      • Cisplatin 병용화학요법시 발생한 오심, 구토에 대한 Ondansetron과 Metoclopramide의 효과에 대한 비교

        Inspite of possible effects for emesis following chemotherapy including cisplatin, nausea and vomiting are the most unpleasant side effects of cancer chemotherapy. None of the currently available antiemetic agents is entirely effective preventing emesis. Ondansetron, a 5-HT(5-hydroxytryptamine)_(3) receptor antagonist, has been shown to be effective antiemetic agent in the control of cisplatin induced emesis. Twenty solid tumor patients who are scheduled to receive cisplatin containing combination chemotherapy participated in a prospectively randomized study to compare the antiemetic efficacy and safety of ondansetron and metoclopramide. Ondansetron was given in dose of 8mg intravenous 30min before cisplatin and then 4hours and 8hours after cisplatin on day 1, and Ⅰ.Ⅴ. every 8hours from day 2 to 5. In the metoclopramide group, metoclopramide was given in dose of 2 ㎎/㎏ intravenous every 8hours from day 1 to 5. The control of emesis was graded in the following way : complete response, no vomiting : major response, 1-2 emetic episodes : minor response, 3-5 emetic episodes : failure, above 5 emetic episodes. Complete or major response of emesis was achieved 10 of 12(83.2%) patients receiving ondansetron and in 5 of 8(62.5%) patients receiving metoclopramide, but effectiveness was not significantly different. Ondansetron was at least as effective as metoclopramide therapy in controlling cisplatin induced emesis. Side effects in ondansetron was less than in Metoclopramide.

      • RAPD 표지자 분석에 의한 가시아메바속 한국분리주의 유전적 지위

        가시아메바 속(Acanthamoeba spp.)의 DNA 염기 구성 정보와 관계없이 임의의 10개의 염기로 구성된 프라이머를 사용하여 random amplified polymorphic DNA-polymerase chain reaction(RAPD-PCR)에 의해 게놈 상의 DNA를 무작위로 증폭하여 확인된 표지자로써 한국 분리주 및 외국 분리주와 기존의 알려진 4개 가시아메바 종간의 유전적 근연관계 분석을 통해서 4개 한국분리주의 분류상의 성상을 규명하였다. 본 연구에서 A. culbertsoni, A. hatchetti, A. triangularis, A. polyphaga와 한국 분리주인 YM-2, YM-3, YM-4, YM-5, 그리고 외국 분리주인 HOV의 게놈 DNA는 18종류의 프라이머에 의하여 다양한 양상의 증폭산물을 보였으며, 그 중 9개 프라이머는 한국 분리주간에도 특이성을 보이는 RAPD 표지자를 제공하였다. 총 18개의 프라이머에 대한 증폭산물을 대상으로 각 시료의 유사도를 조사한 결과, A. culbertsoni는 A. hatchetti, A. triangularis, A. polyphaga와 유사도가 각각 0.300, 0.308, 0.313이었고, A. hatchetti와 A. triangularis간의 유사도는 0.838이었다. 한국 분리주 YM-2, -3, -4간의 평균 유사도는 0.959이었고, YM-2, -3, -4와 A. hatchetti, A. triangularis간의 평균 유사도는 0.832이었다. 한국 분리주 YM-5는 YM-2, -3, -4 간의 비교에서 평균 0.237의 유사도를 보인 반면, A. culbertsoni와 유사도 0.857을 보여, 다른 한국 분리주보다 A. culbertsoni와 유전적으로 유사함을 알 수 있었다. UPGMA법에 의한 유전적 근연관계 분석 결과 phenogram 상에 두개의 분지군이 존재하는데, A. hatchetti, A. triangularis 및 3개 한국 분리주 (YM-2, -3, -4)가 하나의 분지군을, A. culbertsoni, A. polyphaga, HOV주, 및 YM-5가 다른 분지군을 형성하는 것으로 나타났다. 게놈 DNA 상의 변이에 근거하여 볼 때, YM-5 주는 유전적으로 A. culbertsoni와 거의 유사한 분리주이며 한국 분리주는 최소 2종 이상의 가시아메바로 분류할 수 있다고 사료된다. Genetic status of Acanthamoeba spp. were tested on the basis of random amplified polymorphic DNA(RAPD) marker analysis. Four previously established Acanthamoeba species. 4 Korean isolates of Acanthamoeba sp.. and one American isolate of Acanthamoeba sp. were analyzed by RAPD-PCR using an arbitrary decamer primers. Amplification products were fractionated by agarose gel electrophoresis and stained by ethidium bromide. Eighteen primers producted DNA amplification profiles revealing clear differences among 4 species. Nie of them also produced DNA amplification profiles which included some isolate-specific amplification products. On the basis of amplified fragments by 18 primers, the pairwise similarity indices between A. culbertsoni and other species (i.e., A. hatchetti, A. triangularis, A. polyphaga) were 0.300, 0.308, and 0.313, respectively. Similarity index between A. hatchetti and A. triangularis was 0.833. The mean similarity index among the 3 Korean isolates (YM-2,-3,-4) was 0.959 and 0.832 among them and 2 other species (A. hatchetti and A. triangularis). The mean similarity index among YM-5 and other Korean isolates (YM-2,-3,-4) was 0.237. However, the similarity index between YM-5 and A. culbertsoni was 0.857, which suggests that YM-5 is genetically more similar to A. culbertsoni than other Korean isolates. Phenogram reconstructed by UPGMA method revealed that there are two groups: one group consists of A. hatchetti, A. triangularis, and 3 Korean isolates (YM-2,-3,-4), and the other group consists of A. culbertsoni, A. polyphaga, HOV, and YM-5.

        일 농촌지역의 우울 증상에 관한 역학연구 -위험요인 및 요인구조분석

        연구목적 : 세계적으로 널리 사용되는 우울증 척도인 the Center for Epidemiologic Studies Depression Scale(CES-D)을 이용하여 지역사회 주민의 우울증상의 정도를 평가하고, 우울증상의 위험요인을 분석하고, 한국인에서 우울증상의 요인구조를 알기 위해 본 연구를 실시하였다. 방 법 : 경기도 일 농촌 지역의 30세 이상 주민에 대해 무작위 군집 집락 추출을 실시하였고, 훈련된 면담원들이 가정 방문을 통하여 면접 및 자기보고 검사인 CES-D를 실시하였다. 총 1,315명에서 응답을 하였고 응답율은 59.3%이었다. 결 과 : CES-D에 의한 우울증 점수는 0∼5점이 57%정도를 차지하였고, 평균 점수는 남자 7.8점 여자 9.5점으로 비교적 낮은 편이었다. ANCOVA 분석 결과 비결혼, 저학력, 여성이라는 위험요인은 독립적으로 우울증상을 악화시키는 요인이었으나 연령은 통계적으로 유의하지 않았다. CES-D 결과를 요인분석을 시행한 결과 4가지 요인이 추출되었는데 우울증상 및 신체증상이 한 개의 요인으로, 다른 나라에서 보고되지 않는 정서적 고통이 별도의 요인으로 추출되었다. 결 론 : 국내 역학 조사에서도 외국과 유사한 우울점수 및 위험요인이 확인되었으나 우울증의 요인에서는 특이한 요인이 추출되었다. 한국인에 독특한 우울 반응에 관한 추가 연구가 필요하다. Objectives :The rates, correlates, and factor structure of depressive symptoms in a Korean rural sample were examined using Korean version of the Center for Epidemiologic Studies Depression Scale(CES-D). Methods : Door to door visiting survey was conducted by trained interviewers. A random cluster sample of 1,315 respondents, aged 30 and over completed the CES-D scale along with additional sociodemographic questionnaire. Results : In this sample, mean CES-D score of male and female were 7.8 and 9.5, respectively which were lower than those reported previously. Symptoms of depression were most common among the less educated, non-married and female according to ANCOVA. Age was not a significant risk factor for depressive symptoms. Different factor structures emerged from this sample as compared to other studies using this approach. Somatic symptoms and affective symptoms were combined as one factor and emotional hardship emerged as a separate factor. Conclusion : Symptoms scores and risk factors were similar to the results reported in other countries but unique factor structures were found in this study. Further research is needed about the Korean depressive symptomatology and related areas.

        Anti-Platelet Effect of the Constituents Isolated from the Barks and Fruits of Magnolia obovata

        In the course of our work on anti-platelet constituents from plants, five phenolic compounds, magnolol, honokiol, obovatol, methyl caffeate, and syringin, were isolated from the methanol extracts of the barks and fruits of Magnolia obovata. The compounds were identified based on the spectroscopic data. Methyl caffeate was isolated for the first time from the genus Magnolia and it showed 3∼4-folds higher potency than ASA. The activities of obovatol and honokiol were comparable to ASA. Magnolol and syringin showed only very mild inhibitory effects to all the stimulators.

        Novel Glycolipoproteins from Ginseng

        Ginseng has been used as a general tonic agent to invigorate human body. In the present study, we isolated novel glycolipoproteins from ginseng that activate $Ca^{2+}$-activated $Cl^-$ channel (CaCC) in Xenopus oocytes and transiently increase intracellular free $Ca^{2+}$ concentration ($[Ca^{2+}]_i$) in mouse Ehrlich ascites tumor cells. We named the active ingredients as gintonin. Gintonin exists in at least six different forms. The native molecular weight of gintonin is about 67 kDa but its apparent molecular weight is about 13 kDa, indicating that gintonin might be a pentamer. Gintonin is rich in hydrophobic amino acids. Its main carbohydrates are glucose and glucosamine. Its lipid components are linoleic, palmitic, oleic, and stearic acids. Gintonin actions were blocked by U73122, a phospholipase C inhibitor, 2-aminoethxydiphenyl borate, an inositol 1,4,5-trisphosphate receptor antagonist, or bis (o-aminophenoxy) ethane-N,N,N0,N0-tetracetic acid acetoxymethyl ester, a membrane permeable $Ca^{2+}$ chelator. In the present study, we for the first time isolated novel gintonin and showed the signaling pathways on gintonin-mediated CaCC activations and transient increase of $[Ca^{2+}]_i$. Since $[Ca^{2+}]_i$ as a second messenger plays a pivotal role in the regulation of diverse $Ca^{2+}$-dependent intracellular signal pathways, gintonin-mediated regulations of $[Ca^{2+}]_i$ might contribute to biological actions of ginseng.

        Isolation and Anti-thrombotic Activity of Citric acid 1,5-dimethyl ester from Gastrodia elata

        In the course of continous work for the discovery of anti-thrombotic constituents from tubers of Gastrodia elata, citric acid 1,5-dimethyl ester was isolated from ethylacetate soluble fraction prepared from the methanol extract. The structure of the compound was determined by the spectroscopic data. The anti-thrombotic effect of this compound was observed with prolonging the bleeding time in thrombin-induced thrombosis model of mice.
