The subjects were classified into subgroups according to sex, age, number of nails involved, nail area affected, duration of nail disease, specific diseases (e.g. onychomycosis, psoriatic nail and etc.) and groups of diseases (e.g. traumatic, infectious, and etc.). Statistical analysis was carried out using T-test, ANOVA analysis to compare the values between subgroups Results: Comparison between various disease groups showed a statistically significant higher impact for traumatic, infectious, inflammatory and structural diseases and a lower impact for pigmentary diseases. QOL was statistically significantly more affected in patients having multiple involved nails, women, widely affected nail area, longer duration of diseases and in people aged 60-79 years. There was no statistically significant difference on QOL impact between patients having only fingernails or toenails involved Conclusion: Even though published literature tends to focus on the impact of nail psoriasis and onychomycosis on QOL, other nail diseases induced similar frustration to patients. Dystrophic appearance of the nails has a significant influences on patients irrespective of diseases

Corticotropin-releasing factor(CRF) and neuropeptide Y(NPY) are known to play important roles in mediating stress responses and stress-related behavior. To elucidate the role of neuropeptides in response to the condition that had paired with traumatic event, we observed the changes of CRF and NPY by immunohistochemistry using a conditioned footshock paradigm. Male Sprague-Dawley rats were placed in a shuttle box and exposed to 20 pairings of a tone(< 70dB, 5sec) followed by a footshock(FS, 0.8mA, 1sec) over 60min. A second group was exposed to the tone-footshock pairings, returned to the homecage for 2days, and then reexposed to the test chamber and 20tones alone for 60min, prior to sacrifice. Control groups were : a) sacrificed without exposure to FS ; b) exposed to the tone-footshock pairings and then sacrificed two days later ; or c) exposed to the chamber and tones alone, returned to the homecage for 2days and then reexposed to the chamber and 20tones over 60min prior to sacrifice. CRF was increased in animals exposed to FS or the aversive condition(context and tone) that had paired to FS in bed nucleus of the stria terminalis (BNST) compared to the control. NPY was increased by FS in amygdala and PVN, but the condition previously associated with FS results in slight increase only in amygdala area. These results suggest that the BNST appears to be the mostly involved neural circuit in response to explicit cues previously paired with footshock. Moreover, this study raise the possibility that increased CRF peptide in the BNST in response to re-exposure to the aversive condition may underlie, in part, the experience of conditioned fear-related anxiety behavior.

IgG4-related disease (IgG4-RD) is a newly designated entity which is characterized by elevated serum IgG4 level and infiltration of IgG4+plasma cell in target tissue. IgG4-RD can involve various end organs including skin as well. The cutaneous manifestations of IgG4-RD are not characteristic, however it usually exhibits pruritic multiple erythematous nodules or plaques. We report a case which was considered as Kimura disease initially, however finally diagnosed as IgG4-RD. 47 year-old male patient had indurated bulging plaque on left cheek. The skin lesion appeared 3 years ago, and have been getting worse. He also had left periorbital swelling and intermittent respiratory symptom that didn’t diagnosed accurately at ophthalmology and pulmonology. Serum IgG4 level was elevated, and pathologic findings showed lots of IgG4+plasma cells infiltration with mononuclear cells in dermis. Here, we report the IgG4-RD patient who had not been diagnosed accurately without pathologic confirmation for a long time that showed dramatic improvement after treatment with systemic steroid, cyclosporin.

Terra firma forme disease (TFFD) is a little known keratinization maturation disorder that shows brownish to pigmented patches clinically. The Latin phrase ‘Terra firma’ is translated to ‘solid land’ or ‘dry land’ in current English indicating typical dirt-like skin lesion on inspection. It is understood as a keratinocyte maturation disorder, rather than a proliferation one, resulting in interrupted shedding process of differentiated keratinocyte with a prolonged intercellular adhesion. Characteristically, the lesion in TFFD is easily removed by a maneuver of scrubbing with 70% isopropyl or ethyl alcohol, simply designated as ‘alcohol swab test’. ‘Alcohol swab test’ is a simple test that can be done on routine medical in-office environment, but performing the test with high enough pressure in every patient with dirt-like pigmented patches is not rational, especially in younger childs, infants. Dermoscopy is a non-invasive method that could help to visualize microstructure and pattern which cannot be seen on nake eye examination. We herein would like to present the dermoscopic feature of Terra firma forme disease as an ancillary method in the diagnostic process.

Notalgia paresthetica (NP) is a chronic localized itch, affecting mainly the inter-scapular area especially T2-T6 dermatomes, but occasionally with a more widespread distribution, involving shoulders, back, and upper chest. There are no specific cutaneous signs, apart from those attributed to scratching and rubbing. Various etiologies have been reported, but the cause of NP is not established. The current view on etiology is that neuropathic itch due to nerve entrapment of the posterior rami of spinal nerve arising at T2-T6. Another recent documented case showed an increase in the number of intradermal nerves by neural immunochistochemistry staining of S-100 protein, protein gene product 9.5 (PGP 9.5). Herein, we experienced an uncommon case of NP of the back and tried to clarify pathogenesis by using quantitative sensory testing such as neurometer and von-frey filaments. Also, this case suggests that pregabalin may be useful as a symptomatic treatment for neuropathic itch associated with NP.

Idiopathic pulmonary fibrosis (IPF) is a progressive, fibrotic, fatal lung disease associated with inevitable loss of lung function. Pirfenidone is a novel agent for the treatment of IPF. It has some adverse effects and among them, photosensitivity has been reported infrequently in a few reports. The patient was a 74-year-old fisherman who presented with itchy erythematous patches on the face, neck, and dorsal aspects of both hands since 1 month ago. He had been diagnosed with IPF five months before and received pirfenidone therapy with gradually increasing dose until nine capsules of 200mg per day (total: 1800mg/day) with good tolerability. After 3 months of treatment, itchy erythema occurred on the face, and showed rapid exacerbation. A skin biopsy was performed on the facial lesions, and it showed spongiotic dermatitis with basophilic degeneration of upper dermis. The results of photo-patch test were consistent with photo-allergic reaction to pirfenidone. Pirfenidone therapy was discontinued and thereafter, the skin lesion showed improvement leaving post-inflammatory hyperpigmentation. Herein, we describe a patient with IPF who presented with photo-allergic contact dermatitis after administration of pirfenidone therapy. Our aim in presenting this case is to increase the awareness of clinicians regarding the potential severe photo-allergic effects of oral pirfenidone therapy.

Hypopigmented mycosis fungoides is one of atypical, rare clinical variants of mycosis fungoides (MF) characterized by solely hypopigmented to achromic patches or more commonly in combination with studding erythematous papules or plaques. The epidemiologic features that characterize hypopigmented MF from classical MF comprise its high prevalence among younger patients such as childhood and adolescence, and high skin phototype (usually Fitzpatrick skin scale IV-V). In the respect of treatment, psoralen plus ultraviolet A (PUVA) got the majority of the child patients treatment in the previously reported literatures. And the reports on the narrowband ultraviolet B phototherapy have recently been come out as a successful treatment alternative for PUV A therapy. We herein would like to present a Korean child of hypopigmented mycosis fungoides successfully treated with 308nm excimer laser which has not been reported before.
