<P>The prognostic value of the left ventricle ejection fraction (LVEF) after acute myocardial infarction (AMI) has been questioned even though it is an accurate marker of left ventricle (LV) systolic dysfunction. This study aimed to examine the prognostic impact of LVEF in patients with AMI with or without high-grade mitral regurgitation (MR). A total of 15,097 patients with AMI who received echocardiography were registered in the Korean Acute Myocardial Infarction Registry (KAMIR) between January 2005 and July 2011. Patients with low-grade MR (grades 0-2) and high-grade MR (grades 3-4) were divided into the following two sub-groups according to LVEF: LVEF ≤ 40% (n = 2,422 and 197, respectively) and LVEF > 40% (n = 12,252 and 226, respectively). The primary endpoints were major adverse cardiac events (MACE), cardiac death, and all-cause death during the first year after registration. Independent predictors of mortality in the multivariate analysis in AMI patients with low-grade MR were age ≥ 75 yr, Killip class ≥ III, N-terminal pro-B-type natriuretic peptide > 4,000 pg/mL, high-sensitivity C-reactive protein ≥ 2.59 mg/L, LVEF ≤ 40%, estimated glomerular filtration rate (eGFR), and percutaneous coronary intervention (PCI). However, PCI was an independent predictor in AMI patients with high-grade MR. No differences in primary endpoints between AMI patients with high-grade MR (grades 3-4) and EF ≤ 40% or EF > 40% were noted. MR is a predictor of a poor outcome regardless of ejection fraction. LVEF is an inadequate method to evaluate contractile function of the ischemic heart in the face of significant MR.</P>

We reviewed 14 patients(22 hips) to whom had been performed total hip arthroplasty using CLW(CementLess weill, Protek, Switzerland)acetabular cup at the department of orthopaedic surgery, National Medical Center from February 1988 to August 1990. The purpose of this retrospective follow-up study was to analyze the radiological findings such as loosening or migration at the acetabular component and to assess the usefulness of the CLW cup. The CLW acetabular cup is made of self-tapping, double-threaded anchoring ring with deep pitch and low profile and it is made of pure titanium with rough corundum blasted surface. The average age of the patients at operation was 49 year(23-62 years) and all were male. The mean follow-up period was 6years and 3months(from Syears 2months to 7years 9months). The operation had been performed by a single surgeon(J-M Lee) using transgluteal approach. We analyzed the loosening and migration on the immediate postoperative and last follow-up plain X-ray film by Engh's criteria, which demonstrated no loosening and migration. While we need long term follow-up, we think that threaded cups which are well designed biomechanically and have good surface with bone on-growth capability should make different results compared to the poor results of other threaded cups.

Successful total hip arthroplasty in its current form was made by Sir John Charnley. The incidence of acetabular loosening appears to increase at about 5-7 years after surgery. Over the last decade, a number of modifications in acetabular component design have been undertaken to solve this problem. In 1971, for purpose of reduction of peak stress occurring in cement and trabecular bone of the acetabulum, Harris reported a series of metal-backed acetabrlar components with removable polyethylene liners that could be replaced if excessive wear ocurred. Although long-term survivorship analysis has not demonstrated substantial improvement with the use of metal backing, most current acetabular designs retain the matal backing as a means of acetabular fixation. Even more, recent reports of porous-ingrowth of cementless cup have generally been favorable and few revision have been reported. The increasing use of cementless metal-backed acetabular components in total hip arthroplaty produced another mechanical interface that may fail: Mechanical failure of polyethylene liner in metal backed cup. We report two cases of early failure of polyethylene liner in cementless metal backed cup, and review previously reported articles.

A retrospective follow-up study was perfomend with clinical and radiographic results of uncemented total hip arthroplasty using CLS(Cementless Spotorno, Protek, Berne, Switzerland) in 40 patients(53 hips) with osteonecrosis of the femoral head at the department of orthopaedic surgery, National Medical Center from November 1987 to November 1991. The average age at surgery was 45.5 years (range, 30-55 years). 15 Weill threaded cup, 19 expansion cup and 19 bipolar cup were used for the acetabular components. The Average Harris hip score improved from 49 to 96 points and revealed mild thigh pain in 2 hips (3.8%) and intermittent groin pain in 3 bipolar hips(5.7%). The radiographic evaluation demonstrated that significant reactive lines showed in 8 femoral components (15%) and in 4 acetabular components( I Weill cup, 3 expansion cup), endosteal bone formation in 42 hips (79%), cortical hypertrophy in 29 hips (55%) and calcar atrophy in 29 hips (55%), and also stem subsidence more than 4 mm, in 1 hip, heterotopic ossification, in 11 hips(21%). No revision was performed for aseptic loosening, failure, or infection.

Purpose: Femoral intertrochanteric fractures are common in the elderly. Appropriate surgical fixation of trochanteric fracture fragments can restore normal anatomical structure and ambulation, and can aid in the recovery of biomechanical function of the hip. We evaluated clinical outcomes of bipolar hemiarthroplasty using a wiring technique for trochanteric fracture fragment fixation. Materials and Methods: From September 2006 to February 2015, a total of 260 cases underwent simultaneous bipolar hemiarthroplasty and wire fixation. A total of 65 patients (69 hips) with an average age of 78 years and more than one year of follow-up was included in the study. Using pre-, postoperative and follow-up radiograms, we evaluated wire fixation failure and also assessed changes in walking ability. Results: Loosening or osteolysis around the stem was not observed; however, we did observe bone growth around the stem (54 cases), cortical hypertrophy (6 cases), a wide range of sclerotic lines but no stem subsidence (1 case), wire breakage (9 cases), and fracture fragment migration with no significant functional deficiency (2 cases). Conclusion: Our study showed that additional wiring for trochanteric fracture fragment fixation following bipolar hemiarthroplasty can help restore normal anatomy. The added stability results in faster rehabilitation, and good clinical and radiographic outcomes. We recommend this procedure in this type of fracture.

An assessment of the nutritional aspects on irradiated garlic bulbs, stored for 10 months at low(3±1℃, 75∼85% R.H.) and room(12±5℃, 75∼85% R.H.), was performed by means of investigations on the changes in some chemical composition. Ionizing energy treatment at 0.1 kGy gamma-ray brought about no significant changes in the moisture content of stored garlic, but it contributed to the reduction of the changes in total available carbohydrate content of stored garlic. After 8 months of storage, the content of free sugars was significantly higher in nonirradiated garlic than in irradiated garlic, especially in low temperature storage. The 0.1kGy irradiation at the sprout-inhibition dose had no apparent effect on the content of ascorbic acid in stored garlic, and sulfurcontaining amino acids appeared to be little affected by the irradiation at 0.1kGy, even though there were same sensitive amino acids like glutamic and aspartic acids.
