levels and activities. This program is just one example of the "Education for Non-disabled Preschoolers and Elementary School Children to Understand Disabilities". I have arranged it in such a way as to allow teachers to teach the children by upgrading or downgrading the program's level as well as modifying or replacing the contents of this program by using various materials that can be obtained in everyday lives. In 2004, I am planning to develop and distribute the "Education for non-disabled Middle-and High-school Students to Understand Disabilities".
