        Acute visual loss resulting from an uncomplicated cranioplasty is rare. A 52-year-old man presented with computed tomography (CT)-proven acute subdural hematoma in the right frontotemporoparietal area. The patients got a standard craniotomy in supine position. But on postoperative day 24, he was diagnosed postoperative epidural and subdural empyema. The patient underwent infected bone flap removal and evacuation of epidural and subdural empyema. Four months later, he got right frontotemporoparietal cranioplasty with artificial bone (Medpor) after controlling of infection. But on postoperative day 15, he complained of inability to see with his right eye. Brain and orbital CT, magnetic resonance imaging scan performed postoperatively did not show any new intracranial or intraorbital pathology. There were not any adverse events such as excessive pressure placed on the eyeball or bulky retraction of frontal skin flap. Visual evoked potential revealed prechiasmatic lesion. Fundoscopy showed slightly pallor optic disc of right side. The patient’s vision in the right eye remained no light perception permanently. The reason and pathophysiology of the mechanism of this complication should be discussed through reporting more cases.

        The Effects of Acupotomy Therapy on Carpal Tunnel Syndrome: A Report of 4 Cases

        The aim of this report was to show the effects of acupotomy in patients with carpal tunnel syndrome. Four patients were treated with acupotomy twice. Visual analogue scale (VAS), Tinel's sign, Phalen's test, Boston carpal tunnel syndrome questionnaire (BCTQ), muscular strength test, and a cross-sectional area of median nerve was measured using ultrasound before and after treatment. In all 4 cases, the VAS score, BCTQ score and cross-sectional area of median nerve, all decreased and muscular strength test score increased. Tinel's sign and the Phalen's test changed from a positive to a negative in most cases. This report shows that acupotomy is an effective treatment for carpal tunnel syndrome. Further larger are needed to fully evaluate the beneficial effects of this treatment.

      • IL-32-induced Inflammatory Cytokines Are Selectively Suppressed by α1-antitrypsin in Mouse Bone Marrow Cells

        <P>The induction of interleukin (IL)-32 in bone marrow (BM) inflammation is crucial in graft versus host disease (GvHD) that is a common side effect of allogeneic BM transplantation. Clinical trials on α-1 antitrypsin (AAT) in patients with GvHD are based on the preliminary human and mouse studies on AAT reducing the severity of GvHD. Proteinase 3 (PR3) is an IL-32-binding protein that was isolated from human urine. IL-32 primarily induces inflammatory cytokines in myeloid cells, probably due to PR3 expression on the membrane of the myeloid lineage cells. The inhibitory activity of AAT on serine proteinases may explain the anti-inflammatory effect of AAT on GvHD. However, the anti-inflammatory activity of AAT on BM cells remains unclear. Mouse BM cells were treated with IL-32γ and different inflammatory stimuli to investigate the anti-inflammatory activity of AAT. Recombinant AAT-Fc fusion protein inhibited IL-32γ-induced IL-6 expression in BM cells, but failed to suppress that induced by other stimuli. In addition, the binding of IL-32γ to PR3 was abrogated by AAT-Fc. The data suggest that the specific anti-inflammatory effect of AAT in mouse BM cells is due to the blocking of IL-32 binding to membrane PR3.</P>

        CNBP acts as a key transcriptional regulator of sustained expression of interleukin-6

        <P><B>Abstract</B></P><P>The transcription of inflammatory genes is an essential step in host defense activation. Here, we show that cellular nucleic acid-binding protein (CNBP) acts as a transcription regulator that is required for activating the innate immune response. We identified specific CNBP-binding motifs present in the promoter region of sustained inflammatory cytokines, thus, directly inducing the expression of target genes. In particular, lipopolysaccharide (LPS) induced <I>cnbp</I> expression through an NF-κB-dependent manner and a positive autoregulatory mechanism, which enables prolonged <I>il-6</I> gene expression. This event depends strictly on LPS-induced CNBP nuclear translocation through phosphorylation-mediated dimerization. Consequently, <I>cnbp</I>-depleted zebrafish are highly susceptible to <I>Shigella flexneri</I> infection <I>in vivo</I>. Collectively, these observations identify CNBP as a key transcriptional regulator required for activating and maintaining the immune response.</P>

        본 연구는 우리나라 최초의 유치원인 이화여자대학교 사범대학 부속 이화유치원의 교육과정을 구성하고 있는 이론적 근거를 역사적ㆍ철학적으로 고찰하는 것을 그 목적으로 하였다. 본 연구는 문헌연구를 통하여 이화유치원 설립 당시부터 현재까지의 교육과정의 성립 및 변천과정과 철학적ㆍ심리적 사조를 살표보고 이들의 영향이 이화유치원 교육과정에 나타난 부분을 탐구하였다. 연구결과 1914년 한 미국인 선교사에 의해 설립된 이화여자대학교 사범대학 부속 이화유치원은 설립 이후 지금까지 우리나라 유치원 교육의 모델을 제시하며 선도적 역할을 담당해오고 있음을 알 수 있었다. 교육과정의 측면에서는 유아들의 전인발달 특히 사회ㆍ정서적인 발달을 중요시하고 우리의 전통사상과 기독교정신, 진보주의 이론 및 구성주의가 그 철학적ㆍ심리적 기초를 이루고 있음을 알 수 있었다. The purpose of this study was to investigate historical, philosophical, and psychological foundations of the curriculum for Ewha Lab Kindergarten. This study was conducted through mainly literature review and interview. The results of this study are as follows. First, the Ewha Lab Kindergarten, which was founded in 1914, is the very first kindergarten for Korean children. It has played the leader's role of the field of early childhood education in Korea since then. Second, the curriculum of Ewha Lab Kindergarten was differentiated from other kindergartens' from its beginning in the sense that it was based on the firm belief of child-oriented philosophy. Third, essence of the curriculum for Ewha Lab Kindergarten has remained the same despite of its several changes. It has put emphasis on the development of the whole child, especially social-emotional development, and child-oriented curriculum which is based on the child's interests and experiences. Fourth, the curriculum for Ewha Lab Kindergarten was influened by several philosophies and psychological theories. Among those, Korean traditional thoughts, Christianity, Progressivism, and Constructivism are the most salient. Because Ewha Lab Kindergarten was founded by American missionary, Christianity was the basis for the curriculum. Examples can be that hymns, prayer, and bible stories are included in the curriculum and Easter, Thanksgiving, and Christmas are also celebrated. Perspectives on the purpose of education, development of young children, and the teacher's role were influenced by Korean traditional thoughts. The contents, teaching strategies, the teacher's role, and educational environments were heavily influenced by both Progressivism and Constructivism.

        The effect of long working hours on developing type 2 diabetes in adults with prediabetes: The Kangbuk Samsung Cohort Study

        Background: Long working hours are known to account for approximately one-third of the total expected work-related diseases, and much interest and research on long working hours have recently been conducted. Additionally, as the prevalence of prediabetes and the high-risk group for diabetes are increasing worldwide, interest in prediabetes is also rising. However, few studies have addressed the development of type 2 diabetes and long working hours in prediabetes. Therefore, the aim of this longitudinal study was to evaluate the relationship between long working hours and the development of diabetes in prediabetes. Methods: We included 14,258 prediabetes participants with hemoglobinA1c (HbA1c) level of 5.7 to 6.4 in the Kangbuk Samsung Cohort Study. According to a self-reported questionnaire, we evaluated weekly working hours, which were categorized into 35–40, 41–52, and > 52 hours. Development of diabetes was defined as an HbA1c level ≥ 6.5%. Hazard ratios (HRs) and 95% confidence intervals (CIs) for the development of diabetes were estimated using Cox proportional hazards analyses with weekly working 35–40 hours as the reference. Results: During a median follow-up of 3.0 years, 776 participants developed diabetes (incidence density, 1.66 per 100 person-years). Multivariable-adjusted HRs of development of diabetes for weekly working > 52 hours compared with working 35–40 hours were 2.00 (95% CI: 1.50–2.67). In subgroup analyses by age (< 40 years old, ≥ 40 years old), sex (men, women), and household income (< 6 million KRW, ≥ 6 million KRW), consistent and significant positive associations were observed in all groups. Conclusions: In our large-scale longitudinal study, long working hours increases the risk of developing diabetes in prediabetes patients.

        High Performance Liquid Chromatographic Method for Determination of Metazosulfuron Residue in Representative Crops

        BACKGROUND: This study was performed to develop a single residue analytical method for new herbicide metazosulfuron in crops. METHODS AND RESULTS: Brown rice, apple, mandarin, Kimchi cabbage and soybean were selected as representative crops, and clean-up system, partition solvent and extraction solvent were optimized. Instrumental limit of quantitation (ILOQ), linearity of calibration curve and method limit of quantitation (MLOQ) were determined based on the chromatography and whole procedures. For recovery tests, brown rice, apple, mandarin, Kimchi cabbage and soybean samples were macerated and fortified with metazosulfuron standard solution at three levels (MLOQ, 10 MLOQ and 100 MLOQ). And then those were extracted with acetonitrile, concentrated, and partitioned with ethyl acetate. Then the extracts were concentrated again and cleaned-up through $NH_2$ (aminopropyl) SPE cartridge with acetone : dichloromethane (1% acetic acid) (20 : 80, v/v) before concentration and analysis with HPLC. CONCLUSION(S): ILOQ of metazosulfuron was 2 ng (S/N${\geq}$10) and good linearity was achieved between 0.05 and 12.5 mg/Kg of metazosulfuron standard solutions, with coefficients of determination of 0.9999. MLOQ was 0.02 mg/Kg. Good recoveries from 74.1 to 116.9% with coefficients of variation (C.V.) of less than 10% were obtained, regardless of sample type, which satisfies the criteria of Korea Food and Drug Administration (KFDA). Those results were reconfirmed with LC-MS (SIM). The method established in this study is simple, economic and efficient to be applied to most of crops as an official and general method for residue analysis of metazosulfuron.
