Blood tests have been used to develop treatment plans, such as disease diagnosis, treatment effect, and prognosis determination in livestock. The present study examined changes in the blood count, including RBCs, WBCs, and platelets, before and after grazing among the Korean native cattle grazing from spring to autumn. The study compared the blood count of livestock group (A, n=34) that returned from the rangeland to cowshed every evening and livestock group (B, n=21) that were not returned from the rangeland to cowshed every evening during the grazing period. Hematological parameters such as RBC, hemoglobin, hematocrit, MCV, and WBC, neutrophil, eosinophil, monocyte, and lymphocyte were determined using a blood cell analyzer. Livestock group (A) showed significant differences in the values of RBC, MCV, MCHC, WBC, EOS, and LYM. Livestock group (B) showed significant differences in the values of RBC, Hb, HCT, MCV, MCH, PLT, NEU, and BAS. In particular, the RBC count decreased after grazing when compared with that before grazing. Compared with that before grazing, low RBC count was maintained from the first to fifth month of grazing. The WBC count increased from the fourth month of grazing, whereas the EOS count increased from the second month of grazing. These results suggest that the values of RBC and WBC vary before and after grazing, and also with the grazing period. Furthermore, it is necessary to develop a detailed feeding management based on these values of blood analysis for livestock that are raised continuously in the rangeland.

Background: This retrospective comparative study aims to evaluate the surgical outcomes and complications of two surgical methods for simple fractures of the humeral shaft; minimally invasive anterior plating and open reduction combined with internal fixation. Methods: A total of 26 patients with humeral shaft simple fractures, who had surgery between June 2009 and September 2013 and were followed-up at least 12 months, were included in our analysis. They were divided into two groups; group 1 comprised of 12 patients who underwent minimally invasive anterior plating and group 2 comprised of 14 patients who underwent an open reduction and internal fixation. The clinical outcomes, radiological results, and complications were compared and analyzed. Results: We found that bone union was achieved in all patients, and the mean union periods were 20.7 ± 3.34 and 20.3 ± 3.91 weeks for groups 1 and 2, respectively. In most patients, we found that shoulder and elbow functions were recovered. At 12 months post-operation, we found that the Korean Shoulder Scoring system, the University of California at Los Angeles score and Mayo elbow performance score were 91.4 ± 7.97, 33.4 ± 1.15, and 90.8 ± 2.23 for group 1, and 95.2 ± 1.53, 33.3 ± 1.43, and 90.17 ± 1.85 for group 2. In terms of complications, we found that 2 patients had radial nerve palsy after open reduction and internal fixation, but all cases spontaneously resolved within 6 months. Complications such as infection and loss of fixation were not reported. Conclusions: Both minimally invasive anterior plating and open reduction with internal fixation produced satisfactory outcomes in the treatment of simple fractures of the humeral shaft.

Background/Aims: To improve the eradication rate of a first-line therapy for Helicobacter pylori infection, alternate regimens such as sequential, concomitant, and hybrid therapies have been tried. The aim of this study was to evaluate the eradication rate of the 10-day hybrid therapy as a first-line therapy.Materials and Methods: This retrospective study enrolled 124 patients from the Korea University Ansan Hospital between April 2016 and December 2019. The 10-day hybrid therapy comprised 5 days of dual therapy (proton pump inhibitor [PPI] standard dose and amoxicillin 1 g, twice daily) followed by 5 days of quadruple therapy (PPI, amoxicillin 1 g, clarithromycin 500 mg, and metronidazole 500 mg, twice daily). We compared the 10-day hybrid therapy with the 10-day concomitant therapy comprising PPI, amoxicillin 1 g, clarithromycin 500 mg, and metronidazole 500 mg, twice daily. Eradication was assessed by a <sup>13</sup>C-urea breath test or gastroscopic biopsy at least 4 weeks after treatment completion.Results: The eradication rates of the 10-day hybrid and concomitant therapies were 74.2% (46/62) and 67.7% (42/62), respectively, in the intention-to-treat (ITT) analysis and 88.5% (46/52) and 82.4% (42/51), respectively, in the per-protocol (PP) analysis. There was no significant difference in the eradication rates between the two groups in the ITT (P=0.429) and PP analysis (P=0.380). Adverse events developed in 75.0% and 70.6% of patients in the hybrid and concomitant groups, respectively, but there was no significant difference (P=0.615).Conclusions: The 10-day hybrid therapy can be an option for a first-line therapy of Helicobacter pylori infection.
A Case Report of Fast Growing Mediastinal Desmoid Tumor

Desmoid tumors (DTs) are rare and benign soft tissue tumors that result from monoclonal proliferation of well-differentiated fibroblasts. Since DTs tend to infiltrate and compress adjacent structures, the location of DTs is one of the most crucial factors in determining the severity of the disease. Furthermore, DTs can further complicate the clinical course of the patients whenthe growth is remarkably rapid, especially for DTs occurring in anatomically critical compartments including the thoracic cavity. We report a case of a 71-year-old man with a known mediastinal mass incidentally detected 4 months ago, presenting dyspnea with right-sided atelectasis and massive pleural effusion. Imaging studies revealed a fibrous, 16.4 cm×9.4 cm-sized anterior mediastinal mass with high glucose metabolism. The mass infiltrated into the chest wall and also shifted the mediastinum contralaterally. Interestingly, the tumor had an extremely rapid doubling time of 31.3 days. En bloc resection of the tumor was performed as a curative as well as a diagnostic measure. Histopathological examination showed spindle cells with low cellularity and high collagen deposit in the stroma. Immunohistochemical staining showed the positivity for nuclear β -catenin. Based on these pathologic findings, the mass was diagnosed as DT. After the surgery, there has been no evidence of recurrence of disease in this patient.

The best way to prevent foot-and-mouth disease (FMD) constantly occurring in Korea will be vaccination. In this study, FMD vaccines were given to Korean native cattle (Hanwoo), dairy cattle, and pigs to investigate the antibody positive rate of FMD vaccine by age in year and month. Hanwoo, dairy cattle, and pigs showed antibody positive rates of 99.5%, 97.7%, and 95.9%, respectively. High antibody positive rates more than 95% were found in Hanwoo and dairy cattle. In particular, high antibody positive rates were found in Hanwoo and dairy cattle regardless of age. Pigs showed a relatively low antibody positive rate of 57.6% at 3 months of age and then constantly maintained a high antibody positive rate of above 95.0% after 4 months of age. As a result of this study, high antibody positive rates were found when regular FMD vaccination was given to newborn calves and piglets after FMD vaccination twice to them. Therefore, it is considered the most important to receive vaccination thoroughly according to vaccination plan in order to prevent FMD.

Purpose: In patients with altered mentality caused by drugs or unknown causes, ammonia is checked to facilitate differential diagnosis or diagnose hepatic coma. This helps early prevention and treatment of brain damage due to hyperammonemia. This study was conducted to evaluate clinical characteristics of intoxicated adult patients with hyperammonemia. Methods: We evaluated 95 patients with hyperammonemia among intoxicated patients above the age of 15 who visited our ED from January 2013 to December 2015. We analyzed the demographic characteristics and type of poisoning substance, reason for ingestion, toxicological characteristics such as elapsed time from ingestion to hospital visit, lab, clinical progression and complications. Data were evaluated using the student`s t test or Mann-Whitney U test for continuous variables, and Chi-square test and Fisher`s exact test for frequency analysis of categorical variables. Results: When compared to healthy individuals, patients with hyperammonemia showed statistical significance on their SOFA score (p=0.016) and poison severity score (p<0.001). Additionally, patients with hyperammonemia showed significantly different initial serum AST level (p=0.012) and maximum serum AST level during the hospital stay (p=0.026) when compared to healthy individuals. Moreover, individuals with sustained hyperammonemia compared to transient hyperammonemia showed clinically significant SOFA scores (p<0.001), poison severity scores (p=0.007), mortality rates in the ICU (p=0.021), as well as different duration of hospital stay (p=0.037), serum creatinine level (p=0.002), erythrocyte sedimentation rate (p=0.025), and serum myoglobin (p=0.015). Conclusion: Most poisoning-induced hyperammonemia cases were transient and recovered without special treatment. Therefore, hyperammonemia is almost non-specific among poisoning patients.
