The novel PWM(pulsewidth modulation) strategy of three-phase voltage source inverter in the overmodulation range is proposed. Generally, the space vector PWM strategy in the overmodulation range is widely utilized due to its simple and effective digital implementation and increased voltage transfer ratio. A unique algorithm manages the transition form the onset of overmodulation to six-step operation. This proposed technique maximizes the utilization of the dc-link voltage and maintain the voltage linearity between the reference and output voltages, and it decreases the necessary memory quantity for the look-up table of the angle.
Actinic cheilitis treated with topical ingenol mebutate gel (Picato®) 0.015%: A case report

Actinic cheilitis is a common, premalignant lesion often affecting the lower lip that requires early and effective treatment. There are several treatment options including surgical excision, cryotherapy, carbon dioxide laser, and topical agents such as 5-fluorouracil or 5% imiquimod. A 79-year-old woman presented with a painful patch on her lower lip for 1 month duration. On physical examination, erythematous keratotic patches on the lower lip were shown. Histopathological findings revealed parakeratosis and atypical keratinocytes in the epidermis with marked solar elastosis in the dermis. The patient was diagnosed as actinic cheilitis. We applied topical ingenol mebutate gel (Picato, Leo Pharma, Inc.) 0.015% for the three consecutive days. After treatment with ingenol mebutate gel (Picato®), the patient experienced painful erythematous scaly patches and erosion at day 1. Despite daily wet dressing, the local treatment site reaction persisted for two months. Ingenol mebutate gel (Picato®) has proven to be effective for the field treatment of actinic keratosis. Contrary to our case, in a report of four caucasian actinic cheilitis patients treated with ingenol mebutate gel (Picato®) 0.015%, three patients showed minimal local reaction. Some differences regarding drug action and efficacy may exist between Caucasian and Asian patients, but more studies and long-term follow-up are necessary.

Background: Inflammatory bowel disease (IBD) is chronic disease of the gastrointestinal tract attributed to aberrant activity of the immune system. Increasing evidence suggests that IBD patients are at an increased risk of inflammatory skin diseases (ISDs). Objectives: We sought to clarify the association between IBD and ISDs using a nationwide health claims database maintained in Korea. Methods: We interrogated Korean health claim database data from 2009 to 2013. We enrolled all patients with IBD, and age- and sex-matched controls, and evaluated the risks of ISDs, including psoriasis, rosacea, and atopic dermatitis, and the risks of autoimmune skin diseases (ASDs), including vitiligo and alopecia areata. We used multivariable logistic regression to this end. Results: ISDs including rosacea, psoriasis, and atopic dermatitis were significantly associated with IBD, whereas the associations between IBD and ASDs including vitiligo and alopecia areata were less marked or non-existent. Ulcerative colitis and Crohn’s disease were both associated with ISDs. Conclusion: IBD was significantly associated with ISDs, but less so or not at all with ASDs.
Localization of Broca's Area Using Functional MR Imaging: Quantitative Evaluation of Paradigms

Objective : Functional magnetic resonance imaging (fMRI) is frequently used to localize language areas in a non-invasive manner. Various paradigms for presurgical localization of language areas have been developed, but a systematic quantitative evaluation of the efficiency of those paradigms has not been performed. In the present study, the authors analyzed different language paradigms to see which paradigm is most efficient in localizing frontal language areas. Methods : Five men and five women with no neurological deficits participated (mean age, 24 years) in this study. All volunteers were right-handed. Each subject performed 4 tasks, including fixation (Fix), sentence reading (SRI. pseudoword reading (PR), and word generation (WG). Fixation and pseudoword reading were used as contrasts. The functional area was defined as the area(s) with a t-value of more than 3.92 in fMRI with different tasks. To apply an anatomical constraint, we used a brain atlas mapping system, which is available in AFNI, to define the anatomical frontal language area. The numbers of voxels in overlapped area between anatomical and functional area were individually counted in the frontal expressive language area. Results : Of the various combinations, the word generation task was most effective in delineating the frontal expressive language area when fixation was used as a contrast (p<0.05). The sensitivity of this test for localizing Broca's area was 81 % and specificity was 70%. Conclusion : Word generation versus fixation could effectively and reliably delineate the frontal language area. A customized effective paradigm should be analyzed in order to evaluate various language functions.
Idiopathic acquired punctate palmoplantar keratoderma: A report of four cases

Palmoplantar keratodermas(PPKs) refer to a thickening of skin of the palms and soles caused by excessive keratin. Four forms can be seen: diffuse, focal, punctate, and palmoplantar ectodermal dysplasias. Acquired punctate palmoplantar keratodermas(PPPKs) occur in later life, without family history, and tend to be attributable to an underlying etiology. Idiopathic, acquired PPPKs should be reserved as a diagnosis of exclusion, when all the other etiologies have been investigated and ruled out. Four patients were referred to our clinic, presenting with multiple 0.2cm-sized, round papules on the both palms for several months. The lesions were scattered but discretely distributed. None of the patients had past history and laboratory findings were normal. Histopathological findings were nonspecific, with marked hyperkeratosis of the stratum corneum. HPV stain was negative. One of the patients was treated with topical tretinoin and after two weeks, the lesions were slightly improved. The others had not received any treatment. As in these cases, PPPKs resemble verruca plana or lichen nitidus in clinical findings. Therefore, dermatologists should consider PPPKs as possible diagnosis to avoid unnecessary treatment.

Basal cell carcinoma (BCC) is usually effectively managed with surgical removal. Ingenol mebutate gel (Picato, Leo Pharma, Inc.) is a new topical drug extracted from the latex sap of a plant Euphorbia peplus approved by Food and Drug Administration (FDA) for field therapy of actinic keratosis in 2012 for its chemoablative and immune-stimulatory properties. We describe a case of a 87-year-old Korean woman presenting with 1.5 x 1.5 cm-sized blackish crusted plaque and erosion on her scalp for a year. Histopathologically, the lesion showed peripherally palisading basaloid tumor cells budding from the epidermis and limited to upper dermis with partial retraction of stroma from the tumor nest. The patient was diagnosed as superficial BCC. We applied topical ingenol mebutate gel (Picato®) 0.05% for 2 consecutive days. The patient complained of pain, erythema and erosion on the application site on the second day. After 2 weeks of application, blackish plaque disappeared, but 4 weeks later, two blackish papules reappeared. Biopsy specimen of one of the papules showed remnant tumor cells. We conducted cryotherapy on the remaining lesion. Although ingenol mebutate gel (Picato®) monotherapy is insufficient for complete elimination of superficial BCC, we suggest that application of ingenol mebutate gel (Picato®) combined with other modalities such as punch biopsy and cryotherapy may be a useful treatment option for reducing tumor burden.

Background: Second-generation tyrosine kinase inhibitors (2nd TKIs) are novel, well-tolerated and promising class of anticancer drugs, commonly used to treat imatinib resistant and intolerant chronic myeloid leukemia (CML). While cutaneous reactions of traditional TKIs are one of the most common adverse effects, fewer cutaneous adverse effects have been reported for the 2nd TKIs. Objectives: The purpose of this study was to evaluate the 2nd TKIs induced cutaneous adverse effects in CML patients. Methods: We performed a retrospective study of the medical records of 32 CML patients who developed various cutaneous eruptions caused by 2nd TKIs. We analyzed the characteristics of the patients, the duration of TKIs therapy, the type of TKIs, the cutaneous adverse events and the treatment. Results: Among the 31 patients, TKIs used for treatment included radotinib(n=15), dasatinib (n=10) and nilotinib (n=5), bosutinib (n=2). Various cutaneous adverse effects such as acneiform eruptions, eruptive melanocytic nevus, rosacea-like eruptions, dysplastic, alopecia and keratosis pilaris are showed. Conclusion: Better knowledge of these cutaneous adverse events caused by 2nd TKIs, and early management will help patients adhere to long-term treatment and improve their quality of life.
