This paper presents the constituent-based approach for aligning bilingual multiword expressions, such as noun phrases, by considering the relationship not only between source expressions and their target translation equivalents but also between the expressions and constituents of the target equivalents. We only considered the compositional preferences of multiword expressions and not their idiomatic usages because our multiword identification method focuses on their collocational or compositional preferences. In our experimental results, the constituent-based approach showed much better performances than the general method for extracting bilingual multiword expressions. For our future work, we will examine the scoring method of the constituent-based approach in regards to having the best performance. Moreover, we will extend target entries in the evaluation dictionaries by considering their synonyms.

A 54-year-old man presented with frequent episodes of sudden palpitation. Electrocardiogram demonstrated a narrow QRS complex tachycardia with a rate of 150/min. During the tachycardia that J point of the QRS complex occasionally became deeper and sharper(figure1). Electrophysiologic study was performed. A narrow QRS complex tachycardia was reproducibly induced by a single atrial extrastimulus. The tachycardia showed an intermittent change of the J point (figure 2). Atrial activity at the high right atrium (HRA) always preceded those of the His bundle and coronary sinus (CS) during the fi rst SVT with an atrial cycle length of 344 ms, but retrograde atrial activation sequence in the CS electrograms changed occasionally. The activation of right anterolateral and left lateral accessory pathway (AP) made the J point deeper and sharper than right anterolateral AP activation alone. Radiofrequency catheter ablation could abolish conduction over right side AP. A single ventricular extrastimulus induced the second tachycardia (388 ms) with the earliest, consistent retrograde atrial activity at distal CS. Electrocardiographic or electrophysiologic fi ndings that demonstrate the multiple AP include 1) changing antegrade delta waves; 2) evidence of multiple routes of retrograde atrial activation; 3) orthodromic SVT with intermittent antegrade fusion complexes. 1 In our instance, additional AP could be recognized by more than one atrial breakthrough points. Here were intermittent changes of retrograde atrial activation sequence in the CS while retrograde atrial activation in the HRA was earliest. The constantly earliest activation in the HRA suggests the clinical SVT uses the right anterolateral AP as a reentry circuit and intermittently the left lateral AP as a bystander. Atrial fusion due to intermittent activation of the two accessory pathways changed the J point on the electrocardiogram. Careful attention to the J point during the tachycardia could help identify multiple accessory pathways before electrophysiologic study.

Purpose: This study examined emergency medical service (EMS) transportation patterns for adult trauma patients before and after establishing a level 1 regional trauma center (RTC) and to evaluate the transportation approach after prehospital severity screening. Methods: This was a retrospective observational study of trauma patients aged ≥18 years admitted via EMS to the emergency department or a level 1 RTC, 1 year before to 3 years after RTC establishment. Patients with an Injury Severity Score (ISS) in the patient registration system were selected. Analyses were performed to determine transportation pattern changes by comparing patients pre- and post-RTC establishment and by yearly comparisons over the 4-year study period using the Mann-Whitney U test and chi-square test. Results: Overall, 3,587 patients were included. The mean ISS was higher in the post-RTC group (n=2,693; 10.63±8.90, median 9.00) than in the pre-RTC group (n=894; 9.44±8.20, median 8.00; p<0.001). The mean transportation distance (9.84±13.71, median 5.80 vs. 13.12±16.15 km, median 6.00; p<0.001) was longer in the post-RTC group than in the pre-RTC group. Furthermore, proportionally fewer patients were transported from an area in the same city as the RTC after establishment (86.1% vs. 78.3%; p<0.001). Yearly comparisons revealed a gradually increasing trend in the hospital death rate (p<sub>trend</sub>=0.031). Conclusions: After establishing a level 1 RTC, the EMS transportation of severe trauma patients increased gradually along with the long-distance transportation of minor trauma patients. Therefore, improved prehospital EMS trauma severity assessments and level 1 RTC involvement in patient classification in the prehospital phase are necessary.

Spontaneous spinal epidural hematoma is extremely rare. It can cause fatal complication such as permanent neurologic deficit. Even though the exact cause of epidural hematoma is not known in most cases, anticoagulation treatment is associated in some cases. In this situation, prompt reversal of anticoagulation and immediate decompression surgery is required. Seventy-year-old woman presesnted with sudden hemiplegia. She had history of hypertension, dyslipidemia, and paroxysmal atrial fibrillation and had taken antihypertensive agent, statin, and warfarin. She did not have any history of trauma and her international normalized ratio was within therapeutic range during follow-up and at the time of event. Physical examination revealed decreased right extremities’ motor and sensory function but cognitive function was normal. Detailed neurologic examination and spinal magnetic resonance imaging identified extensive, spontaneous spinal epidural hematoma. After the reversal of anticoagulation with vitamin K injection, the patient underwent emergent hematoma removal surgery and recovered completely. Our case suggests epidural hematoma should be considered in sudden hemiplegia patients with anticoagulation therapy. Immediate reversal of anticoagulation and decompression surgery can avoid neurologic deficit.

본 논문은 인공신경망(artificial neural network)의 한 종류인 자기조직화 지도(self-organizing map)를 이용하여 비교말뭉치(comparable corpora)로부터 이중언어사전(bilingual lexicon)을 자동으로 구축하는 방법에 대하여 기술한다. 일반적으로 우리가 대상으로 하는 언어 쌍마다 말뭉치 혹은 초기사전과 같은 언어 자원을 수집하고 그것을 필요에 맞게 가공하는 것은 매우 어려운 일이다. 이런 관점에서 볼 때, 비지도학습(unsupervised learning) 방법 중 하나인 자기조직화 지도를 이용하여 사전을 구축하면 다른 방법에 비해 적은 노력으로도 더 높은 성능을 얻을 수 있다. 본 논문에서는 한국어와 불어에 대하여 실험을 하였고, 그 결과 적은 양의 초기사전으로도 주목할 만한 정확도를 얻을 수 있었다. 향후 연구로는 학습 파라미터에 대해 좀 더 다양한 실험을 하고, 다른 언어 쌍으로의 적용 및 기존의 평가사전을 확장하여 더 많은 경우에 대해 실험하는 것을 들 수 있다.
