        On filtrated and autoclaved water samples from Lake Jinyang, algal growth potential(AGP) test using Selenastrum capricornutum Printz were performed from February, 1999 to October, 1999 in order to assess fertility potential of the lake and to identify possible limiting nutrients. Algal growth potentials of the filtrated and filtrated after autoclaved samples were 0.018 ㎎/ℓ ~9.965 ㎎/ℓ and 0.018 ㎎/ℓ ~28.788㎎/ℓ respectively during surveyed period. AGP of the filtrated after autoclaved were higher than those of the filtrated only except some specific stations. The points of maximum growth were measured after addition of N, P and unenriched samples (control). The limiting factor was nitrogen only at site 5 in spring and phosphate appeared to be the major limiting nutrients in spike test throughout the year, especially during spring and autumn. Accoding to trophic level by Sudo(1980), the water quality of lake Jinyang were estimated mesotrophic at site 2, 4, 5, 6 in spring, at site 3, 6 in summer and at site 6 in autumn but all other sites were oligotrophic. But it is suggested that if we have not manigement of lake Jinyang, the water quality is possible to convert eutrophic.

        한국의 발톱진균증 환자의 삶의 질에 관한 연구

        Background: Onychomycosis, especially toenail onychomycosis has become one of the common fungal infection and has historically been regarded as a cosmetic rather than medical problem by many patients, even by physicians. Recently, however, there are several reports that is a refractory disease which may cause a deleterious effect on patiets quality of life (QOL). Objective: The purpose of this study was to investigate the impact of toenail onychomycosis on QOL in Korea and to assess the chandes of QOL after treatment. Methods: Total 1004 patients with toenail onychomycosis which was confirmed by clinical findings and KOH preparation were were enrolled at 47 dermatologic centers in Korea, and interviewed with standardized QOL questionnaire before and after sytemic antifungal treatment. Responses to the questionnaire were scored by 5-point scale (1∼4) and averaged, and were analyzed for 5 dimensions of emotional impact, social impact, symptorn and functional impact, patients views concerning treatment, and relationship with doctor. Results: 1. Before and after treatment, the most serious impact was emotional dimension showing 1.90 and 1.30 in average score (AS), and social (AS: 1.14 and 0.83) and symptom and functional impact (AS: 1.05 and 0.92) was also affected. 2. In female rather than male, statistically more significant impact on patients QOL was observed in all dimensions. 3. After treatment, 3 of 5 dimensions were improved significantly - emotional dimension (AS : from 1.90 to 1.30), social dimension (AS: from 1.14 to 0.83), patients view concerning treatment (AS: from 1.34 to 1.02). 4. The drgree of patients satisfaction at the therapeutic effect was very high- 62.4% (immediately after treatment) and 65.8% (9 months affer initiation of treatment) of patients answered excellent or good. Conclusion: This study confirms that toenail onychomycosis has significant impact on the overall QOL of patients. Also the effect of antifungal therapy on patients QOL were watisfactory. Therefore, both doctor and patient should pay more attention to the treatment of onychomycosis. [Kor J Med Mycol 3(1): 115∼124]

      • FCP 17 : Effect of three sea salt type hot spring (HS) water on cytokine expression in primary human keratinocyte (KC) under Toll-like receptors (TLRs) stimulation

        Background: Balneotherapy is widely used for the management of inflammatory skin diseases, but the mechanism is partly understood. Korea has abundant resources of hot spring(HS) and among them are sea salt type of Na(Ca)-Cl. We previously reported that different HS water affected the secretion of proinflammatory cytokines from HaCaT cells. Objectives: We evaluated effect on proinflammatory cytokine secretion in primary keratinocytes(KCs) of three different sea salt type HS water, i.e., Haeundae, Magumsan, and Seokmodo HS area, selected by salt concentration gradient. Methods: To know whether primary KCs express Toll-like receptors(TLRs), we isolated mRNA from KCs and evaluated the expression of TLR1 to 10 by RT-PCR. Next, we investigated the effect of three different HS waters on theexpression of cytokines(IL-8, TNF-α, IL-1β, IL-6, and GM-CSF) in the primary KCs under TLR1, 2, 3, 5, and TLR6 stimulation by ELISA. Results: The treatment of HS waters induced suppression of proinflammatory cytokines under TLR stimulation in the primary KCs. Cytokine secretion from primary human KCs to TLR3 ligand was the most prominent. Decrease of IL-8, TNF-α, IL-1β, IL-6, or GM-CSF secretion under TLR stimulation were observed to all HS waters. Conclusion: Three different sea salt type HS water treatment affected the secretion of proinflammatory cytokine production from KCs under TLR stimulation. These parameters and protocols can be used to evaluate anti-inflammatory effect of sea salt type HS water.

      • FC 1-1 : Immunomodulatory effects of Ul-Jin thermomineral water on Imiquimod-induced psoriasis like murine model

        Background: Balneotherapy is considered to have therapeutic properties for psoriasis, however the immunologic mechanism of balenotherapy is not established. Objectives: A imiquimod-induced psoriasis model was used for evaluating the therapeutic effects of balneotherapy with Ul-Jin (Baek-Am) thermomineral water. Methods: Clinical improvements were evaluated using modified SCORAD index. Confocal microscopic imaging analysis was performed for lesional expression of IL-17, CD-4, Foxp3, and TNF-a. Lesional mRNA expression of IL-17, IL-15, IL-6, and Foxp3 was evaluated by real-time reverse transcriptase polymerase chain reaction method. Histologic evaluation was also performed. Results: balneotherapy with Ul-Jin (Baek-Am) thermomineral water showed immunomodulatory effects in psoriasis-like murine model by decreasing inflammatory cytokines and increasing lesional regulatory helper T cells. Conclusion: These results suggest that balneotherapy can be an effective and safe treatment option in psoriasis.

      • Topographic Variance of Contact Sensitivith Reaction in Guinea Pigs

        There have been published manuscripts which fully suggest that there may be a topographic variance of contact sensitivity in experimental animals such as guinea pigs, hamsters and mice, probably due to a large extent to the topographic difference of Langerhans cell population and to a little extent to impact on the induction site. This assay was done to establish the possibility of involvement of a certain topographic variance in contact sensitivity reaction. Three different anatomical sites were chosen fur the induction in three groups of five guinea pigs: the conventional site of the nuchal region, and the sacral and abdominal skin regions. For the elicitation, two different concentrations of DNCB were applied on three sites along both sides of the spine in the back. The contact sensitivity reactions elicited by DNCB in the group of the nuchal and abdominal skin induction were definitely stronger at the site closer to the nuchal region than that found at site closer to the sacral region, but all animals of the group sensitized on the sacral region showed comparatively uniform reaction. Evidently, contact sensitivity reaction is influenced mainly by topographic variance but shows a little difference depending upon induction sites.

      • Merkel cell carcinoma of the arm with spontaneous regression

        Merkel cell carcinoma is a rare malignant cutaneous neuroendocrine carcinoma of aggressive behavior with high rate of metastasis and recurrence. The carcinoma presents as a solitary, non-tender, firm, reddish orskin-colored, dermal papule or nodule commonly affecting the head and neck or the extremities in older people. It grows rapidly within 1 to 3 months in spite of its deceptively innocuous appearance. A 75 year-old women presented with a tender, solitary, 3 x 3 cm-sized, erythematous, firm, dermal nodule on the left arm for months. A 3 mm-sized punch biopsy was performed. Histopathologic evaluation showed dense dermal infiltrates of hyperchromatic lymphoid cells and medium sized bluish cells containing scanty amount of cytoplasms. Mitotic figures were also observed. The tumor cells stained positive for pancytokeratin, CK20, synaptophy in, CD56a, and NSE and negativity for LCA, CK7, and S-100. Diagnosis of merkel cell carcinoma was made on the clinical and histopathological grounds. Two months after biopsy, the primary tumor lesion was downsized to 1 x cm without any intervention. Spontaneous regression of merkel cell carcinoma is a rare event with incidence estimated to be 1.5%. Herein, we report a rare and interesting case of merkel cell carcinoma which has spontaneously regressed after biopsy with no activemedical or surgical treatment.

      • Disseminated cutaneous gout: a rare clinical presentation

        Gout is a form of arthritis which culminates from a buildup of uric acid crystals in the joints. Four distinct stages of gout are generally recognized. A 58-year-old man with chronic gouty arthritis presented with tender, multiple, firm, erythematous and yellowish nodules over erythematous patches on both upper and lower extremities for 1 week. He also had been complaining of general weakness and fever. Blood tests revealed elevated renal function test, hyperkalemia (7.9 mEq/L), leukocytosis (25.36 109/L), and elevated segment neutrophils (91%). A deep punch biopsy of the nodule revealed chalk-like whitish materials. Histologic examination revealed amorphous pinkish crystalline material surrounded by granulomatous inflammation in the dermis. On polarized light microscopy, strong, negatively-birefringent needle-shaped crystals were seen. Unfortunately, the patient’s renal function deteriorated, with the laboratory makers pointing to development of sepsis. The patient ultimately expired from ventricular tachycardia. In our patient, disseminated gout lesions were broadly distributed including nonarticular sites, and severe leukocytosis implying bacterial infection was present. Although the exact cause of sepsis is unclear, there is a possibility that cutaneous infection of gouty nodules may have played a significant role. Herein, we report a case of severe, disseminated tophaceous gout that may have led to systemic infection.

      • Association of total plasma IgE level with MIF (macrophage migration inhibitory factor) polymorphisms in Atopic dermatitis patients

        Background: Macrophage migration inhibitory factor (MIF) is a proinflammatory cytokine and significantly elevated in patients with AD. The MIF gene polymorphisms such as -173 G to C and -794 CATT5-8 MIF have been implicated in AD. Objectives: The aim of this study is to evaluate the relationship between -173 G to C and -794 CATT5-8 MIFgene polymorphisms and total plasma immunoglobulin E (IgE) level in AD patients. Methods: We performed polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) analysis from 178 patients with AD and 80 controls. The total plasma MIF level was determined by ELISA. Logistic regression analyses were performed. Results: Compared with GG homozygotes of MIF-173G/C polymorphism, odds ratio (OR) for CC homozygotes was 9.3 (p=0.048), and OR for GC heterozygotes was 2.5 (p=0.031). Compared with MIF G/5-CATT haplotype, OR for MIF C/5-CATT haplotype was 9.7 (p=0.009), and OR for MIF C/7-CATT haplotype was 4.5 (p=0.005). The IgE level was highly associated with age and sex among AD patients with the MIF -794 7-CATT polymorphism (p=0.043), while MIF C/7-CATT haplotype was associated with decreased IgE level (p=0.036). Conclusion: Our results shows that the MIF promoter polymorphisms in -173 C allele and MIF C/5-CATT and MIF C/7-CATT haplotype are associated with increased risk for AD. In contrast, the -794 7-CATT locus and MIF C/7-CATT haplotype was associated with decreased total plasma IgE level in AD patients.

      • Cutaneous malignant mesothelioma

        Malignant mesothelioma is a rare malignant tumor that arises from pleural, pericardial or peritoneal mesenchymal cells. Though extremely rare, cutaneous mesothelioma mayoccur through regional spread, distant metastasis, or direct extension within surgical scars. Clinically, cutaneous lesions present as violaceous papules, nodules, or plaques. Histologically, mesothelioma are divided into epithelial (60%), sarcomatoid (15%), and mixed types (25%). These histological features may simulate metastatic adenocarcinoma, a primary adnexal neoplasm or angiosarcoma. Immunohistochemistry can aid in diagnosis, as the malignancy yields positive staining for calretinin, WT-1, Cytokeratin 5/6, mesothelin, and podoplanin. A 66-year-old man presented with asymptomatic, solitary, palpable, skin-colored, subcutaneous mass on the Rt. flank detected 3 days ago. He had been previously diagnosed with mesothelioma of the Rt. chest wall 3 years prior and undergone mass excision along with adjuvant chemotherapy. A follow up abdomen CT showed a small nodule on the right abdominal wall, which was referred to our department for biopsy. Histopathological findings were remarkable for neoplastic, epithelial cells exhibiting irregular, tubular and papillary growth pattern in the dermis. The cells were positive for calretinin, cytokeratin 5/6, and wt-1 and negative for MOC-31. This to our knowledge is the first report of cutaneous malignant mesothelioma in Korea.
