        청소년기에 발생한 경부 지방육종의 치험례

        Liposarcomas are common malignant mesenchymal neoplasms, comprising approximately 15% of all soft-tissue sarcomas. They rarely occur in the head and neck region, the reported incidence being 2 to 6%. They make up only 4% of childhood soft tissue sarcomas, in contrast to adults, in whom they account for 15 to 20%. Less than 15 cases of liposarcomas in the head and neck have been reported in patients under the age of 18 years. Although the tumor is radiosensitive, wide surgical excision with postoperative radiotherapy is the treatment of choice at present. The prognosis is dependent of the type and grade. Five year survival is reported as over 80% for myxoid types and well-differentiated types but less than 50% for round cell types and pleomorphic types. We found a case of liposarcoma in the submandibular region in a 16-year-old female patient. In this case, surgical excision with postoperative radiotherapy was perfomed.

        Accurate acquisition of surface geometries such as machined surfaces, biological surfaces, and deformed parts have been very important technique in scientific study an engineering, expecially for system design, manufacturing and inspection. Two camera method keeps accuracy more than double than mechanical method. In this paper, we describe the processes surface data of teeth for automatic processing, transformation that changes to coping data of possible numeric data form, and processed inlay teeth. As the result of the inlay manufacture, which can process with high resolution, therefore we construct the automatic processing system that depends on manual.

      • The Effect of CD18 Monoclonal Antibody on Tissue and Serum Interleukin-8 Concentration in Reperfusion Injury

        It remains a problem that successful revascularization and reperfusion after ischemia are associated with high systemic complication rates and sever local tissue injuries. With prolonged ischemia, there is damage to tissue from anoxia, but further injury may occur after reperfusion. The activation of leukocytes and endothelial cells during reperfusion causes neutrophil adhesion in capillaries, resulting in pluggin and further ischemia, Alternatively, neutrophil adhesion to endothelium leads to the migration neutrophils with local edema formation, hemorrhage, and thrombosis. Some chemotactic and activating factors are needed to propel neutrophils to the site of local inflammation. The chief cytokines that induce a pro-adhesive state in endothelium are tumor necrosis factor-α(TNF-α), interleukin-1β(IL-1β) and and endotoxin or lipopolysaccharide(LPS). Similarly, TNF-α, and to a lesser extent interleukin-8(IL-8), is the important stimulus that acts on neutrophils and other leukocytes to alter their adhesion. The purpose of this study was to evaluate the patholgenetic role of IL-8 after perfusion with CD18 monoclonal antibody(CD18 mAb), the blocking antibody of neutrophil adherence, on reperfusion injury in rat epigastric island skin flap. A 6×3 cm sized island skin flap was made on the abdomen. The epigastric pedicle was occluded for six hoiurs with ambient temperature. Fifteen minutes before reperfusion, the flap was perfused with saline and CD18 mAb(1 mg/kg). For evaluation of IL-8 levels, tissue fluid and serum were obtained at 4, 8, 12, and 24 hours after reperfusion. IL-8 concentrations of the CD18 mAb group in the tissue fluid were significantly decreased at 8, 12, and 24 hours compared to the control group(p < 0.01), but the difference between the two groups was not significant at 4 hours after reperfusion. IL-8 concentrations of the Cd18 mAb group I the serum were significantly decreased over time compared to the control group(p < 0.05, p < 0.01). from The above results, we conclued that blocking neutrophil adherence using CD18 mAb within the peak level of IL-8 at 4 hours after reperfusion may be a better method of reducing reperfusion injury to the island skin flap.         The Effects of Glucose and UV-Irradiation on Stability of Acellular Dermal Matrix

        There are some literatures about the synergistic effects of glucose and UV on collagen crosslinking with two-dimensional collagen film model, but there were no experiment or literature about three-dimensional model. So, this study was proceeded to investigate the possibility of synergistic effects of glucose and UV irradiation in the formation of crosslinking in three-dimensional structure of multi-components, human acelluar dermal matrix(ADM). ADM were assigned to four groups: group I was no treatment as control; group II was treated with glucose only; group III was treated with ultraviolet only; group IV was treated with glucose and ultraviolet. To determine the stability of ADM; differential scanning calorimetric, tensile strength test, amount of released hydroxyproline after acidinduced lysis and weight change after collagenase digestion were measured. Compared with the controlled group and other experimental groups, group IV showed characteristic results: lowest swelling ratio, highest in shrinkage temperature, lowest peak width, and greatest resistance to acid and collagenase. Consequently, these results imply that glucose and UV contribute to enhancing the stability of ADM strikingly.

        Current Methods for the Treatment of Alveolar Cleft

        Alveolar cleft is a tornado-shaped bone defect in the maxillary arch. The treatment goals for alveolar cleft are stabilization and provision of bone continuity to the maxillary arch, permitting support for tooth eruption, eliminating oronasal fistulas, providing an improved esthetic result, and improving speech. Treatment protocols vary in terms of the operative time, surgical techniques, and graft materials. Early approaches including boneless bone grafting (gingivoperiosteoplasty) and primary bone graft fell into disfavor because they impaired facial growth, and they remain controversial. Secondary bone graft (SBG) is not the most perfect method, but long-term follow-up has shown that the graft is absorbed to a lesser extent, does not impede facial growth, and supports other teeth. Accordingly, SBG in the mixed dentition phase (6-11 years) has become the preferred method of treatment. The most commonly used graft material is cancellous bone from the iliac crest. Recently, many researchers have investigated the use of allogeneic bone, artificial bone, and recombinant human bone morphogenetic protein, along with growth factors because of their ability to decrease donor-site morbidity. Further investigations of bone substitutes and additives will continue to be needed to increase their effectiveness and to reduce complications.

        초저온 급속동결과 일반 동결방법에 의해 처리된 무세포진피의 안정성 비교

        To preserve human tissue for a long time, freezing at -40 C has generally been used. However, this method causes tissue damage due to ice crystal formation. We tried to minimize the tissue damage and stabilize the structure by rapidly freezing human acellular dermal matrix(ADM) at ultra low temperature. ADM was rapidly frozen with -196oC nitrogen liquid and went into freeze drying process. The structural stability of quick frozen ADM was compared with that of conventional frozen ADM as a control group by thermodynamic evaluation, biochemistry analysis, and physical property studies. With differential scanning calorimetry, we measured denaturation calories and shrinkage temperature. To assess the degradation of collagen by collagenase, dried weights were measured every 5 hours until 24 hours. Finally, to estimate physical properties, tensile strength of the specimen was checked. Denaturation calories significantly increased and degradation rate significantly decreased, compared to the control group(p<0.05). Evaluation of tensile strength showed superior results in the rapid freezing method. In the process of ADM, rapid ultra-freezing method made the tissue water into amorphous small ices that minimizes the tissue damage. Quick frozen ADM revealed less tissue damage than conventional frozen ADM. It suggests that quick frozen method of ADM contributes the structure stability that the conventional method.

Form the above results, we concluded that blocking neutrophil adherence using CD18 mAb within the peak level of IL-8 at 4 hours after reperfusion may be a better method of reducing reperfusion injury to the island skin flap.
