        노인전문요양시설에서 발생한 옴 유행 역학조사

          Objectives: To investigate an outbreak of scabies that occurred in geriatricfacilities located in Kyounggi-province, Korea, between September 2004 and September 2005.   Methods: We carried out an epidemiologic investigation on the scabies outbreak. All workers except two were interviewed and the charts of the patients were reviewed.   Results: Among workers, the attack rate was 58.6% (17/29). The rate was 86.0%(37/43) in patients. Secondary cases also occurred among the family members of workers, with a secondary attack rate of 15.8%. The 95% confidence interval was 6o/~31.3%. The transmission of scabies began from one index case who had scabies and moved from another facility. She was hospitalized on the 4th floor of this facilitywhere dependent patients resided. After the activities (picnic and yard sale) in May 2005, and the ward rotation of care givers between the 3rd and 4th floor in July 2005, the incidence of scabies increased. The last cases occurred in Sep. 2005.   Conclusions: This outbreak investigation has importance because it is the first report of ascabies outbreak in geriatric long-tenn care facilities in Korea. Recently, the reports on a scabies epidemic are increasing "With the augmentation of geriatric long-tenn care facilities.   It is important to educate and inform workers and staff in long-tenn care facilities about scabies and other contagious skin diseases. More facilities for senility and dementia "Will be opened and we need to lookinto contagious diseases in long-term care facilities.

        Neuroprotective Effect of L-Theanine on Aβ-Induced Neurotoxicity through Anti-Oxidative Mechanisms in SK-N-SH and SK-N-MC Cells

        Amyloid beta (Aβ)-induced neurotoxicity is a major pathological mechanism of Alzheimer`s disease (AD). In this study, we investigated the inhibitory effect of L-theanine, a component of green tea (Camellia sinensis) on Aβ1-42-induced neurotoxicity and oxidative damages of macromolecules. L-theanine inhibited Aβ1-42-induced generation of reactive oxygen species, and activation of extracellular signal-regulated kinase and p38 mitogenic activated protein kinase as well as the activity of nuclear factor kappa- B. L-theanine also significantly reduced oxidative protein and lipid damage, and elevated glutathione level. Consistent with the reduced neurotoxic signals, L-theanine (10-50 μg/ml) concomitantly attenuated Aβ1-42 (5 μM)-induced neurotoxicity in SK-N-MC and SK-N-SH human neuroblastoma cells. These data indicate that L-theanine on Aβ-induced neurotoxicity prevented oxidative damages of neuronal cells, and may be useful in the prevention and treatment of neurodegenerative disease like AD.

        난소암 세포주에서 Mullerian Inhibiting Substance의 증식 억제 효과

        목적: 임신 7주경 태아 고환의 미숙 Sertoli 세포에서 생산되어 Muller관을 퇴행시키는 Mullerian inhibiting substance (MIS)는 성 발달과 가임기 여성의 생식생리 기전을 조절하는 이외에 Muller관으로부터 발생하는 일부 종양과 세포주의 성장을 in vivo와 in vitro에서 억제한다는 사실이 밝혀졌다. 이에 본 연구자들은 난소상피암 세포주들에서 MIS type II 수용체 (MISR II)의 발현을 확인하고, MIS를 투여하여 MIS의 세포 증식 억제효과와 그 기전을 밝힘으로서 종양치료제로서 MIS의 임상적 사용에 대한 가능성을 알아보고자 하였다. 연구 방법: 난소암 세포주인 SKOV-3, OVCAR-3 및 OVCAR-8에서 면역조직화학염색법으로 MISR II의 발현여부를 확인하였다. 난소암 세포주의 생존도는 고순도 MIS를 투여하여 24시간과 48시간동안 배양한 후 MTT assay로 측정하였다. MIS에 의한 세포주기 변화는 DNA 염색 후 flow cytometer로 분석하였으며 세포자멸사는 annexin-V-FITC 염색방법을 이용하여 flow cytometer로 평가하였다. MIS에 의한 난소암 세포주의 성장억제 감수성 차이와 세포자멸사 기전을 알아보기 위해 western blot 분석법을 이용하였다. 결과: 모든 난소암 세포주에서 MISR II 발현이 관찰되었지만, 발현정도는 OVCAR-8에서 제일 강했다. 세포 생존도는 OVCAR-8만이 MIS의 처치 용량과 시간에 따라 감소하였고, OVCAR-3은 고농도 MIS로 48시간 투여한 경우에만 의의 있게 감소하였으나, SKOV-3은 반응하지 않았다. Flow cytometer로 조사한 세포주기 변화는 OVCAR-8에서 10 μg/mL MIS로 24시간 처치한 경우에 G1세포주기 정지 소견을 보였으며, 48시간 후에는 세포자멸사를 의미하는 sub G0G1분기 분획이 7.02%로 나타나, MIS는 세포주기의 변화를 먼저 일으킨 후 세포자멸사를 유발하였다. OVCAR-3에서는 48시간 후 sub G0G1분기가 3.32%였으나 SKOV-3에서는 변화가 없었다. Annexin-V-FITC 염색방법으로, OVCAR-8과 OVCAR-3에서 48시간 MIS 처치 후에 각각 8.05%와 3.67%의 세포자멸사가 관찰되었다. Western blot 분석으로, OVCAR-8에서 MIS에 의한 세포 증식 억제에 이은 세포자멸사는 p16단백 증가와 연관이 있다고 판단된다. 또한 MIS는 OVCAR-8에서 MISR II와 결합한 후 pRb 비의존적인 세포내 경로를 통해 간접적으로 E2F1 활성 증가로 세포자멸사를 유도할 가능성이 있다. 결론: 본 연구에서 MIS가 난소암 세포주에서 증식 억제와 세포자멸사를 일으키는 기전을 완전히 규명하지는 못하였지만, MIS가 MIS 수용체를 발현하는 난소암에 대하여 in vitro에서 효과적인 항종양능을 나타낸다는 사실을 확인할 수 있었다. 향후 더 많은 연구로 MIS가 MIS 수용체를 발현하는 종양의 생물학적 조절제 혹은 치료제로 개발될 것이라고 예상된다. Objective: In order to explore Mullerian inhibiting substance (MIS) effects on the ovarian neoplasia, the expression and localization of the MIS type II receptor (MISR II), the growth inhibitory effects of MIS, and the underlying molecular mechanisms were investigated in the ovarian cancer cell lines. Methods: Expression of MISR Ⅱ were studied in SKOV-3, OVCAR-3, and OVCAR-8 cell lines by immunohistochemical staining. The antiproliferative effects of MIS in these cell lines were investigated by methylthiazoletetrazolium (MTT) assay, fluorescence-activated cell sorting (FACS) analysis, annexin-V-FITC binding, and western blot analysis. Results: All cell lines showed strong specific staining for MISR II, although staining in OVCAR-8 cells was more intense than that in SKOV-3 and OVCAR-3. Treatment of OVCAR-8 cells with MIS led to a dose- and time-dependent inhibition of cell growth and survival was determined use by MTT assay. But OVCAR-3 cells exhibited growth inhibition at higher doses after 48 hours of treatment and SKOV-3 cells did not demonstrate response. Using FACS analysis, exposure of OVCAR-8 cells to MIS (71 nM) resulted in G1 arrest after 24 hours of treatment. This pattern was changed by time-dependent increase in the percentage of cells with a sub G0G1 DNA content, suggesting apoptosis, after 48 hours of treatment. These results suggested that cell death be preceded by cell cycle arrest. Time-related induction of apoptosis was also observed in this cell line as measured by annexin-V-FITC binding. In OVCAR-8 cells, the growth inhibitory effects of MIS were mediated through specific induction of CDKI p16 protein expression and via regulation of E2F1 in the absence of detectable levels of pRb. We estimated that OVCAR-3 cells were affected by MIS through p16-independent, alternative mechanistic pathways, since the growth inhibitory effects of MIS were minimal. SKOV-3 cells did not express p16 protein. Conclusion: We have demonstrated that ovarian cancer cells express the MISR II. Epithelial ovarian cancer cells respond to MIS by growth inhibition. Although the precise mechanisms of MIS mediated inhibition of ovarian cancer cell growth have not been fully defined, these data suggest that MIS has activity against ovarian cancers in vitro and may also be an effective targeted therapy for ovarian cancer.

      • APE1/Ref-1 Inhibits Phosphate-Induced Calcification and Osteoblastic Phenotype Changes in Vascular Smooth Muscle Cells

        <P>Vascular calcification plays a role in the pathogenesis of atherosclerosis, diabetes, and chronic kidney disease; however, the role of apurinic/apyrimidinic endonuclease 1/redox factor-1 (APE1/Ref-1) in inorganic phosphate (Pi)-induced vascular smooth muscle cell (VSMC) calcification remains unknown. In this study, we investigated the possible role of APE1/Ref-1 in Pi-induced VSMC calcification. We observed that Pi decreased endogenous APE1/Ref-1 expression and promoter activity in VSMCs, and that adenoviral overexpression of APE1/Ref-1 inhibited Pi-induced calcification in VSMCs and in an ex vivo organ culture of a rat aorta. However, a redox mutant of APE1/Ref-1(C65A/C93A) did not reduce Pi-induced calcification in VSMCs, suggesting APE1/Ref-1-mediated redox function against vascular calcification. Additionally, APE1/Ref-1 overexpression inhibited Pi-induced intracellular and mitochondrial reactive oxygen species production, and APE1/Ref-1 overexpression resulted in decreased Pi-induced lactate dehydrogenase activity, pro-apoptotic Bax levels, and increased anti-apoptotic Bcl-2 protein levels. Furthermore, APE1/Ref-1 inhibited Pi-induced osteoblastic differentiation associated with alkaline phosphatase activity and inhibited Pi-exposure-induced loss of the smooth muscle phenotype. Our findings provided valuable insights into the redox function of APE1/Ref-1 in preventing Pi-induced VSMC calcification by inhibiting oxidative stress and osteoblastic differentiation, resulting in prevention of altered osteoblastic phenotypes in VSMCs.</P>

        The Prevalence and Risk Factors for Hepatitis B Surface Ag Positivity in Pregnant Women in Eastern Region of Ghana

        Background/Aims: The aim of this study was to evaluate the prevalence and risk factors for hepatitis B surface antigen (HBsAg) positivity in pregnant Ghanaian women. Methods: We surveyed 1,500 pregnant women in Eastern region of Ghana. Direct interviews were performed by trained nurses using standardized questionnaires. Pregnant women were screened for human immunodefi ciency virus (HIV) and hepatitis B infections, hemoglobin levels and sickle cell anemia as part of the antenatal check-up. Results: The overall HBsAg positive rate was 10.6%, which varied among districts (13.8% for Kwahu West, 12.4% for Upper Manya, and 2.2% for Yilo Krobo). HBsAg positivity was significantly higher in women with depression (odds ratio [OR], 3.74; 95% confidence interval [CI], 2.13 to 6.57) and HIV (OR, 2.03; 95% CI, 1.06 to 3.89). Age, education, and gravidity were not related to HBsAg positivity. Anti-hepatitis B immunoglobulin for newborns of HBsAg-positive mothers is not provided at birth in public health facilities in Ghana. However, hepatitis B vaccination is provided as part of a routine vaccination schedule starting at 6 weeks of age. Conclusions: To prevent mother-tochild transmission of hepatitis B, screening tests for HBsAg in pregnant women and hepatitis B vaccination of newborns immediately after birth need to be performed in this region.         Mycobacterium marinum에 의한 피부감염증 2예

        We report two cases of cutaneous Mycobacterium(M.) marinum infections in a 43-year-old man and a 31-year-old woman who had multiple subcutaneous nodules on the left upper extremity and an erythematous ulcer on the right elbow, respectively. Skin biopsy specimens revealed chronic granulamatous inflammation and AFB positive bacilli in the dermis. The culture from tissue specimens on Lowenstein-Jensen media at 30'C for 10 days, yielded cream-colored colonies, which became yellow when exposed to light. The organism was identified as M. marinum by selected biochemical reactions. The patients were treated with surgical excision followed by administration of minocycline and administration of minocycline, respectively. (Korean J I)ermatol 1998;36(S): 932-937)

        전염병 설명 모형과 모의시험

          Modeling and simulations of infectious diseases are useful tools to estimate epidemic size and determine effective intervention methods. Models are idealized, simplified representations of reality. Therefore, a simple model should be the first design, with complex models always able to be added later to better represent reality. The following recommendations are made to ensure accurate simulations: (1) minimal feasible complexity, (2) mathematically well-defined, models should be constructed "With well characterized stochastic processes rather than "ad-hoc"behavioral models, (3) if predictions are being made, unknown parameters need to be robustly estimated using the model from epidemiological data, (4) where data is unavailable, a detailed sensitivity analysis needs to be undertaken, and (5) even if models are only being used to explore dynamics, sensitivity analysis is important to understand mechanisms.   For the effective control of infectious diseases "With models and simulations, we need effective information systems (laboratory and clinical data) for communicable disease control as a permanent infrastructure, closer collaboration between academic and service providers on new developments in communicable disease control, and enhanced monitoring of veterinary infections and "minor" zoonoses, particularly in resources poor settings.

        The hexane fraction of Naematoloma sublateritium extract suppresses the TNF-α-induced metastatic potential of MDA-MB-231 breast cancer cells through modulation of the JNK and p38 pathways.

        <P>Naematoloma sublateritium (Fr.) P. Karst is a basidiomycete that has been used as traditional medicine. N. sublateritium produces a triterpenoid antitumor compound, clavaric acid, but, in general, the effects of N. sublateritium constituents against tumor invasion and metastasis have been poorly studied. To investigate the inhibitory effect of N. sublateritium constituents on highly invasive and metastatic tumor cells, the TNF-alpha-stimulated human breast cancer cell line, MDA-MB-231 was treated with the hexane fraction of an N. sublateritium extract (HFNS). Non-cytotoxic concentrations of HFNS markedly inhibited the invasion and migration of the MDA-MB-231 cells in the Matrigel invasion assay and wound-healing analysis, respectively. Gelatin zymography showed that HFNS suppressed the activity of MMP-9, but not of MMP-2. Immunoblotting demonstrated that treatment with HFNS had decreased the level of MMP-9 and urokinase plasminogen activator-1 (uPA-1), but had upregulated expression of the endogenous inhibitor proteins, including TIMP-1,-2, and PAT-1, in a dose-dependent manner. Furthermore, HFNS suppressed the phosphorylation of p38 and JNK1/2, but not that of ERK1/2. This was confirmed by pretreatment of cells with specific inhibitors prior to stimulation with TNF-alpha. HFNS treatment also led to a dose-dependent inhibition of the DNA-binding activities of AP-1 and NF kappa B, which are downstream targets of JNK and p38. These data suggested that HFNS inhibits the metastatic potential of MDA-MB-231 cells by inhibiting the phosphorylation of JNK/p38 and reducing AP-1 and NF kappa B DNA-binding activities. Therefore, HFNS may be a potential therapeutic agent against metastasis of breast cancer.</P>
