Primary unruptured appendiceal mucocele is a rare disease. It is very difficult to diagnose preoperatively because of the rarity of classical symptoms and low incidence. Usually it is discovered incidentally during abdominal ultrasonography(US), computed tomography(CT), or appendectomy for acute appendicitis. If untreated, one type of mucoceles may rupture producing a potentially fatal entity known as pseudomyxoma peritonei, therefore preoperative diagnosis is very important. With the advent of colonoscopy, abdominal US and CT, it has been possible to preoperatively diagnose mucocele of appendix. We experienced a case of appendiceal mucocele in a 60 year-old woman who had symptoms of lower abdominal discomfort and right lower quadrant abdominal pain, and it was diagnosed preoperatively by colonoscopy, abdominal US and CT. So we present this case with review of literature.
당뇨병환자에서 뇨증 Fibronectin농도의 증가에 관한 연구

Plasma fibronectin is an α₂-glyoprotein, which is produced by vascular endothelial cells. Raised level of plasma fibronectin has been observed in diabetic patients particularly in the presence of microvascular complications. However, no available data exist about urine level of this glycoprotein in diabetic patients. We measured urine fibronectin level by ELISA methods in 54 diabetic patients who have microvascular comlications or not. The following results were obtained. 1) Urine fibronectin level(㎍/g creatinine) in diabetic group(1740.0±678.0) is increased compaired with those in normal control group(471.0±59.0). but this results were not significantly different among two groups(P>0.05). 2) Urine fibronectin level(㎍/g creatinine) was significantly increased in patients with nephropathy (6188.0±3144.0) compared with those in normal control group(471.0±59.0) and patients without nephropathy(645.0±251.0)(P<0.001). 3) There were a significant correlation between BUN, creatinine, creatinine clearance, 24hr urine total protein and urine fibronectin level in diabetic patients. Our data suggest that urine fibronectin excretion level might be used as a sensitive guide for diabectic nephropathy.

Mastoparan B (MPB)는 벌독으로부터 정제된 양친매성α-helical 구조를 취하면서 14개의 아미노산 잔기로 구성된 염기성 항균성 펩타이드로서 여러 가지 생물막과 작용한다. 본 연구에서는 우리 나라 연안의 적조 (HABs, harmful algal blooms)를 일으키는 4종의 적조생물 (Alexandrium tamarense, Chattonella catenatum)에 대한 MPB의 살조효과를 조사하였다. MPB의 4종의 적조생물에 대한 살조효과는 31.3㎍/㎖에서 세포의 lysis또는 ecdysis와 같은 형태로 현미경으로 관찰할 수 있었다. 또한 MPB는 C. marina 및 C. polykridorides에 대해서 A. tamarense와 G. catenatum보다 더욱 강한 살조효과를 나타내었다. 이러한 MPB의 HABs에 대한 살조효과연구는 새로운 살조물질을 개발하기 위한 자료가 될 것으로 생각된다. Mastoparan B (MPB), an antimicrobial cationic peptide isolated from the venom of the hornet Vespa basalis, is a basic amphipathic a-helical peptide composed of fourteen amino acid residues. In this study, we have investigated the algicidal effect of MPB against hatmful algae blooms (HABs) casative Alexandrium tamarense, Chattonella marina, Cochlodinium polykrikoides and Gymnodinium catenatum. The algicidal effect of MPB showed in the concentration of 31.3 ㎍/㎖ to 500 ㎍/㎖ against 4 HAB species and observed cell lysis or cell ecdysis by microscopy. MPB reacted more sensitive to C. marina and C.polykrikoides than A. tamarense and G. catenatum. The algicidal study of MPB against HABs will provides much insight into development of new algicidal substances.
Lymphocyte-to-monocyte ratio on day 7 is associated with outcomes in acute ischemic stroke

<P>The main features of stroke-induced immunosuppression are lymphopenia and deactivation of monocytes in peripheral blood. We hypothesized that lymphocyte-to-monocyte ratio (LMR) in peripheral blood may represent the degree of stroke-induced immunosuppression. To prove this hypothesis, we evaluated whether LMR is associated with risk of post-stroke infection and clinical outcome at 3 months in patients with acute ischemic stroke. We selected patients with stroke in anterior circulation within 24 h from onset. Peripheral blood sampling for differential blood count was performed on days 1 and 7. The LMRs on days 1 and 7 were analyzed to determine associations with excellent outcomes (modified Rankin Scale of score 0-1 at 3 months). One hundred and two patients were included. The initial National Institutes of Health Stroke Scale score (adjusted odd ratio [OR] 0.89; 95% confidence interval [CI], 0.83-0.95; P = 0.001) and LMR on day 7 (adjusted OR 1.49; 95% CI, 1.09-2.02; P = 0.011) were associated with excellent outcomes. LMRs on day 1 were significantly lower in stroke patients with pneumonia (P = 0.007) and pneumonia or urinary tract infection (P = 0.012) than those without infections. LMRs on day 7 were also significantly lower in stroke patients with infection (P = 0.005 in pneumonia, P = 0.003 in urinary tract infection, and P < 0.001 in pneumonia or urinary tract infection) than those without infections. Lower LMRs on day 7 are associated with worse outcomes at 3 months after stroke onset. LMR may be a useful marker for assessing the stroke-induced immunosuppression.</P>

Quantitative trait locus (QTL) mapping is a highly effective approach for studying genetically complex forms of plant shattering. With QTLs mapping, the shattering loci can be described. SSR marker is based on the imformation of Simple Sequence Repeat and easy to analyze using PCR and has high reproducibility. For analyzing QTLs associated with shattering, we selected 219 SSR markers from 254 SSR markers and used them for implementing Mapmaker(Ver. 3.0) and Mapchart(Ver. 2.2). Mapmaker help to calculate distances between each markers and Mapchart is a program for drawing Genetic map. This Genetic map of rice (Oryza sativa L.) covering 2082.4 cM with 9.5 cM between makers in the Kosambi function has been constructed using 120 F1 DH plants from a single cross between the indica variety Chungchung and the japonica variety Nagdong.

<P>Prolonged elevations of glucose concentration have deleterious effects on beta-cell function. One of the hallmarks of such glucotoxicity is a reduction in insulin gene expression, resulting from decreased insulin promoter activity. Small heterodimer partner (SHP; NR0B2) is an atypical orphan nuclear receptor that inhibits nuclear receptor signaling in diverse metabolic pathways. In this study, we found that sustained culture of INS-1 cells at high glucose concentrations leads to an increase in SHP mRNA expression, followed by a decrease in insulin gene expression. Inhibition of endogenous SHP gene expression by small interfering RNA partially restored high-glucose-induced suppression of the insulin gene. Adenovirus-mediated overexpression of SHP in INS-1 cells impaired glucose-stimulated insulin secretion as well as insulin gene expression. SHP downregulates insulin gene expression via two mechanisms: by downregulating PDX-1 and MafA gene expression and by inhibiting p300-mediated pancreatic duodenal homeobox factor 1-and BETA2-dependent transcriptional activity from the insulin promoter. Finally, the pancreatic islets of diabetic OLETF rats express SHP mRNA at higher levels than the islets from LETO rats. These results collectively suggest that SHP plays an important role in the development of beta-cell dysfunction induced by glucotoxicity.</P>
PDK4 Deficiency Suppresses Hepatic Glucagon Signaling by Decreasing cAMP Levels

<P>In fasting or diabetes, gluconeogenic genes are transcriptionally activated by glucagon stimulation of the cAMP-protein kinase A (PKA)-CREB signaling pathway. Previous work showed pyruvate dehydrogenase kinase (PDK) inhibition in skeletal muscle increases pyruvate oxidation, which limits the availability of gluconeogenic substrates in the liver. However, this study found upregulation of hepatic PDK4 promoted glucagon-mediated expression of gluconeogenic genes, whereas knockdown or inhibition of hepatic PDK4 caused the opposite effect on gluconeogenic gene expression and decreased hepatic glucose production. Mechanistically, PDK4 deficiency decreased ATP levels, thus increasing phosphorylated AMPK (p-AMPK), which increased p-AMPK-sensitive phosphorylation of cyclic nucleotide phosphodiesterase 4B (p-PDE4B). This reduced cAMP levels and consequently p-CREB. Metabolic flux analysis showed that the reduction in ATP was a consequence of a diminished rate of fatty acid oxidation (FAO). However, overexpression of PDK4 increased FAO and increased ATP levels, which decreased p-AMPK and p-PDE4B and allowed greater accumulation of cAMP and p-CREB. The latter were abrogated by the FAO inhibitor etomoxir, suggesting a critical role for PDK4 in FAO stimulation and the regulation of cAMP levels. This finding strengthens the possibility of PDK4 as a target against diabetes.</P>
