        토끼의 협골궁을 이용한 유경골이식과 유리골이식의 비교연구

        The membranous bone is new used frequently to fill the defect made during the craniofacial osteotmy. The membranous bone is resolved less than the endochondral bone after the free graft, and maintains its greater preportion when it is transfered with its vascular pedicle. It is also known that there is less resorption in the orthotopic graft than in the heterotopic graft. We have stuided the amount of resorption of the orthotopically grafted bone in the vascularized and nonvascularized model. A zygomatic arch of the rabbit was resected completely and was fixed orthotopically using a wire. The contralateral one was resected and fixed with its muscle attachment(masseter). After 8 weeks the animals were sacrificed. The volume of the grafted bone was compared. There 10±2% volume loss in the vascularized modol and 18±4% volume loss in the free graft model with a statistically significant difference(P<0.001). Histologically, in the vascularized graft, primary bony union was observed and the original architecture was preserved well and the periosteal activity way great. The free graft was also healed by bony union except in 1 case. But the architectural irregularity was more prominent in the free graft than in the vascularized graft.

      • GO-53 : Total vaginal necrosis after radiation for recurrent cervical cancer: A case report

        Total vaginal necrosis (TVN) is a rare, late complication after radiation treatment that requires prolonged management. A 75-year-old woman with a history of whole pelvis and intracavitary radiotherapy (ICR) for squamous cell carcinoma stage IIB in 1981 (complete remission) and another 6th cycles of chemotherapy due to cervical recurrence in 2009 developed second local recurrence confirmed by cervical biopsy in 2012. The vaginal length had been extremely shortened due to atrophy. The patient received ICR (3000 cGy) for a month. Seven months after radiation, the patient suffered pelvic pain, foul-odored vaginal discharge and urinary leakage. Pelvic examination showed total necrotic change of vagina. Vaginal biopsy showed no evidence of malignancy. Despite a month of conservative treatment, symptoms aggravated and thus necessitated intensive inpatient treatment. Therapies included daily vaginal irrigation, dressing with chlorohexidine gluconate and saline, intravenous antibiotics, and pain control with oral opioid and NSAIDs. Only minimal surgical debridement was possible due to diffusely necrotic and friable epithelium. Two kinds of vaginal suppositories (estradiol/ lyophilized lactobacilli, and neomycin sulfate / nystatin / polymyxin B sulfate) were inserted daily alternatively. Medication dosage was tapered gradually after 3 months. Five months after treatment, the vaginal wall was significantly improved with pinkish normal-looking epithelium. Although no definitive treatment is proven optimal for TVN so far, several conservative options may help alleviate symptoms.

      • Association of Diagnostic Lung Biopsy on the Risk of Spread Through Air Spaces (STAS) and Its Prognostic Impact in Resected Stage I Non-Small Cell Lung Cancer (NSCLC)

        Background Spread through air spaces (STAS) is a recently determined pathologic phenomenon of lung cancers with significant impact on prognosis. However, the possible influence of mechanical forces caused by diagnostic interventions on the occurrence of STAS has not yet been explored. The purpose of this study was to evaluate the association of diagnostic lung biopsy on the risk of STAS, and its prognostic impact in resected stage I NSCLC. Methods We performed a retrospective review of 2,238 patients who underwent surgery for stage I NSCLC, from January 2011 to December 2019 in Seoul National University Bundang Hospital. The risk factors for STAS was assessed by logistic regression. In addition, the association between previous diagnostic lung biopsy and incidence of recurrence, lung cancer-related mortality were assessed stratified by STAS positivity with Multivariate Cox’s regression analyses and the Kaplan-Meier method. Results Of the 2,238 patients analyzed, 677(30.2%) patients had positive STAS. Lung cancer with STAS tended to be solid tumors with greater tumor size and T stage. In the univariate logistic regression analysis for the risk of STAS, previous percutaneous needle biopsy (OR=1.81, P<0.001), bronchoscopic biopsy (OR=1.42, P<0.030), bronchoscopic washing and BAL(OR=1.41, P<0.005) were all significantly associated with the risk for STAS. However, in the multivariate analysis, only solid type(OR=2.54,P<0.001) and tumor size (OR=1.76, P<0.001) remained significant for STAS positivity. Multivariate Cox proportional hazards models showed that after adjustments for previous diagnostic procedures, STAS was a significant risk factor for cancer recurrence(OR=1.62, P=0.005). In addition, for STAS positive patients, sublobar resection remained a significant risk factor for recurrence(OR=2.93, P<0.001) and lung cancer related mortality(OR= 9.58, P<0.001) after adjustments for previous diagnostic procedures. Conclusions Previous lung biopsy and intervention in stage I NSCLC are not associated with the development of STAS, and do not affect the prognosis of NSCLC related to STAS.

        Which Cognitive Tasks Predict Traffic Accidents in Professional Drivers? Comparison between Middle-Aged and Older Drivers

        While most research on safe driving has focused on young adults and the elderly, professional drivers are typically middle-aged or older adults. To address this gap, we examined differences in cognitive functions between middle-aged and older adult drivers and identified which tasks predicted traffic accidents for each group. We administered tasks related to Useful Field of View, Visuomotor Speed, Spatial Judgment, Visual Working Memory, and Multitasking to each group and compared their performance with traffic accident records from the past 5 years. Results showed that older drivers performed significantly worse than middle-aged drivers on all cognitive tasks except Visuomotor Speed (p < .10), which was measured using a foot-pedal. We also found that cognitive function tasks could predict traffic accidents in both groups. Our findings suggest the need to include middle-aged drivers in research on safe driving, and to develop tailored driving aptitude tests that reflect the needs of this population.

        In this paper, we performed the customization of CAE analysis in the process of product development and verification. The purpose of customized analysis is to implement collaboration framework of inter-corporate, inter-division for knowledge process based on product development. Customized analysis methodology using ANSYS Workbench API is presented for the web-based and workbench-based application. Cellular phone was selected as a CAD model, and template based process of analysis is conducted. By using this system, customized analysis is suitable to site or user oriented specific analysis and for user who has little experience on CAE analysis.

      • Auditory and Speech Performance in Deaf Children With Deaf Parents After Cochlear Implant

        OBJECTIVE: To evaluate the auditory and speech outcome in deaf children with deaf parents (CDP) after cochlear implantation (CI), emphasizing both the presence of additional caregiver and patients’ main communication mode. STUDY DESIGN: Retrospective case review. SETTING: Cochlear implant center at a tertiary referral hospital. PATIENTS: Fourteen CDP and 14 age- and sex-matched deaf children with normal-hearing parents (CNH). MAIN OUTCOME MEASURE(S): The Korean version of Ling’s stage (K-Ling) and Category of Auditory Perception (CAP) were administered to the children to assess the speech production and auditory perception abilities, preoperatively and 3, 6, 12, and 24 months after CI. To elucidate the effects of the additional caregiver and main communication mode of the implanted child, the patients were divided into the following groups: with additional caregiver(s) who have normal hearing (n = 11) versus without additional caregiver (n = 3); sign language plus oral communication (S+O) group (n = 9) versus oral communication only (O) group (n = 5). RESULTS: CAP scores and K-Ling stages improved remarkably in both CDP and CNH, and no significant differences were found between the 2 groups. Within the CDP group, CAP scores and K-Ling stages improved significantly in CDP with an additional caregiver than those without. Auditory perception and speech production performances in the S+O group were similar to those in the O group. CONCLUSION: CDP can develop similarly to CNH in auditory perception and speech production, if an additional caregiver with normal hearing provides sufficient support and speech input. In addition, using sign language in addition to oral language might not be harmful, and these children can be a communication bridge between their deaf parents and society.

        인슐린을 함유한 키토산-폴리카프로락톤 복합 나노섬유 시트형 창상피복재의 특성평가

        창상피복용 구조체는 생체적합성과 물리적 강도가 보장되고 외부의 감염균으로부터 상처부를 보호하면서, 상피층과 진피층의 재생을 촉진할 수 있도록 설계되어야 한다. 본 연구에서는 키토산과 poly(ε-caprolactone)(PCL) 및 인슐린 혼합물의 전기방사를 통해 나노섬유 시트형 창상피복재를 제조하였다. 혼합비율에 따른 각 나노섬유 시트의 미세구조분석, 원소조성분석, 인장시험, 생분해성, in vitro 약물방출능, 항균능, 세포증식 거동, in vivo 동물실험을 통하여 물리화학적 특성분석 및 창상치유능을 평가하였다. 키토산의 비율이 증가할수록 시트의 변형률이 감소하였고, 생분해성, 약물방출능 및 항균능은 증가하는 경향을 보였다. 동물실험 및 조직분석결과에서 키토산-PCL-인슐린 나노섬유 시트는 다른 나노섬유 시트보다 촉진된 상처회복률을 보였으며 창상피복용 소재로의 활용이 기대된다. The construct for wound dressings should be designed to promote the regeneration of the epidermal and dermal layers while ensuring biocompatibility and physical strength with protecting the wound from external infectious bacteria. In this study, nanofibrous sheet type wound dressings were prepared by electrospinning of a mixture of chitosan, poly(ε- caprolactone) (PCL) and insulin. The nanofiber sheets according to the mixing ratio were analyzed for physicochemical properties through structural analysis, elemental composition, tensile test, biodegradability, drug release behavior, antibacterial activity, cell proliferation behavior, and wound healing ability via in vivo animal testing. As the content of chitosan increased, the elongation of the nanofiber sheets decreased, and the biodegradability, drug release ability, and antibacterial activity tended to increase. In animal experiments and histological analysis, the accelerated wound healing rate of the chitosan-PCL-insulin nanofibrous sheet was verified when compared to other nanofiber sheets, and thus it is expected to be used as a wound dressing material.

        Immunohistochemical study of the distribution of sodium-dependent vitamin C transporters in adult rat brain

        <P>Sodium-dependent vitamin C transporters (SVCTs) is known to transport the reduced form of ascorbic acid into the cell, whereas the oxidized form of vitamin C (VC) is moved through a facilitative sugar transporter, such as glucose transporter (GLUT). With regard to the distribution of SVCT1 and -2 within the various organs, they were reported to be expressed in different types of cells. Especially in the central nervous system, only SVCT2 mRNA was expressed mainly in neurons and some types of neuroglial cells. However, data on the expression of SVCT proteins in the brain are scant. Therefore, we tried to develop comprehensive data on the distribution of SVCT proteins in adult rat brain by using immunohistochemical techniques for the first time. In our study, SVCT2 immunoreactivities (IRs) were intensely localized in the neurons of cerebral cortex, hippocampus, and Purkinje cells of cerebellum, and much weaker SVCT2 IRs were found in the other brain regions. Judging from double-immunohistochemical data, most of the cells expressing SVCT2 IRs were likely to be neurons or microglia, even though the cells in choroids plexus or ependymal cells around the ventricles also exhibited SVCT2 IRs. Complete mapping of the distribution of SVCT2 IRs was available by using a semiquantitative method. The subcellular localization of SVCT proteins is necessary for understanding the exact role of the protein, so the current overall mapping of SVCT IRs in the rat brain could be the basis for further studies on related subjects. © 2006 Wiley-Liss, Inc.</P>
