        Spontaneous retroperitoneal hemorrhage due to hemophilia A with impaired coagulopathy is very rare. Spontaneous retroperitoneal hemorrhage has been recorded as having originated from many retroperitoneal organs and blood vessels, and it may be due to local and/or systemic factors. In the majority of the patients, kidney and adrenal gland were the major site of hemorrhage. The systemic causes of spontaneous retroperitoneal hemorrhage are anticoagulation therapy and chronic hemodialysis. During the course of these treatments, hemorrhagic complications may occur at many site, including the retroperitoneal space. Blood dyscrasias including leukemia, polycythemia, sickle cell trait and hemophilia have been reported associated with spontaneous retroperitoneal hemorrhage.         Fibrous Dysplasia Associated with Primary Hyperparathyroidism Absent of McCune-Albright Syndrome : Tc-99m MIBI and Tc-99m MDP Findings

        섬유성 골이형성증은 비정상적인 섬유성 골조직으로 대체되며 느리게 진행하는 골병변이다. 섬유성 골이형성증과 부갑상선기능항진증은 흔하게 관찰되는 질환이나 McCune-Albright 증후군 없이 일차성 부갑상선기능항진증에 동반된 섬유성 골 이형성증은 거의 보고된바가 없다. Tc-99m MDP 골스캔이 섬유성 골이형성증의 진단에 유용하다고 알려져 있으나, Tc-99m MIBI 영상은 아직 보고된 바가 없다. 저자들은 McCune-Albright 증후군이 없이 부갑상선기능항진증에 동반된 섬유성 골 이행성증의 Tc-99m MIBI 스캔과 Tc-99m MDP 골스캔의 영상을 비교하였다. Tc-99m MDP 골스캔상 병변 부위에서 증가된 섭취 소견을 보였으며, Tc-99m MIBI 조기영상에서도 동일한 위치에서 섭취증가가 관찰되었다. 2시간 지연 Tc-99m MIBI 영상에서는 Tc-99m MIBI의 섭취가 배출되는 소견을 보였다. 섬유성 골이형성증 병변에서 Tc-99m MIBI 스캔이 유용할 것이라고 생각하며, 섬유성 골이형성증 병변에서 Tc-99m MIBI의 섭취를 증가시키는 인자에 대한 더 많은 연구가 필요할 것으로 생각한다.

        Associations of serotonergic genes with poststroke emotional incontinence

        <P><B>Objectives</B></P><P>Poststroke emotional incontinence (PSEI) has been associated with serotonergic dysfunction. Polymorphisms of serotonin transporter (5‐HTT) and serotonin 2a receptor (5‐HTR2a) genes may regulate serotonergic signaling at brain synapses, and this study was to investigate associations with PSEI in an East Asian population.</P><P><B>Methods</B></P><P>In 276 stroke cases, PSEI was diagnosed by Kim's criteria. Covariates included age, gender, education, history of depression or stroke, current depression, and stroke severity and location. Genotypes were ascertained for 5‐HTT gene‐linked promoter region (5‐HTTLPR), serotonin transporter intron 2 variable number tandem repeat, 5‐HTR2a 1438A/G, and 5‐HTR2a 102 T/C. Associations with PSEI were estimated by using logistic regression models, and gene–gene interactions were investigated by using the generalized multifactor dimensionality reduction method.</P><P><B>Results</B></P><P>PSEI was present in 37 (13.4%) patients. The 5‐HTT gene‐linked promoter region <I>s</I>/<I>s</I> genotype was independently associated with PSEI. No associations with STin2 VNTR and 5‐HTR2a genes were found, and no significant gene–gene interactions were identified.</P><P><B>Conclusions</B></P><P>Stroke patients with 5‐HTTLPR <I>s</I> allele had higher susceptibility to PSEI, which underlines the potential role of serotonergic pathways in its etiology.         원발성 유방암에서 Sodium Iodide Symporter의 발현과 ^(99m)Tc-MIBI 유방스캔의 관계

        목적: hNIS는 갑상선 조직 외에 다른 조직들에서도 발현된다고 알려져 있다. 유방암 세포는 그런 조직들 중에 하나이며, 그에 의한 유방암의 방사성옥소치료의 가능성이 제시되고 있다. 본 연구에서는 유방암 조직에서 hNIS의 발현정도를 알아보고, ^99mTc-MIBI 유방스캔과 hNIS의 발현 정도와의 관계를 조사하여 보았다. 대상 및 방법: 본 연구에서는 유방암으로 수술을 시행했던 56명의 환자를 대상으로 하였다. hNIS의 발현은 면역조직화학염색에 의해서 평가되었으며, 그 결과를 ^99mTc-MIBI 유방스캔의 판정 결과와 비교하였다. 결과: 전체 56명의 환자에서 hNIS의 발현율은 41.1%였다. 병리학적 진단에 의해 침윤성관상피암종이었던 49명에서의 발현율은 42.9%, 관상피내암종이었던 7명에서의 발현율은 28.6%였다. ^99mTc-MIBI 유방스캔에서 국소섭취 소견이 관찰되었던 41명에서의 hNIS의 발현율은 31.7%였다. 비정상적인 섭취 소견이 관찰되지 않았던 15명의 환자에서의 hNIS의 발현율이 의미있게 높게 관찰되었다(66.7%, p>0.05). 결론: 유방암 환자에서의 hNIS의 발현율은 그다지 높지 않았다. ^99mTc-MIBI 유방스캔에서 섭취증가 소견이 없을 때 hNIS의 발현율이 더 높았다. Purpose: Human Na^+/I- symporter (hNIS) is known to be expressed in many tissues other than thyroid gland. The breast cancer cells are one of them and the possibility of radioiodine therapy in treatment of the breast cancer may be suggested. We investigated the expression rate of hNIS and the relationship between the expression of hNIS and the finding of 99mTc-MIBI scintimammography in the breast cancer. Materials and Methods: Surgically proved 56 patients with breast cancer were the subjects of this study. The expression of hNIS were evaluated by immunohistochemistry and the results were compared to the findings of 99mTc-MIBI scintimammography. Results: Overall expression rate of hNIS was 41.1% in 56 patients. According to the pathologic diagnosis, it was 42.9% in 49 patients with invasive ductal carcinoma and 28.6% in the 7 patients with ductal carcinoma in situ. The expression rate of hNIS in the 41 cases with a focal increased uptake at he breast lesion on 99mTc-MIBI scintimammogram was 31.7%. That in the 15 cases without any abnormal uptake on the scan was significantly higher(66.7%, p<0.05). Conclusion: The expression rate of hNIS in the patients with breast cancer was not so high. The rate was higher in the patients with no increased uptake at the breast lesion on 99mTc-MIBI scintimammography. (Korean J Nucl Med 2002;36;325-32)

      • S-542 Indomethacin aggravates the renal injury by inhibition of adenosine-medited renal protection in AKI

        Background: Ischemia-reperfusion injury (IRI) is a leading cause of acute kidney injury with high morbidity and mortality due to limited therapy. AKI emerges in various clinical settings and is complex with outcome linking oxidative stress, inflammation, and cell death. Therefore protection of AKI is still an unsolved problem. Indomethacin is generally known that it inhibits the production of prostaglandins through the inhibition both cyclooxygenase (COX) 1 and 2. Prostaglandins have a wide variety of effect such as regulation of vasodilation, inflammation, regeneration, pain, fever. Therefore effect of indomethacin in AKI is different according to injury model. We investigated whether indomethacin which inhibits the production of prostaglandins aggravate the renal injury in AKI mouse model. Methods: Male C57/BL6 mice (8-10 weeks old, weight 20~25 g) were used. Acute kidney injury is induced by bilateral kidneys pedicle clamping which were subjected to 20 min or 30 min at both kidneys. Mice were treated with indomethacin at before and after injury. Blood and kidney samples were collected at 24 hr after IRI. The expression level of creatinine, N-gal & Kim-1 were detected in serum. And the expression level of PGE2, cAMP and adenosine were detected in kidney. Kidney Injury score were measured by HE staining and TUNEL. Results: In bilateral AKI model, Serum NGAL level and creatinine level were significantly highest in indomethacin treated group compared to non-treated group (NGAL, p<0.05; creatinine, p<0.01). Indomethacin treated group showed significantly more necrosis and apoptosis compared to non-treated group. Furthermore, Indomethacine inhibited the production of prostaglandins, cAMP and adenosine. Conclusions: Indomethacin inhibits adenosine-mediated renal protection by inhibition of prostaglandin production in AKI.         『고산유고』 소재 시조(時調)의 구조(句調) 통계 및 분석

        고산 윤선도의 『고산유고』소재 시조 35수의 구조(句調)를 통계ㆍ분석해 본 결과, 각 구(句)의 주조(主調)를 이루는 구조는, 초장(初章)의 경우 전구(前句)는 3ㆍ4조, 후구(後句)는 3ㆍ4조와 4ㆍ4조였고, 중장(中章)의 경우 전구(前句)는 3ㆍ4조, 후구(後句)는 4ㆍ4조와 3ㆍ4조였으며, 종장(終章)의 경우 전구(前句)는 3ㆍ5조와 3ㆍ6조, 후구(後句)는 4ㆍ3조였다. 고산 시조의 구조 형성 양상을 살필 때 음수 ‘1음’과 ‘6음’은 절(節)을 이루는 구성 음수가 아니다. 그러므로 구조를 형성하는 ‘절음범주(節音範疇)’는 ‘2음’부터 ‘5음’까지가 된다. 다만 종장 제1절은 ‘3음’ 고정화(固定化) 양상을 보이며 종장 제2절은 결합절(結合節)의 양상을 보인다. 고산 시조의 구조의 시작점과 끝점을 살펴보면 2ㆍ2조와 5ㆍ5조는 나타나지 않는다. 즉 시작점은 2ㆍ3조이며 끝점은 4ㆍ5조, 5ㆍ4조이다. 다만 2ㆍ3조가, 특수하게 중장 전구에서만 나타나는 분할구(分割句)임을 감안하면 ‘구음범주(句音範疇)’는 ‘6음’부터 ‘9음’까지라고 할 수 있다. 고산 시조의 통계?분석을 통하여 ‘구조의 율형(律型)’을 확인한다. 다시 말해서 구조를 이루는 전절(前節)과 후절(後節) 사이의 ‘+, 0, -’의 수차(數差)에 의해서 발현되는 순조(順調), 평조(平調), 역조(逆調)를 발견한다. 순조는 ‘생성ㆍ변화’, 평조는 ‘유지ㆍ안정’, 역조는 ‘종결ㆍ변화’로서의 율성(律性)을 보여준다. This paper is statistics and analysis on ‘Gujo(句?)’ of ‘Sijo(??)’ in 『Gosanyugo』. The ‘Jujo(主調)’ of each ‘Guwi(句位)’ are as follows: ‘Chojang(初章) Jeongu(前句)’ 3ㆍ4jo(3ㆍ4調), ‘Chojang(初章) Hugu(後句)’ 3ㆍ4jo(3ㆍ4調) and 4ㆍ4jo(4ㆍ4調), ‘Jungjang(中章) Jeongu(前句)’ 3ㆍ4jo(3ㆍ4調), ‘Jungjang(中章) Hugu(後句)’ 4ㆍ4jo(4ㆍ4調) and 3ㆍ4jo(3ㆍ4調), ‘Jongjang(終章) Jeongu(前句)’ 3ㆍ5jo(3ㆍ5調) and 3ㆍ6jo(3?6調), ‘Jongjang(終章) Hugu(後句)’ 4ㆍ3jo(4ㆍ3調). In aspects of Gujo formation of Gosan(孤山)"s Sijo, ‘one syllable’ and ‘six syllables’ can not make up the ‘Jeol(節)’. In other words, ‘Jeoleum(節音)’ is ‘2∼5’. However, ‘three syllables’ of ‘Jongjang(終章) Jeongu(前句) Jeonjeol(前節)’ is immobilized(固定化). And ‘Jongjang(終章) Jeongu(前句) Hujeol(後節)’ is ‘Gyeolhapjeol(?合?)’. In aspects of Gujo formation of Gosan(孤山)"s Sijo, ‘2ㆍ2jo(調)’ and ‘5ㆍ5jo(調)’ can not make up the ‘Gu(句)’. The starting point is ‘2ㆍ3jo(調)’. The endpoint is ‘4ㆍ5jo(調)’ and ‘5ㆍ4jo(調)’. ‘2ㆍ3jo(調)’ is ‘Bunhalgu(分割句)’. ‘Bunhalgu(分割句)’ is the ‘Jungjang(中章) Jeongu(前句)’ appears only. In other words, ‘Gueum(句音)’ is ‘6∼9’. This was confirmed through analysis ‘metrical pattern(律型) of Gujo(句?)’. This study finds ‘Sunjo(順?)’ and ‘Pyeongjo(平?)’ and ‘Yeokjo(逆?)’. This is expressed by ‘+, 0, -’ of the quantity difference(?差) in ‘Jeolgan(節間)’ of ‘Gujo(句調)’. Metrical feature(律性) of ‘Sunjo(順?)’ is ‘creation?change(生成?變化)’. Metrical feature(律性) of ‘Pyeongjo(平?)’ is ‘keeping?stability(維持?安定)’. Metrical feature(律性) of ‘Yeokjo(逆?)’ is ‘close?change(終結?變化)’.

        Novel Repair Technique for Articular Cartilage Defect using a Fibrin and Hyaluronic acid Mixture

        We evaluated the cartilage repair potential of a hyaluronic acid and fibrin mixture when transplanted into cartilage defects. Circular, articular, cartilage defects 4-mm in diameter were made in the trochlear region in 21 New Zealand white rabbits divided into three groups. The seven rabbits in the control group underwent microfracture (M group), the seven rabbits in the experimental group underwent microfracture with subsequent injection of hyaluronic acid mixed with fibrin (MH group), and seven rabbits in the other experimental group underwent microfracture followed by injection of bone marrow concentrate and hyaluronic acid mixed with fibrin (MBH group). At week 12 following surgery, the cartilage was observed and histologically compared in the three groups. The surface of the newly generated cartilage was very smooth and even, and we noticed that the entire area was completely regenerated in both experimental groups. The control group showed incomplete and irregular cartilage formation in the defect. In histologic scoring, comparison of the MBH group (M= 2.333) and the M group (M= 9.000) differed significantly (P= 0.046). Therefore, injection of a mixture of bone marrow concentrate, hyaluronic acid and fibrin to treat articular cartilage defects of the knee appears to be an effective method of cartilage regeneration.

      • 고혈압백서 대동맥에서 Ca에 의한 수축에 미치는 Na/Ca교환기전의 영향

        We investigated the contraction by sodium-calcium(Na-Ca) exchanges in the aortic strips of spontaneously hypertensive rats(SHR) and Wistar-Kyoto (WKY) rats. Resting tension of aortic strips in SHR was decreased by the removal of extracellular Ca and those were recovered by the addition of Ca. However, resting tone of aortic strips in WKY was not changed by Ca removal or Ca addition. Ca-induced contraction after exposure to Ca-free solution was suppressed by verapamil 10^-6M to 90% or more. In the presence of verapamil 10^-5M, phentolamine 10^-6M, caffeine 5mM and atropine 10^-6M, the reduction of extracellular sodium concentration([Na]o) from normal(158 mM) to 0 mM(replaced by choline chloride) caused the contraction(Ca entry by Na-Ca exchange). The amplitude of contraction was more greater in SHR than in WKY rats. In the presence of verapamil 10^-5M, phentolamine 10^-6M, caffeine 5mM and atropine 10^-6M, aortic strips were not contracted by Naand Ca-free solution, but was contracted by the addition of Ca. In the presence of verapamil 10^-5M and phentolamine 10^-6M and ouabain 10^-5M, Ca did not induce contraction after exposure to Ca-free Tyrode ' s solution, but in the presence of verapamil 10^-5M and phentolamine 10^-6M and monensin 10^-5M a sodium ionophore, Ca induced a little contraction in the aortic strips of SHR. These observations suggest that Na-Ca exchange does not contribute the development of Ca-induced contraction after exposure to Ca-free Tyrode ' s Solution in the presence of normal extracellular Na concentration. However, Na/Ca exchange in vascular smooth muscle is increased in SHR and may be involved in the mechanism for hypertension in SHR.
