Short-rib polydactyly syndromes(SRPS) comprise a group of rare, generally lethal skeletal dysplasias. This group is manifested by short-limb short stature, short ribs with thorasic hypoplasia, and polydactyly. This heterogenous group of recessively inherited disorder has distinct imaging findings and ancillary findings on both pre-and postnatal assessments may enable individual cases to be classified into one of four subtypes : SRPS Ⅰ(Saldino-Noonan); SRPS Ⅱ (Majewski); SRPS Ⅲ (Verma-Naumoff); and SRPS Ⅳ (Beemer-Langer)1,2,3). All forms of the SRPS described to date are thought to be inherited in an autosomal recessive manner. There are difficulties in the clssification of these disorders because of the frequent overlap. We experienced one case of recurrent short-rib polydactyly syndrome and presented with review of literature.

This research was carried out to find out the possibility of using Faraday's law in analyzing the electrocoagulation system. Bench-scale reactor equipped with aluminum electrode plates was operated using synthetic wastewater which received sodium chloride for conductivity adjustment. Phosphate was added to the wastewater to induce the precipitation with Al. The amount of aluminum dissolved from the electrodes could be predicted by Faraday's law with a difference less than 5%. This difference was greater at a higher electric current, probably due to the increased solution temperature. However, effect of pH on the dissolution of the aluminum was negligible. The result of this study suggested that the operating condition of electrocoagulation system could be developed using the Faraday's law when the pollutant concentration is given.

Background : About 30% of patients in the hospital are under nourishment. A large portion of people are undernourished when they are admitted to the hospital and in the majority of these, undernutrition develops further while in hospital. Patient-Generated subjective global assessment(PG-SGA), a method of nutritional assessment based on clinical judgment, has been widely used to assess the nutritional status of adults for both clinical and research purposes. Modified PG-SGA can be used as a nutrition screening tool, and a comprehensive nutrition assessment is desirable for those who are malnourished. The purpose of this study is to develop a simple, reliable and valid nutrition screening program(NSP). Methods : We applied the nutrition screening program(NSP) to adult inpatients who are at risk of malnutrition using the Modified Patient-Generated subjective global assessment(PG-SGA) and objective assessment(electrical medical record data). Results : Therefore simple and beneficial NSP is developed for patients with severe malnutrition. Using NSP. nutritional information of the severe malnutrition patient should be shared with the physicians and they should be taken care of by clinical dietitians to improve their nutritional status. Conclusion : The Modified PG-SGA and nutrition screening tool are simple, reliable and valid nutrition screening program(NSP).

Radioactive iodine and surgery are options in the treatment of patients presenting with antithyroid drug-resistant hyperthyroidism. It is necessary to normalize thyroid function prior to any procedure in order to reduce complications. Cholestyramine is effective as a short-term adjunctive agent in the treatment of thyrotoxic patients who are resistant to conventional therapy. A 22-year-old woman with severe Graves` disease was referred from a local clinic because of her refractory hyperthyroidism. She presented with exophthalmos, diffuse goiter, and tachycardia. She was treated with a maximal dose of methimazole and a beta-blocker for 2 months. However, her thyroid function test(TFT) did not improve. TFT showed a free T4 level of 74.7 ng/dL (normal range: 0.93.1.7 ng/dL) and a thyroid stimulating hormone (TSH) level of 0.007 μIU/mL (normal range: 0.4.4.0 μIU/mL). She was then administered cholestyramine (4 g thrice daily), hydrocortisone (300 mg/day) and methimazole (100 mg/day) which prepared the patient for surgery by reducing the free T4 level (4.7 ng/dL). The patient underwent a total thyroidectomy without experiencing thyrotoxic crisis. This case describes the use of cholestyramine for the first time in Korea in treating Graves` disease and provides limited evidence that cholestyramine can be an effective option.

<P>LTA from <I>E. faecalis</I> inhibits LPS-induced IL-8 in human periodontal cells</P><P>Periodontitis is caused by multi-bacterial infection and <I>Aggregatibacter actinomycetemcomitans</I> and <I>Enterococcus faecalis</I> are closely associated with inflammatory periodontal diseases. Although lipopolysaccharide (LPS) of <I>A. actinomycetemcomitans</I> (Aa.LPS) and lipoteichoic acid of <I>E. faecalis</I> (Ef.LTA) are considered to be major virulence factors evoking inflammatory responses, their combinatorial effect on the induction of chemokines has not been investigated. In this study, we investigated the interaction between Aa.LPS and Ef.LTA on IL-8 expression in human periodontal ligament (PDL) cells. Aa.LPS, but not Ef.LTA, substantially induced IL-8 expression at the protein and mRNA levels. Interestingly, Ef.LTA suppressed Aa.LPS-induced IL-8 expression without affecting the binding of Aa.LPS to Toll-like receptor (TLR) 4. Ef.LTA reduced Aa.LPS-induced phosphorylation of mitogen-activated protein kinases, including ERK, JNK and p38 kinase. Furthermore, Ef.LTA inhibited the Aa.LPS-induced transcriptional activities of the activating protein 1, CCAAT/enhancer-binding protein and nuclear factor-kappa B transcription factors, all of which are known to regulate IL-8 gene expression. Ef.LTA augmented the expression of IL-1 receptor–associated kinase-M (IRAK-M), a negative regulator of TLR intracellular signaling pathways, in the presence of Aa.LPS at both the mRNA and protein levels. Small interfering RNA silencing IRAK-M reversed the attenuation of Aa.LPS-induced IL-8 expression by Ef.LTA. Collectively, these results suggest that Ef.LTA down-regulates Aa.LPS-induced IL-8 expression in human PDL cells through up-regulation of the negative regulator IRAK-M.</P>
