Fiddler`s Neck Accompanied by Allergic Contact Dermatitis to Nickel in a Viola Player

Fiddler`s neck is an irritant contact dermatitis that frequently affects violin and viola players. The etiology of the skin changes associated with this condition are probably attributable to a combination of factors-including increased pressure, friction, poor hygiene, and excessive perspiration. Clinically, the lesions generally consist of a localized area of lichenification on the left side of the neck just below the angle of the jaw. Herein, we report a case of fiddler`s neck in a viola player, attended by allergic contact dermatitis to the nickel in the metal fixtures of a viola. We hope that our case report draws the attention of dermatologists toward this, and many other skin problems that affect musicians. (Ann Dermatol 22(1) 88∼90, 2010)

Background: Acne is a chronic inflammatory disease of the pilosebaceous unit and usually affects adolescents when the peak concentrations of growth hormone, insulin-like growth factor 1, and androgen are demonstrated. The activity of alkaline phosphatase (ALP), which increases physiologically in growing children and adolescents, in the pilosebaceous unit has been reported. However, the correlation between the serum level of ALP and the number of acne lesions has not been studied. Objective: The present cross-sectional study was designed to evaluate the correlation between serum level of ALP and the numbers of non-inflammatory and inflammatory acne lesions in children and adolescents. Methods: For this study, 202 pediatric and adolescent patients clinically diagnosed with acne vulgaris were included. Age, sex, serum level of ALP, number of non-inflammatory acne lesions, number of inflammatory acne lesions, and number of total acne lesions were evaluated. Additionally, the serum level of dehydroepiandrosterone sulfate was evaluated in 117 patients. Multiple regression analysis was performed. Multicollinearity was quantified using the variance inflation factor. Results: In the 202 patients, serum level of ALP was the only independent factor that significantly affected both the number of non-inflammatory acne lesions and of total acne lesions (regression coefficient=0.089 and 0.086, respectively, p<0.001). Conclusion: There was a significant correlation between serum level of ALP and the extent of acne (non-inflammatory acne lesions and total acne lesions). (Ann Dermatol 32(3) 206∼212, 2020)
Childhood Lichen Planus with Palmoplantar Involvement

Lichen planus (LP) commonly involves the flexor aspects of the wrists, legs, and oral and genital mucous membranes. But it rarely occurs on the palms and/or soles. It mainly affects people in the age range 30∼60 years. Childhood LP is reported to constitute only 1∼4% of total cases of LP. In the literature, a few cases of LP on the palms and soles of pediatric patients have been reported. Here we report an interesting case of childhood LP with palmoplantar involvement. The patient was a 7-year-old boy who for 6 months had variously sized and shaped, pruritic, violaceous, polygonal papules and plaques on the whole body. The skin biopsy specimens taken from four sites of the patient (dorsum of the hand and foot, sole and chest) showed typical features of LP on histopathology. A diagnosis of generalized LP with palmoplantar involvement was made. (Ann Dermatol 22(1) 51∼53, 2010)
The Lateral Tarsal Strip for Paralytic Ectropion in Patients with Leprosy

Background: In patients with leprosy, paralysis of the facial nerve results in the lower eyelid ectropion and lagophthalmos as a sequela even when the leprosy is cured. Paralytic ectropion causes many functional and cosmetic eye problems, leading to blindness if left untreated. Objective: The purpose of this retrospective study is to evaluate the efficacy of surgical correction of paralytic ectropion, the lateral tarsal strip, in patients with leprosy. Methods: Between 2010 and 2015, 40 Korean patients (44 eyelids) with paralytic ectropion who had visited Korean Hansen Welfare Association Hospital were treated with the lateral tarsal strip. Four-point patients’ global assessment scale, local complications, and recurrence were assessed at the end of follow-up period. The average follow-up period was 12 months. Results: In the 44 eyelids, recurrence was observed in 5 cases (5/44, 11.4%). There were no serious postoperative complications except mild size discrepancy of both eyes. Most patients were satisfied with the results and mean satisfaction scale was 2.6/3. Conclusion: The lateral tarsal strip is a simple, safe, and effective treatment method for the dermatologic surgeon to correct paralytic ectropion of mild to moderate degree in patients with leprosy. Kaposi sarcoma is a lympho-angioproliferative disease of the skin and viscera. Because the natural history of Kaposi sarcoma is variable, the assessment of therapy may be difficult and optimal therapy has yet to be determined. Pentoxifylline has been used to treat various dermatologic diseases, but its effectiveness in Kaposi sarcoma has not been reported. We now report a case of Kaposi sarcoma in a 76-year-old man who improved after treatment with oral pentoxifylline. (Korean J Dermatol 2009;47(7):811~814)
Basal cell carcinoma at the umbilicus

Basal cell carcinoma (BCC) is a common malignant skin tumor. Risk factors for development of BCC compose of sun exposure, radiation therapy, family history of BCC, immunosuppression, genetic syndromes, and a fair complexion. Occasionally, BCC can also occur in sun-protected sites such as axilla or genitalia. However, BCC that arises at the umbilicus is rare. Few cases have been reported about the metastatic tumor on the umbilicus, which is often called Sister Mary Joseph``s node. It is mainly metastases from the stomach, large and small bowel, and the ovary as a result of primary adenocarcionoma. However, primary skin cancer on the umbilicus is rarely reported in the literature. Herein, we report a rare case of primary basal cell carcinoma on the umbilicus with no prior history of malignancy. The patient was a 60-year-old male and complained of pigmented plaque with surface ulceration on the umbilicus which first appeared six years ago. He reported that he had frequently scratched and irritated the umbilicus during baths. And the lesion aggravated a year ago with intermittent bleeding and discharge. The biopsy from the umbilicus confirmed basal cell carcinoma and the tumor was surgically removed. PET-CT showed no lymph node invasion or any signs of metastasis.
