        Omeprazole 투여시 위산분비 억제능에 대한 CYP2C19 의 영향

        Background/Aims: CYP2C19 has an important role in the catabolism of several proton pump inhibitors. However, the relative contribution of CYP2C19-mediated metabolism varies among the different proton pump inhibitors. We investigate to determine the effect of CYP2C19 genotype status on intragastric pH during dosing with omeprazole. Methods: The subjects were 16 healthy volunteers. Their CYP2C19 genotype status was determined by a polymerase chain reaction-restriction fragment length polymorphism method. Twenty-four-hour monitoring of intragastric acidity were performed on the day without medication and day 4 after omeprazole administration. Results: A single dose of omeprazole significantly decreased 24 hour intragastric acidity (1.7±0.3 vs. 5.1±0.4). Subjects were divided into three groups on the basis of their CYP2C19 genotype status: homozygous extensive metabolizers (Ho-EMs, n=5), heterozygous extensive metabolizers (Ht-EMs, n=6), and poor metabolizers (PMs, n=5). Median 24 hour intragastric pH in the Ho-EM group was 3.1 compared with 5.5 in Ht-EM group and 5.9 in PM group(P<0.05). The median pH during omeprazole administration was influenced by CYP2C19 genotype. On the other hand, the Helicobacter pylori infection did not influence the median intragastric pH during omeprazole administration (p= NS). Conclusions: The effects of omeprazole on intragastric acidity is influenced by the CYP2C19 polymorphism. (Korean J Gastroenterol 2001;38:399-404)

        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

The educational curricula, educational method, and educational contents in the information society will be drastically different from today's and developed into the information-oriented ones.

      • GPS/GLONASS 결합에 의한 차량의 이동궤적의 정확도 분석

        Satellite visibility, accuracy, and availability were increased by the combined GPS/GLONASS. But, there are some problems such as differences in the time frame, differences in the coordinate datum, and the problem of solving carrier phase ambiguities in the combined carrier frequency solutions due to different GLONASS frequency. Therefore, the author will access the accuracy analysis of positioning using the combined PGS/GLONASS, and intend to study the characteristics of the combined GPS/GLONASS with considered the rate of data acquisition according to satellite visibility, elevation mask angle and kinematic relative position at the combined GPS/GLONASS.

      • 키위(Actinidia chinensis)의 休眠枝揷에 關한 硏究

        키위 苗木을 短期間內에 大量으로 生産할수 있는 方法을 究明하기 위하여 'Hayward' 品種을 供試하여 休眠枝揷을 實施한 바 그 重要 結果는 다음과 같다. 1. 揷木床土의 加溫 (24℃와 28℃)은 揷穗의 發根 및 地下部 生育에 도움을 주지 못하였다. 2. 揷穗 基部의 Rooton 塗布는 揷穗의 發根 및 뿌리의 生育을 촉진하였다. 3. 揷木時期에 있어서 4月 上旬 보다는 3月 中旬에 揷木하였을 때가 發根率, 根의 生長 그리고 出芽率이 良好하였다. This study was conducted to establish the effective methods of dormant wood cutting of kiwifruit tree(Actinidia chinensis Planch. cv. 'Hayward'). Cuttings from one-year old shoots of six-year old tree were taken in mid Feb. and planted under mist in polyethylene film house. Results obtained are summarized as follows : 1. Heating of soil temperature (24℃ and 28℃) didn't improve rooting and shooting percentage of cuttings. 2. Treatment of the basal cut end with Rooton increased rooting percentage and accelerated root growth. 3. Based on the rooting percentage, number of roots per cutting and shooting percentage, the suitable time for dormant wood cutting was found to be mid March rather than early April.

      • LIS를 이용한 數値地圖와 數値地積圖의 重疊에 따른 正確度 分析에 관한 연구

        The cadastral administration is centered around cadastral maps. It increasing that demand of the public for effective administration and higher quality of service through the development of application program of cadastral maps maintenance. Also, there has been much demands for the digital information of cadastral maps in other systems. Land Information System is the answer to meet with the public for effective administration and higher quality of service. In this study, The graphic and attribute data of cadastral maps were integrated on the same DBMS. Also, pointing out difficulties of database implementation and many experiences in the development of application program will be used effectively for further expansion of the automation of cadastral administration.

      • 교량의 실시간 거동 모니터링을 위한 GPS의 적용

        Real Time Kinematic GPS will allow a multitude of monitoring, navigation and engineering applications. Applications inclued real time deformation and movement monitoring and real time automated plant control. In a low dynamic environment, with a short baseline length, it is possible to position a roving receiver to an approximate accuracy of less than 5mm within a few seconds of thestart of the ambiguity. One Engineering application is the monitoring of the movement of cable suspension bridges. Such structures are capable of movements of up to a few meters under adverse wind conditions or heavy traffic loads. The use of relative OTF positioing at strategic points upon the bridge would allow close monitoring of the bridge's movements and calculate deformation and long-term deterioration factors for the bridge. Future bridge designs and traffic management schemes could also benefit from the monitoring system.

      • 구동 전동기 입력 전류 및 전력신호에 의한 전동 게이트밸브 진단연구

        The motor-operated valves are used extensively within nuclear power plants in safety-related and balance-of-plant systems and located in almost all plant fluid systems. Their failures have resulted in significant plant maintenance efforts. More important, the operational readiness of nuclear plant safety-related systems has often been affected by MOV degradation and failure. The performance of motor-operated valves should be evaluated to ensure the nuclear safety. It will be accomplished by understanding the state of the art of the diagnostic method and developing the appropriate new methodology to evaluate the measuring parameters to predict the valve performance Measured and evaluated were the changes of the motor current signature taken by simulating the abnormal conditions on the gate MOVs used extensively in the nuclear power plants. Based on the evaluation results, the conclusion can be as follows: 1.The DC signature of motor power shows more sensitive than the current. Both of them have similar behavior during the simulation. 2.The changes of the valve stroke and magnitude with appropriate management of the signature occured during the stroke can be used as an index showing the valve failure and degradation. 3.The diagnosis of valve by simultaneous use of the valve current and power is more useful than the method using the motor current only.

      • HDPE 관의 TEE 성형에 대한 유한요소해석

        고밀도 폴리에틸렌(HDPE) 파이프 중 TEE 성형 공정이 강소석 유한요소해석 툴인 DEFORM을 이용하여 해석되었다. 많은 HDPE 파이프 중 TEE는 주관을 가지관과 연결하여 온수의 유동방향을 변화시키는데 사용된다. HDPE 파이프는 온수의 온도를 유지하기 위한 절연재로서 사용된다. HDPE 파이프에 대한 TEE 성형과 맞대기 용접 공정은 기존의 압출용접 공정에 비해 용접부의 강도를 향상시키는 개성된 공정이다. 초기의 구멍 형상과 같은 설계 인자들이 실험과 해석을 통하여 얻어졌고, 실제 제조공정에 적용되었다. TEE forming process on HDPE(High Density PolyEthylen) pipe has been analyzed by using rigid plastic finite element analysis code. DEFORM. TEE of HDPE pipes is necessary to connect a main pipe with a branch pipe and change the flow direction of hot water. A HDPE pipe is used as insulator to maintain the temperature of hot water. TEE forming and butt welding process on a HDPE pipe is a updated process improving the strength of welding part compared to conventional extrusion welding process. The design parameters, such as an initial hole, shape have been obtained from the experiment and rigid-plastic finite element analysis, which is applied to the actual manufacturing process.
