      • 전자선 증착장치 및 ITO 박막의 제작과 그 특성에 관한 연구

        태양 전지의 제작시 투명 전극막으로 쓰이는 ITO 박막을 제조하기 위하여 cathode 접지 방식의 전자총을 가진 정전 편향 집속형의 전자선 증착 장치를 제작하였다. 가속전압이 0∼15kV, 필라멘트 전류가 0∼28A, 두 전극판 사이의 간격이 11mm, anode와 deflector 사이의 간격이 30mm인 최적 실험조건하에서 생성된 전자선 spot의 크기는 길이가 20mm 폭이 1mm였으며, 전자선의 출력은 약 400W였다. 이 전자선 증착 장치를 이용하여 1600℃까지의 증발 온도를 가진 도체와 절연체 등을 쉽게 증착시킬 수 있었으며, 만드어진 ITO 박막은 약 5000Å의 두께에서 10^-1∼10^-2Ω·㎝의 비저항과 95%의 광투과율을 갖는다. An electron beam evaporator with a cathode grounded type electron gun was designed and constructed in order to deposit ITO thin films for photovoltaic applications. The optimum condition was observed with electode spacing of 11mm, 30mm between anode and deflector, a cathode inner diameter of 3mm and an anode inner diameter of 5mm. As the accelerating voltage varies from 0 to 15kV and ghe filament current from 0 to 28 A, the minimum electron beam spot has a 20mm length, and 1mm width and the output power ranges from 0 to 400W under the above optimum conditions. We can deposit materials whose evaporation temperature is over 1600℃ using this evaporator. And as a result ITO thin film showed a low resistivity of 10^-1∼10^-2Ω·㎝ and the transmittance of 95%.

        Tourette 장애 소년에서 자기공명영상을 이용한 기저핵의 용적 분석

        Objectives : The purpose of this study was to investigate the regional differences of the basal ganglia volume between the boy subjects who have Tourette's disorder and the control group who don't have this disorder by using Brain MRI. Methods : Sixteen subjects who were diagnosed as Tourette's disorder using DSM-IV criteria, and sixteen healthy subjects were selected. For analysis the coronal slice images of the basal ganglia were collected and collected MR images were analyzed with NIH Image (version 6.1 ppc). Resuls : The total brain volume in the subjects with Tourette's disorder were smaller than control group by 6.2% (F=6.2, d.f.=1, P=0.019). The right putamen volume in boys with Tourette's disorder were significant smaller than control group statistically (F=3.2, d.f.=3, P=0.040). The asymmetry (1eft>right) in the globus pallidus decreased significantly in the boys with Tourette's disorder (p=0.06) were found from this experiment. Conclusion : These findings suggest that boys with Tourette's disorder may have neuropathological abnormalities in the basal ganglia. Moreover, the present findings point out the important role of basal ganglia, especially putamen, in Patho-physiology of Tourette's disorder. More studies about the structural and functional analysis of the basal ganglia in Tourette's disorder are highly required.

      • Free Paper Session : Biliary Diseases ; Risk Factors And Management Of Biliary Complications In Adult Living Donor Liver Transplantation Patients

        Background/Aims: The major morbidity after adult living donor liver transplantation (ALDLT) is due to biliary complications. The aim of the present study is to evaluate 1) the kinds of biliary complications and their incidence, 2) the risk factors of biliary stricture and 3) the clinical outcomes of management. Methods: Between January 2000 and December 2006, 259 adult patients underwent ALDLT at Seoul National University Hospital. Among 259 recipients, 20 patients died during the postoperative period. We retrospectively analyzed the remaining 239 patients. Results: The mean follow-up period was 40 months (range, 7-90 months), and the graft failure rate was 3%. The 1-year survival rate was 91%. The main causes of death were hepatocellular careinoma recurrence (65%), acute or chronic rejection (10%), sepsis (10%) and HCV reactivation (5%). The overall incidence of biliary complications was 28.9% for biliary stricture, 6.2% for bile leakage, 1.6% for bile duct stone and 0.8% for biliary cast. On multivariate analysis, the risk factors of biliary stricture were multiple bile duct anastomosis (OR 3.25, CI 1.2-8.7, p=0.02), bile leakage (OR 28.1, CI 3.3-238.1, p-0.02) and hepatic artery stenosis (OR 9.7, CI 2.3-41.4, p=0.002). All strictures were initially treated nonsurgically (endoscopic cholangioplasty, n=27; percutaneous cholangioplasty, n=41; and observation, n=1). However, 6 patients ultimately required biliary reconstruction. The patients with bile leakage were treated with endoscopic nasobiliary drainage (n=3), percutancous transbiliary drainage (n=11) or biliary reconstruction (n=1). Biliary casts and stones that developed after ALDLT were cured with endoscopic or percutaneous removal. Conclusions: The most common biliary complication after ALDLT was biliary stricture. Hepatic artery stenosis, multiple bile duct anastomosis and bile leakage were significantly associated with biliary stricture. Biliary complications can be successfully managed with nonsurgical methods.

      • Saccharomyces cerevisiae에서 Trichoderma Endoglucanase의 발현과 분비

        The endoglucanase gene, egl6, of Trichoderma sp. was connected with the yeast ADH1 promoter, and the resultant plasmid, pVT-C4, was introduced into three S. cerevisiae host strains (YNN27, 2805, and SEY2102). Among each 80 transformants, the cell growth and expression level of endoglucanase were compared in test-tube cultivation, and three respective transformants for each host cells showing the highest expression level and cell growth were selected. When three recombinant yeast cells were batchwise cultivated for 48 hr in flask, the total activities of endoglucanase expressed were about 1140 unit/1 with 2805/pVT-C4, 1020 unit/l with SEY2102/pVT-C4, and 590 unit/l with (YNN27/pVT-C4. Irrespective of host strain, about 80% of the expressed endoglucanase was detected in the extracellular medium. In addition, it was also found that the recombinant enzyme was secreted into the culture medium as two major forms of lightly and heavily glycosylated proteins.

        Anti-inflammatory effects of a traditional Korean medicine: Ojayeonjonghwan

        <P><B>Abstract</B></P><P><B>Objective:</B> To study the anti-inflammatory properties of OJ.</P><P><B>Context:</B> Ojayeonjonghwan (OJ) is a traditional Korean prescription, which has been widely used for the treatment of prostatitis. However, no scientific study has been performed of the anti-inflammatory effects of OJ.</P><P><B>Materials and methods:</B> Peritoneal macrophages were isolated 3–4 days after injecting a C57BL/6J mouse with thioglycollate. They were then treated with OJ water extract (0.01, 0.1, and 1 mg/mL) for 1 h and stimulated with lipopolysaccharide (LPS) for different times. Nitric oxide (NO), inducible nitric oxide synthase (iNOS) and cyclooxygenase (COX)-2, and proinflammatory cytokine levels were determined by NO assay, Western blotting, RT-PCR and ELISA.</P><P><B>Results:</B> NO generation and iNOS induction were increased in the LPS-activated mouse peritoneal macrophages. However, NO generation and iNOS induction by LPS were suppressed by treatment with OJ for the first time. The IC<SUB>50</SUB> value of OJ with respect to NO production was 0.09 mg/mL. OJ did not influence LPS-stimulated COX-2 induction, but did significantly decrease LPS-stimulated secretions and mRNA expressions of tumour necrosis factor (TNF)-α, interleukin (IL)-6, and IL-1β. Inhibition rates of TNF-α, IL-6, and IL-1β at an OJ concentration of 1 mg/mL were 77%, 88%, and 50%, respectively. OJ also suppressed the LPS-induced nuclear translocation of NF-κB. High-performance liquid chromatography showed schizandrin and gomisin A are major components of OJ.</P><P><B>Conclusions:</B> OJ reduces inflammatory response, and this probably explains its positive impact on the prostatitis associated inflammation.</P>

        Open versus laparoscopic surgery for mid-rectal or low-rectal cancer after neoadjuvant chemoradiotherapy (COREAN trial): survival outcomes of an open-label, non-inferiority, randomised controlled trial

        <P><B>Summary</B></P> <P><B>Background</B></P> <P>Compared with open resection, laparoscopic resection of rectal cancers is associated with improved short-term outcomes, but high-level evidence showing similar long-term outcomes is scarce. We aimed to compare survival outcomes of laparoscopic surgery with open surgery for patients with mid-rectal or low-rectal cancer.</P> <P><B>Methods</B></P> <P>The Comparison of Open versus laparoscopic surgery for mid or low REctal cancer After Neoadjuvant chemoradiotherapy (COREAN) trial was an open-label, non-inferiority, randomised controlled trial done between April 4, 2006, and Aug 26, 2009, at three centres in Korea. Patients (aged 18–80 years) with cT3N0–2M0 mid-rectal or low-rectal cancer who had received preoperative chemoradiotherapy were randomly assigned (1:1) to receive either open or laparoscopic surgery. Randomisation was stratified by sex and preoperative chemotherapy regimen. Investigators were masked to the randomisation sequence; patients and clinicians were not masked to the treatment assignments. The primary endpoint was 3 year disease-free survival, with a non-inferiority margin of 15%. Analysis was by intention to treat. This trial is registered with ClinicalTrials.gov, number NCT00470951.</P> <P><B>Findings</B></P> <P>We randomly assigned 340 patients to receive either open surgery (n=170) or laparoscopic surgery (n=170). 3 year disease-free survival was 72·5% (95% CI 65·0–78·6) for the open surgery group and 79·2% (72·3–84·6) for the laparoscopic surgery group, with a difference that was lower than the prespecified non-inferiority margin (–6·7%, 95% CI −15·8 to 2·4; p<0·0001). 25 (15%) patients died in the open group and 20 (12%) died in the laparoscopic group. No deaths were treatment related.</P> <P><B>Interpretation</B></P> <P>Our results show that laparoscopic resection for locally advanced rectal cancer after preoperative chemoradiotherapy provides similar outcomes for disease-free survival as open resection, thus justifying its use.</P> <P><B>Funding</B></P> <P>National Cancer Center, South Korea.</P>

      • Molecular analysis of S gene of spike glycoprotein of winter dysentery bovine coronavirus circulated in Korea during 2002–2003

        <P><B>Abstract</B></P><P>Since the molecular analysis of spike (S) glycoprotein gene of bovine coronavirus (BCoV) has been conducted and compared mainly among American and Canadian isolates and/or strains, it is unclear whether BCoV circulated in the other countries are distinctive in genetic characteristics. In the present study, we analyzed the S glycoprotein gene to characterize 10 winter dysentery (WD) coronavirus strains circulated in Korea during 2002–2003 and compared the nucleotide (nt) and deduced amino acid (aa) sequences with the other known BCoV. The phylogenetic analysis of the entire S glycoprotein gene revealed that the aa sequences of all Korean WD strains were more homologous to each other and were very closely related to respiratory bovine coronavirus (RBCV) strain OK and enteric bovine coronavirus (EBCV) strain LY-138, but were distinct from the other known BCoVs. Based on the phylogenetic analysis of the hypervariable region of the S1 subunit, all Korean WD strains clustered with the respiratory strain OK, BCQ3994 and the enteric strain LY-138, while the Canadian BCQ calf diarrhea and WD strains, and the American RBCV LSU, French EBCV F15 and avirulent VACC, L9, and Mebus strains clustered on a separate major branch. These data suggest that the WD strains circulated in Korea had a genetic property of both RBCV and EBCV and were significantly distinct from the ancestral enteric strain.</P>

        당뇨병성 케톤산증을 동반한 십이지장소마토스타틴종 1 예

        The syndrome induced by excessive somatostatin secretion in somatostatinoma is characterized by steatorrhea, hypochlorhydria, cholelithiasis, and mild non-ketotic hyperglycemia. Since the first case of pancreatic somatostatinoma reported by Larsson et al in 1977, approximately 25 cases of pancreatic somatostatinama have been reported. Extrapancreatic somatostatinoma(mainly gastrointestinal somatostatinoma) was relatively rare, and first described by Kaneko et al in 1979. Gastrointestinal somatostatinomas are somewhat different from pancreatic somatostatinomas. Somatostatinoma syndrome is rarely seen in gastrointestinal somatostatinoma because of its low level of somatostatin. Due to the fact that somatostatin inhibits the secretion of insulin and glucagon simultaneously, the absence of diabetic ketoacidosis has been regarded. Jackson et al first described the diabetic ketoacidosis in the patient with malignant extrapancreatic somatostatinoma originated from the lung. As far as we can determine, intestinal somatostatinoma presenting with diabetic ketoacidosis has not been reported previously. In our case, the increased circulating level of somatostatin was detected and fasting insulin and C-peptide levels were suppressed, but plasma glucagon was not suppressed below normal level. Diabetic ketoacidosis was promptly corrected with insulin replacement and supportive fluid therapies, and after operation, the hyperglycemia was disappeared without insulin replacement. The mass was located at the second portion of duodenum and oval shaped, solid mass was seperated from the head of pancreas by a thick fibrous capsule. Light microscopic examination revealed that each tumor cells were polygonal shaped and had plenty of finely granular cytoplasm. The immunohistochemical stainings of tumor cells showed positive immunoreactivity for chromogranin and somatostatin but insulin, glucagon, calcitonin, and serotonin were negatively stained. The electronicroscopy of tumor cells showed electron-dense secretory granules and tonofilament bundles in cytoplasm. We presented a case of duodenal somatostatinoma in whom the diagnosis was recognized after presentation with diabetic ketoacidosis.

        Effect of Insamyangyoung-tang on the Skin Barrier Function of Hairless Mice

        Objective : To study the effect of the Insamyangyoung-tang(ISYT) extract on the skin barrier function, the skin pH, skin humidity and transepidermal water loss(TEWL) were measured and histological changes were observed in DNCB(2,4-dinitrochloro-benzen)-induced contact dermatitis(CD) hairless mice. Methods : The male hairless mice were divided into three groups. Each group consisted of 15 mice. The normal group which had acetone- olive oil applied. The control group which had intentionally induced CD by DNCB and it was fed normal saline orally. The ISYT group which had intentionally induced CD by DNCB and it was fed ISYT extract orally for 7 days. The three groups were checked 24h, 48h and 72h later after inducing CD, and the skin pH, skin humidity and TEWL were observed. Tissue samples were taken, and damage to the epithelial cell was observed. Statistical analysis was performed by using one way-ANOVA: significance was set at p values less than 5% (p<0.05). Results : ISYTextract efficiently maintained the pH balance, it kept the skin humidity at a normal level, and it inhibited TEWL of the DNCB-induced CD hairless mouse. The damage to the epithelium was decreased and the regeneration power of the skin was increased in the ISYT group. Conclusion : Insamyangyoung-tang has a good effect on the skin barrier function of DNCB induced contact dermatitis hairless mice.
