        RNA-Seq for Gene Expression Profiling of Human Necrotizing Enterocolitis: a Pilot Study

        <P>Necrotizing enterocolitis (NEC) characterized by inflammatory intestinal necrosis is a major cause of mortality and morbidity in newborns. Deep RNA sequencing (RNA-Seq) has recently emerged as a powerful technology enabling better quantification of gene expression than microarrays with a lower background signal. A total of 10 transcriptomes from 5 pairs of NEC lesions and adjacent normal tissues obtained from preterm infants with NEC were analyzed. As a result, a total of 65 genes (57 down-regulated and 8 up-regulated) revealed significantly different expression levels in the NEC lesion compared to the adjacent normal region, based on a significance at fold change ≥ 1.5 and <I>P</I> ≤ 0.05. The most significant gene, <I>DPF3</I> (<I>P</I> < 0.001), has recently been reported to have differential expressions in colon segments. Our gene ontology analysis between NEC lesion and adjacent normal tissues showed that down-regulated genes were included in nervous system development with the most significance (<I>P</I> = 9.3 × 10<SUP>−7</SUP>; <I>P<SUB>corr</SUB></I> = 0.0003). In further pathway analysis using Pathway Express based on the Kyoto Encyclopedia of Genes and Genomes (KEGG) database, genes involved in thyroid cancer and axon guidance were predicted to be associated with different expression (<I>P<SUB>corr</SUB></I> = 0.008 and 0.020, respectively). Although further replications using a larger sample size and functional evaluations are needed, our results suggest that altered gene expression and the genes' involved functional pathways and categories may provide insight into NEC development and aid in future research.</P>

        A Multi-Center Educational Research Regarding Breastfeeding for Pediatrics Residents in Korea

        Purpose: Pediatricians have a significant responsibility to educate mothers about the importance of breastfeeding. However, there have been minimal efforts in the courses of resident training in Korea. The purpose of this study is to evaluate the change in knowledge and attitude before and after a 4-week breastfeeding educational intervention among multicenter residents. Methods: Prospective interventional educational research was designed for residents at eight training hospitals in Korea. Institutional reviews were obtained in each hospital. The education curriculum consisted of 14 courses regarding breastfeeding theory and practice. These materials were used to teach pediatric residents for 4 weeks. Knowledge-based tests were administered before the course, and re-tests were administered after the course using different test items of similar levels. Test scores and survey responses were compared before and after the intervention. Results: A total of 73 residents (1st year 20, 2nd year 23, 3rd year 16, and 4th year residents 14) from eight training hospitals completed the intervention. Their average age was 30.3±2.9 years, 17 (23.3%) were male, 22 (30.1%) were married, and eight had more than one child of their own. The mean pre-test score was 61.8±13.4 and the mean post-test score was 78.3±7.5 (P<0.001). The inter-grade difference in the score was significant in the pre-test (P=0.005), but not significant in the post-test (P=0.155). There were more responses of obtaining confidence after the intervention (P<0.001). Conclusion: In our study, pediatric residents showed improvement in their knowledge and confidence level after 4 weeks of the breastfeeding curriculum.         대학의 기술지식 생산 속성 및 동적 변화 분석

        대학교의 역할과 가치는 시대적 요구에 따라 변화하고 있다. 과거 교육, 연구, 봉사 중심의 대학교에서 교육과일치를 중심으로 하는 연구중심대학으로 그리고 연구와 기업가적 가치 생산을 요구받는 현재의 기업가적 연구중심 대학의 모습으로 변화하였다. 또한, 대학교가 생산하는 지식도 변화하여 왔다. 과거에는 대학교에서 과학지식(학술논문) 생산에 큰 힘을 쏟았다면, 최근에는 기술지식(특허) 생산에도 많은 관심을 보여주고 있다. 본고에서는 기업가적 연구중심대학들이 생산하는 기술지식의 속성을 정의하고 그 속성에 따라 국내 대학교들이 생산하는 기술지식의 방향성을 연구하였다. 국내외에서 진행된 기업가적 대학과 지식흐름의 관한 연구를 선행연구로 보여주었고, 국내 대학에서 등록한 대학특허를 기반하고 재조정하여 도전적 기술지식과 혁신적 기술지식을 수치로 나타내었다. 도전적 기술지식은 특허에서 과학문헌을 인용하여, 과학지식에서 기술지식으로 도전적으로 전환한 기술지식을 가리키며, 혁신적 기술지식은 해외의 특허를 인용하며, 해외의 기술지식을 먼저 국내로 도입한 기술지식을 가리킨다. 연구 결과, 과학기술중점대학교의 도전적, 혁신적 기술지식 생산은 국내 종합대학보다 많았고, 시간 변화에 따라 국내 대학들은 모두 기업가적 연구중심대학의 기술생산에 많은 결과를 만들고 있음을 확인하였다. 다만, 대학에서 암묵적으로 생산 및 전달되는 가치를 평가에 반영하지 못하였고, 해외 대학의 결과값도 포함하지 못한 점은 연구 한계점으로 남지만, 대학의 기술지식 속성을 고려한 새로운 장을 열었다는 점에 의의가 있다. The roles and values of universities are changing according to the needs of the times. The roles of universities in the past were to provide education, research, and service. Later, research-oriented universities emerged with the convergence of education and research. Currently, this role has been transformed into that of an entrepreneurial research-oriented university. The knowledge that universities produce has also changed. In the past, universities focused on producing scientific knowledge (academic papers), but recently they are showing a lot of interest in producing technical knowledge (patents). In this paper, the attributes of the technical knowledge produced by entrepreneurial research-oriented universities were defined, and the direction of the knowledge produced by domestic universities was studied according to its attributes. Research on entrepreneurial universities and knowledge flow conducted at home and abroad was used as a preceding study, and both challenging and innovative technical knowledge were expressed numerically based on the university patents registered by domestic universities. Challenging technology knowledge refers to technical knowledge that has transitioned from scientific knowledge to technical knowledge by citing scientific literature in patents. Innovative technology knowledge refers to technical knowledge that cites foreign patents and introduces foreign technical knowledge for the first time in Korea. As a result of this study, it was confirmed that the production of challenging and innovative technical knowledge by the Korea Institute of Science and Technology (KIST) was higher than that of domestic universities and that all domestic universities were delivering results in their roles as entrepreneurial research-oriented universities producing technology over time. Unfortunately, the technical knowledge of universities was analyzed only based on patent information, so the value implicitly produced and delivered by them could not be reflected in the evaluation. Also, overseas universities were not included. Nevertheless, this study is meaningful in that it opened a new chapter in the research of technical knowledge produced by universities.

        A Novel COL4A1 Mutation in a Neonate with Intrauterine Intraventricular Hemorrhage and Porencephaly

        Collagen type IV alpha 1 (COL4A1) plays an important role in construction of the basement membranes of all human tissues, especially vessels. Mutations in COL4A1 lead to various multisystemic dysfunctions, including hereditary porencephaly, hemorrhagic stroke, hemiplegia, cerebral small vessel disease, and nephropathy. In this study, we describe a neonatal case featuring a novel de novo COL4A1 mutation, manifesting as fetal intraventricular hemorrhage and porencephaly. This patient is one of the youngest to have been diagnosed with the most severe phenotype. Our experience may assist clinicians in the diagnosis and management of this extremely rare genetic condition.

        Case of mucinous adenocarcinoma of the lung associated with congenital pulmonary airway malformation in a neonate

        Congenital pulmonary airway malformation (CPAM), previously known as congenital cystic adenomatoid malformation, is a rare developmental lung abnormality associated with rhabdomyosarcoma, pleuropulmonary blastoma, and mucinous adenocarcinoma of the lung. We report an unusual case of a 10-day-old male newborn with a left lower lobe pulmonary cyst who underwent lobectomy, which revealed type II CPAM complicated by multifocal mucinous adenocarcinoma. KRAS sequencing revealed a somatic mutation in Codon12 ($GGT{\rightarrow}GAT$), suggesting the development of a mucinous adenocarcinoma in the background of mucinous metaplasia. Mucinous adenocarcinoma is the most common lung tumor associated with CPAM, but it generally occurs in older children and adults. Further, all cases in the literature are of type I CPAM. This case in a neonate indicates that malignant transformation can occur very early in type II CPAM.

        The Impact of Introduction of Fetoscopic Laser Surgery on Twin-to-Twin Transfusion Syndrome

        Purpose: Fetoscopic laser surgery (FLS) is considered an optimal therapeutic strategy for twin-to- twin transfusion syndrome (TTTS). We aimed to determine the clinical outcomes of TTTS patients since the introduction of FLS. Methods: A retrospective study of TTTS patients born between January 2005 and December 2015 was conducted. Mortality and morbidity were compared in the FLS and non-FLS groups. The FLS group was divided into selective FLS and Solomon technique group, and subgroup analysis was performed. Results: Of 70 pregnancies diagnosed with TTTS during the study period, FLS was performed for 35 (50%). Survival of at least one infant at discharge was achieved in 100% (35/35) of pregnancies in the FLS group and 91.4% (32/35) in the non-FLS group (P=0.028). Subgroup analysis revealed that the Solomon technique group had a higher dual survival rate than the selective FLS group (81.3% vs. 57.9%, P=0.036). Neonatal mortality was significantly lower in the FLS group than in the non-FLS group (1.7% vs. 16.4%, P=0.005). Severe intraventricular hemorrhage more than grade 3 was less frequent in the FLS group than in the non-FLS group (0% vs. 18.2%, P= 0.001). The FLS group showed smaller inter-twin differences in birth weight (24.4% vs. 33.7%, P= 0.032) and lower incidence of twin anemia-polycythemia sequence (0% vs. 43.8%, P< 0.001). Conclusion: Since the introduction of FLS, the survival rate of TTTS patients has improved. The Solomon technique resulted in better dual survival rates than selective FLS.

        Survival and Morbidities in Infants with Birth Weight Less than 500 g: a Nationwide Cohort Study

        Background: This study aimed to investigate the survival and morbidities of infants in the Korean Neonatal Network (KNN) with birth weight (BW) < 500 g. Methods: The demographic and clinical data of 208 live-born infants with a BW < 500 g at a gestational age of ≥ 22 weeks who were treated in the neonatal intensive care units of the KNN between 2013 and 2017 were reviewed. Results: The survival rate of the infants was 28%, with a median gestational age and BW of 243/7 weeks (range, 220/7–336/7) and 440 g (range, 220–499), respectively. Multivariable Cox proportional hazards analysis demonstrated that survival to discharge was associated with longer gestation, higher BW, female sex, singleton gestation, use of any antenatal corticosteroids, and higher Apgar scores at 5 minutes. The overall survival rates were significantly different between the BW categories of < 400 g and 400–499 g. However, there was no significant difference in the incidence of any morbidity between the BW groups. Half of the deaths of infants with BW < 500 g occurred within a week of life, mainly due to cardiopulmonary and neurologic causes. The major causes of death in infants after 1 week of age were infection and gastrointestinal disease. Among the surviving infants, 79% had moderate to severe bronchopulmonary dysplasia, 21% underwent surgical ligation of patent ductus arteriosus, 12% had severe intraventricular hemorrhage (grade III–IV), 38% had sepsis, 9% had necrotizing enterocolitis (stage ≥ 2), and 47% underwent laser treatment for retinopathy of prematurity. The median length of hospital stay was 132 days (range, 69–291), and 53% required assistive devices at discharge. Conclusion: Despite recent advances in neonatal intensive care, the survival and morbidity rates of infants with BW < 500 g need further improvement.
