      • Large Volume Sample Stacking 방법과 Head Column Stacking 을 이용한 프로틴과 펩타이드의 시료 농축

        현재 세포 성분 내에서의 단백질들의 기능규명, 새로운 신약의 개발 등은 매우 중요함을 차지하고 있다. 그러나 현재의 분석 기술인 2-D 젤 모세관 전기 영동 장치, 액체 크로마토그래피 그리고 질량 분석기 등의 여러 가지 방법들은 세포 내에서 극 미량의 단백질들을 분석하는데 있어서 충분한 감도를 제공하지 못한다. 이 실험에서는 모세관 전기 영동 장치를 이용한 단백질과 펩타이드의 분석에서 시료의 낮은 농도의 감도를 향상시키기 위하여 시료 농축 방법인 Large Volume Sample Stacking 방법과 Head column stacking 방법을 이용하여 분석을 하였다. α - lactalbumin과 trypsin inhibitor들과 같은 단백질들을 위의 시료 농축 방법을 이용하여 낮은 시료의 농도 0.01 ㎍/㎖까지 농축이 가능하였다. 또한 효소로 처리한 잘려진 단백질들의 절편들도 극 미량의 low femtomole 수준까지 검출이 가능하였다. 그리고 low femtomole 수준의 단백질 절편들을 규명하기 위하여 CE-MS를 사용하였다 The identification proteins in the cell is very important for Proteomics research. However, current analytical techniques such as 2-D gel electrophoresis, 1iquid chromatography and mass spectrometry do not provide enough sensitivity for cell proteins especially for low-abundant proteins. Therefore, protein and peptide concentration method should be developed for the easy operation of current identification technologies. In order to enhance the concentration sensitivity in capillary electrophoresis of proteins deptides, large volume sample stacking using electroosmotic flow pump and LVSEP combined with Head column stacking were applied. Proteins such as α -Lactalbumin and trypsin inhibitor were successfully stacked and their limits of detection were below 0.01㎍/㎖. A large volume of digested protein samples was also stacked and the enhanced sensitivity allowed one to analyze protein digests at low femtomole levels. CE-MS was employed for the identification of protein digests at low femtomole levels.

      • 갑상선암에서 MAGE 1-6 mRNA를 검출하는MAGE common primer의 의의

        Background: MAGE (melanoma antigen gene) was named as cancer/testis specific antigens since their expression is only detected in the testis or cancer cells. Because of their relatively specific expression in the cancer cells, they have been considered as the early diagnosis of several cancers, or the appropriate targets for the specific immunotherapy mediated by cytotoxic T lymphocytes. Therefore, there were many reports about the expression of MAGE genes in various types of malignant tumors, but only a few reports in human thyroid neoplasms. The purpose of this study was to determine whether the MAGE common primers expressed in different histological types of thyroid tumors and to see the clinical usefulness of MAGE common primers on the thyroid tumors. Methods: Fifty-nine patients who underwent thyroidectomy at Kosin Medical Center from January, 2001 to April, 2002 were included in the study. Of the 59 patients 40 were papillary carcinoma, 4 follicular carcinoma, 3 medullary carcinoma, 1 anaplastic carcinoma, 4 nodular hyperplasia, 3 adenomatous goiter, and 4 follicular adenoma. Total cellular mRNA was extracted from those 48 cancer tissues, 11 benign tissues and additional eleven normal control thyroid tissues. Nested reverse transcriptase-polymerase chain reactions (nested RT-PCR) with the MAGE common primers designed to detect MAGE 1-6 genes were employed in this study. With probes confirmed by DNA sequencing. Results: Of those 48 cases of thyroid carcinomas 10 cases (20.8%) were expressed MAGE common primers. In the 40 papillary carcinoma, MAGE common primers were expressed in 17.5%. In the four follicular carcinoma, they were expressed in two cases, and in one case of one anaplatic carcinoma. However, in the three cases of medullary carcinoma, MAGE gene was not expressed. None of 11 benign tumors and 11 normal tissues expressed any of these MAGE common primers. The sensitivity of MAGE common primers in thyroid tumors was 20.8% and the specificity was 100%. Conclusion: These results suggested that MAGE common primers were expressed low in the malignant thyroid tumors but it was not expressed in benign tumors and normal tissues. It is suggested that MAGE common primers can be used for preoperative differential diagnosis of follicular carcinoma from benign follicular adenoma which cannot be made by fine needle aspiration cytology of thyroid tissue. However, further study is needed for clinical application of MAGE common primers expression as tumor markers in the thyroid tumors.

      • 항결핵 치료과정에서 호전되었다가 악화된 뇌 결핵종 1예 보고

        In spite of nationmide tuberculosis control program, tuberculosis is still prevalent in Korea. Accordingly, tuberculous meningitis also poses one of the main trouble some problems. Recently, We experienced a perplexing case of tuberculous menin-gitis and tubercdoma in a 24-year old female patient. She was admitted to our hospital owing to meningeal irritation sign, aphasia, VI nerve palsy, and ,fever. CSF analysis showed a finding consistent with that of presumptive tuberculous meningitis.. Initial MRI of the brain revealed a large mass(tuhercu1oma) on left parieto-temporal area. Antitubeculous regimen(1soniazid (INH), rifampin(RMP), ethambutol(EMB), pyrazinamide(PZA)} of meningeal dose was given immediately, and her symptoms and signs were dramatically disappeared with a few days after treatment. She was discharged with improved condition and under maintenance treatment of tuberculosis. Follow-up one year after treatment. revealed that the previous lesion was nearly diminished. She showed no symptom at all until she recognized a total blindness of sudden onset on her left eye a t the 18th month of treatment. Brain MRI checked at that time showed multiple masses of tuberculoma scattered over the brain base and one of them invaded left optic nerve. In our opinion, the cause of relapse in this patient might he explained by the selection and ultimate replication of a minor population of resistant strain. Although such relapse is an extremely rare event, we think that we should always pay attention to this possible resistance problem every time we meet patients with tuberculous meningitis.

        Experiments were conducted to investigate the atomization characteristics of rice-bran oil which is an alternative fuel. For the study, an ultrasonic system, an injection system, and six different nozzles(three kinds of single-hole type and three kinds of pintle-type) were made To investigate the effects of ultrasonic energy on a high viscous fuel, an immersion method and a phase doppler particle analyzer were used for the measurements of droplets distribution. Diameters of rice-bran oil droplets were apparently decreased by applying the ultrasonic energy, with the increasing open pressure, and when samples were collected at more distant position from the nozzle.

        Toluene, Xylene, Trichloroethylene 투여가 흰쥐의 시상하부와 뇌하수체의 GnRH, GnRH Receptor, Pit-1 유전자 발현에 미치는 영향

        The workplace exposure of chemicals has steadily increased, therefore the concern for subsequent effect on reproductive outcome has been an important issue in occupational medicine. In previous studies, higher rates of spontaneous abortion, reduced fertility and menstrual disorder among women, and an impairement of sperm quantity and quality among men have been associated with a wide variety of chemical agents. This study was conducted to evaluate the effects of toluene, xylene and trichloroethylene(TCE) injection on the mRNA levels of GnRH, GnRH receptor and Pit-1 genes in male rats hypothalamus and pituitary and the effects on the plasma levels of FSH, LH, prolactin and testosterone. Sprague-Dawley male rats were divided into five groups of five each according to concentration of toluene, xylene and TCE. The rats were injected subcutaneously to 0, 50, 100, 200, 400 mg/kg body weight/day of toluene, xylene and TCE, respectively for 6 days. Rat brains were excised and hypothalamus and pituitary were separated. Reverse transcription-polymerase chain reaction (RT-PCR) and RNase protection assay(RPA) were used to evaluate the GnRH, GnRH receptor and Pit-1 mRNA levels. Plasma concentrations of FSH, LH, prolactin and testosterone were assayed by radioimmunoassay(RIA). The results were as follows; 1. GnRH, GnRH receptor and Pit-1 mRNA levels in toluene and xylene injected groups, and GnRH receptor mRNA levels in TCE injected group were lowered dose-dependently. Especially, GnRH receptor and Pit-1 mRNA levels in 200mg/kg of toluene injected group, and GnRH, GnRH receptor and Pit-1 mRNA levels in 400mg/kg of toluene injected group were significantly lower than control group (p<0.05). GnRH receptor and Pit-1 mRNA levels in 400mg/kg of xylene injected group, and GnRH receptor mRNA levels in 400mg/kg of TCE injected group were significantly lower than control group(p<0.05). 2. The plasma levels of prolactin and testosterone in 400mg/kg of toluene injected group, and LH in 100, 200 and 400mg/kg of xylene injected group, and testosterone in 400 mg/kg of TCE injected group were significantly lower than control group(p<0.05). In conclusion, we speculated that toluene and xylene affected reproductive system secondarily through hypothalamus-pituitary axis, and TCE affected directly through steroidogenesis. And we recommended that further study for assessment of the reproductive toxicity of mixed organic solvent exposures should be conducted.

        Kyphoscoliosis is a disease manifested as lateral curvature of spine and accompanying rotation of the vertebrae, resulting in distortion of rib cage. Restrictive lung disease and pulmonary hypertension progressing to cor pulmonale are the major cause of mortality in patients with kyphoscoliosis. We had experienced a successful spinal and caudal epidural anesthesia for the open reduction and internal fixation with plate of fractured femur shaft in two patients with severe kyphosoliosis. We had chosen regional anesthesia as an anesthetic method because of there poor pulmonary function and severe deformities of thoracolumbar spine. We could achieve the adequate anesthetic sensory dermatome level, T_(5) and T_(8), for operation without any significant deterioration of blood pressure and respiration. We recommended that regional anesthesia is a useful anesthetic method for a patients with severe kyphoscoliosis when the operation on hip or lower extremity is required.

        Clinical Score System to Differentiate Severe Fever with Thrombocytopenia Syndrome Patients from Patients with Scrub Typhus or Hemorrhagic Fever with Renal Syndrome in Korea

        Background: Severe fever with thrombocytopenia syndrome (SFTS) is an emerging infectious disease with high mortality in East Asia. This study aimed to develop, for primary care providers, a prediction score using initial symptoms and basic laboratory blood tests to differentiate between SFTS and other endemic zoonoses in Korea. Methods: Patients aged ≥ 18 years diagnosed with endemic zoonoses during a 3-year period (between January 2015 and December 2017) were retrospectively enrolled from 4 tertiary university hospitals. A prediction score was built based on multivariate logistic regression analyses. Results: Of 84 patients, 35 with SFTS and 49 with other endemic zoonoses were enrolled. In multivariate logistic regression analysis, independent predictors of SFTS included neurologic symptoms (odds ratio [OR], 12.915; 95% confidence interval [CI], 2.173–76.747), diarrhea (OR, 10.306; 95% CI, 1.588–66.895), leukopenia (< 4,000/mm3 ) (OR, 19.400; 95% CI, 3.290– 114.408), and normal C-reactive protein (< 0.5 mg/dL) (OR, 24.739; 95% CI, 1.812–337.742). We set up a prediction score by assigning one point to each of these four predictors. A score of ≥ 2 had 82.9% sensitivity (95% CI, 71.7%–87.5%) and 95.9% specificity (95% CI, 88.0%–99.2%). The area under the curve of the clinical prediction score was 0.950 (95% CI, 0.903–0.997). Conclusion: This study finding suggests a simple and useful scoring system to predict SFTS in patients with endemic zoonoses. We expect this strategic approach to facilitate early differentiation of SFTS from other endemic zoonoses, especially by primary care providers, and to improve the clinical outcomes.

      • Effects of melatonin on in vitro maturation of porcine oocyte and expression of melatonin receptor RNA in cumulus and granulosa cells

        <P>Abstract: </P><P>Melatonin is a multifunctional molecule that mediates several circadian and seasonal processes in animal reproduction. Melatonin and its metabolites are antioxidants and free radical scavengers. We investigated the effects of melatonin on porcine oocyte maturation and embryo development. We then investigated the local expression of the melatonin receptor 1 (MT1) gene in cumulus cells, granulosa cells, and the oocytes with the reverse transcription-polymerase chain reaction (RT-PCR) method. We further evaluated the antioxidant effects [reactive oxygen species (ROS) levels in cumulus-oocytes complexes] of melatonin supplementation during in vitro maturation (IVM). Compared with control, melatonin supplementation (10 ng/mL) during IVM resulted in a greater proportion of oocytes extruding the polar body (75.6% versus 84.6%). Significantly greater proportion of parthenogenetically activated oocytes developed to blastocysts when the in vitro medium was supplemented with melatonin; however, cleavage frequency and blastocyst cell number were not affected by the treatment. RT-PCR analysis revealed the expression of MT1 gene in cumulus and granulosa cells but not in oocytes. Melatonin-treated oocytes had significantly lower levels of ROS than did control (untreated) oocytes. We conclude that exogenous melatonin has beneficial effects on nuclear and cytoplasmic maturation during porcine IVM. Some of the observed effects may be mediated by receptor binding and while others may have been receptor independent, e.g., direct free radical scavenging.</P>
