Spindle cell lipoma classically occurs as a solitary, subcutaneous, circumscribed lesion in the posterior back, neck, or shoulders of older men. Histologically, classic spindle cell lipoma is variably composed of ropey collagen, myxoid stroma, CD34-positive spindled cells, and mature fat. The variable proportion of these elements among different cases of the neoplasm makes diagnosis difficult. Herein, a 53-year old man presented for several years with an asymptomatic 2cm sized subcutaneous mass on left periauricular area. Skin biopsy specimen revealed prominent collagen bundles and focal myxoid stroma with mixed spindle cells and mature adipocytes which was noted to be present in <5% of the tumor. Immunohistochemical studies showed that spindle cells are positively stained with CD34 and myxoid stroma is positively stained with alcian-blue pH 2.5. We concluded that this is consistent with low-fat, fibrous variant of spindle cell lipoma.
Neutrophilic eccrine hidradenitis: associated with eosinophilic granulomatosis with polyangiitis

Patients with eosinophilic granulomatosis with polyangiitis (EGPA) may develop palpable purpura and nodules, typically located on the limbs and scalp, which showed leukocytoclastic vasculitis mostly involving venules with the findings of numerous eosinophils in the infiltrate, in addtion to neutrophils, lymphocytes and macrophages. A 39-year-old Korean female presented with a 2-week history of localized, bilateral, multiple, and polycyclic to annular, smooth erythematous to reddish edematous papules and plaques with indurated subcutaneous nodules on the both palms. Ten days before visit our dermatology clinic, the patient was diagnosed with EGPA and had done only workup for EGPA diagnosis without any treatments or medications until visit dermatology clinic. She had no medications and did not do anything which can stimulate her hand. Hematoxylin and eosin staining showed perivascular patchy mixed inflammatory infiltrates of eosinophils, lymphocytes and plasma cells in the dermis, and focal dense neutrophils, eosinophils and lymphocytes infiltrates in the subcutaneous layer with eccrine glands area. We report a case of EGPA with unusual palmar eruption that revealed the clinicopathologic findings of neutrophilic eccrine hidradenitis (NEH). Despite the absence of previous cases, NEH may be considered as an additional skin manifestation that can be seen in patients with EGPA.
A clinical study of a novel noninvasive radiofrequency technology for the reduction of submental fat

Background: Recently, Novel radiofrequency (RF) technologywas introduced as a promising tool for non-invasive body and face contouring, which can heats subcutaneous fat up to 450C continuously without surface damage and induces adipocytes apoptosis. Objectives: To evaluate the efficacy and safety of novel noninvasive RF technology for the reduction of submental fat. Methods: Twenty-two subjects with an excessive submental fat received twice treatments at 4-weeks intervals. The thickness of the submental fat, the change of facial contour and subjective satisfaction were assessed at baseline, after treatment, and 8-weeks after last treatment. Results: All subjects showed a decrease in thickness of the submental fat objectively. Subjective satisfaction is also very high and adverse problems were minimal. Conclusion: A novel noninvasive RF technology is effective and safe modality for reduction of submental fat.

Background: Eyelid tumors are common neoplasms in daily dermatology practice and encompass a wide variety of benign and malignant tumors, sometimes mimicking common inflammatory conditions. Still, there have been insufficient clinical investigations of these conditions. Objectives: This study was performed to investigate the epidemiological aspects of eyelid skin tumors. The aim of this study was to provide dermatologist with recent data on the relative frequency and the clinical features of the various eyelid skin tumors. Methods: We retrospectively reviewed data of 111 patients biopsied for the eyelid skin tumors at Kangbuk Samsung hospital between January 2009 and July 2014. Results: The mean age of the patients was 56.8 years old. Benign tumors largely predominated over malignant ones, representing 81% of cases in this series, and the 3 mostfrequent subtypes were melanocytic nevus (18%), seborrheic keratosis (17%), and epidermal cyst (14%). Basal cell carcinoma was the most frequent malignant tumor (15%), followed by squamous cell carcinoma (3%). Among 111 cases, 8 cases (7%) are connected to eyelash. Conclusion: This study suggests that various tumors including malignancy can affect the eyelid areas. This study provides useful data on the incidence and characteristics of eyelid skintumors. Further investigation with a large group of patients is necessary to better understand the epidemiology of eyelid skin tumors.
P111 : Various local flaps for nasal reconstruction after resection of nonmelanoma skin cancer

Background: Aesthetic facial reconstruction requires understanding regional anatomy and tissue movement and theability to use innovatively the tissue adjacent to the defect to create a reconstruction that preserves the function of the area and the cosmetic facial units. Objectives: Our purpose is to describe various local flaps for nasal reconstruction of nonmelanoma skin cancer according to the aesthetic units, and to evaluate postoperative aesthetic andfunctional results. Methods: A retrospective cohort review of patients who underwent nasal reconstruction with local flaps from 2009 to 2014. Patients were categorized by nasal aesthetic subunits, complications recorded, and the postoperative esthetic results were evaluated by photographs. Results: Total 26 patients were treated with local flaps for nasal defects after resection of nonmelanoma skin cancer. The surgical defect confined to one cosmetic unit was present on the dorsum in 38.9%, lateral sidewall in 22.2%, tip in 16.7%, root in 16.7%, and ala in 5.6%. In each group, most common used local flap was rhombic flap, bilobed flap, banner flap, bilobed flap, and island pedicle flap. Most common reconstruction method for two or more units was island pedicle flap. The results of postoperative esthetic evaluation were good. Conclusion: Reconstruction of the nasal defect using various local flaps according to the aesthetic subunit was useful to provide consistently satisfying aesthetic results.

Background: High intensity focused ultrasonography (HIFU) is proved to be a safe, effective, noninvasive procedure to tighten the facial skin. Recently novel HIFU is introduced, but there has not been research about comparison between novel and conventional HIFU. Objectives: This study was performed to compare the efficacy and safety of conventional HIFU and novel HIFU in tightening the facial skin. Methods: 20 patients with moderate to severe facial wrinkles were randomized to receive HIFU, with one side of the face treated by conventional HIFU and the other side treated by novel HIFU. After 8 weeks of treatment, efficacy of the treatment was evaluated using three forms of assessment: two blinded observers’ evaluation, measuring the width of face and participant’s satisfaction score. All data was subjected to statistical analysis using independent or paired t-test. Results: All 20 patients showed clinical improvement on both treated side. There were no significant difference on both sides in terms of clinical efficacy and patients satisfaction but significantly less pain with novel HIFU treated side. No subject was noted to have any severe complication on both sides. Conclusion: This study suggest that the novel HIFU can be the safe treatment option for the facial lifting.
Monopolar grid fractional radiofrequency therapy for the treatment of periorbital wrinkles

Background: Radiofrequency (RF) is the widely used technique for skin rejuvenation and several reports have shown improvement of the periorbital wrinkles with fractional microneedle RF devices. For minimizing those possible side effects of pain or bleeding, novel grid fractional radiofrequency (GFRF) device has beenintroduced. Objectives: To examine the clinical efficacy and safety ofGFRF system for the treatment of periorbital wrinkles. Methods: The authors performed a single-center, prospective, self-controlled study of 15 Korean patients (Fitzpatrick skin type III or IV) with periorbital wrinkles. The patients received a series of 4 treatment sessions at 2 week intervals with the GFRF device at setting of a power of 200 Watt, spot size of the regular square tip (15】15mm), on-time of 0.3 second, and off-time of 0.2 second. Results: Clinical improvement was evaluated using the 5-point wrinkle assessment scale (WAS). The WAS scores showed statistically significant improvements comparing baseline and 4 weeks after the last treatment (p< 0.001). Patients undergoing treatment reported minimal pain and no erythema, crusting, erosion, edema, pigmentary change, and scarring was reported. Conclusion: The GFRF treatment was clinically effective, safe and well-tolerated therapy for the treatment of periorbital wrinkles.
