Background/Aims: Left-sided diverticulitis is increasing in Japan, and many studies report that left-sided diverticulitis is more likely to be severe. Therefore, it is important to identify the features and risk factors for left-sided diverticulitis. We hypothesized that left-sided diverticulitis in Japan is related to obesity and conducted a study of the features and risk factors for this disorder in Japan. Methods: Right-sided di-verticulitis and left-sided diverticulitis patients (total of 215) were compared with respect to background, particularly obesity-related factors to identify risk factors for diverticulitis. Results: There were 166 (77.2%) right-sided diverticulitis patients and 49 (22.8%) left-sided diverticulitis patients. The proportions of obese patients (body mass index ≥25 kg/m2, p=0.0349), viscerally obese patients (visceral fat area ≥100 cm2, p=0.0019), patients of mean age (p=0.0003), and el-derly patients (age ≥65 years, p=0.0177) were significantly higher in the left-sided-diverticulitis group than in the right-sided-diverticulitis group. The proportion of viscerally obese patients was significantly higher in the left-sided-diverticulitis group than in the left-sided-diverticulosis group (p=0.0390). Conclusions: This study showed that obesity, particularly visceral obesity, was a risk factor for left-sided diverticulitis in Japan. (Gut Liver 2013;7:532-538)
Analgesic Effects of Toad Cake and Toad-cake-containing Herbal Drugs

Objectives: This study was conducted to clarify the analgesic effect of toad cake and toad-cake-containing herbal drugs. Methods: We counted the writhing response of mice after the intraperitoneal administration of acetic acid as a nociceptive pain model and the withdrawal response after the plantar surface stimulation of the hind paw induced by partial sciatic nerve ligation of the mice as a neuropathic pain model to investigate the analgesic effect of toad cake and toad-cake-containing herbal drugs. A co-treatment study with serotonin biosynthesis inhibitory drug 4-chloro-DL-phenylalanine methyl ester hydrochloride (PCPA), the catecholamine biosynthesis inhibitory drug α-methyl-DL-tyrosine methyl ester hydrochloride (AMPT) or the opioid receptor antagonist naloxone hydrochloride was also conducted. Results: Analgesic effects in a mouse model of nociceptive pain and neuropathic pain were shown by oral administration of toad cake and toad-cake-containing herbal drugs. The effects of toad cake and toad-cake-containing herbal drugs disappeared upon co-treatment with PCPA, but not with AMPT or naloxone in the nociceptive pain model; the analgesic effect of toad-cake-containing herbal drugs also disappeared upon co-treatment with PCPA in the neuropathic pain model. Conclusion: Toad cake and toad-cake-containing herbal drugs have potential for the treatments of nociceptive pain and of neuropathic pain, such as post-herpetic neuralgia, trigeminal neuralgia, diabetic neuralgia, and postoperative or posttraumatic pain, by activation of the central serotonin nervous system.

Objective: In this study we sought to investigate the clinical factors that affect post progression survival (PPS) in patients with recurrent or persistent clear cell carcinoma (CCC). We utilized the JGOG3017/Gynecological Cancer InterGroup data to compare paclitaxel pluscarboplatin (TC) and irinotecan plus cisplatin (CPT-P) in the treatment of stages I to IV CCC. Methods: We enrolled 166 patients with recurrent or persistent CCC and assessed the impactof variables, including platinum sensitivity, treatment arm, crossover chemotherapy, primarystage, residual tumor at primary surgery, performance status, ethnicity, and tumor reductionsurgery at recurrence on the median of PPS in patients with recurrent or persistent CCC. Results: A total of 77 patients received TC, and 89 patients received CPT-P. The median PPSfor patients with platinum-resistant disease was 10.9 months, compared with 18.8 monthsfor patients with platinum-sensitive disease (hazard ratio [HR]=1.88; 95% confidence interval[CI]=1.30–2.72; log-rank p<0.001). In the multivariate analysis, the platinum sensitivity(resistant vs. sensitivity; HR=1.60; p=0.027) and primary stage (p=0.009) were identified asindependent predictors of prognosis factors for PPS in recurrent or persistent CCC. Conclusions: Our findings revealed that platinum sensitivity and primary stage are clinicalfactors that significantly affect PPS in patients with recurrent or persistent CCC as well as other histologic subtypes of ovarian cancer. PPS in patients with recurrent CCC shouldestablish the basis for future clinical trials in this population.
Effects of Income Inequality on Growth through Efficiency Improvement and Capital Accumulation

In the present paper, the inverted-U shape relationship between economic growth and inequality found in Chen (2003) is re-examined. We decompose productivity growth into efficiency improvement, capital accumulation and technological progress and then ascertain their determinants by employing fixed effects and dynamic panel models. In particular, this paper focuses on the question of how economic inequality affects capital accumulation and efficiency improvement. Key findings are that inequality enhances efficiency improvement as well as capital accumulation and then undermines them as inequality widens. However, other factors such as human capital, openness and government consumption have different effects on them.

In this paper we consider a regional monetary cooperation in East Asia after we look at recent movements of exchange rates of East Asian currencies and exchange rate policies in East Asian countries. We regard that the recent movements of exchange rates have been related with their reactions to the US dollar depreciation that has been caused by the current account deficit of the United States and changes in capital flows between the United States and the rest of the world. We found that East Asian currencies were classified into at least two groups; one group’s currencies have been appreciated against the US dollar while the other’s currencies have been pegged to the US dollar. We stress coordination failure in exchange rate policies among East Asian countries that causes biased change in exchange rates among the intra-regional currencies. It is necessary to make regional coordination in exchange rate policies for a desirable exchange rate system in East Asia. At last, we suggest some policy recommendations related with regional cooperation in exchange rate policies in East Asia.

This paper presents a novel prototype of an active auxiliary quasi-resonant snubber(Auxiliary Quasi-Resonant Commutation Block-Link; ARCB)-assisted three voltage source soft switching space voltage vector modulated PFC rectifier, which uses Zero Voltage Soft Switching (ZVS) commutation. The operating principles of this digitally-controlled three phase soft switching PWM-PFC rectifier system with an instantaneous power feedback scheme are illustrated and its steady-state performance is evaluated using computer-aided simulation analysis.
