        The Risk of Acute Myocardial Infarction in Patients With Gastroesophageal Reflux Disease

        Background/Aims A number of inflammatory mediators have been documented to be elevated in gastroesophageal reflux disease (GERD). Similar inflammatory mediators are involved in coronary artery disease. Thus, the aim of the study is to determine if GERD is a risk factor for developing acute myocardial infarction (AMI). Methods We used Explorys, a private cloud-based data store to which a number of health care systems feed information. We identified a cohort of GERD patients who have undergone an esophagogastroduodenoscopy compared to those without GERD. Incidence of AMI was studied after statistically controlling for known AMI risk factors. Results Total of 200 400 patients were included in the GERD group and 386 800 patients in non-GERD group. The primary event of AMI occurred in 17 200 patients in the GERD group (8.6%) vs 24 300 in non-GERD group (6.3%). Using logistic regression analysis and controlling for 6 major risk factors which included male gender (OR, 1.09; 95% CI, 1.07-1.11; P < 0.001), hypertension (OR, 6.53; 95% CI, 6.21-6.88; P < 0.001), hyperlipidemia (OR, 3.08; 95% CI, 2.96-3.20; P < 0.001), diabetes mellitus (OR, 1.72; 95% CI, 1.69- 1.76; P < 0.001), obesity (OR, 1.02; 95% CI, 1.00-1.04; P = 0.044), and smoking (OR, 1.38; 95% CI, 1.35-1.41; P < 0.001). The odds of developing AMI in the GERD population was 1.11 (95% CI, 1.08-1.13; P < 0.001). GERD had higher odds of developing AMI than male gender or obesity in our study. Conclusions This study demonstrated that GERD is a risk factor for AMI, higher than male gender and obesity. However, the increased risk may be clinically insignificant.         Synthesis of certain 2-aminoadmantane derivatives as potential antimicrobial agents

        N-(2-Amamantyl)-N-(5-arylhydrazono-6-methyl-4-oxopyrimidin-2-yl) guanidines (IIIa, b), 2-(2-admantyl-amino)-4-amino-s-triazine (IVa) and its 6-chloromethyl derivative (IVb) were prepared by cylization of 1-(2-admantyl) biguanide HCl (I) with ethyl 2-arylhydrazono-3-oxobutyrates (II), ethyl formate and ethyl chloroacetate, respectively. Where 1-(2-admantyl)-3-(4, 5-dioxo-2-imidazolidinylidene)guanidine (V) was used as intermediate for the synthesis of amides (VIIa, b), hydrazide (VIII) and azomethine derivatives (IX, b) of alkyl 2-(2-admantyl-amino)-4-amino-2-triazine-6-carboxylates (VI a, b). The antimicrobial testing of the prepared compounds proved that compound 1Xb was the most active. It showed a marked bacteriostatic effect against Staphylococcus aureus and Bacillus subtilis.

        Alternatives to Enhance Flat Slab Ductility

        Flat slab systems are vastly used in multi-story buildings because of their savings in story height and construction time, as well as for their flexibility in architectural remodeling. However, they frequently suffer brittle punching-shear failure around columns, especially when subjected to lateral loads. Therefore, seismic codes labeled flat slabs as non-ductile systems. This research goal is investigating some construction alternatives to enhance flat slab ductility and deformability. The alternatives are: adding different types of punching-shear reinforcement, using discreet fibers in concrete mixes, and increasing thickness of slab around columns. The experimental study included preparation and testing of seven half-scale interior slab-column connections up to failure. The first specimen is considered a reference, the second two specimens made of concrete mixes with different volumetric ratios of polymer fibers. Another three specimens reinforced with different types of punching-shear reinforcement, and the last specimen constructed with drop panel of inverted pyramidal shape. It is found that using the inverted pyramid-shape drop panel of specimen, increases the punching-shear capacity, and the initial and the post-cracking stiffnesses. The initial elastic stiffnesses are different for all specimens especially for the slab with closed stirrups where it is experienced the highest initial stiffness compared to the reference slab.

        Nickel nanorods over nickel foam as standalone anode for direct alkaline methanol and ethanol fuel cell

        <P><B>Abstract</B></P> <P>A highly electroactive nickel nanorod (NNR)/nickel foam (NF) electrode was fabricated for direct alcohol fuel cells (DAFCs) using a simple and cost-effective hydrothermal process. The Ni/NiO nanorods were successfully grown on the surface of an NF electrode, which strongly enhanced the anode wettability and increased surface area by 18 times (11.9 m<SUP>2</SUP> g<SUP>−1</SUP>), resulting in interfacial polarization resistance reduction. The NNR/NF electrode shows high electro-catalytic activity and great stability during alcohol oxidation. The current densities obtained for NNR/NF were four (479 mA cm<SUP>−2</SUP>) and six (543 mA cm<SUP>−2</SUP>) times higher than that for pristine NF in the cases of methanol and ethanol oxidations, respectively. This high current density can be attributed to the superhydrophilic surface of the Ni/NiO nanorods and corresponding high mass transfer capability between the electrolytes and Ni/NiO nanorods embedded on the surface of the electrodes. This study presents a new approach for using the novel NNR/NF as a cheap and high performance anode in DAFCs.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Unique Ni/NiO nanorods coated nickel foam (NNR/NF) electrodes were synthesized. </LI> <LI> Ni/NiO nanorods were grown on nickel foam through a simple hydrothermal process. </LI> <LI> Ni/NiO nanorods on the surface of NF enhanced wettability and area by 18 times. </LI> <LI> NNR/NF demonstrated 5-fold higher current generation than pristine NF electrode. </LI> <LI> Highly electrocatalytic NNR/NF showed efficient MeOH and EtOH electrooxidation. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        The Changing Epidemiology of Gastroesophageal Reflux Disease: Are Patients Getting Younger?

        Background/Aims Gastroesophageal reflux disease (GERD) is a common disease globally with increasing prevalence and consequently greater burden on the Healthcare system. Traditionally, GERD has been considered a disease of middle-aged and older people. Since risk factors for GERD affect a growing number of the adult population, concerns have been raised that increasingly younger people may develop GERD. We aim to determine if the proportion of younger patients has increased among the GERD population. Methods The incidence of GERD as well as several variables were evaluated during an 11-year period. Explorys was used to evaluate datasets at a “Universal” and Healthcare system in northern Ohio to determine if trends at a local level reflected those at a universal level. GERD patients were classified into 7 age groups (15-19, 20-29, 30-39, 40-49, 50-59, 60-69, and ≥ 70 years). Results The proportion of patients with GERD increased in all age groups, except for those who were ≥ 70 years in the universal dataset (P< 0.001) and those who were ≥ 60 years in the Healthcare system (P < 0.001). The greatest rise was seen in 30-39 years in both datasets (P < 0.001). Similarly, the proportion of GERD patients who were using proton pump inhibitors increased in all age groups except for those who were ≥ 70 years in both datasets (P < 0.001), with the greatest increase being the group 30-39 years (P <0.001). Conclusion Over the last decade, there has been a significant increase in the proportion of younger patients with GERD, especially those within the age range of 30-39 years.         Some Nucleophilic Substitutions in 2,4- and 2,4,8-Trichloro- pyrido [3,2-d] pyrimidines

        Reaction of 6-(acetoxymethyl) -2,4-dichloropyrido [3,2-d] pyrimidine (I) with some nucleophiles was investigated. When I reacted with sodium azide afforded 2,4-diazido derivative (II). Treatment of II with sodium hydroxide underwent cyclization of the 2-azido group to tetrazolo, replacement of 4-azido group by hydroxide ion, and hydrolysis of 6-ace-foxy moiety to hydroxy methyl derivative (III). While, reaction of I with hydrazine hydrate resulted in the formation of 2,4 dihydrozino-6-hydroxymethyl derivative (lV).
