        <i>In vitro</i> and <i>in vivo</i> evaluation of a novel polymer-free everolimus-eluting stent by nitrogen-doped titanium dioxide film deposition

        <P><B>Abstract</B></P> <P>Inflammation and thrombosis are linked to the use of polymer-based drug-eluting stents (DES). The aim of this study was to develop a polymer-free everolimus (EVL)-eluting stent using nitrogen-doped titanium dioxide (N-TiO<SUB>2</SUB>) and verify its efficacy by <I>in vitro</I> and <I>in vivo</I> assessment in a porcine coronary model. Various analytical approaches such as scanning electron microscopy and atomic force microscopy, electron spectroscopy, Fourier transform infrared spectrometry and contact angle measurement were employed for the characterization. As a part of biocompatibility assessment, platelet adhesion and smooth muscle cell (SMC) proliferation were examined. Bare metal stent (BMS), N-TiO<SUB>2</SUB> stent, everolimus-eluting N-TiO<SUB>2</SUB> (N-TiO<SUB>2</SUB>-EVL) stent, and commercialized EVL-eluting stent (EES) were randomly placed in forty coronary arteries in twenty pigs. After four weeks of implantation, the stents were subjected to histological and quantitative analysis. The N-TiO<SUB>2</SUB> film used in this study was well coated without any cracks or peeling. Surface hydrophilicity (88.8% of angle decrement) could be associated with the decrease in surface roughness post N-TiO<SUB>2</SUB> deposition (37.0%). The platelet adhesion on the N-TiO<SUB>2</SUB> surfaces was less than that on the BMS surface. The proliferation of SMC was suppressed in the N-TiO<SUB>2</SUB>-EVL group (30.2%) but not in the BMS group. In the animal study, the percent area restenosis was significantly decreased in the N-TiO<SUB>2</SUB>-EVL group compared to that in the BMS group. The results (BMS; 47.0 ± 11.00%, N-TiO<SUB>2</SUB>-EVL; 31.7 ± 10.50%, and EES; 29.1 ± 11.21%, <I>n</I> = 10, <I>p</I> < 0.05) were almost at par with those of the commercialized EVL-eluting stent. The introduction of N-TiO<SUB>2</SUB> deposition during fabrication of polymer-free DES may be an efficient accessorial process for preventing in-stent restenosis and thrombosis.</P> <P><B>Highlights</B></P> <P> <UL> <LI> N-TiO<SUB>2</SUB> surfaces can help to reduce the platelet adhesion. </LI> <LI> In porcine model, N-TiO<SUB>2</SUB> everolimus decreased in-stent restenosis and fibrin deposition. </LI> </UL> </P>

        만성 궤양성 대장염의 추적관찰 - 단기적 치료 반응 및 재발율을 중심으로

        N/A Our retrospective study on 240 cases with chronic ulcerative colitis in Seoul National Uni- versity Hospital spanning from January,1975 to December,1993 revealed the following results. According to the extent of disease, there was 27.5% in proctitis, which was more frequent than that of the previous domestic reports, 15.8% in distal, 22.9% in left and 31.7% in exten- sive colitis. According to the severity of disease, there was 47.5% in mild, 32.9% in moderate and 19.6% in severe disease. There was a positive correlation between severity and extent of disease. The extraintestinal complications were not related with the extent, or severity. Howev- er 3 cases(1.3%) of toxic megacolon appeared in moderate or severe extensive colitis, one of which progressed to perforation. Two cases(0.8%) of mortality were noticed, one caused by perforation, the other by operation related complication. There was not a single case of colon cancer developed during follow up period. Most cases showed excellent short term response to medical treatment; the symptomatic remission rates were 90.3%, 85.7%, 72.9% and 58.9% in order of proctitis, distal, left and extensive colitis, and the endoscopic remission rates were 78.8%, 50.0%, 59.1% and 30.0% respectively. Higher remission rates corresponded with lower extent of the disease. According to the severity, the symptomatic remission rates of milder dis- ease were significantly higher. The cumulative relapse rates were 38.7% in 1 year, 62.4% in 2 years, 73.3% in 3 years and 81.9% in 5 years, which were not related to the extent but in- creased according to the severity. The cumulative operation rates were 5.0% in 1 year, 7.6% in 2 years, 10.2% in 3 years and 15.9% in 6 years. The emergency operation accounted for only 22.2% of all operation and the others were elective ones. The most frequent indication of surgery was medical intractability which was determined after mean 35 days of intensive med- ical therapy. We concluded that severity and extent of ulcerative colitis had predictive value of the responses to medical treatment or relapses, furthermore despite the high relapse rates, medical therapy was the first line treatment because of its high remission rates on a short- term base.         Genetic Variations of the Apolipoprotein B Gene in Korean People and Its Association with Hypercholesterolemia

        Genetic polymorphisms at the apolipoprotein B have been associated with elevated plasma concentrations of LDL, atherosclerosis and increased risk of coronary artery disease. 106 individuals composed of 46 hypercholesterolemic patients and 60 controls were analyzed with Ins/Del length polymorphism, four RFLPs (HincII, PvuII, AluI, EcoRI) and 3`-VNTR to clarify the characteristics of the apolipoprotein B gene and to determine their influence on the lipid profile of hypercholesterolemic patients and controls in Korea. A total of 212 apolipoprotein B alleles from six markers were identified in this study. Heterozygosityranges from zero to 0.32, and the genotyope frequencies in the case of 3`-VNTR are significantly different between two groups. Analysis of linkage disequilibrium revealed paired nonrandom associations between four pairs of polymorphic sites of the apolipoprotein B gene (p<0.05). Moreover, addition of all 4 polymorphic sites provided the highest PIC value. This study also investigated the association of these polymorphisms with the patients and with variation in lipid levels. A significant association between 3`-VNTR genotypes and Lp(a) was observed in hypercholesterolemic patients (p<0.05). The results suggest that genotypes of six polymorphic markers were not significantly associated with TC, TG or LDL-chol level between the patients and controls in the Koreans.

        Prevalence of Latent Tuberculosis Infection among Health Care Workers in South Korea: A Multicenter Study

        Background: We investigated the prevalence of latent tuberculosis infection (LTBI) among the health care workers (HCWs) and analyzed its risk factors in South Korea. Methods: A standard questionnaire regarding the baseline demographics and risk factors for LTBI was given to each participant and tuberculin skin test (TST), QuantiFERON-TB GOLD In-Tube (QFT-GIT) assay, and chest radiography were performed. Results: A total of 493 participants, 152 (30.8%) doctors and 341 (69.2%) nurses were enrolled in eight tertiary referral hospitals. The mean age of the subjects was 30.6 years old, and 383 (77.7%) were female. Of the 152 doctors, 63 (41.4%) and 36 (23.7%) were positive by TST and by QTF-GIT, respectively, and among the 341 nurses, 119 (34.9%) and 49 (14.4%) had positive TST and QFT-GIT results, respectively. Overall, the agreement between the two tests was 0.22 by the chance corrected proportional agreement rate (kappa coefficient) in 493 subjects. Experience of working in tuberculosis (TB)-related departments was significantly associated with positive LTBI test results by QFT-GIT assay, not by TST. In multivariate analysis, only age was independently associated with increased risk of a positive TST result, while age and experience of working in TB-related departments (odds ratio, 2.29; 95% confidence interval, 1.01-5.12) were independently associated with increased risk of a positive QFT-GIT result. Conclusion: A high prevalence of LTBI was found among South Korean HCWs. Considering the association between the experience of working in TB-related departments and high risk of LTBI, QFT-GIT may be a better diagnostic test for LTBI than TST in HCWs.

      • 151종 생약제 추출물이 B형 간염 바이러스 표면 항원(HBsAg)과 항체(HBsAb)의 결합반응 그리고 HBV DNA polymerase 활성에 미치는 효과 : 한국산 각종 생약제를 이용한 B형 간염치료제의 개발

        세계인구 43억명 중 B형 간염 바이러스 보균자는 2억 1천 5백만명이며 이중 78%인 1억 6천만명 이상이 아시아 대륙에 집중되어 있다. 한국도 B형 바이러스 간염의 침윤지대이다. 본 연구는 B형 바이러스 보균자 혹은 만성간염을 치료할 수 있는 생약제를 탐색할 목적의 일환으로 한국에서 시판하는 생약제 151종을 구입하여 증류수로 추출하고 여과한 후 냉동건조하여 HBs 항원에 대한 결합능, HBV DNA polymerase억제능, tumor necrosis factor(TNF) 유리에 미치는 영향, 그리고 galactosamine 유발 간염에 대한간장보호 작용을 조사하였다. 151종의 생약제중 0.16㎍/㎖ 농도에서 간염 vaccine 내에 함유된 HBs 항원과 결합하는 생약제는 지유, 가자, 복분자, 대황, 정향, 빈량, 산수유, 소목, 마황, 쇄양, 후박, 목과, 양강, 오약, 산사, 목단피 등 16종이었다. 이들 생약제중 대황(Rheum palmatum L. 大黃), 가자(Terminalia chebula R. 訶子), 지유(Sanguisorba officinalis L. 地楡), 복분자(Rubus coreanus M. 覆盆子)는 HBV DNA polymerase 활성도를 비교적 강하게 억제하였으며 빈량(Areca catechu L. 빈랑), 정향(Eugenia caryophyllata T. 丁香), 목과(Chaenomeles japonica L. 木瓜), 산수유(Cornus officinalis S. 山茱萸)는 약하게나마 HBV DNA polymerase 활성을 억제하였다. 또한 HBV DNA polymerase를 억제한 대황, 가자, 지유, 복분자, 빈량, 정향, 마황, 소목, 산수유를 대상으로 TNF 유리에 미치는 영향을 조사한 결과 소목을 제외한 8종의 생약제 모두가 대식세포를 자극하여 TNF를 유리시켰다. 그리고 장차 임상에 사용함을 전제로 HBV DNA polymerase를 억제한 대황, 가자, 지유, 복분자, 빈량, 정향, 마황, 소목, 산수유를 galactosamine 유발 간염(백서)에 투여하여 간기능 검사에 미치는 효과를 조사하였으나 간기능 검사상으로는 특별한 보호작용을 나타내지 못하였으며 생약제 단독 투여로서는 간기능에 대하여 나쁜 영향을 주지 않았다. Hepatitis B virus infection is major cause of acute and chrome hepatitis, cirrhosis of liver, and hepatocellular carcinoma. There are over 200 million carriers of hepatitis B virus in the world, nearly 80 % of whom live in the South East Asia, Korea is also highly endemic area of hepatitis B virus infection. This study was performed to evaluate the effect of extracts of traditional Korean herbal medicines on hepatitis B virus. 151 traditional herbs were extracted, filtered and lyophilized. Using 151 herbal extracts we tested binding capability to the HBs Ag, inhibition of HBV DNA polymerase, release of tumor necrosis factor (TNF), and protection against galactosamine induced liver damage. Among 151 herbs, 16 herbs (Sanguisorba officinalis L., Terminalia chebula R., rubus coreanus M., Rheum palmatum L., Eugenia caryophyllata T., Areca catechu L., Cornus officinalis S., Caesalpinia sappan L, Ephedra sinica S., Cynomorium songaricum R., Machilus thunbergii S., Chaenomeles japonica L., Alpinia officinarum H., Lindera strychnifolia V., Crataegus pinnatifida B., Paeonia suffruticasa A. ) had binding capability to HBs Ag which contained in the commercial hepatitis vaccine, at 16 ug/㎖ of concentratioa Among the above 16 herbs, Rheum palmatum L., Terminalia chebula R., Sanguisorba officinalis L., Rubus coreanus M. significantly inhibited the HBV DNA polymerase activity, and Areca catechu L., Eugenia caryophyllata T., Ephedra sinica S., Caesalpinia sappan L., Chaenomeles Japonica L., Cornus officinalis S. also inibited the HBV DNA polymerase activity but less effective. All of the herbs, except Caesalpinia sappan L., which inhibited the HBV DNA polymerase activity, actively stimulated the mononuclear cells to release tumor necrosis factor(TNF) in whole blood culture system. The herbs which inhibited the HBV DNA polymerase activity, did not show any protective effect in galactosamine-induced hepatitis in rats, but there was no direct toxic effect when herbs were administered alone.

      • 신생아 중증 세균성 감염에 대한 Augmentin(clavulanate potentiated amoxycillin)의 치료효과 및 안전성 연구

        Combination therapy of β-lactam antibiotics has been used widely to treat severe infections in neonates. The widespread use of β-lactam antibiotics has resulted in emergence of a broad variety of plasmid or chromosome mediated β-lactamases. Also, there is a great propensity for these enzymes to spread among various species of bacteria. Thus, the use of β-lactamase inhibitor antibiotics may be of great interest and has potential therapeutic value in severe neonatal infections. We tried to find out the efficacy and safety of clavulanate potentiated amoxycillin (Augmentin) in treatment of severe neonatal infections. Total thirty-five neonates with sepsis(26) or severe organ infections(9) were involved. Each 30mg/kg dose of Augmentin was injected at 12hr-or 8hr-or 6hr-intervals according to the age and weight of neonates. Clinically 77.1% of the cases were cured, and microbiologically 80.0% were eliminated. 88.9% of eighteen gram positive organisms(S. aureus, S. epidermidis, a -hemolytic strep., S. pneumoniae , E. faecalis) isolated from study group were susceptible to Augmentin. 47.0% of gram-negative organisms (K. pneumoniae, E.coli, P. mirabilis, S. marcescens, C. freundii) were suceptible. Thus, it was confirmed that Augmentin was very effective in ampicillin-or ceftriaxone-resistant gram positive cocci infections, but less effective in isolated K. pneumoniae and S.marcescens cases. The serum levels of amoxycillin in the neonates were maintined about 6㎍/ml until 1hr. at each injection. We observed that alkaline phosphatase level was elevated above 1,000IU/ℓ in three neonates and that skin rash developed after injection in one neonate. But, these side effects were mild and transient. These results suggest that Augmentin may be tolerable and effective in treating severe gram positive cocci infections in neonates.

        In recent years, the rolling stock is being increasingly developed. However, higher axle loads have increased the incidence of fatigue defects in railroad rails. Many of these fatigue-initiated defects develop in the head of the rail. The head defects are very important because they are generally not visible until they reach quite large size and they can lead to catastrophic rail fractures. In this paper, stress intensity factors(SIF) of a embedded crack in the head of the rail were analysed along the wheel positions relative to the crack plane and along the running condition using the finite element method.
