소아간질로 오인된 Paroxysmal Kinesigenic Chreoathetosis 1례

Paroxysmal kinesigenic choreoathetosis is a rare, involuntary movement disorder. It is characterized by a sudden onset of unilateral or occasionally bilateral choreoathetosis or dystonic posturing of limbs and precipitated by sudden movement. Sporadic and more, commonly, familial cases have been reported. Onset occurs most often during childhood. Response to anticonvulsant such as phenytoin is usually excellent. We experienced a case of Paroxysmal kinesigenic choreoathetosis in a 7 years-old male patient who had sudden dystonic posture of upper limbs and choreoathetoid movement whenever he started to move suddenly. These episodes occurred 6-7 times in a day , each lasting 10 seconds , during which time the patient was alert, perfectly. The neurologic examination was normal and brain magnetic resonance imaging study and electroencephalograph were normal. The laboratory findings including serum electrolyte, fasting blood sugar were normal range. He was taken phenytoin 100㎎ per day and the frequency of attacks diminished. The phenytoin dosage was increased to 150㎎ per day and attacks completely controlled with this dosage.

Phenobarbital is a long-acting barbiturate causing generalized depression of neuronal activity in the brain. Its effect is primarily achieved through enhanced GABA-mediated synaptic inhibition. Its use as an antiepileptic agent was first described in 1912. Before the introduction of phenytoin, phenobarbital is used as sedative-hypnotics. It is used for the treatment of epilepsy and status epilepticus. All barbiturates, including phenobarbital, have a high potential for abuse. They were frequently used for suicide attempts in the past, but they have in large part been replaced by benzodiazepines. The onset of symptoms depends on the drug and the route of administration. Mild to moderate barbiturate intoxication resembles ethanol inebriation with slurred speech, ataxia, and lethargy. Severe acute barbiturate intoxication is life threatening. Early deaths are generally cardiovascular-related. Hypotension, shock, pulmonary edema, and cardiac arrest that occurs with large doses are caused by depression of central sympathetic tone and as well as by direct depression of cardiac contractility. The potentially fatal oral dose of phenobarbital is 6-10g. We describe an 23-year-old woman with pulmonary edema, and cardiac arrest ingestion of 18grams of phenobarbital. She was completely recovered by successful cardiopulmonary resuscitation and homoperfusion. We report a case with literature review.
경피신경전기자극 후 상부 승모근 활성도와 EMG gap의 변화

The purpose of this study was to compare visual analogue scale (VAS), pain threshold (PT), %RMSRVC, and EMG gaps before and after applying transcutaneous electrical nerve stimulation (TENS) on the upper trapezius muscle at the patients with myofascial pain syndrome (MPS). The subjects were 4 men and 10 women composed of both the inpatients and outpatients who were diagnosed as MPS at Wonju Medical Center. VAS and PT measurements were performed to assess the subjective pain level. The reference voluntary contraction (RVC) test was performed for 15 seconds for normalization on the bilateral trapezius muscle using surface electromyography (sEMG). After 3-minute resting time, the EMG signal was recorded while performing a typing activity for 2 minutes and then TENS was applicated with a comfortable intensity for 10 minutes. The EMG activity of the upper trapezius muscle was recorded during typing for 2 minutes. The results of study were as follows: 1) VAS score was significantly decreased on the more painful side after treatment, however, it was not significantly different on the less painful side. 2) PT was increased after treatment on both sides, however, it was not significantly different between before and after the TENS application. 3) The EMG activity during typing was significantly decreased after treatment, and 4) The EMG gaps were significantly increased after TENS treatment compared to before it. Consequently, the study showed that TENS was effective in decreasing VAS, %RMSRVC, and in increasing EMG gaps. The EMG gap analysis could be a useful method to measure pain in patients with MPS in the upper trapezius.

Insect cell expression system using baculovirus has several benefits from high capacity, flexibility, and safety to humans and glycosylation capability. Thus, the baculovirus insect cell system has been widely used for production of recombinant protein. The HC and LC genes of the mAb CO17-1A were cloned under the control of two different promoters, P10 and PPH, respectively, on baculovirus expression pFastBacTM Dual vector. The gene expression cassettes carrying HC and LC genes were transferred into a parent bacmid in DH10Bac. Immunoflouorescence analyses and Western blot confirmed expression and secretion of mAb CO17-1A in the virus infected insect cells. The optimum condition for mAb expression was optimized at 24, 48 and 72hr after the virus infection with MOI ranging (0.2, 1 and 5). HPLC analysis revealed that the insect-derived mAb CO17-1A had insect specific glycan structures. Cell ELISA showed that the purified mAb from insect cell cultured media had a specific binding activity to SW948 cell. These results indicated that the baculovirus insect cell system is able to express, assemble, and secrete functional full size monoclonal antibody with insect specific glycosylation.
Expression of Anti-Colorectal Cancer Monoclonal Antibody CO17-1A in Insect Cells

Insect cell expression system using baculovirus has several benefits from high capacity, flexibility, safety to humans and glycosylation capability. Thus, the baculovirus insect cell system has been widely used for production of recombinant protein. In this study, the insect cell expression system was designed to produce anti-cancer mAb CO17-1A, which recognizes the antigen GA733 highly expressed on the surface membrane of colorectal carcinoma cells. The heavy (HC) and light chain (LC) genes of the mAb CO17-1A were cloned under the control of two different promoters, P10 and PPH, respectively, on baculovirus expression pFastBacTM Dual vector. The gene expression cassettes carrying HC and LC genes were transferred into a parent bacmid in E.coli (DH10Bac). The bacmid was transfected to Sf9 insect cells to generate the baculovirus expressing mAb CO17-1A. Western blot and immunoflouorescence analyses confirmed expression and secretion of mAb CO17-1A in the virus infected insect cells. The optimum condition for mAb expression was optimized at 24, 48 and 72hr after the virus infection with MOI (optimum virus concentration) ranging (0.2, 1 and 5). Expression of mAb CO17-1A in insect cells significantly increased at both 48 and 72hr after transfection with the MOI 1. HPLC chromatography revealed that the insect-derived mAb CO17-1A had insect specific glycan structures. Cell ELISA showed that the purified mAb from insect cell cultured media had a specific binding activity to SW948 human colorectal cancer cell. These results indicated that the baculovirus insect cell system is able to express, assemble, and secrete functional full size monoclonal antibody with insect specific glycosylation.

oagulation, usually with heparin, is required to prevent thrombosis in the blood circuit of hemodialysis. In patients at high bleeding risk, strategies to minimize the bleeding risk include heparin-free or regional anticoagulation methods. Nafamostat mesilate with conventional dose (35 mg/ hr) has been used for this purpose. But it is an expensive anticoagulant to use conveniently for the dialysis therapy. Application of low-dose nafamostat mesilate has almost never been tried yet on hemodiaysis management. In this study, we examined the effect of low-dose nafamostat mesilate compared to heparin-free in hemodialysis patients with high risk of bleeding. Methods: The current study was conducted on 35 hemodialysis patients with high bleeding risk (ongoing bleeding, hemorrhage, surgery or severe thrombocytopenia). In the low-dose nafamostat group (n=17, mean age: 59±15 years), 238 sessions were performed with continuous infusion of nafamostat mesilate (12.5 mg/hr). In the control group with saline-flushing no heparin methods (n=18, mean age: 57±17 years), 247 sessions were analyzed. Results: No significant differences were found in baseline characteristics between the low-dose nafamostat group and the saline group. In the progress of bleeding complications, there were no significant differences between the two groups (11.8% vs. 11.1%). In saline group, however, massive clotting occurred in 44.5 per 1000 sessions, while it occurred in 4.2 per 1000 sessions in the low-dose nafamostat group (p=0.006). Conclusion: In patients at high bleeding risk, low-dose nafamostat mesilat can be used as an inexpensive, effective, and safe anticoagulant for hemodialysis.
다양한 영상 데이터 수집을 위한 3차원 공간데이터 기반 자율주행 시뮬레이션 시스템 구현

최근 실세계에서 수집되는 영상 데이터를 활용하여 딥러닝 기반 자율주행 성능을 개선하기 위한 연구가 활발히 수행되고 있다. 그러나 딥러닝 모델을 생성하기 위해 요구되는 많은 양의 영상 데이터를 실제 주행환경에서 수집하는 것은 시공간적 및 비용적인 한계가 존재한다. 이에 본 연구에서는 3차원 공간정보를 활용하여 실세계를 모사한 가상환경을 구축하고, 다양한 시나리오 환경에서 영상 데이터를 수집할 수 있는 자율주행 시뮬레이션 시스템을 제안하고자 한다. 제안된 시스템은 사용자가 원하는 가상의 주행환경 설정과 차량 수집 센서 설정 기능을 제공하여 다양한 영상 데이터 수집이 가능할 뿐만 아니라, 가상환경에서 시공간적 한계 없이 실세계에서는 수집이 어려운 영상 데이터의 수집도 가능할 것으로 기대된다. Recently, there has been so much research to improve deep learning-based autonomous driving performance using imagery data collected in the real world. However, it has spatiotemporal and cost limitations to collect a large amount of imagery data required to create a deep learning model in a real driving environment. Therefore, in this paper, we propose an autonomous driving simulation system that can virtualize the real world using 3D spatial data and can collect imagery data in various scenario environments. It is expected that it is possible for the proposed system not only to collect various kinds of imagery data using the virtual environment setting and the vehicle’s sensor setting, but also can collect imagery data that is difficult to collect in the real world without spatiotemporal limitations.
