Nasotracheal intubation is commonly and safely used in the anesthetic management of patients undergoing oral cavity surgery. But endotracheal tube obstruction causes serious complications, including pneumothorax, pulmonary edema, brain injury and death. We experienced a case of endotracheal tube obstruction due to mucous crust after general anesthesia. A 5-year-old woman with cleft palate was intubated with a 4.5 mm amored tube for general anesthesia. During the surgery, vital signs were within normal limits. In the intensive care unit, she exhibited sign of complete airway obstruction. We exchanged the tube with another tube after failure of suction. The airway obstruction was due to a dried mucous crust attached to the bevel of the endotracheal tube. We should keep in mind the presence of an amored endotracheal tube cannot be regarded as a guarantee of a patent airway.
Leuconostoc mesenteroides Producing Bifidogenic Growth Stimulator via Whey Fermentation

Two whey culture supernatants of CJNU 0147and CJNU 0400 were found to effectively enhance the growth of Bifidobacterium longum FI10564 by 1.58 fold compared to non-fermented whey medium. The 2 isolates were identified to be Leuconostoc mesenteroides (99%identity) by 16S rRNA gene sequence analysis. To determine whether the whey culture supernatant of CJNU 0147 selectively stimulate the growth of bifidobacteria, the growth rates of Escherichia coli DH5α, Enterococcus faecalis KFRI 675, Listeria monocytogenes ATCC 19111,and Staphylococcus aureus ATCC 14458 with the supernatant were measured. In these experiments, the supernatant slightly inhibited the growths of bacteria except for E. coli,indicating that the whey culture supernatant had very little influence on the growth of these bacterial strains.

1,4-Dihydroxy-2-naphthoic acid (DHNA), a precursor of menaquinone (vitamin K2), has an effect on growth stimulation of bifidobacteria and prevention of osteoporosis, making it a promising functional food material. Therefore, we tried to clone the menB gene encoding DHNA synthase from Leuconostoc mesenteroides CJNU 0147. Based on the genome sequence of Leu. mesenteroides ATCC 8293 (GenBank accession no., CP000414), a primer set (Leu_menBfull_F and Leu_menBfull_R) was designed for the PCR amplification of menB gene of CJNU 0147. A DNA fragment (1,190 bp), including the menB gene, was amplified, cloned into pGEM-T Easy vector, and sequenced. The deduced amino acid sequence of MenB (DHNA synthase) protein of CJNU 0147 had a 98% similarity to the corresponding protein of ATCC 8293. The menB gene was subcloned into pCW4, a lactic acid bacteria - E. coli shuttle vector, and transferred to CJNU 0147. The transcription of menB gene of CJNU 0147 (pCW4::menB) was increased, when compared with those of CJNU 0147 (pCW4) and CJNU 0147 (−). The DHNA was produced from it at a detectable level, indicating that the cloned menB gene of CJNU 0147 encoded a DHNA synthase which is responsible for the production of DHNA, resulting in an increase of bifidogenic growth stimulation activity.
Detection of 1,4-Dihydroxy-2-Naphthoic Acid from Commercial Makgeolli Products

To support beneficial effects of makgeolli for human health, we investigated for the presence of 1,4-dihydroxy-2-naphthoic acid (DHNA), a bifidogenic growth stimulator (BGS), from commercial makgeolli products. Among eleven makgeolli products (A~K), four showed positive peaks for DHNA in high performance liquid chromatography analysis. Makgeolli product A in particular contained the highest concentration of DHNA (0.44 ppm), as confirmed by liquid chromatography-mass spectrometry. Furthermore, BGS activity of the makgeolli product A was higher than those of products in which DHNA was not detected. These results indicate that makgeolli can be a good source for DHNA and that DHNA-enriched makgeolli could be developed by modifying manufacturing procedures and controlling its microbiota.

Previous studies have confirmed that fermented whey produced by Leuconostoc mesenteroides CJNU 0147 or Lactobacillus casei CJNU 0588 display bifidogenic growth stimulator (BGS) activity. The present study sought to determine if the strain itself can improve the growth of bifidobacteria in co-cultures. In reinforced clostridial medium (RCM), both strains stimulated the growth of a Bifidobacterium lactis strain during the exponential phase and also stimulated the growth during almost all growth phases in whey broth. Fermented whey containing viable Leu. mesenteroides CJNU 0147 and L. casei CJNU 0588 cells maintained viability of the B. lactis strain stored at 10℃ in MRS broth. Viable cell count of the B. lactis strain without the fermented whey was decreased to 5.6 log cfu/mL after 15 days, whereas that of the strain with the fermented whey was slightly increased to 7.1 log cfu/mL as compared with initial viable cell count of 6.9 log cfu/mL.
Increased Bacteriocin Activity of a Recombinant Pediococcus acidilactici

Pediocin PA-1, mainly produced from Pediococcus spp., is a well-known class IIa bacteriocin showing strong anti-listerial activity. Pediococcus acidilactici K10 producing the bacteriocin was transformed with pLR5cat(S)_PSAB in which the pediocin PA-1 structural gene (pedA) was translationally fused with a deduced signal sequence of the bifidobacterial ${\alpha}$-amylase gene for secretion. The P. acidilactici K10 transformant presented increased bacteriocin activity when compared with that of native P. acidilactici K10. The P. acidilactici K10 mutant (K10M), in which the plasmid for pediocin PA-1 production is cured, also showed bacteriocin activity when transformed with pLR5cat(S)_PSAB, indicating that the pediocin gene was expressed in hosts. An increase in pedA gene transcription level and the detection of both transcripts from the pediocin operon and PSAB were also confirmed. The P. acidilactici K10 [pLR5cat(S)_PSAB] transformant was more inhibitory against Listeria monocytogenes than that of the P. acidilactici K10 wild type in co-cultures.
