        The purpose of this study was to provide the basic data for the planning of education of patients by exploring and comparing patients', nurses' and doctors' perception of the educational needs and degree of providing education. 66 hospitalized patients undergoing chemotherapy and 44 nurses, 42 doctors assigned for the direct care were sampled in 1 university hospital in Taejon from August 30, 1993 to September 30, 1993. For the comparing patients', nurses' and doctors' perecption of the educational needs and degree of providing education, the researcher used Kims' Educational need scale which is composed of 20 item Likert type 5 point scale. Data were analyzed by means of percentile score, SPSS, t-test, ANOVA, Pearson correlation. The result were as follows 1. Comparision of total Mean score patients', nurses' and doctors' perception of the educational need ordering nurses(M=4.0465), patients (M=3.9803), doctors (M=3.8309). The difference between the three group's total Mean score is showed no statistical significant difference. 2. Comparision of total Mean score patients' educational need, patients' percetion of nurses' and doctors' degree of providing education, patients' educational need is scored 3.9803, patients' perception of nurse is scored 2.3431, patients' perception of doctor is scored 2.5022. The difference between the patients' educational need and medical teams' degree of providing education is showed statistical significant difference(P<.001). 3. Comparision of total mean score nurses' educational need is scored 4.0465, nurses' perception of nurses' degree of providing education is scored 2.9920, patients' perception of nurses' degree of providing education is scored 2.3431. The difference of between the nurses' educational need and nurses' perception of nurses' degree of providing education is showed statistical significant difference(P<.001) 4. Comparision of total mean score doctors' educational need is scored 3.8309, doctors' perception of doctors' degree of providing eduation is scored 3.1726, patients' perception of doctors' degree of providing education is scored 2.5022. The difference of between the doctors' educational need and doctors' perception of doctors' degree of providing education is showed statistical difference(P<.001).

      • 젊은 남자에게 발생한 통상성 간질성 폐렴과 동반된 폐암 1예

        Usual interstitial pneumonia(UIP) is relatively rare disease of lung parenchyme under the age of 50. Lung cancer is reported to occur as a complication of UIP, but has not been reported in young age patient with UIP in Korea. A 35-year-old man was admitted to the hospital because of dyspnea, cough and mild fever. He had been suffered from dyspnea during 3 years. The dyspnea worsened recent 1 month ago. He had been treated with pneumonia at another hospital during 4 months before this admission. Chest radiographs and High resolution CT showed subpleural and basal dominant reticular opacities in both lung with no change until now. He was smoker and his occupation was car engine-man dealing with benzene. UIP is suspected by history and radiologic findings. Operation for open lung biopsy was performed to obtain lung specimen via video-assisted thoracoscopic surgery. Patient's pathologic examination showed UIP combined with adenocarcinoma in RLL superior segment and posterobasal segment and RLL lobectomy was done. After operation, the patient has been in cancer free condition so far. We report a case of UIP combined with lung cancer in young man with brief review of the literature.

      • 암환자가 지각한 사회적지지, 희망과 삶의 질과의 관계

        The Relationship among Percieved Social Support, Hope and Quality of Life of Cancer Patients Tae, Young Sook. Kong, Eun Sil. Lee, Myung Hwa. Park, Geum Ja The purpose of this study was to investigate the relationship among percieved social support, hope and Quality of life of the cancer patients and to gain the baseline data for development of nursing intervention program for promoting quality of life in cancer patients. The design of this study was a cross sectional correlational survey. The subjects were 220 out and in-cancer patients in 5 general hospitals in Pusan The data were collected from July 2 to August 1,2001. The instruments were the Percieved social support scale(16 items, 5 point scale) had developed by Tae(1986), Hope scale(12 items, 4point scale) developed by Nowotny(1989) and Quality of life scale(31 items, 10 point scale) developed by Tae et. al(2000). The data was analyzed by the SPSS/PC+ program using frequency & percentage, item mean & standard deviation, t-test, ANOVA & Scheffe test, Pearson's correlation coefficient. The results of this study was as follows : 1) The item mean score of quality of life was 6.05±1.16 (range 0-10) .The heighest score of subarea of the quality of life was the spiritual wellbeing area (7.09±1.63) and the lowest score was social wellbeing area(5.53±1.65). The mean score of perceived social support was 52.65±10.32 (최고 1, 최저 80). The mean score of family support was32.71±6.66 (range 1- 40) and the mean score of medical team support was 19.93±5.95 (range 1- 40). The mean score of Hope was 37.02±5.64 (range 1-48). 2) There were statistically significant difference in the score of quality of life according to the life effect of religion (F=3.97, p=0.00), treatment method(F=2.94, p=0.01), area of diagnosis (F=3.48, p=0.01), stage of disease (F=13.74, p= 0.00). 3) There was significant correlation between perceived social support(r=0.44, p= 0.000 ; family support: r=0.334, p=0.000, medical support; r=0.395, p=0.000), hope(r=0.563, p= 0.000) and quality of life. In conclusion, there was a significant relationship among perceived social support, hope and quality of life. Therefore perceived social support, hope intervention programs should be developed to improve the quality of life in cancer patients.

        간호실무에서 신봉이론(Espoused theories)에 대한 탐색적 연구

        As a nursing practice involves nurses' actions in a specific context of health care, this study has focused on exploring the espoused theories in nursing practice within the action science perspectives, Espoused theories are the belief, principles, and rationale expressed by the practitioner as guiding her/his actions in a situation of practice. The data were analysed qualitatively and 25 elements of espoused theories of nursing action were identified and clustered into 6 categories. The 25 elements of espoused theories are as follows: The clinical nurse worked in wholistic and individual nursing, focussed on the patient's needed, comfort and supportive nursing (5 theories of nursing goal) ; excellent skills, knowledge based, assessment and data collection, explaining, educating or a scientific basis(6 theories of nursing intervention): advocacy, value oriented, treatment, account- ability and commitment(4 theories of nursing ethics) ; human respect. partnership, trust(3 theories of patient-nurse relationship) : knowledgable, accumulated clinical experiences and personally lived experiences. positive perspectives(4 theories of nurse), role of intervention. rewarding peer relationship(3 theories of situations) The above mentioned espoused theories are similar to that of nursing textbooks which students learned through basic nursing education and almost the same as the Acts ofa Nurse in Korean. However, we are doubtful whether nurses actually do as they think. Therefore. it is recommended to review the theories-in-use in order to and any discrepancies between the espoused theories and the reality of nursing actions

        Today human needs for health promotion and maintenance are increasing. The nursing progression has a great responsibility for helping clients to practice good health behavior. self efficacy was found to be potent predictor in initiating and maintaining the health-related behavior. Therefore, the concept is important in nursig intervention for change of health behavior. The purpose of this paper was to analyze and clarify the meaning of the concept, self efficacy. This study used Walder & Avant's process of concept analysis. Critical attributes of self efficacy were : 1) positive perception of his won capability; 2) intrapersonal strength; 3) possibility of being learned; 4) individuality; 5) specificity; 6) generalizability. Antecedents of self efficacy consist of those facts that 1) new situation occurs; 2) individuals have needs to change the new situation; 3) there are some required actions in new situation. Consequences occurring as a result of self efficacy consist of those facts that 1) individual initiates the of failure; 5) cope with the situation; 6) controls the situation; 7) the level of self efficacy is enhanced; 8) the level of self-esteem is enhanced.

        Familial adenomatous polyposis (FAP) is an autosomal dominant syndrome, typically characterized by multiple colorectal adenomas and increased incidence of colorectal carcinomas if it is not treated. Moreover, a variety of extracolonic manifestation are seen. The prevalence of thyroid tumors developing in patients with FAP is about 1∼2%. Generally, papillary thyroid cancer indicates low mortality but ,however, as people get older, the cancer becomes more aggressive. So, the operation is required. Recently we experienced a case of FAP, presenting with papillary thyroid carcinoma, and reported with a brief review of literatures.

        청소년 소비자의 구매의사결정과 수입상품 선호도

        The purposes of this study were to investigate the decision making process & preference for imported products and to suggest the systematic education programs for adolescent consumers. The data for this research were attained from 426 middle & high school students in Pohang. The data were analyzed by frequency distribution, mean. standard deviation. analysis of variance, t-test. χ2-test and pearson's correlation with SAS Program. The results of this study were as follows : 1.Preference for imported products was significantly different according to sex, amount of pocket money, region, T.V. advertising. friends and the attitude of their mothers. 2.Decision making Process was composed of 4 dimensions in this study: information sources. criteria for Products. problem experience of consumption and type of problem solving. In case of information sources, there was a significant difference according to sex, gratae, level of parents' education. Criteria for products were significantly different according to sex. tirade. level of father's education, type of father's job. amount of pocket money and how they gained their pocket money Consumption problem experience & type of problem solving were significantly different according to sex and grade. 3.Preference for imported products was significantly different according to criteria for products and consumption problem experience out of adecision making process.

      • 農村託兒所의 效果分析

        This research is carried out to ensure the significances of rural day care centers by analysis of the effect of the centers and to provide the suggestible data for more effective establishment of the day care services in rural area. Intentional sampling was made from the rural demonstration day care center which are running only for several busy months with some snacks, the full-time day care centers which are running all the year around with lunch, and the rural villages with-out any day care centers as controlled group. 337 preschool children in three groups and their mothers were interviewed and chi-square(χ²) analysis was done under the following 5 hypotheses. 1. The current rural day-care centers affect food habits, dining attitudes and the type of diet of the day-care children. 2. The day-care have an influence on physical and intellectual development and social maturity of the children. 3. the day-care centers present safety isle for the children and reduce the time of child care for their mothers. 4. The day-care centers have an educational influence on the mothers of the children. 5. The day-care affects the attitudes of the mothers toward the day care centers. The major results are as follows: 1. The day-care center children had higher frequency of eating with their family compared with the children of the controlled group, which indicates the favorable influence of the center on children's eating environment. The children of the demonstration day-care centers were fed more regularly at home with all food groups except potatoes and cereals groups than the children of the full-time day care center and the controlled group. 2. The day-care centers, regardless of the kind, had favorable influences on the preference for all food served and on the dining attitudes. 3. The test of significance for the difference amog three meal patterns shows that the balanced diet was served most frequently at the full-time day-care centers, followed by the demonstration day-care centers and the controlled group. The difference among the three groups in the nutritional balance of the diet was particularly significant for lunch. 4. weight/height ratio of children from the full-time and demonstration day-care center was slightly higher than that of children from the controlled group, although the difference between the two was statistically insignificant. 5. It appears that experience in day-care centers have a significant influence on the intellectual development (p<0.01) and social maturity(p<0.05) of the children. Day-care children marked higher IQ on average than controlled group and average IQ was highest in full-time day-care centers among three groups. Social maturity of demonstration day-care children was hither than any other groups. 6. The yearly frequencies of hurts and accidents of the children was highest in the controlled group and lowest in the demonstration day-care and the difference among three groups was stat-istically significant(p<0.01). 7. Day-care reduce the time of child care for mothers by 2-3 hours per day and the difference of time spending on the childcare appears significant among three groups(p<0.01). 8. Knowledge of children's mother about nutrition and child care was highest in the full-time day-care centers followed by the demonstration day-care centers and was lowest in the controlled group. The difference among three groups were statistically significant(p<0.01). 9. Mothers of children at the demonstration day-care centers showed higher concern over the expenses for the center lunch, and stronger desire for participation in the lunch preperation at the centers. 10. Mothers of day-care children more strongly recognize the needs for the day-care services in the rural area and the educational effects, particularly on intellectual development and the formation of good habits of the children.

      • 장기간의 복합트레이닝이 중년 남녀의 스포츠 의학적 변인에 미치는 영향

        The purpose of this study was to investigate the changes of sports medicine variables after 20weeks combined training. Subjects were consisted of 130 middle-aged males and females. The training program was separated treadmill running and muscular resistance training. The treadmill running was composed of 60-70% HRmax(Intensity), 30 minutes per day(Duration), 5 days per week(Frequency). The muscular resistance training was composed of 70% of 1RM(Intensity), 40 minutes per day(Duration), 5 days per week(Frequency). multiple ANOVA was used to evaluate change times. We found that weight, BMI %body fat, SBP and DBP were significantly decreased and LBM was significantly increased after 20 weeks combined training. In conclusion, combined training was shown to positive influence on body composition and blood pressure. Therefore, it is maybe though that 20 weeks combined training will benefit the prevention of life style related diseases.
